Hysterosalpingo contrast sonography (HyCoSy) with SH U 454 (Echovist) for the assessment of tubal patency.

A total of 88 Fallopian tubes from 44 patients was examined with hysterosalpingo contrast sonography (HyCoSy), hysterosalpingogram (HSG), and laparoscopic chromopertubation (LC) in order to assess their relative accuracy for measuring tubal patency. HyCoSy was done by transvaginal ultrasound and the contrast was SH U 454 (Echovist). The flow of multiple fractions of the contrast medium through each Fallopian tube was observed in real time in appropriate imaging planes by means of a transvaginal probe. Compared with laparoscopic results, we found a sensitivity of 85.2%, a specificity of 85.2%,

[Ultrasonographic hysterosalpingography].

The objective of the investigation was to compare US hysterosalpingography with methods currently used to examine tubal patency, i.e. hysterosalpingography and laparoscopy with chromoperturbation. The examination was made in 39 women with the diagnosis of primary or secondary sterility. Consistent with data in the literature, the authors proved a high concordance (86.8%) with reference methods. US HSG is an ambulatory minimally-invasive method of examination of the uterine cavity and patency of the oviducts, which is well tolerated by patients.

[Diagnosis and treatment of cornual obstruction by transcervical fallopian tube cannulation under hysteroscopy].

Transcervical cannulation of the proximal oviduct was performed with a tube catheter and flexible guidewire under hysteroscopic guidance to evaluate and treat intramural fallopian tube obstruction in 37 patients. Hysteroscopic cannulation with direct visualization by laparoscopy was successful in 69.2% of the obstructed tubes, 77.4% of the cases. 74% of the obstructed tubes diagnosed by HSG and/or laparoscopy was patent after hydrotubation and cannulation by hysteroscopy. Transcervical cannulation of the proximal oviduct is an effective method for evaluation and treatment of cornual obstructio

Accuracy of endovaginal sonography for the detection of fallopian tube blockage.

The patency of 814 fallopian tubes in 414 patients was evaluated by endovaginal sonography immediately prior to hysterosalpingography. In the 659 fallopian tubes that were normal with free spillage, endovaginal sonography did not reveal any tubal or peritubal abnormality (specificity 100%). Of the 64 fallopian tubes with definite hydrosalpinx on hysterosalpingography, only 22 were detected on endovaginal sonography (sensitivity 34%). Four of 57 (7%) fallopian tubes with definite proximal blockage on the hysterosalpingogram showed hydrosalpinx on the same side on endovaginal sonography, indicat

Fallopian tube recanalization in an unrestricted patient population.

PURPOSE: To review the effectiveness of fallopian tube recanalization (FTR) when performed without restriction based on history or tubal condition. MATERIALS AND METHODS: From October 1989 to July 1992 in 37 consecutive women, aged 22-44 years (mean, 35 years), 42 FTRs were performed (five patients each underwent two FTRs). Water-soluble contrast material and selective ostial salpingography and/or microcatheter technique were used exclusively. Eighty tubes were evaluated, since four patients had previously undergone unilateral salpingectomy. Sixty-three tubes (79%) were occluded at the outset

Transvaginal fallopian tube catheterization--diagnostic and therapeutic usefulness.

Fifty-two patients with 84 proximal Fallopian tube obstructions underwent Fallopian tube recanalization (FTR). The successful catheterization rate for Fallopian tubal ostia was 100%, and the successful recanalization rate was 64%. After FTR, hysterosalpingography or selective tubography showed no evidence of adhesion (without adhesion) in 31 tubes, perifimbrial adhesion in 17 tubes, hydrosalpinx in six tubes, and intratubal adhesion in 30 tubes (unsuccessful recanalization). Six patients (11%) achieved intrauterine pregnancies and two had extrauterine pregnancies. Follow-up studies in seven pa

Sterilization failures and their causes.

To say that a sterilization failure rate is expected does not answer why. Forty-seven cases of repeat sterilization have undergone such surgical and pathologic scrutiny. Resection methods failed most frequently because of spontaneous reanastomosis or fistula formation. Fimbriectomy was particularly vulnerable to reanastomosis because the fimbria ovarica was not removed. Mechanical devices failed when the device was defective, placed improperly, or placed in an improper location. Tissue damage was evident but incomplete when the bipolar electrocoagulation method failures were reviewed, and the

A novel concept for female infertility exploration: the Fertiliscan©, a dedicated all-in-one 3D ultrasound exploration.

The investigation of the probable cause of infertility is mandatory to propose an accurate therapeutic option to the infertile couple i.e. good chance of pregnancy. Usually, this investigation in woman includes at least hormonal dosages to estimate the ovarian function and reserve, a pelvic ultrasound scan and a hystero-salpingography to determine tubal patency. We introduce a unique investigation based on the realization of a high quality 3D ultrasound scan that involves the assessment of tubal patency. It is called Fertiliscan as opposed to the standard pelvic scan. The Fertiliscan assesses

Transvaginal hydrolaparoscopy for diagnosis of tubal infertility.

OBJECTIVE: Infertility problem affects more than 70 million couples worldwide, 5-15% of which are couples in their reproductive age. Less and less invasive endoscopic methods like transvaginal hydrolaparoscopy have been developed by technological progress. This method enables not only precise identification, but is now increasingly used for treatment of tubal and peritoneal factor pathology, which cause approximately 35 per cent of female infertility. AIM: Evaluation of transvaginal hydrolaparoscopy (HLTV) usefulness for diagnosis of tubal infertility comparing to standard laparoscopy and hys

Tubal disease and fertility outcome.

A consequence of assisted reproduction technology in infertility management has been a decline in tubal surgery. Microsurgery to correct localized damage has the advantage of long-standing restoration of fertility. A simple prognostic classification is lacking. The severity of the tubal damage and the health of the mucosa is key in determining outcome. Visualization of the tube by hysterosalpingography (HSG) or by hysterocontrastsonography (HyCoSy) has limitations. Laparoscopy has the advantage of inspecting the tube and its relation to other pelvic organs. Differentiating between anatomical o

Sonographic hydrotubation using agitated saline: a new technique for improving fallopian tube visualization.

Vaginal sonographic hydrotubation (SHT) was assessed in the evaluation of uterine configuration and tubal patency in 71 women undergoing investigation for infertility. In addition, a new technique using "agitated" saline during transvaginal sonography was evaluated in 50 of the patients. Ultrasound was more sensitive in detecting uterine abnormalities (89% compared with 6% for hysterosalpingography (HSG). Of 142 individual fallopian tubes studied, the diagnosis on SHT and HSG were the same in 120 (85%). Laparoscopic results were available on 15 patients. The findings on all 19 tubes in which t

Is color Doppler necessary in the evaluation of tubal patency by hysterosalpingo-contrast sonography.

BACKGROUND: To evaluate the diagnostic efficacy of hysterosalpingo-contrast sonography (HyCoSy) and to establish whether, in doubtful cases, the additional use of transvaginal Color Doppler can improve the tubal diagnosis reached with gray scale imaging alone. STUDY DESIGN: Ninety-six cases of unknown tubal function with infertility complaints were included. Within four weeks after sonographic hydrotubation, hysterosalpingography was performed. The diagnostic efficacy of gray scale and Color Doppler were compared with each other and against HSG. RESULTS: We calculated a sensitivity value of

[Evaluation of the value of clinical doppler sonography and sonographic contrast media for assessment of tubal patency].

The tubal factor still remains one the most often reason of female infertility. The applying of colour Doppler sonography and up-to-date constant medium (Echovist-Schering) make possible the comparison of clinical value between colour Doppler imaging and others presently used diagnostic methods. The aim of this study was the comparison of results obtained by means of hysterosalpingography, laparoscopy and hysterosalpingosonography (HSSG) directly observing tubal flow as an estimation of tubal patency.

[Tuboscopy--new developments for accurate tubal diagnosis].

A specially developed linear everting catheter (LEC) in combination with a microendoscope, enables for the first time the visualisation of the complete tubal mucosa from a vaginal approach. Preliminary results in using this technique are described. Physiological and pathological results of tubal mucosa can be presented, which will influence the diagnostic and therapeutic part of subsequent sterility treatment.

The intussusception salpingostomy technique for the therapy of distal oviductal occlusion at laparoscopy.

A new technique of salpingostomy by laparoscopy for treatment of distal oviductal occlusion is described. This technique uses sharp dissection and can be performed rapidly. The intussusception salpingostomy is undertaken by making a relatively short incision in the hydrosalpinx and then prolapsing the tubal mucosa through this neostium. The borders of the incision thus act as a restrictive collar to maintain the mucosa in this newly everted configuration. In a series of 40 women subjected to this procedure, 22 were followed for more than 12 months postoperatively; the term pregnancy rate was 2

Transcervical tubal cannulation and salpingoscopy in the treatment of tubal infertility.

Transcervical tubal cannulation and salpingoscopy are two recent techniques used for the evaluation of tubal patency and pathology. Selective tubal cannulation cannot determine the nature of the obstructive process, although it does allow for the demonstration of potential tubal patency. This may be important in deciding whether therapies requiring tubal patency may be used in specific clinical situations. Because currently there are no controlled studies in the literature evaluating this technique, the therapeutic value of the procedure must be considered unproven at this time. Salpingoscopy
