Between trust and traceability: How immigrant-serving organizations become sites of both care and surveillance.

This article examines how immigrant-serving community-based organizations become implicated in state monitoring systems as they partner with public health programs. Drawing on ethnographic research and interviews with medical professionals, frontline workers, and administrators in a city in Upstate New York, I analyze how data requirements tied to state health programs incorporate immigrant-serving organizations into systems of oversight. Using the concept of system embeddedness, I show that (1) the financial precarity of immigrant-serving organizations, combined with their trusted relationshi

Large Community Outbreak of Legionnaires Disease Potentially Associated with a Cooling Tower - Napa County, California, 2022.

Legionnaires disease is a serious infection acquired by inhalation of water droplets from human-made building water systems that contain Legionella bacteria. On July 11 and 12, 2022, Napa County Public Health (NCPH) in California received reports of three positive urinary antigen tests for Legionella pneumophila serogroup 1 in the town of Napa. By July 21, six Legionnaires disease cases had been confirmed among Napa County residents, compared with a baseline of one or two cases per year. NCPH requested assistance from the California Department of Public Health (CDPH) and CDC to aid in the inve

Pediatric Developmental Safety Assessment and New Approach Methodologies: US FDA/Triangle CERSI Workshop Report.

Pediatric developmental safety assessment presents distinct challenges in drug development. Children undergo dynamic changes in organ systems and drug disposition pathways during development, and drug-related effects may be delayed or manifested differently than in adults. Pharmacovigilance data, primarily from adults, is not always predictive of the response in children. Historically, juvenile animal studies have been used for nonclinical pediatric drug safety assessments exclusively, but the reduction in animal testing is a goal of public health agencies. To address this concern, the Office

Cancer Screening and Follow Up Among Individuals With Serious Mental Illness.

PURPOSE: Cancer is the second leading cause of death in the United States. Individuals with serious mental illness (SMI) have higher cancer mortality, which may be due to lower screening rates and barriers to timely, effective care. The current study explored how individuals with SMI perceive cancer screening and follow up to identify barriers and facilitators to care. METHOD: Using a qualitative interpretive description approach, we conducted individual interviews with 10 adults with SMI about their cancer screening experiences. RESULTS: The most frequently reported barriers were distrust o

Pandemic (H1N1) 2009: setting up a multi-agency regional response centre--a toolkit for other public health emergencies.

The H1N1 pandemic emerged from Mexico in April 2009. In the UK, local Health Protection Units were quickly overwhelmed with calls from health professionals seeking public health advice on this novel virus. The Health Protection Agency (HPA) led the initial response and established regional flu response centres (FRCs). In London, the HPA's flu response moved swiftly from four in-house emergency operations centres to a fully functional multiagency response centre in rented office space, working with a new database, IT and telephone systems to provide a regional response. Surge capacity was sough

A comparison of coronavirus disease 2019 and seasonal influenza surveillance in five European countries: France, Germany, Italy, Spain and the United Kingdom.

BACKGROUND: In response to the coronavirus disease (COVID-19) outbreak that unfolded across Europe in 2020, the World Health Organisation (WHO) called for repurposing existing influenza surveillance systems to monitor COVID-19. This analysis aimed to compare descriptively the extent to which influenza surveillance systems were adapted and enhanced and how COVID-19 surveillance could ultimately benefit or disrupt routine influenza surveillance. METHODS: We used a previously developed framework in France, Germany, Italy, Spain and the United Kingdom to describe COVID-19 surveillance and its imp

American Academy of Pediatrics Community Access to Child Health (CATCH) Program: a model for supporting community pediatricians.

Advocacy on behalf of children who are medically underserved and the pediatricians who care for them has been a long-standing core commitment of the Royal College of Paediatrics and Child Health and the American Academy of Pediatrics. Although different in etiology, barriers to adequate health care exist in both nations. In the United States, almost 18 million children have either no health insurance or inadequate coverage, whereas in the United Kingdom, parents can, in most cases, readily enroll their youngsters in a universal health insurance program that is not dependent on employers or emp

Policy actors' perspectives on improving federal grants to promote the implementation success of evidence-based behavioral health practices.

BACKGROUND: To promote high-quality behavioral health service delivery, federal agencies often invest in evidence-informed and evidence-based practice (EBP) implementation through discretionary (i.e., competitive) grants. However, gaps remain in understanding how federal grant mechanisms can lead to large-scale reach (i.e., the extent of EBP integration into service systems). To understand EBP reach through federal grant mechanisms and provide actionable data, we conducted a series of focus groups with relevant federal and state policy actors to gather their perspectives on how to improve fede

Leveraging public health's participation in a Health Information Exchange to improve communicable disease reporting.

BACKGROUND: Infectious diseases can appear and spread rapidly. Timely information about disease patterns and trends allows public health agencies to quickly investigate and efficiently contain those diseases. But disease case reporting to public health has traditionally been paper-based, resulting in somewhat slow, burdensome processes. Fortunately, the expanding use of electronic health records and health information exchanges has created opportunities for more rapid, complete, and easily managed case reporting and investigation. To assess how this new service might impact the efficiency and

Establishing an independent HTA agency in Ukraine: a conceptual framework for governance, operations, and long-term sustainability.

OBJECTIVES: Health technology assessment (HTA) has become an integral part of Ukraine's health system since its formal introduction into national legislation in 2017. By 2020, HTA was mandated for evaluating publicly funded medicines, laying the groundwork for more evidence-based healthcare decisions. Although the creation of an independent HTA agency was initially planned for 2022, implementation was delayed due to the COVID-19 pandemic and Russia's ongoing invasion. The relevant Cabinet Resolution calls for the establishment of an autonomous agency by January 2026. This commentary outlines a

Practical insights for regional multi-sectoral exercise planning: The Greater Toronto experience.

Exercise GTA Unified was a functional, multi-agency, cross-jurisdictional, health-sector focused mass casualty preparedness exercise conducted in the Greater Toronto Area (GTA) on 28th November, 2019. With over 1,000 unique paper-based and electronic injects and 34 participating agencies, including 22 separate hospital sites, Exercise GTA Unified is likely the largest health-sector focused mass casualty preparedness exercise ever conducted in Canada. The exercise design approach supported a successful, objective-based functional exercise, with elements of marked realism for participants. The e

Water quality laws and waterborne diseases: Cryptosporidium and other emerging pathogens.

Waterborne diseases, such as cryptosporidiosis, cause many cases of serious illness in the United States annually. Water quality is regulated by a complex system of federal and state legal provisions and agencies, which has been poorly studied. The authors surveyed state and territorial agencies responsible for water quality about their laws, regulations, policies, and practices related to water quality and surveillance of cryptosporidiosis related to drinking water. In this commentary they review the development and current status of federal drinking water regulations, identify conflicts or g

A look at state-level risk assessment in the United States: making decisions in the absence of federal risk values.

State environmental agencies in the United States are charged with making risk management decisions that protect public health and the environment while managing limited technical, financial, and human resources. Meanwhile, the federal risk assessment community that provides risk assessment guidance to state agencies is challenged by the rapid growth of the global chemical inventory. When chemical toxicity profiles are unavailable on the U.S. Environmental Protection Agency's Integrated Risk Information System or other federal resources, each state agency must act independently to identify and

Responding to the Fentanyl Overdose Crisis With a Tri-Government Incident Command Structure in Portland, Oregon, 2024.

CONTEXT: The increasing use of illicit fentanyl has contributed to a drastic increase in drug overdose fatalities throughout the United States. The rate of fatal fentanyl-related overdoses in Multnomah County, Oregon, was 1.6 times higher than the United States' average in 2022 and continued to rise in 2023. In January 2024, the Governor of Oregon, the Multnomah County Chair, and the Portland Mayor declared a 90-day tri-government emergency to augment and enhance the ongoing response to fentanyl use and overdose in Portland city center. EMERGENCY RESPONSE: Between January 31, 2024, and April

Bioterrorism preparedness for local health departments.

Bioterrorism preparedness has not traditionally been an everyday concern of local public health departments. The likely first responders to a biological bioterrorism event will be local public health personnel. The events of September 11, 2001, and the anthrax crisis that followed tested the capabilities of the public health system and demonstrated its fragility. Little federal funding has trickled down to local health departments, and they have not been included in planning or training for bioterrorism preparedness. Now local health departments must develop detailed bioterrorism response plan

Homelessness Among Acute Care Patients Within a Large Health Care System in Northern California.

The impacts of homelessness on health and health care access are detrimental. Intervention and efforts to improve outcomes and increase availability of affordable housing have mainly originated from the public health sector and government. The role that large community-based health systems may play has yet to be established. This study characterizes patients self-identified as homeless in acute care facilities in a large integrated health care system in Northern California to inform the development of collaborative interventions addressing unmet needs of this vulnerable population. Drought and armed conflict as drivers of overcrowding in Sudan's refugee camps.

Forced migration is a result of variable changes, including human-made factors, environmental factors or both. Climate change has been a significant cause of increased forced migration in Sudan. The prolonged drought attributed to climate change affects Sudanese citizens, with 80% of the population relying on agriculture and pastoralism as their main source of income. Since April 2023, the conflict has displaced over 14.5 million people. Four million have sought safety in neighbouring countries. The refugee camps lack essential resources, and the displaced people face a health crisis, with out

[Adverse drug reaction surveillance system in Korea].

Despite extensive researches and pre-market clinical trials, only limited information on the adverse drug reactions (ADRs) of a drug can be collected at the time of market approval from regulatory agency. ADRs constitute a major public health problem. Post-marketing surveillance of drugs is important to detect signals for ADR. In Korea, one of the main methods for monitoring the safety of marketed drugs is spontaneous reporting system of suspected ADRs. Re-examination and re-evaluation system are in force for monitoring safety of new market approval drugs and currently under marketing drugs, r

Practical insights at a critical time: Lessons from Illinois on implementing family planning state plan amendments.

Family planning Medicaid programs via state plan amendments can extend preventative care coverage to broader populations, generate cost savings for states, and safeguard against changes to Title X and to Medicaid and its financing structure. Contraceptive access initiatives, as subject matter experts and coalition builders, are uniquely positioned to serve as "systems orchestrators", working across "the 4 Ps"-providers, patients, payers, and policymakers-to fulfill the promise of these policies to improve access to critical sexual and reproductive health services.
