Patient perceived value of teleophthalmology in an urban, low income US population with diabetes.

Dilated eye exams are the standard of care to detect advancing, vision threatening, but often asymptomatic retinopathy in a timely fashion, allowing for vision preserving treatments. Annual exam rates are suboptimal, especially in underserved populations. Although teleophthalmology programs tremendously improve annual exam rates in low income/under resourced settings, widespread adoption is limited. Using a mixed methods approach, three focus groups and individual interviews were conducted for patients with type 2 diabetes (N = 23) who had a teleophthalmology exam or a dilated eye exam. A surv

An Artificial Intelligence-Based App for Self-Management of Low Back and Neck Pain in Specialist Care: Process Evaluation From a Randomized Clinical Trial.

BACKGROUND: Self-management is endorsed in clinical practice guidelines for the care of musculoskeletal pain. In a randomized clinical trial, we tested the effectiveness of an artificial intelligence-based self-management app (selfBACK) as an adjunct to usual care for patients with low back and neck pain referred to specialist care. OBJECTIVE: This study is a process evaluation aiming to explore patients' engagement and experiences with the selfBACK app and specialist health care practitioners' views on adopting digital self-management tools in their clinical practice. METHODS: App usage ana

Decision fatigue in low-value prostate cancer screening.

BACKGROUND: Low-value prostate-specific antigen (PSA) testing is common yet contributes substantial waste and downstream patient harm. Decision fatigue may represent an actionable target to reduce low-value urologic care. The objective of this study was to determine whether low-value PSA testing patterns by outpatient clinicians are consistent with decision fatigue. METHODS: Outpatient appointments for adult men without prostate cancer were identified at a large academic health system from 2011 through 2018. The authors assessed the association of appointment time with the likelihood of PSA t

Evaluation of "point of care" devices in the measurement of low blood glucose in neonatal practice.

BACKGROUND: Low blood glucose in newborns is difficult to detect clinically. Hence a reliable "point of care" device (glucometer) for early detection and treatment of low glucose is needed. OBJECTIVE: To evaluate the performance of five readily available glucometers for the detection of low blood glucose in newborn infants. METHOD: Glucostix measurements were taken for newborns with risk factors using a Reflolux S (Boehringer) glucometer. If the initial reading was low (< 2.6 mmol/l), further measurements were taken with two other glucometers (phase I, Advantage and Glucotrend (Roche); phase

A Multidisciplinary Assessment of Standard Low Dose Versus Ultra-low Dose Chest CT Scans for Pectus Excavatum Imaging.

AIM: Evaluation of ultra-low dose chest CT imaging for the assessment of pectus excavatum severity as determined by pediatric radiologists and pediatric surgeons using Haller (HI) and Correction indices (CI). METHODS: A single institution, prospective evaluation of patients being evaluated for pectus excavatum were scanned with a standard low-dose chest CT protocol (CARE) followed by a consecutive ultra-low dose CT scan (ULTRA). 3 surgeons and 4 radiologists were instructed to determine HI and CI in each series. The Intraclass Correlation Coefficient (ICC) was used to calculate the agreement

Development of the SAVE study protocol to use lived experience to assess the value of serious health-related suffering and palliative care.

BACKGROUND: Suffering generated by life-limiting or life-threatening conditions is a complex phenomenon inextricably linked with human experience. Most of this suffering can be ameliorated with adequate access to palliative care that is unavailable in much of the world, especially low-resource settings. Exploration of the definitional boundaries of and needs pertaining to suffering due to illness suggests that patients and families assign specific importance to the compassionate and supportive aspects of palliative care. However, the intrinsic and personal value of suffering alleviation has no

Diagnostic performance of a faecal immunochemical test for patients with low-risk symptoms of colorectal cancer in primary care: an evaluation in the South West of England.

BACKGROUND: The faecal immunochemical test (FIT) was introduced to triage patients with low-risk symptoms of possible colorectal cancer in English primary care in 2017, underpinned by little primary care evidence. METHODS: All healthcare providers in the South West of England (population 4 million) participated in this evaluation. 3890 patients aged ≥50 years presenting in primary care with low-risk symptoms of colorectal cancer had a FIT from 01/06/2018 to 31/12/2018. A threshold of 10 μg Hb/g faeces defined a positive test. RESULTS: Six hundred and eighteen (15.9%) patients tested positive

Ventilatory strategies in intensive care: Balancing clinical outcomes and cost-effectiveness.

Ventilatory strategies shape both outcomes and healthcare expenditures in acute respiratory failure. In resource-limited settings, choosing interventions that provide the greatest value is crucial. Evidence from India indicates that non-invasive ventilation may reduce mortality, intensive care unit stay, and treatment costs to nearly one-fifth of those associated with invasive mechanical ventilation. Comparative data from other low-income and middle-income countries reinforce the importance of prioritizing scalable, protocolized strategies with favorable cost-effectiveness profiles. This edito

Low self-esteem: a refined cognitive behavioural model.

Melanie Fennell's (1997) seminal cognitive approach to low self-esteem was published in Behavioural and Cognitive Psychotherapy. The current paper proposes a refined model, drawing on social theories, and research with people with socially devalued characteristics. This model emphasises how self-esteem relates to perceptions of one's value in the eyes of others. It is proposed that core beliefs typical of low self-esteem relate to one's value in relation to personal adequacy (e.g. having worth or status) and/or to social connection (e.g. being liked, loved, accepted or included). In each of th

Incidence, associated risk factors, and the ideal mode of delivery following preterm labour between 24 to 28 weeks of gestation in a low resource setting.

BACKGROUND: Preterm labour, between 24 to 28 weeks of gestation, remains prevalent in low resource settings. There is evidence of improved survival after 24 weeks though the ideal mode of delivery remains unclear. There are no clear management protocols to guide patient management. We sought to determine the incidence of preterm labour occurring between 24 to 28 weeks, its associated risk factors and the preferred mode of delivery in a low resource setting with the aim of streamlining patient care. METHODS: Between February 2020 and September 2020, we prospectively followed 392 women with pre

Adherence to oxygenation and ventilation targets in mechanically ventilated premature and sick newborns: a retrospective study.

BACKGROUND: Ventilator treatment exposes newborns to both hyperoxemia and hyperventilation. It is not known how common hyperoxemia and hyperventilation are in neonatal intensive care units in Norway. The purpose of this study was to assess the quality of current care by studying deviations from the target range of charted oxygenation and ventilation parameters in newborns receiving mechanical ventilation. METHODS: Single centre, retrospective chart review that focused on oxygen and ventilator treatment practices. RESULTS: The bedside intensive care charts of 138 newborns reflected 4978 hours

Digital Versus In-Person Physical Therapy in Adults With Musculoskeletal Conditions: Retrospective Matched-Cohort Analysis of Surgery and Low-Value Surgical Rates.

BACKGROUND: Musculoskeletal (MSK) disorders are leading causes of disability worldwide, with clinical guidelines recommending physical therapy-based interventions. Digital MSK programs offer an alternative to address logistical and socioeconomic barriers to regular in-person care. However, evidence comparing surgical use between digital and in-person physical therapy remains limited, particularly for low-value procedures. OBJECTIVE: This study aimed to evaluate the 12-month incidence of MSK surgery and low-value surgical procedures among participants initiating a multimodal Digital Care Progr

Management of low back pain among Medicaid beneficiaries: modalities, patterns, and perspectives across states.

INTRODUCTION: Low back pain is the leading cause of disability worldwide and Medicaid beneficiaries are disproportionally impacted. No studies have comprehensively examined patterns of treatment among Medicaid beneficiaries with low back pain. METHODS: We quantitatively described modalities of treatment and low-value care received following a low back pain diagnosis among Medicaid beneficiaries across the United States. We then qualitatively explored factors that influence treatment patterns by interviewing chronic pain experts. RESULTS: On average, 39.6% of patients received a prescription

The usefulness of the STarT back screening tool and single-item general health measures when predicting future disability in patients with low back pain treated in Danish primary care physiotherapy.

INTRODUCTION: The extent to which disease specific screening tools or other health measures add to the predictive value of common clinical factors (pain, disability and socio-demographics) has been sparsely investigated. The aim of this study was to investigate whether a disease specific screening tool and a single-item general health measure adds predictive value to basic information collected in primary physiotherapy care when predicting future disability in patients with low back pain. MATERIAL AND METHODS: This longitudinal cohort study included 354 patients with low back pain from Danish

Nursing-related time toxicity: An evolutionary concept analysis.

BACKGROUND: Time toxicity has been used in cancer care to describe the time patients lose to treatment and healthcare-related activities. In nursing work, time-related problems have been described across several areas, including documentation burden, electronic health record use, non-nursing tasks, low-value care, time pressure, missed nursing care and recovery loss. However, these phenomena have not been integrated into a coherent nursing concept. OBJECTIVE: To clarify the concept of nursing-related time toxicity by identifying its surrogate terms, related concepts, defining attributes, ante

Marginal competence, risk assessment, and care decisions: a comparison of values of health care professionals and older adults.

Using a case scenario involving a marginally competent elderly woman living alone at risk, we assessed the care decisions made by older adults (n = 82) and health care professionals (HCPs, n = 87), and identified differences in the values underlying the care decisions. Overall, participants did not place a high value on independence when they appraised the risk to the client as high and safety as low. Under these conditions, elderly respondents tended to be more paternalistic in their decisions about care, while HCPs tended to be more beneficient. If the values of HCPs differ from those of eld

Patterns of better breast cancer care in countries with higher human development index and healthcare expenditure: Insights from GLOBOCAN 2020.

BACKGROUND: The huge burden of breast cancer (BC) necessitates the profound and accurate knowledge of the most recent cancer epidemiology and quality of care provided. We aimed to evaluate BC epidemiology and quality of care and examine the effects of socioeconomic development and healthcare expenditure on disparities in BC care. METHODS: The results from the GLOBOCAN 2020 study were utilized to extract data on female BC, including incidence and mortality numbers, crude rates, and age-standardized rates [age-standardized incidence rates (ASIRs) and age-standardized mortality rates (ASMRs)].
