A retrospective study on epidemiological analysis of pre-hospital emergency care in Hangzhou, China.

Out-of-hospital cardiac arrest (OHCA) is a leading cause of global mortality, with numerous factors influencing the patient survival rate and prognosis. This study aimed to evaluate the OHCA epidemiology in China and elaborate on the current Hangzhou emergency system status. This retrospective analysis was based on the medical history system of the Hangzhou Emergency Center registered from 2015-2021. We provided a detailed description of OHCA characteristics and investigated the factors affecting the success rate of emergency treatment in terms of epidemiology, causes of onset, bystander rescu

Where Trouble Starts: Communication Breakdown in a Complex Emergency Call.

Emergency calls require efficient communication between caller and call taker to establish a need for assistance and dispatch help quickly. Analyzing communication processes at this first link in the emergency medical care chain has important implications for improving the quality of emergency care across the health system. This paper examines an interaction between a call taker and a caller requesting assistance at the scene of a family murder, using a hybrid interactional sociolinguistic approach to analysis. We also draw from court testimony. We demonstrate how several factors contribute to

System implications of the ambulance arrival-to-patient contact interval on response interval compliance.

BACKGROUND: In some emergency medical services (EMS) system designs, response time intervals are mandated with monetary penalties for noncompliance. These times are set with the goal of providing rapid, definitive patient care. The time interval of vehicle at scene-to-patient access (VSPA) has been measured, but its effect on response time interval compliance has not been determined. PURPOSE: To determine the effect of the VSPA interval on the mandated code 1 (< 9 min) and code 2 (< 13 min) response time interval compliance in an urban, public-utility model system. METHODS: A prospective, ob

Multiple-Casualty Incident Following Lightning Strike at Mount Giewont: An Analysis of Disaster Rescue Response.

On August 22, 2019, several cloud-to-ground currents struck the top of the popular hiking mountain Giewont (Tatra Mountains, Poland). At the scene, first aid and evacuation were provided by mountain emergency rescue services. The injured patients received care and were initially stabilized at a local general hospital. Some of the victims were then relocated to other nearby hospitals, regional trauma centers, or regional burn centers. This study is a retrospective analysis of regional health system response. The official records of the disaster response from the institutions involved were exami

Symptoms presented during emergency telephone calls for patients with spontaneous subarachnoid haemorrhage.

BACKGROUND: A spontaneous subarachnoid haemorrhage (SAH) is one of the most critical neurological emergencies a dispatcher can face in an emergency telephone call. No study has yet investigated which symptoms are presented in emergency telephone calls for these patients. We aimed to identify symptoms indicative of SAH and to determine the sensitivity of these and their association (odds ratio, OR) with SAH. METHODS: This was a nested case-control study based on all telephone calls to the medical dispatch center of Copenhagen Emergency Medical Services in a 4-year time period. Patients with SA

Sudden unconsciousness during local anesthesia.

This case demonstrates the necessity of adequate emergency management during dental treatment. A 57-year-old man loses consciousness immediately after the injection of a dental local anesthetic. For medical assistance, a neighboring general practitioner is first telephoned without success. After approximately 15 minutes, an ambulance is telephoned, but the caller gave no indication of the situation being life threatening. Finally, an emergency doctor is dispatched and arrives approximately 35 minutes after the emergency situation began. The electrocardiogram shows a supraventricular tachycardi

Prehospital Management of Peripartum Neonatal Complications by Helicopter Emergency Medical Service in the South West of the Netherlands: An Observational Study.

OBJECTIVE: Emergency medical service (EMS) is responsible for prehospital care encompassing all ages, irrespective of injury cause or medical condition, which includes peripartum emergencies. When patients require care more advanced than the level provided by the national EMS protocol, an EMS physician-staffed Dutch helicopter emergency medical service (HEMS) may be dispatched. In the Netherlands in 2016, there were 21.434 planned home births guided by midwives alone without further obstetric assistance, accounting for 12.7% of all births that year. However, there are no clear data available t

The Estimated Prevalence of Multiple Record Encounters in the National Emergency Medical Services Information System.

OBJECTIVES: The National Emergency Medical Services Information System (NEMSIS) is a large repository of de-identified Emergency Medical Services (EMS) data supported by the National Highway Traffic Safety Administration and is a cornerstone of large database EMS research. A known concern with large EMS datasets like NEMSIS is that multiple records may exist for a single episode of patient care. However, the extent of potentially related records is not well described. We evaluated potential EMS episodes of care contained within NEMSIS. METHODS: We used an observational study design and conduc

Grady Health System's Mobile Integrated Health Program: A Statistical Analysis of Low-Acuity 911 Calls.

Grady's Mobile Integrated Health (MIH) program works to manage outpatient health concerns that otherwise burden EDs, improve quality of care, and connect patients to the appropriate level of care and resources. This prospective study collected data from 09/01/2019-03/31/2020 to analyze Grady's MIH response to low-acuity 911 calls compared to a traditional EMS (ACLS/BLS) response. A total of 2,759 EMS calls were reviewed. These calls comprised the four most common emergency medical dispatch codes for Grady's MIH response: i) "sick person other pain," ii) "diabetic alert behaving normally," iii)

Automated loss of pulse detection on a consumer smartwatch.

Out-of-hospital cardiac arrest is a time-sensitive emergency that requires prompt identification and intervention: sudden, unwitnessed cardiac arrest is nearly unsurvivable1-3. A cardinal sign of cardiac arrest is sudden loss of pulse4. Automated biosensor detection of unwitnessed cardiac arrest, and dispatch of medical assistance, may improve survivability given the substantial prognostic role of time3,5, but only if the false-positive burden on public emergency medical systems is minimized5-7. Here we show that a multimodal, machine learning-based algorithm on a smartwatch can reach performa

Opioid-Associated Out-of-Hospital Cardiac Arrest: Distinctive Clinical Features and Implications for Health Care and Public Responses: A Scientific Statement From the American Heart Association.

Opioid overdose is the leading cause of death for Americans 25 to 64 years of age, and opioid use disorder affects >2 million Americans. The epidemiology of opioid-associated out-of-hospital cardiac arrest in the United States is changing rapidly, with exponential increases in death resulting from synthetic opioids and linear increases in heroin deaths more than offsetting modest reductions in deaths from prescription opioids. The pathophysiology of polysubstance toxidromes involving opioids, asphyxial death, and prolonged hypoxemia leading to global ischemia (cardiac arrest) differs from that

Regional disparities in emergency medical services in Poland: a preliminary study.

INTRODUCTION: Equitable access to emergency medical services (EMS) is a core principle of health system performance. However, in territorially heterogeneous countries, resource allocation often fails to reflect interacting demographic and spatial determinants. In Poland, systematic nationwide evidence integrating demographic, spatial, and operational determinants of EMS utilisation remains limited. This study examines whether EMS allocation aligns with actual demand patterns and identifies structural imbalances across regions. METHODS: A nationwide cross-sectional analysis was conducted using
