Recovery cycles of posterior root-muscle reflexes evoked by transcutaneous spinal cord stimulation and of the H reflex in individuals with intact and injured spinal cord.

Posterior root-muscle (PRM) reflexes are short-latency spinal reflexes evoked by epidural or transcutaneous spinal cord stimulation (SCS) in clinical and physiological studies. PRM reflexes share key physiological characteristics with the H reflex elicited by electrical stimulation of large-diameter muscle spindle afferents in the tibial nerve. Here, we compared the H reflex and the PRM reflex of soleus in response to transcutaneous stimulation by studying their recovery cycles in ten neurologically intact volunteers and ten individuals with traumatic, chronic spinal cord injury (SCI). The Gray Zone of H-Reflex in Runners: When Should We Suspect Pathology? A Pilot Study.

Background/Objectives: Spinal excitability may undergo adaptive modulation in response to training load, sport-specific demands, and fatigue. While high-impact sports are known to influence reflex responsiveness, the extent to which these changes differ from athletes in lower-impact disciplines remains unclear. This study aimed to investigate post-exercise changes in Hmax/Mmax ratio among trained runners with varied sport backgrounds, and to identify emergent physiological profiles that may reflect differential spinal adaptation to fatigue. Methods: Twenty-two trained athletes underwent unilat

Synaptic dynamics linked to widespread elevation of H-reflex before peripheral denervation in amyotrophic lateral sclerosis.

Changes in Hoffmann reflex (H-reflex) exhibit heterogeneity among patients with amyotrophic lateral sclerosis (ALS), likely due to phenotype diversity. Current knowledge primarily focuses on soleus H-reflex, which may demonstrate an initial increase before subsequent decline throughout the disease course. The main objective was to investigate other muscles, to determine whether H-reflex changes could be associated with patient phenotype (onset site, functional disabilities). Additional experiments were performed to elucidate the neurophysiological mechanisms underlying H-reflex modifications.

Soleus Muscle H-reflex: Reference Data of Adult Population From a Tertiary Care Center in India.

Introduction The Hoffmann reflex (H reflex) is one of the most studied reflexes in human neurophysiological literature. Detection of the H reflex is useful in the diagnosis of early polyneuropathy, S1 radiculopathy, early GBS, tibial neuropathy and sciatica, and sacral plexopathy. The H reflex is also used as a tool to measure the excitability of the nervous components of the arc, regardless of the sensitivity of the sensory organs. The monosynaptic nature of reflex circuits makes H-reflex an attractive tool for clinical neurophysiology and research. Objective The objective is to create refere

Diagnostic advantage of S1 foramen-evoked H-reflex for S1 radiculopathy in patients with diabetes mellitus.

Hoffmann reflex to tibial nerve stimulation at the popliteal fossa (tibial H-reflex) is routinely used to evaluate S1 radiculopathy. However, it lacks sensitivity because other lesions along this reflex circuit affect the H-reflex bilaterally. This study was undertaken to determine whether the H-reflex evoked by stimulating proximally at the S1 foramen (S1 foramen H-reflex) could improve S1 root lesion evaluation sensitivity in patients with diabetes mellitus. A randomized paired study was designed to evaluate tibial and S1 foramen H-reflexes; bilateral H-M interval (HMI) and H-reflex latency

Inhibitory effects of circumferential pressure on flexor carpi radialis H-reflex in adults with neurological deficits.

The effects of circumferential air-splint pressure on the flexor carpi radialis H-reflex in 22 participants with cerebrovascular accident and 5 with spinal cord injury were examined. H-reflexes were assessed by measuring the peak-to-peak amplitude change before, during, and after circumferential pressure was applied to the forearm. 12 H-reflexes (H/M ratio: M = 25%; SD = 14) were recorded before pressure application to obtain a baseline value for comparison of data. A pneumatic 34-cm air splint inflated to 51-60 mmHg provided pressure around the forearm. H-reflex recordings were taken at 1, 3,

Hip angle induced modulation of H reflex amplitude, latency and duration in spinal cord injured humans.

OBJECTIVES: To investigate the modulation of the soleus H reflex in spinal cord injured (SCI) subjects resulting from imposed changes in hip angle and to establish whether changes in H reflex amplitude co-vary with changes in reflex latency and duration. METHODS: H reflexes were recorded using conventional methods in 7 SCI subjects in the supine position. The right leg was secured by a leg brace and positioned at various angles of hip flexion (30 degrees, 40 degrees ) and at 10 degrees of hip extension. RESULTS: We found that imposing 10 degrees of hip extension resulted in a significant fac

Motoneuron pool excitability of hemiplegic patients: assessing recovery stages by using H-reflex and M response.

OBJECTIVES: To compare the excitability of the motoneuron pools of both the spastic and the unimpaired sides of patients with hemiplegia with a new method by using H-reflexes and M responses. The method determines the ratio of the developmental slope of the H-reflex (Hslp) to the slope of the M response (Mslp). We also examined the relation between the Brunnstrom stages and the Hslp/Mslp. DESIGN: Experiment. SETTING: Electrophysiologic experimental laboratory in a Japanese medical school. PATIENTS: Fifteen hemiplegic patients (9 men, 6 women; age range, 48-71yr; mean, 60yr) with spasticity

Independence of soleus H-reflex tests in control and spastic subjects shown by principal components analysis.

Different soleus H-reflex tests are used in the study of neurophysiological mechanisms of motor control. We studied the interdependence pattern of a number of soleus H-reflex tests, i.e., vibratory inhibition, the ratio of the reflex response to direct muscle potential (H/M ratio) and the homonymous recovery curve with a principal components analysis in 48 healthy controls and 38 patients with signs of the upper motoneuron syndrome. In controls, the analysis showed 3 independent principal components (PCs). Vibratory inhibition and H/M ratio loaded on separate components. Late facilitation and

Normal values for the soleus H-reflex in newborn infants 31-45 weeks post conceptional age.

Normal values for the soleus H-reflex were established in 83 preterm and term infants 31 to 45 weeks postconceptional age. Infants at conceptional ages 31 to 34 weeks (n = 30) had a mean H-latency (msec) of 19.2 +/- 2.16; infants 35 to 39 weeks (n = 26), 16.67 +/- 1.48; and infants 40 to 45 weeks (n = 27), 15.94 +/- 1.45. Simple linear regression shows a significant relationship between conceptional age and H-latency (Pearson correlation = .68, R2 = 0.47, f = 71.31, p = .0001). Adjusting H-latency values for leg length, the regression of H-soleus latency/leg length (msec/cm) against conception

Adaptive gait responses to varying weight-bearing conditions: Inferences from gait dynamics and H-reflex magnitude.

This study investigates the effects of varying loading conditions on excitability in neural pathways and gait dynamics. We focussed on evaluating the magnitude of the Hoffman reflex (H-reflex), a neurophysiological measure representing the capability to activate motor neurons and the timing and placement of the foot during walking. We hypothesized that weight manipulation would alter H-reflex magnitude, footfall and lower body kinematics. Twenty healthy participants were recruited and subjected to various weight-loading conditions. The H-reflex, evoked by stimulating the tibial nerve, was asse

Effects of age and leg length upon central loop of the gastrocnemius-soleus H-reflex latency.

BACKGROUND: central loop of the gastrocnemius-soleus H-reflex latency (Tc) that looks promising in the diagnosis of S1 radiculopathy; has been investigated in a few studies and only two of them have focused on the constitutional factors affecting it. Although leg length has been shown to contribute to the Tc, the role of age is controversial. More confusing, none of the previously performed studies have used strict criteria to rule out subclinical neuropathy, so the results could be misleading. This study has been performed to determine the influence of leg length and age on Tc among a careful

The central loop of H-reflex in the S1 spinal nerve: normal values and constitutional influencing factors.

OBJECTIVE: To investigate the central loop of H reflex in the S1 nerve and the constitutional influencing factors in normal healthy individuals. MATERIALS AND METHODS: The study was performed on 39 apparently healthy volunteers. To obtain central H-reflex, the cathode electrode was inserted at a point 1 cm medial to the posterior superior iliac spine, perpendicular to the frontal plane. After the needle touched the sacrum, it was slightly retracted to avoid direct contract to the sacrum. The anode electrode was placed over the anterior iliac spine. The active pick-up electrode was placed at t

Effects of postural and voluntary muscle contraction on modulation of the soleus H reflex by transcranial magnetic stimulation.

Modulation of spinal reflexes depends largely on the integrity of the corticospinal tract. A useful method to document the influence of descending tracts on reflexes is to examine the effects of transcranial magnetic stimulation (TMS) on the soleus H reflex elicited by posterior tibial nerve electrical stimuli (PTS). In 12 healthy volunteers, we investigated how postural or voluntary muscle contraction modified such descending modulation. We first characterized the effects of TMS at 95 % of motor threshold for leg responses on the H reflex elicited by a preceding PTS at inter-stimuli intervals

Abnormal flexor carpi radialis H-reflex as a specific indicator of C7 as compared with C6 radiculopathy.

OBJECTIVE: The H-reflex of the flexor carpi radialis (FCR H-reflex) has not been commonly used for the diagnosis of cervical radiculopathy when compared with the routinely tested soleus H-reflex. Although both S1 and S2 roots innervate the soleus, the H-reflex is selectively related to S1 nerve root function clinically. Flexor carpi radialis is also innervated by two nerve roots which are C6 and C7. Although they are among the most common roots involved in cervical radiculopathy, few studies reported if the attenuation of the FCR H-reflex is caused by lesions affecting C7 or C6 nerve roots, or

The effect of operant-conditioning balance training on the down-regulation of spinal H-reflexes in a spastic patient.

Spasticity in chronic hemiparetic stroke patients has primarily been treated pharmacologically. However, there is increasing evidence that physical rehabilitation can help manage hyper-excitability of reflexes (hyperreflexia), which is a primary contributor to spasticity. In the present study, one chronic hemiparetic stroke patient operantly conditioned the soleus H-reflex while training on a balance board for two weeks. The results showed a minimal decrease in the Hmax-Mmax ratio for both the affected and unaffected limb, indicating that the H-reflex was not significantly altered with trainin
