Hepatic venous malformation after modified Fontan procedure with partial hepatic vein exclusion.

Partial hepatic vein exclusion, as an alternative to baffle fenestration, has been performed as a modification of the Fontan procedure. We report a case of severe cyanosis resulting from massive right-to-left shunting via a hepatic vascular malformation that developed in a patient after a modified Fontan procedure with partial hepatic vein exclusion.

Early and intermediate results of the Fontan procedure at moderately high altitude.

At higher elevations, alveolar hypoxia increases pulmonary vascular resistance and may limit the cardiac output of individuals without a subpulmonary ventricle. Thus, we reviewed the outcome of definitive palliation for tricuspid atresia and other forms of single ventricle in 60 consecutive Fontan patients living at a mean elevation of 1,370 m (range, 910 to 2,130 m). There were four early deaths (6.7%; 70% confidence limits, 4.1% to 10.7%) and six late deaths. Kaplan-Meier actuarial survival (+/- standard error) is 79.6% +/- 8.2% at 5 years. Survival was significantly decreased in patients wi

Factors influencing survival of patients with heterotaxy syndrome undergoing the Fontan procedure.

OBJECTIVES: This study was undertaken to determine those factors that may influence survival in patients with heterotaxy syndrome undergoing the Fontan procedure. BACKGROUND: The Fontan procedure remains the preferred palliative procedure for patients with heterotaxy syndrome. Although the mortality rate has improved for patients without this syndrome undergoing the Fontan procedure, it remains high for patients with heterotaxy syndrome. METHODS: The medical records of 20 consecutive pediatric patients with asplenia (n = 12) and polysplenia (n = 8) who underwent the Fontan procedure between

Persistence and effects of sinus rhythm after Fontan procedure for tricuspid atresia.

Four patients who had had a Fontan type of procedure for tricuspid atresia 23, 6, 6, and 11 months previously were investigated by ambulatory electrocardiographic recording and simultaneous recording of the jugular venous pressure and echocardiogram of the conduit or pulmonary valve. All had been considerably improved by the operation. In 1 patient episodes of supraventricular tachycardia were recorded but no rhythm disturbance was detected in the other 3. Pulmonary blood flow was shown to be pulsatile and atrial systole is an important factor in this. The conduit valve showed delayed opening

Mechanical Thrombectomy for Large-Vessel Occlusion after Fontan Procedure in an Adult.

OBJECTIVE: Patients with adult congenital heart disease (ACHD) are at increased risk of thromboembolic events. However, case reports describing the use of mechanical thrombectomy (MT) for acute large-vessel occlusion (LVO) in this population remain limited. This case report describes an LVO caused by cardiogenic embolism in an adult patient who underwent the Fontan procedure for tricuspid atresia. CASE PRESENTATION: A 31-year-old woman with a history of an extracardiac Fontan procedure for tricuspid atresia presented with right-sided paralysis and leftward conjugate deviation. CTA revealed oc

Successful Resection of Mediastinal Paraganglioma in an Adult After a Fontan Procedure.

Paraganglioma is a rare neuroendocrine tumor commonly located in the abdomen, head, and neck but rarely in the mediastinum. An association between the Fontan procedure and paragangliomas has been suggested. We report the case of a 28-year-old female patient incidentally diagnosed with a mediastinal paraganglioma at hypertension evaluation during cardiac catheterization post-Fontan procedure. We performed a preoperative right pulmonary artery occlusion test to ensure safe 1-lung ventilation. The tumor's feeding vessels were embolized to minimize intraoperative blood loss. This case demonstrates

Quality Report: Postoperative Guideline Implementation Reduces Length of Stay after Fontan Procedure.

INTRODUCTION: Patients following the Fontan procedure have a physiology that results in prolonged pleural effusion, often delaying hospital discharge. The hospital length of stay (LOS) of patients following the Fontan procedure at our institution was significantly longer than the Society of Thoracic Surgery benchmark. This quality improvement project aimed to decrease hospital LOS in patients following the Fontan procedure from a baseline of 23 days to 7 days by January 1, 2021, and sustain indefinitely. METHODS: We implemented standardized postoperative clinical practice guidelines in April

Early outcomes of fenestrated intra-extracardiac Fontan procedure: Insights, experiences, and expectations.

BACKGROUND: Intraextracardiac Fontan procedure (FP) aimed to combine the advantages of lateral tunnel and extracardiac conduit modifications of the original technique. Herein, we present our early outcomes in patients with intraextracardiac fenestrated FP. METHODS: A retrospective analysis was performed to evaluate intraextracardiac fenestrated Fontan patients between 2014 and 2021. Seventeen patients were operated on with a mean age and body weight of 9.1 ± 5.5 years and 28.6 ± 14.6 kg. RESULTS: Sixteen patients (94%) were palliated as univentricular physiology with hypoplasia of one of the

Plastic Bronchitis-A Serious Rare Complication Affecting Children Only after Fontan Procedure?

BACKGROUND: Plastic bronchitis (PB) may occur not only in children following palliative Fontan procedure but also in those without underlying heart disease. We aim to assess the clinical course, therapeutic measures, outcome, and follow-up of PB in children with congenital heart disease (CHD) and children without cardiac problems. METHODS: This retrospective case series assessed children with PB admitted to hospital between 2015 and 2019. Parents or guardians of patients were contacted by e-mail or telephone between September 2017 and June 2019 to enquiry about recurrence of PB and strategy o

Chronotropic incompetence causes multiple organ complications in adults after the Fontan procedure.

BACKGROUND: Although patients undergoing the Fontan procedure (FP) present a normal or close‑to‑‑normal function of the systemic ventricle, they cannot generate cardiac output or exhibit similar exercise capacity as their healthy peers. This can be attributed to chronotropic incompetence and multiple organ complications. AIMS: We evaluated the prevalence of chronotropic incompetence in adults after FP and assessed the relationship between heart rate reserve (HRR) and multiple organ complications. METHODS: Data were obtained from 50 post‑FP patients (mean [SD] age, 27 [6.6] years) and 30 heal

Y-graft modification to the Fontan procedure: Increasingly balanced flow over time.

OBJECTIVE: The use of Y-grafts for Fontan completion is hypothesized to offer more balanced hepatic flow distribution (HFD) and decreased energy losses. The purpose of this study was to evaluate the hemodynamic performance of Y-grafts over time using serial cardiac magnetic resonance data and to compare their performance with extracardiac Fontan connections. METHODS: Ten Fontan patients with commercially available Y-graft connections and serial postoperative cardiac magnetic resonance data were included in this study. Patient-specific computational fluid dynamics simulations were used to esti

Impact of bilateral superior venae cavae on outcome of staged Fontan procedure.

BACKGROUND: The presence of bilateral superior venae cavae may add complexity to the performance of a bidirectional Glenn procedure (BDG). Stagnation of blood flow between the two cavopulmonary anastomoses may increase the risk of thrombosis and impair central pulmonary artery growth. METHODS: Forty patients underwent BDG from January 2004 to April 2011. The cohort was divided into two groups: those receiving bilateral BDG (b-BDG, n = 13) and those receiving unilateral BDG (u-BDG, n = 27). Operative, angiographic, and follow-up data were analyzed retrospectively. RESULTS: None of the patient

Oxygen utilization and hemodynamic response during exercise in children after Fontan procedure.

Eight patients, 9.1 to 16.5 years of age, were studied 2.8 to 8.5 years after Fontan operation. Oxygen utilization was determined during upright bicycle exercise. The cardiac index and stroke index were measured by echocardiography and the anaerobic threshold was determined. The results were compared with 10 patients after surgical closure of the atrial septal defect. Anaerobic threshold (AT) in Fontan patients was lower than in the control subjects. Oxygen consumption at each stage of exercise was significantly lower in the Fontan group compared with the control subjects. From the beginning o

Modified Fontan procedure in patients with previous ascending aorta-pulmonary artery anastomosis.

The Fontan procedure has proved useful in the surgical treatment of patients with tricuspid atresia, univentricular heart, and other complicated cardiac anomalies. Previous palliation of these patients with ascending aorta-right pulmonary artery anastomosis (Waterston shunt) may impose severe restrictions in the operability of these patients because of severe distortion, narrowing, and scarring of the proximal right pulmonary artery. A modification of the Fontan operation is described in which repair is facilitated by transecting the right pulmonary artery at the site of anastomosis, trimming

Transcatheter closure of ventriculopulmonary artery communications in staged Fontan procedures.

BACKGROUND: Ventricle-pulmonary artery connections in patients after the Fontan procedure lead to ineffective volume loading and can cause long term problems. In patients with a cavopulmonary shunt anterograde pulmonary blood flow is frequently maintained, but can cause significant volume loading of the heart or complicate the subsequent Fontan procedure. OBJECTIVE: To evaluate the use of transcatheter closure of a ventricle-pulmonary artery communication in the setting of a cavopulmonary shunt or after the Fontan procedure. PATIENTS AND METHODS: Retrospective study at a tertiary referral ce
