Conceptual aspects of attributable risk with recurrent disease events.

The concept of attributable risk is extended to cover disease events which are potentially recurrent. Whereas the conventional attributable risk parameter allows quantification of the proportion of diseased individuals due to a certain risk factor, the parameter introduced here quantifies the proportion of disease events which is attributable to a specific exposure variable. We accomplish this by taking into account not only the probability of ever being affected by the disease but also the exposure-specific number of occurrences of the disease event within a defined time period. Does cannabis onset trigger cocaine onset? A case-crossover approach.

Psychiatric researchers tend to select the discordant co-twin design when they seek to hold constant genetic influence while estimating exposure-associated disease risk. The epidemiologic case-crossover research design developed for the past two decades represents a viable alternative, not often seen in psychiatric studies. Here, we turn to the epidemiologic case-crossover approach to examine the idea that cannabis onset is a proximal trigger for cocaine use, with the power of "subject-as-own-control" research used to hold constant antecedent characteristics of the individual drug user, includ

[Short term effect of urban air pollution on respiratory insufficiency due to chronic obstructive pulmonary disease. Synthesis of studies published from 1962 to January 2000].

This review presents a synthesis of studies published from 1962 to 2000 on the relations between air pollution and chronic obstructive pulmonary disease (COPD): 12 ecological epidemiological studies, 6 epidemiological panel studies, and 11 controlled human exposure trials. The controlled trials, ecological time-based epidemiological studies and panels are examined successively followed by a discussion of their methodology and results. The controlled trials either do no highlight effects or show effects having no clinical significance since variations are similar to physiological variability. F

Surveillance system assessment in Guinea: Training needed to strengthen data quality and analysis, 2016.

The 2014-2016 Ebola virus disease outbreak revealed the fragility of the Guinean public health infrastructure. As a result, the Guinean Ministry of Health is collaborating with international partners to improve compliance with the International Health Regulations and work toward the Global Health Security Agenda goals, including enhanced case- and community-based disease surveillance. We assessed the case-based disease surveillance system during October 1, 2015-March 31, 2016, in the Boffa prefecture of Guinea. We conducted onsite interviews with public health staff at the peripheral (health c

Patterns of research. Oral contraceptives and cervical cancer.

Original papers on the oral contraceptive use-cervical cancer relationship are analysed. The purpose of this study was to ascertain the biases of the original articles collected in relation to various characteristics of any investigation. Papers were located by using MEDLINE, reviewing the references of each article identified by MEDLINE, and then reviewing the contents of those journals in which an original could be published. Fifty-five publications (from 49 original studies) were graded as to quality and classified as biased or unbiased. Nineteen studies were considered unbiased. The most c

Case-control research. Temporal precedence and other problems of the exposure-disease relationship.

We assessed the principle of temporal precedence in recent case-control studies demonstrating the alleged associations between tampon use and toxic shock syndrome and between aspirin use and Reye's syndrome. For both relationships, we considered four components of the exposure-disease association, including: (1) establishing that the agent preceded the disease, (2) selecting an index time, (3) defining criteria for classifying a patient as "exposed," and (4) avoiding the bias that occurs when use of the etiologic agent was influenced by an early manifestation of the disease. The problems can b

Oral contraceptive use and venous thromboembolism: a consideration of the impact of bias and confounding factors on epidemiological studies.

Recent epidemiological studies have reported an approximately two-fold, significant, increased odds ratio for venous thromboembolism in users of third-generation oral contraceptives (OCs) compared to users of second-generation OCs. However, in each study, this association is of borderline statistical significance, and the studies do not indicate an increase in the absolute risk of venous thromboembolism for users of third-generation OCs compared with the data from prior studies primarily involving second-generation products. The data derived from these recent studies show a lower reported inci

Blinding and expectancy confounds in psychedelic randomized controlled trials.

Introduction: There is increasing interest in the potential for psychedelic drugs such as psilocybin, LSD and ketamine to treat several mental health disorders, with a growing number of randomized controlled trials (RCTs) being conducted to investigate the therapeutic effectiveness of psychedelics.Areas covered: We review previous literature on expectancy effects and blinding in the context of psychedelic RCTs - literature which strongly suggest that psychedelic RCTs might be confounded by de-blinding and expectancy. We conduct systematic reviews of psychedelic RCTs using Medline, PsychInfo an

French administrative health care database (SNDS): The value of its enrichment.

SNIIRAM/SNDS, the French administrative health care database, covers around 99% of the population. Its main limitation is the absence of clinical information and biological results. This report exposes the value of SNIIRAM/SNDS enrichment by external databases, and the linkage issues. It is illustrated by examples: the well-known population-based cohort CONSTANCES created to answer to epidemiological research questions with a specific interest on occupational and social factors, chronic diseases, and aging; the CANARI study, a regional-based study that collected Gleason score in all pathology

Research gaps in evaluating the relationship of meat and health.

Humans evolved as omnivores and it has been proposed that cooking meat allowed for evolution of larger brains that has led to our success as a species. Meat is one of the most nutrient dense foods, providing high-quality protein, heme iron, zinc, and vitamins B6 and B12. Despite these advantages, epidemiologic studies have linked consumption of red or processed meat with obesity, type 2 diabetes, cardiovascular diseases, and cancers of multiple organs. Most observational studies report small, increased relative risks. However, there are many limitations of such studies including inability to a

Oral corticosteroid therapy in chronic rhinosinusitis without polyposis: a systematic review.

BACKGROUND: Recognition of inflammation in the pathophysiology of chronic rhinosinusitis (CRS) has caused corticosteroid therapy in CRS to gain favor. A systematic evaluation of oral steroid use in CRS without nasal polyps (CRSsNP) has not been previously conducted. The objective of the study was to assess evidence on oral steroid therapy in CRSsNP, via a systematic literature review. METHODS: Ovid and PubMed databases were searched for studies on oral steroid therapy in CRSsNP. Manuscripts were reviewed and graded by evidence-based medicine (EBM) level. RESULTS: A total of 33 studies met in

How to assess the external validity of therapeutic trials: a conceptual approach.

BACKGROUND: External validity of study results is an important issue from a clinical point of view. From a methodological point of view, however, the concept of external validity is more complex than it seems to be at first glance. METHODS: Methodological review to address the concept of external validity. RESULTS: External validity refers to the question whether results are generalizable to persons other than the population in the original study. The only formal way to establish the external validity would be to repeat the study for that specific target population. We propose a three-way ap

Aggregating disparate epidemiological evidence: comparing two seminal EMF reviews.

Two seminal reviews (IARC, 2002; CDHS, 2002) of possible health effects from power-frequency EMFs reached partly different conclusions from similar epidemiological evidence. These differences have an impact on precautionary policy. We examine the statistical aggregation of results from individual disparate studies. Without consistent exposure metrics, the advantage of meta-analysis to estimate magnitude of effect is lost. However, counting positive and statistically significant results yields important information. This is not a substitute for meta-analysis, but a fall-back when meaningful met

Relative risk for genetic associations: the case-parent triad as a variant of case-cohort design.

The contribution of this paper is to conceptualize the case-parent triad within an epidemiological framework. We propose that the case-parent triad design is a variant of the case-cohort design. The affected offspring of case-parent triads come from a source cohort of all offspring of parents in a population. We first demonstrate that if the source cohort is restricted to offspring of a certain parental mating type then the relative risk in relation to genetic exposure can be estimated simply from the ratio of the number of exposed to the number of unexposed affected offspring. We then extend

The urban traffic environment and the risk of child pedestrian injury: a case-crossover approach.

We conducted a case-crossover study to quantify the effects of traffic volume and speed on the risk of child pedestrian injury on the school-home journey. We identified 46 children injured as pedestrians on the school-home journey. For each case, we compared the traffic volume and speed on the road where the child was struck with the volumes and speeds on the other roads the child would usually cross. We found strong associations between pedestrian injury risk and high traffic volume [relative risk = 6.3; 95% confidence interval (CI) = 2.1-18.8] and high speed (relative risk = 3.6; 95% CI = 1.

Breast milk jaundice in the newborn. A real entity.

I have reviewed clinical trials that provide data relative to the comparative rates, means, or odds ratio of jaundiced normal breast-fed newborns vs jaundiced normal formula-fed newborns. A pooled analysis of 12 studies revealed moderate jaundice (serum bilirubin level, greater than or equal to 12 mg/dL) in 514 of 3,997 breast-fed vs 172 of 4,255 formula-fed newborns. An analysis of six of these 12 studies demonstrated severe jaundice (serum bilirubin level, greater than or equal to 15 mg/dL) in 54 of 2,655 breast-fed vs ten of 3,002 formula-fed newborns.
