Mastoid obliteration with the temporoparietal fascia flap.

Obliteration of persistently discharging open mastoid cavities is one surgical option to achieve a dry ear. All the currently described techniques involve the use of random pattern local tissue flaps or free grafts. Ten patients have undergone obliteration procedures using the vascularized temporoparietal fascia flap. This resulted in rapid epithelialization by six weeks in seven out of 10 cases. The remaining three patients have persistent non healed areas over the medial attic wall, but are not troubled by otorrhoea. The indications, technique and complications of mastoid obliteration by thi

Care of the ear canal and mastoid.

Zinc sulfate has been used for therapy of granulations of the trachea and ear that have not responded to usual treatment. This paper will report on current indications for zinc sulfate and the resulting success in managing granulomata of the ear canal and mastoid bowl. Some mastoid bowls have been large and hard to clean, especially in the posterior and lateral parts. A method of cleaning using indirect otoscopy and curved aspirators in conjunction with adequate lubrication will be described. Treatment of otalgia in both the ear canal and mastoid bowl due to otitis externa sicca and results of

Tuberculous mastoiditis - a case series.

Tuberculous mastoiditis (TBM) is a rare form of extrapulmonary tuberculosis (TB), which may result in catastrophic complications, including mastoid and ossicle destruction, hearing loss and intracranial spread if untreated. Diagnosis is challenging due to the paucibacillary nature of extrapulmonary TB, compounded by limited theatre access for specimen retrieval, resulting in delayed diagnosis and treatment initiation. In this case series, we discuss three cases of TBM (one paediatric and two adults) who presented to the public and private healthcare sectors in the Eastern Cape in 2022, undersc

Pneumocephalus and Meningitis as Complications of Mastoiditis.

Pneumocephalus in the absence of trauma, tumour, or surgery is a rare entity. We report a case of a 73-year-old lady who presented with sepsis leading to confusion and unresponsiveness. A CT of brain revealed mastoiditis, sinusitis, and associated pneumocephalus. Further investigations led to an eventual diagnosis of pneumococcal meningitis. The combination of pneumocephalus and meningitis as complications of mastoiditis is rare with very few cases published in the literature. We describe one such case.

Mastoiditis with concomitant Lemierre's syndrome.

Otalgia with mastoiditis is an infrequently encountered complication of acute otitis media (Pfaff and Moore, 2018). Even more rare is the development of infected jugular venous thrombosis, Lemierre's disease. We present a case of a six year-old girl with otalgia for over two months who presented to our Emergency Department (ED) with clinical mastoiditis, confirmed on CT scan, as well as an incidental diagnosis of complete thrombosis of the internal jugular (IJ) vein, Lemierre's syndrome. The true prevalence of Lemierre's from mastoiditis is difficult to discern. This clinical case highlights t

Primary tubercular mastoiditis in children.

Primary tubercular mastoiditis is a rare clinical entity. We report three cases of it and among those one was first case of silent tubercular in an infant and two presented with facial paralysis. Though the incidence of middle ear cleft involvement with tuberculosis is quite rare, early complications make it important for its early diagnosis. A high index of suspicion should be kept in mind even when classical symptoms and signs are not present and early initiation of antitubercular therapy can prevent the complications.

[Acute mastoiditis: an increasing entity].

BACKGROUND: Mastoiditis used to be the most common complication of acute otitis media. However, once antibiotics became widely available, it was rarely reported. Recently, this complication has become more frequent. OBJECTIVES: To determine the frequency of acute mastoiditis in our center in the last few years and to analyze the clinical and bacteriologic characteristics of the patients with this diagnosis. METHODS: Retrospective analysis of all patients admitted to our hospital with a diagnosis of acute mastoiditis from 1994-2001. RESULTS: One hundred patients were diagnosed with acute mas

[Intracranial complications of acute mastoiditis].

We report a case of extradural abscess after acute mastoiditis in an 8-years-old boy. The clinic diagnostic of an intracranial complication is difficult. The most common present symptoms are fever, otalgia and otorrhea but are not specific. Neurologic symptoms are suggestive signs of an intracranial complication. We believe that, in acute mastoiditis performance of a CT scan of the brain and temporal bones with intravenous contrast, contribues to the diagnostic of intracranial complication. The diagnostic of thrombosis sinus sigmoid is evoked with slight contrast enhanced sinus sigmoid and fai

Acute Salmonella mastoiditis in an infant.

During the last few decades antibiotics have played an extremely important role in the management of otitis media (OM). Nowadays there are only sporadic reports of its sequelae and complications in the developed countries. Nevertheless, complications of OM still arise and the potential seriousness of this problem emphasizes the need for a high degree of monitoring. We report herewith a case of acute mastoiditis in an 18-month-old infant with chronic suppurative otitis media (CSOM). Repeated cultures from the middle ear and mastoid cavity yielded Salmonella type C. The treatment modality and th

The diagnosis and management of mastoiditis in children.

Even though mastoiditis as a complication of AOM is uncommon, its recognition is imperative to institute timely therapy. Acute coalescent mastoiditis generally follows a severe bout of AOM. Intravenous antimicrobial therapy and myringotomy drainage are usually satisfactory measures. However, refractory cases may require a simple mastoidectomy. Chronic mastoiditis in children is treated initially with intravenous antimicrobial therapy and vigorous aural toilet, which is successful in most patients. Mastoidectomy may be required in selected patients. The clinician must be aware of the differenti

Emergency presentation of coalescent mastoiditis.

Since the introduction of sulfonamides in the late 1930s, coalescent mastoiditis is an uncommon complication of otitis media. Swelling, erythema, and tenderness over the mastoid area with associated anterior displacement of the auricle are classic signs and symptoms of coalescent mastoiditis with formation of a subperiosteal abscess. Early recognition of this entity is essential to prevent its complications. Prompt otolaryngologic consultation is indicated and inpatient antibiotic therapy should be begun. Surgical treatment is also needed and may obviate further complications.
