Hospital-wide interventions for reducing or preventing in-hospital prescribing errors: a scoping review.

INTRODUCTION: Prescribing errors (PEs) in hospital care lead to patient harm, prolonged hospital stays, readmissions, and mortality. Despite interventions that successfully target 'high risk' populations in trials, PE rates remain largely unchanged in real-world settings. Existing studies often focus narrowly on specific populations, overlooking the wider complexities of hospital-wide prescribing. This scoping review evaluates interventions for adult inpatients to identify knowledge gaps in how to reduce in-hospital PEs. METHODS: A systematic search of PubMed, EMBASE.com, and the Cochrane Lib

[Medication errors with concentrated potassium intravenous solutions: Data of the literature, context and prevention].

Accidental direct intravenous injection of a concentrated solution of potassium often leads to patient death. In France, recommendations of healthcare agencies to prevent such accidents cover only preparation and intravenous infusion conditions. Accidents continue to occur in French hospitals. These facts demonstrate that these recommendations are insufficient and ineffective to prevent such deaths, especially those occurring during a catheter flushing. This article reviews the measures able to reduce the number of accidents. Countries which removed concentrated ampoules from ward stocks obser

Clinical assessment of a new anaesthetic drug administration system: a prospective, controlled, longitudinal incident monitoring study.

A safety-orientated system of delivering parenteral anaesthetic drugs was assessed in a prospective incident monitoring study at two hospitals. Anaesthetists completed an incident form for every anaesthetic, indicating if an incident occurred. Case mix data were collected and the number of drug administrations made during procedures estimated. From February 1998 at Hospital A and from June 1999 at Hospital B, until November 2003, 74,478 anaesthetics were included, for which 59,273 incident forms were returned (a 79.6% response rate). Fewer parenteral drug errors occurred with the new system th

Potentially inappropriate medication prescribing based on 2019 Beers criteria and the impact of pharmacist intervention in elderly patients with kidney diseases: A report from Iran.

BACKGROUND AND AIMS: A potentially inappropriate medication (PIM) is a pharmaceutical agent that poses a greater risk of harm than potential benefit to elderly patients. This study aimed to detect PIMs and their risk factors in hospitalized elderly patients with kidney disease. METHODS: This cross-sectional study assessed medication orders of elderly patients (≥65 years old) with kidney diseases admitted to the hospital. In the first 6 months, we retrospectively evaluated all medications to identify PIMs according to the 2019 Beers criteria. In the second phase, a clinical pharmacist prospect

Perspective: leveraging the electronic health record to improve sleep in the hospital.

UNLABELLED: Inpatient sleep loss can worsen health outcomes, including delirium and falls. Sleep disruptions in the hospital often originate from provider-patient interactions ordered electronically through computerized provider order entry. These orders contain clinical decision support systems with default schedules. These defaults are often around-the-clock, may not align with patients' needs, and cause iatrogenic sleep loss. Optimizing clinical decision support in the electronic health record can decrease unnecessary sleep disruptions and influence sleep-friendly decision-making. CITATION

Impact of Specific Alerts in Potassium-Increasing Drug-Drug Interactions.

Alerts in potassium(K+)-increasing drug-drug interactions (DDIs) are often ignored due to their low specificity. Although different approaches have been implemented to address DDIs, subsequent clinical studies revealed poor adherence to such alerts. We therefore suggest a novel alert concept currently being evaluated in a randomized clinical trial in a large teaching hospital. Highly specific reminders (to monitor K+) and alerts (of hyperkalaemia) are displayed to the physicians of the intervention group, whereas reminders and alerts are suppressed in the control group. Preliminary analysis sh

Rebooting healthcare information technology downtime management.

Healthcare, like many other industries, has become reliant on technology to manage daily operations. Often, technology downtimes are managed within the information technology departments to ensure the technical staff can return the hardware or software to the end users as quickly as possible. However, if managed strictly through IT departments, such incidents lack critical communication components and an understanding of the impact on end users. This paper argues that such technological incidents need to be managed not just rapidly but also in conjunction with the emergency preparedness depart

[Use of aminopenicillins: first cause of error in the prescription of antimicrobial agents at a hospital].

The patterns of use and quality of prescription of ampicillin and amoxycillin (A/A) as compared with the rest of antimicrobials (AM) was evaluated by means of the review of the therapeutic sheets of 4572 admitted patients, in coincidence with 5 calculations of prevalence in a university hospital. 25.7% of patients received AM. These were used in 865 instances, and 6% were disqualified. A/A participated in 57% of these errors (p = 0.002 as related with the rest of AM). 309 patients received AM as prophylactic drugs, and 70% of them did not comply with the standard rules. The election of A/A for

[Retrocession, a profitable but risky activity].

UNLABELLED: The retrocession (out-patient dispensing of hospital-reserved drugs)is a pharmaceutical critical activity requiring a care security with a territorial approach. In this drug supply chain, the pharmacist is the last step before the drug administration and the economic profitability is questionable. In this context, a risk mapping and an economic evaluation seem necessary. METHODS: The risk analysis was conducted with the adverse events collected. The economic study was realised with the point of view of the hospital and with the microcosting method. RESULTS: Six never events were

New IEEE standard enables data collection for medical applications.

The IEEE has gone to ballot on a "Standard for Medical Device Communications", IEEE P1073. The lower layer, hardware portions of the standard are expected to be approved by the IEEE Standards Board at their December 11-13, 1994 meeting. Other portions of the standard are in the initial stages of the IEEE ballot process. The intent of the standard is to allow hospitals and other users to interface medical electronic devices to host computer systems in a standard, interchangeable manner. The standard is optimized for acute care environments such as ICU's, operating rooms, and emergency rooms. [1

Be well!: a computer-based health care interview for hospital personnel.

We developed a computer-based health care interview for employees at an urban teaching hospital. The interview is part of the hospital-wide, integrated Center for Clinical Computing (CCC) system at the Beth Israel Hospital in Boston, and is available on 1500 terminals located throughout the hospital. The program emphasizes medical problems and patterns of living for which behavioral change is considered desirable. Conducted in private and with protection of confidentiality, the interview offers information about people and facilities available to help members of the hospital community seek bet

Impact on hospital functions following the 2010 Chilean earthquake.

OBJECTIVE: The objective of this study was to assess the impact of the 2010 Chilean earthquake on hospital functions and services. Hospitals functioning in a post-disaster environment must provide emergency medical care related to the event, in addition to providing standard community health services. This study focused on damage to both structural and nonstructural components, as well as to utility services. METHODS: Site visits were made to every hospital in a single province (Bio-Bio). Engineers conducted damage assessments while interviews of hospital administrators were conducted. The su

Comparative Effectiveness of Post-Discharge Strategies for Hospitalized Smokers: study protocol for the Helping HAND 2 randomized controlled trial.

BACKGROUND: Smoking cessation interventions for hospitalized smokers are effective in promoting smoking cessation, but only if the tobacco dependence treatment continues after the patient leaves the hospital. Sustaining tobacco dependence treatment after hospital discharge is a challenge for health care systems. Our previous single-site randomized controlled trial demonstrated the effectiveness of an intervention that facilitated the delivery of comprehensive tobacco cessation treatment, including both medication and counseling, after hospital discharge. We subsequently streamlined the interve

Adverse drug events caused by medication errors in medical inpatients.

PRINCIPLES: In view of growing concern in recent years regarding medication errors as causes of adverse drug events (ADEs), we explore the frequency and characteristics of error-associated ADEs in medical inpatients. METHODS: All patients with ADEs or ADErelated hospital admission in a cohort of medical inpatients identified by "event monitoring" (SAS/CHDM database, Br J Clin Pharmacol 2000:49:158-67) were evaluated independently by two experienced physicians. ADEs were first divided into ADEs occurring during cohort stay (incident ADE) and ADE present prior to/at admission. ADEs were then gr

Medication Adherence and its Related Factors in Patients Undergoing Coronary Artery Angioplasty.

Introduction: Percutaneous Coronary Intervention (PCI) has no effect on coronary artery atherosclerosis, thus the modification of physiological risk factors seems essential to prevent coronary artery disease (CAD). Then PCI patients have to receive multiple drug therapies in an attempt to prevent the recurrence of cardiac events. In spite of the evidence based on medication adherence to prevent post-PCI CAD development, medication adherence is the main concern for health care system. Accordingly, this study aims to determine the medication adherence and its related factors among these patient

Wearable devices for improving medication adherence in older adults with chronic conditions: A systematic review.

BACKGROUND: Medication adherence remains a major public health challenge, especially among older adults managing multiple chronic conditions and complex therapeutic regimens. Poor adherence is linked to increased mortality, complications, hospital readmissions, higher healthcare costs, and reduced quality of life. Wearable devices (WDs), integrated into everyday accessories and equipped with sensors and reminder systems, may offer a practical strategy to support continuous monitoring and improve adherence. Given the still-limited evidence in this area, this review evaluates the impact of weara

Syndromic analysis of computerized emergency department patients' chief complaints: an opportunity for bioterrorism and influenza surveillance.

STUDY OBJECTIVE: Emergency department computerized triage logs might be useful for automated ED surveillance and potentially for early identification of bioterrorism events. We describe a Web-based surveillance program and its feasibility for surveillance. METHODS: A Web-based surveillance program that receives computerized chief complaint data daily from a large academic urban teaching hospital and performs syndromic analysis on these data was developed. On the basis of preset limits, the Web-based surveillance program sends an alert e-mail message when the syndromic analysis reveals an incr
