Frosted branch angiitis in an octogenarian with infective endocarditis.

PURPOSE: To present the difficulties in diagnosing frosted branch angiitis (FBA) in an elderly person, and to present the likely immunogenic association of FBA with alpha-hemolytic streptococci. METHODS: Thorough review of the medical records of an 80-year-old white man diagnosed with FBA. Patient consent was obtained before the writing of this article. RESULTS: Our patient presented with 1 week of decreasing vision and floaters in his right eye and 3 days of floaters in his left eye. Significant medical history included a prosthetic mitral valve. Notable features on examination were bilater

Acute dysphagia in an octogenarian: an unusual case of tetanus.

Tetanus is a major worldwide health problem, and its global incidence has been estimated to be approximately 1 million cases per year. In particular, tetanus is more frequent in the elderly as compared with adults. We report a case of an octogenarian who presented with dysphagia and tremors as the only clinical symptoms. During hospitalization, the patient's clinical conditions worsened rapidly, and, although in absence of the classic clinical presentation (trisma, nuchal rigidity, and opisthotonus), a diagnosis of tetanus was suspected. Thus, the patient underwent a tetanus immunoglobulin imm

A case of poppy tea dependence in an octogenarian lady.

While poppy seed and poppy tea dependence has been described, it is unusual to see such patients actively seek treatment in India. We report the case of an 82-year-old client with dependent use of poppy for 55 years. She was brought for treatment as access to poppy became difficult following legal restrictions. She was successfully maintained on buprenorphine maintainence.

Endovascular aortic aneurysm repair in the octogenarian: is it worthwhile?

HYPOTHESIS: During the past decade, endovascular stent graft repair (EVSG) of abdominal aortic aneurysms has emerged as a less invasive and less morbid alternative to open surgical repair. We hypothesize that EVSG may become the treatment method of choice among patients older than 80 years. DESIGN: Retrospective case series. SETTING: Major academic medical center with extensive experience in endovascular and open aortic aneurysm surgery. PATIENTS AND METHODS: During a 5-year period, EVSG was performed in 595 patients at our institution. One hundred fifty (25.2%) of these patients were older

Colorectal surgery in the elderly: an audit of surgery in octogenarians.

BACKGROUND: Morbidity and mortality rates are higher in elderly compared to younger patients undergoing colorectal cancer surgery. This study was undertaken to see whether this finding applied to all colorectal surgery in the elderly and if so to try to identify the determining factors. METHODS: All patients undergoing colorectal surgery between 1975 and 1990 were entered into a computerized database. Patients were divided into two groups, those less than 80 years (< 80) and those 80 years and more (80+), and compared with regard to the type of surgery performed, the patient's race, the senio

Isolated coronary artery bypass in octogenarians: does gender matter?

BACKGROUND: Coronary artery bypass grafting (CABG) in octogenarians remains a challenging but increasingly common procedure due to extended life expectancy and the growing burden of coronary artery disease in the elderly. This study aimed to evaluate sex-specific differences in perioperative characteristics and early outcomes in patients aged ≥80 years undergoing isolated CABG. METHODS: A total of 104 patients aged ≥80 years who underwent isolated CABG between 2017 and 2023 were retrospectively analysed. Patients were stratified by gender, and comparisons were made regarding preoperative como

Hospitalisation from fractures in New Zealand octogenarians: LiLACS NZ.

UNLABELLED: Longitudinal data quantifying fracture rates beyond 80 years are lacking. Over 5 years, hospitalised fracture incidences increased by 85% in Māori and 73% in non-Māori octogenarians. However, while fracture-related hospital nights increased by 107% in non-Māori, they remained stable for Māori. Hospitalised fracture risk increases markedly with 5 years of advanced ageing. PURPOSE: Fractures become increasingly common in people beyond 80 years, the most serious resulting in hospitalisation. This study examines longitudinal changes in hospitalised fractures in octogenarians. METHODS

Pancreaticoduodenectomy for octogenarians under postoperative rehabilitation enhanced ERAS protocol.

BACKGROUND: Although pancreaticoduodenectomy (PD) for pancreatic or periampullary cancer is the gold standard treatment regardless of patient age, patients aged 80 years or older have poor postoperative short-term outcomes because of their poor functional status and many medical comorbidities. Postoperative rehabilitation in octogenarians could improve postoperative outcomes; however, its effect remains unclear. METHODS: This retrospective study included patients who underwent PD at two institutions between January 2019 and December 2022. All patients were managed using the enhanced recovery

Increased mortality in octogenarians treated for lifestyle limiting claudication.

OBJECTIVE: Treatment for lifestyle limiting claudication (LLC) that is due to infra-inguinal peripheral artery disease relies on either bypass, angioplasty, and/or stenting. Given the enthusiasm and shift toward more endovascular therapy for treatment of LLC, we sought to analyze whether octogenarians benefit from infra-inguinal interventions in the same manner as their younger counterparts. METHODS: We identified all patients admitted for elective treatment of LLC from the Nationwide Inpatient Sample from 2003 to 2012, who received open surgical or endovascular intervention for infra-inguina

Atrial fibrillation ablation in octogenarians: where do we stand?

The prevalence of atrial fibrillation increases significantly with age, affecting nearly 10 % of adults greater than 80 years of age. Complications from atrial fibrillation, including stroke, also increase with age. Medical therapy includes anticoagulation, ventricular rate control, and if symptoms persist, maintenance of sinus rhythm with antiarrhythmic drugs. However, anticoagulation and antiarrhythmic therapy is often challenging in the elderly due to side effects, comorbidities, and heightened sensitivity to medications. Catheter based ablation of atrial fibrillation is an effective treatm

Drug-eluting stents in octogenarians: early and intermediate outcome.

BACKGROUND: Large randomized controlled trials have demonstrated that percutaneous coronary intervention with the routine use of drug-eluting stents is safe and effective. However, octogenarians are usually excluded from these trials. METHODS: We analyzed 2453 consecutive patients who underwent DES implantation at the Mayo Clinic (Rochester, MN). The patients were classified in 2 age groups: patients > or = 80 years of age < or = 79 years of age. RESULTS: Patients > or = 80 years old had significantly more adverse baseline characteristics including more comorbid conditions and more severe ex

Tissue versus mechanical prostheses: quality of life in octogenarians.

BACKGROUND: The aim of this study was to determine whether changes in prognosis and quality of life (QOL) after aortic valve replacement (AVR) in octogenarians differ depending on the choice of mechanical (MP) or tissue (BP) valves. METHODS: Between July 1992 and September 2006, 160 consecutive octogenarians underwent AVR with (18.8%) or without concomitant coronary artery bypass grafting. At follow-up (mean 3.4 +/- 2.8 years, 552 patient-years, 98.3% complete), 121 were still alive and answered the Medical Outcomes Study Short-Form 36 Health Survey (SF-36) QOL questionnaire. [Cardiac surgery in octogenarians: current situation and future perspectives].

BACKGROUND AND OBJECTIVE: Increasing life expectancy makes cardiac surgery in octogenarians not very uncommon. We evaluated the characteristics and outcomes of patients aged over 80 undergoing cardiac surgery in our centre. PATIENTS AND METHOD: We retrospectively analyzed preoperative risk factors, in-hospital morbimortality, and long term survival in 120 patients aged 80 years--mean age (standard deviation): 81.53 (1.83)--who underwent cardiac surgery between 2000 and 2005, in comparison with 2425 younger patients referred to our center during the same period. Carotid artery stenting in octogenarians: is it too risky?

Recent clinical trials have shown that the safety of carotid angioplasty and stenting (CAS) is equivalent to that of carotid endarterectomy (CEA) in high-risk patients. Despite this, the beneficial outcome in octogenarians has come under scrutiny in light of the interim results of the Carotid Artery Revascularization versus Stent Trial (CREST). We report on a single-center experience of patients > or =80 years of age compared to younger cohorts treated with CAS. CAS procedures, performed exclusively by vascular surgeons, from April 2001 to December 2004 were retrospectively reviewed. Mechanica

Carotid endarterectomy in octogenarians: early results and late outcome.

PURPOSE: This study was undertaken to determine the safety and efficacy of carotid endarterectomy (CEA) in the octogenarian population at the Cleveland Clinic. METHODS: From 1989 to 1995, 182 CEAs were performed among 167 octogenarians (98 men, 69 women) with a mean age of 83 years (median, 83 years; range, 80 to 93 years). One hundred procedures (55%) were performed for severe asymptomatic stenosis, whereas 48 (26%) were performed for hemispheric transient ischemic attacks (TIAs) or amaurosis fugax, 24 (13%) for prior stroke, and 10 (5%) for vertebrobasilar symptoms. Major cardiac findings at necropsy in 366 American octogenarians.

We examined the hearts of 366 octogenarians (184 women [50%], 264 white [72%], mean age 84 +/- 4 years). The cause of death was cardiac in 195 (53%), noncardiac but vascular in 47 (13%), and noncardiac and nonvascular in 124 patients (34%). Of the 195 patients with fatal cardiac disease, atherosclerotic coronary artery disease was the cause of death in 127 (65%): acute myocardial infarction in 87 (69%), sudden cardiac arrest outside the hospital in 19 (15%), chronic congestive heart failure with healed myocardial infarction in 15 (12%), and complications of coronary bypass surgery in 6 (4%). Octogenarians with aortic stenosis. Outcome after aortic valve replacement.

To assess the outcome of aortic valve replacement for aortic stenosis in octogenarians, we retrospectively studied 64 patients, aged 80-89 years, who underwent aortic valve replacement for aortic stenosis from 1974 to 1987. Mean aortic valve gradient was 61 +/- 22 mm Hg, and valve area was 0.5 +/- 0.2 cm2. Concurrent coronary artery bypass grafting was performed in 29 patients, mitral valve replacement in two, and both procedures in two. Serious, comorbid, noncardiac conditions were infrequent. In-hospital mortality was 9.4%. Outcomes were classified as 1a) uncomplicated; 1b) complicated (tech

Results of Stress Incontinence Surgery in Octogenarian Women.

Stress urinary incontinence (SUI) is common among octogenarian women, often in association with significant comorbidities that can include functional and cognitive impairment. There is limited evidence in the literature on the efficacy and safety of current surgical options for treating SUI in octogenarians. The few studies available on this topic mainly focus on mid-urethral slings and reveal similar efficacy and safety profiles for octogenarian women in comparison to younger women. These studies are mostly retrospective, with small sample sizes and short follow-up. What data there are sugges
