[Patients at the memory clinic with choreatic movements].

In this case-report we present two patients who were referred to the memory clinic. In both cases there is doubt about the earlier diagnosis. During the physical examination choreatic movements were observed. After repeating diagnostic tests, both patients were diagnosed with Huntington's disease. The dementia with this disease is often preceded by psychiatric symptoms, which makes it quite difficult to diagnose, especially in an early stage of the disease. That is why it is essential that, in case of even the slightest doubt or confronted with new insights, geriatricians should see patients a

Comprehensive geriatric assessment for older people living with chronic kidney disease and frailty: time to set a new goal?

Comprehensive geriatric assessment is an established intervention for older people living with frailty, involving multidimensional assessment and development of a personalized care plan to meet identified needs. The GOAL trial demonstrated that comprehensive geriatric assessment delivered through a single outpatient geriatrician appointment did not improve goal attainment for older people living with chronic kidney disease and frailty. Alternative models of comprehensive geriatric assessment and person-centered care require evaluation. Regardless, embracing person-centered care should be a goa

[Decision making in frail patients with a hip fracture].

BACKGROUND: When an older patient presents herself at the emergency department with a femur fracture different doctors are involved: the geriatrician, the orthopaedic or trauma- surgeon and the anaesthesiologist. Together they form an ad hoc team and are in charge of organising the best medical care for the patient. CASE DESCRIPTION: In this case description we present a patient with cardiovascular comorbidity and dementia. Mis-interpretation or missing information can lead to different treatment decisions and outcomes. CONCLUSION: Effective cooperation and transfer of information in physica

Multidisciplinary development of the Geriatric Core Dataset for clinical research in older patients with cancer: A French initiative with international survey.

BACKGROUND: To define a core set of geriatric data to be methodically collected in clinical cancer trials of older adults, enabling comparison across trials. PATIENTS AND METHODS: Following a consensus approach, a panel of 14 geriatricians from oncology clinics identified seven domains of importance in geriatric assessment. Based on the international recommendations, geriatricians selected the mostly commonly used tools/items for geriatric assessment by domain (January-October 2015). The Geriatric Core Dataset (G-CODE) was progressively developed according to RAND appropriateness ratings and

Psychotic behavior in normal pressure hydrocephalus.

Normal pressure hydrocephalus (NPH) usually presents with progressive gait impairment, urinary incontinence, and cognitive deficits. Many studies have shown that NPH can mimic other disorders like Parkinson disease with tremors and rigidity. In this report, we present our experience with three patients who had psychotic behavior and were treated with psychiatric medications but were subsequently investigated and found to have NPH. After lumboperitoneal shunting, all the three became normal and were even able to stop the psychiatric medications completely. The message is that NPH can mimic a ps

Urinary retention in elderly women: diagnosis & management.

The management of urinary retention in the elderly female can present a challenging conundrum for primary care physicians, geriatricians, and urologists. It is often difficult to diagnose due to concomitant comorbidities and potential cognitive impairments. Evaluation should include a comprehensive history and physical examination, post-void residual, and urinalysis with consideration given to urodynamic testing in select patients. Management varies from conservative to invasive and should be tailored to the individual patient. Primary goals of care include symptom reduction, prevention of uri

Accuracy in Hair Repair: Assessing Sapphire FUE, Forceps Methods, and Treatment Times in Contemporary Hair Transplantation.

BACKGROUND AND OBJECTIVES: There is frequently a degree of variability among different types of dementia specialists in clinical practice in both the clinical diagnosis and the management of patients with Alzheimer's disease and cerebrovascular disease (CVD). This variability may have an adverse effect on the use of medical resources as well as on patients' well-being. The main objective of this study was to describe the current diagnosis and management of patients with Alzheimer's disease and CVD in Spain. Other objectives were to determine whether there were significant differences in the di

Results of transurethral resection of prostate in patients with cerebrovascular disease.

Twenty-two patients with bladder outflow obstruction and a past history of cerebrovascular accident are described. Sixteen patients regained normal micturition control. Poor results occurred in the more handicapped patients, especially in those with additional neurological problems. Patients who present with bladder outflow obstruction should be treated surgically. To determine those patients who will have a poor result full clinical and neurological assessment must be made, together with urodynamic studies. In order to improve these results a team approach is required between the urologist, g

A new model for general practice-led, regional, community-based, memory clinics.

BACKGROUND: Dementia is a major international health issue with high impact on the patient, relatives, and broader society. Routine screening for dementia is limited, despite known benefit of early detection and intervention on quality of care and patient outcomes. Screening is particularly limited in rural and regional areas, despite high burden and projected growth of dementia in these populations. The current study aimed to implement a new general practitioner (GP) led, multidisciplinary, model of care providing dementia detection and referral pathway to a community-based specialist clinic

[Bipolar affective disorders in senescence].

Diagnostic and therapeutic measures in patients with bipolar disorders are significantly different depending on the age of those affected. Given the demographic changes and the fact that approximately one quarter of patients with bipolar disease are in old age, it is important for geriatricians to be aware of the specific aspects of bipolar disease. This review article presents the diagnostics of bipolar disorders in old people. Interactions with somatic comorbidities, which may lead to the occurrence of secondary mania, just to mention one of the characteristics of old age, are elaborated. Fu

Acute geriatrics at the front door.

Older people with frailty and urgent care needs are major uses of health and social care services. Comprehensive geriatric assessment (CGA) is an evidence-based approach to improving their outcomes, as well as improving service outcomes. Geriatricians form a small proportion of the overall workforce and cannot address the population need alone, so all clinicians (doctors, nurses, therapists and so on) need to engage in delivering CGA as a process of care, underpinned by specific competencies - which can be developed. Delivery of this care pathway needs to be measured and improved as rigorously

A comparison of clinical characteristics between old and oldest-old patients hospitalised for SARS-COV2.

OBJECTIVES: Oldest-old patients may have an unusual SARS-COV2 presentation that can jeopardise diagnosis and management. The aim of this study was to compare the clinical characteristics and outcomes of oldest-old (≥85 years) and old patients (75-85 years) admitted with COVID-19 to Belgian hospitals during the first wave of the pandemic. METHODS: We conducted a multicentre, retrospective, observational study in ten Belgian hospitals. We reviewed the electronic clinical records of patients ≥75 years hospitalised with COVID-19 from March to June 2020. RESULTS: A total of 986 patients were plac

Drug Allergy in the Aged: Barriers and Call for Multidisciplinary Collaboration.

There are growing challenges when evaluating drug allergy in the aging global population. Older adults are frequently affected by polypharmacy, multimorbidity, and limited recall of the index reaction. Cultural beliefs, preference for alternative medical systems, and decreased mobility may lead to delays before even scheduling an appointment. Cognitive impairment and inability to provide informed consent can lead to reliance on proxies or family members. Skin aging and physical frailty may present logistical barriers in office evaluation, requiring a personalized or cautious approach. We advoc

[Multimorbidity in Urology].

As people age, they increasingly experience multimorbidity, especially regarding urological conditions. Multimorbidity in elderly patients frequently includes urinary incontinence and subvesical obstruction. While urinary incontinence is often directly associated with other existing diseases or polymedication, subvesical obstruction may be unrelated to other morbidities. Subvesical obstruction often requires polymedication and is therefore a noteworthy condition in urological diagnostics and treatment. First and foremost, a focused diagnostic investigation must be performed, which then enables

[Multimorbidity in elderly rheumatic patients part 1].

An appropriate treatment of elderly rheumatic patients implements comprehensive diagnostics and exclusion diagnostics of e.g. coronary heart disease, osteoporosis, renal failure, diabetes mellitus type 2 and thyroid gland dysfunction. Furthermore, the complex disease situation might require the integration of other faculties or might be a reason for inpatient treatment. The complexity in the treatment of multimorbid elderly patients suffering from rheumatism not only rises with increasing age but also constitutes a considerable challenge due to existing incapacities and preceding as well as cu
