The Use of Janus Kinase Inhibitors in Alopecia Areata: A Review of the Literature.

Alopecia areata (AA) is a chronic, immune-mediated disorder that targets hair follicle epithelium, thereby restricting hair growth in localized patches. Although several therapies for AA have been tested, responses with traditional therapies have been limited. In recent years, numerous reports have been published of patients with AA responding to Janus kinase (JAK) inhibitors. This literature review aims to describe AA pathophysiology, explore how and why JAK inhibitors can be used for AA treatment, and review published case reports, case series, and open-label studies published to date. Janus kinase (JAK) inhibitors in the treatment of inflammatory and neoplastic diseases.

The Janus kinase (JAK) family of non-receptor protein-tyrosine kinases consists of JAK1, JAK2, JAK3, and TYK2 (tyrosine kinase-2). Each of these proteins contains a JAK homology pseudokinase (JH2) domain that regulates the adjacent protein kinase domain (JH1). JAK1/2 and TYK2 are ubiquitously expressed whereas JAK3 is found predominantly in hematopoietic cells. The Janus kinase family is regulated by numerous cytokines including interleukins, interferons, and hormones such as erythropoietin, thrombopoietin, and growth hormone. Ligand binding to cytokine and hormone receptors leads to the activ

Remission of food allergy by the Janus kinase inhibitor ruxolitinib in mice.

To clarify the role of Janus kinase (JAK) in and the efficacy of JAK inhibitors on food allergy, we investigated the effect of the clinically available JAK inhibitor ruxolitinib on mouse food allergy and the functions of cultured mast cells in vitro. Anaphylactic symptoms including diarrhea and decreases in body temperature pursuant to oral ovalbumin (OVA) challenges in food allergy mice were attenuated by the daily oral administration of ruxolitinib. This drug inhibited increases in mouse mast cell protease-1 concentrations in the serum and mast cell numbers in the intestines of these mice as

Janus Kinase and Tyrosine Kinase Inhibitors in Dermatology: A Review of Their Utilization, Safety Profile and Future Applications.

Janus kinase inhibitors, also commonly referred to as JAK inhibitors, are a novel drug class that target and block cytokine signaling mediated by the Janus kinase-signal transducer and activator of transcription (JAK-STAT) pathway, thereby regulating immune response and cell growth. Although JAK inhibitors are mainly used for rheumatological conditions such as rheumatoid arthritis, their application in the field of dermatology is actively being investigated. Tofacitinib is US FDA-approved for psoriatic arthritis and showing promise for treating psoriasis. Most recently, regulatory approvals fo

Janus kinases restrain chronic lymphocytic leukemia cells in patients on ibrutinib: Results of a phase II trial.

Preclinical observations that killing of chronic lymphocytic leukemia (CLL) cells was dexamethasone (DEX) were enhanced by concomitant inhibition of Bruton's tyrosine kinase and janus kinases (JAKs) motivated a phase II trial to determine if clinical responses to ibrutinib could be deepened by DEX and the JAK inhibitor ruxolitinib. Patients on ibrutinib at 420 mg daily for 2 months or with abnormal serum β2M levels after 6 months or with persistent lymphadenopathy or splenomegaly after 12 months were randomized to receive DEX 40 mg on days 1-4 of a 4-week cycle for six cycles alone (three pati

[Advances in the application of Janus kinase inhibitors for the uncommon pediatric dermatological disorders].

Janus kinase (JAK) inhibitors have emerged as a research hotspot in pediatric inflammation-mediated autoimmune skin diseases, owing to the ability to target and block the JAK-signal transducer and activator of transcription (JAK-STAT) signaling pathway. This article summarizes the mechanism of action and approved indications of JAK inhibitors, and systematically reviews the efficacy and safety data in pediatric uncommon skin diseases such as lichen planus, connective tissue diseases, hidradenitis suppurativa, haploinsufficiency of A20, and chronic atypical neutrophilic dermatosis with lipodyst

Janus kinase 2 activation loop as a regulator of catalysis and trans-activation.

Protein kinases regulate essential cellular processes such as apoptosis, proliferation, and growth. Precise regulation of kinase activity is critical for proper signal transduction and is often mediated by activation-loop phosphorylation. Janus kinases (JAKs) employ a unique regulatory mechanism: they remain autoinhibited by their pseudokinase domains until cytokine stimulation triggers dimerization and trans-phosphorylation of two activation-loop tyrosines. Despite its relevance for JAK inhibitor design, the molecular mechanism of JAK activation by activation-loop phosphorylation is not fully

Updated review of Janus kinase inhibitors for the management of inflammatory bowel disease.

Inflammatory bowel disease (IBD) management has evolved with the advent of Janus kinase inhibitors (JAKi), oral small molecules that modulate cytokine-driven inflammation via the Janus kinase-signal transducer and activator of transcription pathway. This narrative review synthesizes current evidence for JAKi in ulcerative colitis (UC) and Crohn's disease (CD), detailing their efficacy, safety, positioning, and special contexts. Tofacitinib has demonstrated robust induction and maintenance efficacy in moderate-to-severe UC, including steroid-refractory and acute severe cases, while failing to m

Janus kinase inhibitors - a role for the treatment of cutaneous T-cell lymphomas?

Despite increases in prevalence, many cutaneous T-cell lymphoma (CTCL) patients still lack effective and safe therapies for their disease. The most prevalent subtype, mycosis fungoides is usually managed with skin directed treatments in early stages, while advanced stages are often targeted with systemic medications. These treatments are all symptomatic except for allogeneic hematopoietic stem cell transplantation, which is associated with its own risks of relapse and potentially fatal complications. A novel class of drugs termed "JAK inhibitors" (JAKi) has recently been developed primarily fo

Adverse effects of Janus kinase inhibitors with relevance for daily practice in dermatology.

Used since 2011 in the USA and 2012 in the EU, Janus kinase inhibitors (JAKi) are gaining increasing acceptance as a treatment for dermatological diseases such as atopic dermatitis, psoriasis, psoriatic arthritis, alopecia areata, chronic hand eczema and vitiligo. Knowledge of their mechanism of action and potential side effects is necessary for a safe and effective use. Their short half-life requires daily administration, enables good controllability and is appreciated by many patients due to the rapid onset of action and the absence of subcutaneous or intravenous injections. Common side effe

Advances in Myelofibrosis Management: New Janus Kinase Inhibitors Beyond Ruxolitinib.

Myelofibrosis is a myeloproliferative neoplasm characterized by the buildup of fibrous scar tissue in the bone marrow occurring secondary to the secretion of inflammatory cytokines, leading to cytopenias, dysfunctional hematopoiesis, and constitutional symptoms. One of the pathologic mechanisms that underlies myelofibrosis is aberrant activation of the Janus kinase (JAK)-STAT pathway. Targeting the JAK-STAT pathway via JAK inhibition can lead to significant improvements in spleen volume reduction and symptom improvement in intermediate- and high-risk myelofibrosis. The first JAK inhibitor appr
