The standard intraocular lens (IOL) that is used to replace a cataractous lens cannot change focus from distant to near objects, so most patients need glasses for near vision after cataract surgery. Limited evidence suggests that accommodative IOLs provide better near vision than monofocal IOLs, but not better than multifocal IOLs. Long-term follow-up is needed to confirm patient outcomes, and determine whether the benefits justify the additional cost.

PURPOSE: To evaluate the results of a large series of secondary implantations using scleral-fixated posterior chamber intraocular lenses (IOLs).
SETTING: Bellevue Eye Hospital, Kiel, Germany.
METHODS: This retrospective review comprised 624 consecutive patients who had secondary implantation of a posterior chamber IOL with scleral fixation between 1988 and 1995. All patients had been aphakic for at least 1 year. An ab interno or ab externo suture technique through the ciliary sulcus was used. A UV-hardening lacquer material based on polyurethane, used in Philips compact disc lens systems, was tested as suitable material for low-cost intraocular lenses (IOLs). Twenty-eight eyes were followed prospectively by goniophotography after cataract surgery. Ten had no intraocular lens (IOL), ten had the "Azar 91Z" lens implanted and eight had the Cilco "Optiflex" lens inserted. In 650 consecutive Binkhorst lens implants, secondary membrane surgery was required more than eight times as frequently after an extracapsular than after an intracapsular cataract extraction.
