Medicare payments for outpatient urological surgery by location of care.

PURPOSE: The cost implications associated with offloading outpatient surgery from hospitals to ambulatory surgery centers and the physician office remain poorly defined. Therefore, we determined whether payments for outpatient surgery vary by location of care. MATERIALS AND METHODS: Using national Medicare claims from 1998 to 2006, we identified elderly patients who underwent 1 of 22 common outpatient urological procedures. For each procedure we measured all relevant payments (in United States dollars) made during the 30-day claims window that encompassed the procedure date. We then categoriz

Office glucose analysis.

The advent of dry chemistry techniques has made inexpensive glucose testing equipment available to physicians and their patients. The ability to measure blood glucose concentration quickly, in almost any setting, has eliminated urinary glucose monitoring as the parameter of choice for managing diabetic patients. Blood glucose monitoring has allowed improved control of diabetic patients who avail themselves of one of the techniques. The physician choosing to offer blood glucose testing in the office laboratory has a variety of test equipment from which to choose. Most physicians will want to ow

A snapshot of tobacco-related messages relayed in pediatric offices in Delaware.

OBJECTIVE: Much research exists demonstrating that pediatricians should counsel patients and families about tobacco. However, few data are available about tobacco-related messages relayed in pediatric offices. Since an anti-tobacco office environment can be a strong component of an active tobacco prevention program, we evaluated pediatric offices in Delaware to characterize tobacco-related messages. METHODS: A convenience sample of 32 of 63 (51%) pediatric offices in Delaware was directly evaluated for the presence of tobacco-related messages. Fifty-five of 63 (87%) pediatric practices in Del

Ambulatory monitoring in the evaluation of blood pressure in patients with borderline hypertension and the role of the defense reflex.

The differences between blood pressure (BP) readings taken in the clinic and during normal daily activities were assessed in two studies using a noninvasive ambulatory BP monitor (Avionics). In the first study 30 untreated hypertensive patients (17 with borderline pressures, average diastolic less than or equal to 95, and 13 established hypertensives, diastolics above 95) and 5 normotensive subjects had 30 readings taken in the physician's office and 30 while at home. Conventional sphygmomanometer BPs were also recorded in the office. In the borderline group home BPs were significantly lower t

A population-based study of paediatric emergency department and office visits for concussions from 2003 to 2010.

BACKGROUND: There is a paucity of information regarding descriptive epidemiology of paediatric concussions over time, and few studies include both emergency department (ED) and physician office visits. OBJECTIVE: To describe trends in visits for paediatric concussions in both EDs and physician offices according to age and sex. A secondary objective was to describe the cause of concussion for children treated in EDs. METHODS: A retrospective, population-based study using linked health administrative data from all concussion-related visits to the ED or a physician office by school-age children

Fecal occult blood testing beliefs and practices of U.S. primary care physicians: serious deviations from evidence-based recommendations.

BACKGROUND: Fecal occult blood testing (FOBT) is an important option for colorectal cancer screening that should be available in order to achieve high population screening coverage. However, results from a national survey of clinical practice in 1999-2000 indicated that many primary care physicians used inadequate methods to implement FOBT screening and follow-up. OBJECTIVE: To determine whether methods to screen for fecal occult blood have improved, including the use of newer more sensitive stool tests. DESIGN: Cross-sectional national survey of primary care physicians. PARTICIPANTS: Parti

Markers for primary care: missed opportunities to immunize and screen for lead and tuberculosis by private physicians serving large numbers of inner-city Medicaid-eligible children.

OBJECTIVE: This study examines coverage levels for immunization, missed opportunities to immunize, and extent of lead and tuberculosis screening in inner-city storefront physician offices and then relates child, visit, and physician characteristics to missed opportunities. METHODOLOGY: With the use of a nested sampling strategy, 232 charts were selected for review in 31 physicians' offices. Charts selected were for children 0 to 35 months of age who had three or more visits in more than 3 months. Physicians were selected from those in specific low-income New York inner-city neighborhoods who

Series: infectious diseases and the office laboratory. Office laboratory diagnosis of sexually transmitted diseases.

The office laboratory can be a tremendous resource for the physician. This is particularly true in the evaluation of STD, where many simple procedures may provide at least a preliminary diagnosis. Instead of waiting for more expensive culture results, the physician can make the diagnosis of Candida, Trichomonas or Gardnerella in the female and N. gonorrhoeae in the male, while the patient is in the office. Cultures for N. gonorrhoeae can be, with the use of the Phadebact or newer enzymatic sugar tests, completed in the office. This, however, is not recommended for an office which performs this

Ambulatory blood pressure measurement and antihypertensive therapy.

The traditional basis for assessing the effect of antihypertensive therapy is the blood pressure reading taken by a physician. However, several recent trials have been designed to evaluate the blood pressure lowering effect of various therapeutic agents during the patients' normal daytime activities, using a portable, semi-automatic blood pressure recorder. The results have shown that in a given patient, blood pressure measured at the physician's office often differs greatly from that prevailing during the rest of the day. This is true both in treated and untreated hypertensive patients. The d

Factors influencing treatment and time spent with physicians in patients with uveitis compared to other ophthalmology subspecialties in the National Ambulatory Medical Care Survey.

BACKGROUND: Cases of uveitis can necessitate long-term treatment resulting in recurrent follow-up appointments. Analysing the demographic distribution and patient factors influencing treatment and time spent with physicians in this population compared to other subspecialties of ophthalmology using the National Ambulatory Medical Care Survey (NAMCS) has not previously been studied. METHODS: Data were extracted from the NAMCS database, a large, nationally representative survey of office-based specialists, entered between 2012-2016 and 2018. Demographics, time with physician, and payor types wer

Ownership or leasing of MRI facilities by nonradiologist physicians is a rapidly growing trend.

PURPOSE: To study growth trends in the ownership of magnetic resonance imaging (MRI) examinations by nonradiologist physicians who either own the equipment outright or are involved in scan leasing arrangements. METHODS AND MATERIALS: Medicare Part B data sets from 2000 through 2005 were reviewed, and procedure codes for MRI examinations were selected. The focus was on only those procedures performed at nonhospital, private-office facilities. Using Medicare's physician specialty codes, all such studies were categorized according to the specialties of the physicians who performed them. Ownershi

No compromise reached on CON extension.

With the federal law requiring state certificate of need laws for expensive medical equipment up for renewal, the 95th Congress debated whether or not to extend the law to cover equipment used in noninstitutional settings. The AHA contended that the exemption for physicians' offices was a loophole that had to be filled; the AMA argued that each state should make its own choice; and Congress couldn't reach a decision. This controversial issue is likely to come up in the 96th Congress.
