Effects of air pollution on adverse birth outcomes and pregnancy complications in the U.S. state of Kansas (2000-2015).

Neonatal mortality and morbidity are often caused by preterm birth and lower birth weight. Gestational diabetes mellitus (GDM) and gestational hypertension (GH) are the most prevalent maternal medical complications during pregnancy. However, evidence on effects of air pollution on adverse birth outcomes and pregnancy complications is mixed. Singleton live births conceived between January 1st, 2000, and December 31st, 2015, and reached at least 27 weeks of pregnancy in Kansas were included in the study. Trimester-specific and total pregnancy exposures to nitrogen dioxide (NO2), particulate matt

Outbreaks of multidrug-resistant Shigella sonnei gastroenteritis associated with day care centers--Kansas, Kentucky, and Missouri, 2005.

Infection with Shigella sonnei that is resistant to antibiotics commonly used in pediatric practice has become more common during the past decade. In 2005, Kansas, Kentucky, and Missouri reported increases in shigellosis cases associated with day care centers caused predominantly by multidrug-resistant (MDR) (i.e., resistant to ampicillin and trimethoprim-sulfamethoxazole [TMP/SMX]) strains of S. sonnei. Pulsed-field gel electrophoresis (PFGE) patterns for isolates from Kansas and Missouri were similar, suggesting a common outbreak in the Kansas City area, whereas isolates from Kentucky had a

Companion animal veterinary personnel have occupational risk for tularemia and One Health role for tularemia prevention in Kansas.

OBJECTIVE: To learn the knowledge, attitudes, and practices of veterinarians and technicians in Kansas regarding the occupational risk of tularemia, personal protection, patient diagnosis and management, zoonotic risk communication, and public health reporting. METHODS: Kansas veterinarians and technicians serving cats and dogs completed an online survey between June and September 2024. RESULTS: The survey was completed by 109 veterinarians and 19 technicians. Forty-three percent (47 of 109) of veterinarians reported diagnosing tularemia in cats and 13% (14 of 109) in dogs. Seven percent (8

Seroprevalence, frequency of leptospiuria, and associated risk factors in horses in Kansas, Missouri, and Nebraska from 2016-2017.

Leptospirosis is a worldwide veterinary and public health concern, and well recognized infectious disease of horses. Seroprevalence rates vary with geography, but many studies have confirmed a high exposure rate. The correlation between seropositivity and shedding status has not been made in horses, however. The aims of this study were to use semi-nested PCR on urine from apparently healthy horses to determine period prevalence of leptospiral shedding and to correlate these findings with MAT results to establish associations with client based survey data regarding horse management and environm

Use of PCR to identify Leptospira in kidneys of big brown bats (Eptesicus fuscus) in Kansas and Nebraska, USA.

Bats have been implicated as potential carriers of Leptospira as a result of surveys, mostly in Australia and South America. We measured the prevalence of pathogenic leptospires in kidneys of bats from Kansas and Nebraska. From 7 August 2012 to 21 August 2012, we extracted DNA from kidneys of 98 big brown bats (Eptesicus fuscus) submitted and found negative for rabies. The DNA was processed in a two-step, seminested PCR assay with a dual-labeled Taqman probe specific for pathogenic leptospires. As a negative control, we used a saprophytic leptospire (Leptospira biflexa Patoc) and, as a pathoge

Assessing environmental injustice in Kansas City by linking paediatric asthma to local sources of pollution: a cross-sectional study.

OBJECTIVE: A grassroots environmental-justice organisation in Kansas City has been examining the disproportionate exposure to air pollution experienced by residents living fenceline to the largest classification railyard in the USA. Prior analyses showed limited increased risk for asthma exacerbation for patients with asthma living closer to toxic release inventory (TRI) facilities and railyards. In this study, we assessed geographical asthma and environmental disparities, to further explore community-level disparities. DESIGN: This is a cross-sectional study of population-level asthma rates,

Experiences with care coordination and backup plans in home and community based services during the COVID-19 pandemic in Kansas.

BACKGROUND: The COVID-19 pandemic was an emergency event during which backup plans became widely relevant. Although backup plans are required for Medicaid-funded Home and Community Based Services (HCBS) as a key risk management strategy, we know little about their effectiveness. OBJECTIVE: The purpose of this study was to explore whether backup plans and care coordination met the needs of HCBS consumers during the COVID-19 pandemic in Kansas. METHODS: An interactive, convergent mixed-methods design within a community-based participatory research framework was used. Data came from 70 in-depth

Promoting Health Equity Through Tax-Funded Zero-Fare Public Transit: A Survey of Voters From Kansas City, MO.

Public transportation has positive impacts for both individual and community health, but funding and scalability are political challenges. Understanding civic processes, such as voting, is key to advancing health equity through large-scale policy change. The purpose of this study is to understand voter support for tax-funded public transportation. This study employed a cross-sectional survey of randomly sampled voters in Kansas City, Missouri, collected from October 2023 to January 2024. Of the 2,577 respondents, 79% participated in the election of interest. Two-sample proportion tests for pai

The Relationship Between the Kansas City Cardiomyopathy Questionnaire and Electrocardiographic Parameters in Predicting Outcomes After Cardiac Resynchronization Therapy.

BACKGROUND: Cardiac resynchronization therapy (CRT) is an essential treatment for patients with symptomatic heart failure and ventricular conduction abnormalities. Low-ejection-fraction (EF) cardiomyopathy often involves a wide QRS complex displaying a left bundle branch block (LBBB) morphology and markedly delayed activation of the LV lateral wall. Following CRT, patients with heart failure and LBBB have better outcomes and quality-of-life improvements. Various electrocardiographic and clinical parameters are thought to be able to predict this improvement. The Kansas City Cardiomyopathy Quest

Health Outcomes of Kansas City's Vulnerable Patients Following Shutdown: An Assessment of Blood Pressure Among Sojourner Health Clinic Patients.

Background In downtown Kansas City, patients who face homelessness or unstable housing situations may have been negatively affected by the shutdown of Sojourner Health Clinic (SOJO), a free student-run clinic that provides primary care predominantly to these patients. Research shows that blood pressure (BP) increases within weeks or months of interruption of antihypertensive therapy, especially in patients with advanced age and polypharmacy. Therefore, this study will examine how patients' blood pressure changed after the closure of Sojourner Health Clinic. Methods The study population consist

Finerenone and quality of life in heart failure: component-level analyses and clinical relevance of the Kansas City cardiomyopathy questionnaire.

AIMS: Finerenone was shown to improve overall health status, as measured by the aggregate 23-item Kansas City Cardiomyopathy Questionnaire (KCCQ) score, in heart failure with mildly reduced or preserved ejection fraction (HFmrEF/HFpEF). This study aimed to contextualize KCCQ changes in a manner that is relevant to patients and clinicians, thereby improving understanding of this metric in HFmrEF/HFpEF. METHODS AND RESULTS: In this prespecified analysis of FINEARTS-HF, a double-blind, randomized, placebo-controlled trial of finerenone in HFmrEF/HFpEF, we performed exploratory assessments of tre

Adapting and usability testing of the Kansas city cardiomyopathy questionnaire (KCCQ) in a heart failure clinic in Tanzania: the Swahili KCCQ.

BACKGROUND: The integration of patient-reported outcome measures (PROMS) into health care delivery systems is being increasingly recognized as an important component of quality, person-centered care, especially for chronic illnesses like congestive heart failure (CHF). However, while PROMS are increasingly being used to follow up CHF patients in high income countries, their use in sub-Saharan Africa is still limited. We adapted the Kansas City Cardiomyopathy Questionnaire (KCCQ-23), an internationally validated, CHF-specific PROM and tested its use in measuring outcomes in an outpatient CHF cl

Mapping Kansas City cardiomyopathy, Seattle Angina, and minnesota living with heart failure to the MacNew-7D in patients with heart disease.

INTRODUCTION: The Kansas City Cardiomyopathy Questionnaire (KCCQ), Seattle Angina Questionnaire (SAQ), and Minnesota Living with Heart Failure Questionnaire (MLHFQ) are widely used non-preference-based instruments that measure health-related quality of life (QOL) in people with heart disease. However, currently it is not possible to estimate quality-adjusted life-years (QALYs) for economic evaluation using these instruments as the summary scores produced are not preference-based. The MacNew-7D is a heart disease-specific preference-based instrument. This study provides different mapping algori

Bacterial abundance and antimicrobial resistance prevalence carried by adult house flies (Diptera: Muscidae) at Kansas dairy and beef cattle operations.

House flies (Musca domestica L.) are filth-breeding pests of urban and rural environments around the world. Frequenting microbe-rich substrates for nutritional and reproductive needs, house flies pose a risk to human and animal health through their carriage and transmission of pathogenic and antimicrobial resistant bacteria (AMR). Adult house flies were collected from Kansas beef and dairy cattle operations to assess factors influencing bacterial abundance and AMR incidence flies. Aerobic culturable bacteria and suspected coliforms (SC) were enumerated from fly homogenate cultured on nonselect

Association of Maternal Language Spoken at Home with Prenatal Care and Delivery Outcomes among Asian and Pacific Islander Populations in Kansas.

INTRODUCTION: In the United States, limited English proficiency (LEP) and provider-patient language discordance are associated with poorer medical outcomes compared with English-proficient, language-concordant patients. Asian American and Pacific Islander (AAPI) populations have higher-than-average rates of LEP and may be more vulnerable to adverse outcomes. The authors of this study examined differences in risk factors, medical care, and delivery outcomes between AAPI individuals who gave birth and spoke English at home versus those who spoke an AAPI language at home. METHODS: De-identified

Prevalence and Genetic Diversity of Adenovirus 40/41, Astrovirus, and Sapovirus in Children With Acute Gastroenteritis in Kansas City, 2011-2016.

BACKGROUND: Most acute gastroenteritis (AGE) episodes in children in the United States are attributed to norovirus, whereas very little information is available on adenovirus 40/41 (AdV40/41), astrovirus, or sapovirus. The New Vaccine Surveillance Network (NVSN) conducted prospective, active, population-based AGE surveillance in young children. METHODS: We tested and typed stool specimens collected between December 2011 and June 2016 from 1 NVSN site in Kansas City for the 3 viruses, and calculated hospitalization and emergency department (ED) detection rates.
