Biomechanical performance of static and NiTiNOL compression devices in first metatarsophalangeal joint arthrodesis.

BACKGROUND: First metatarsophalangeal (MTP) joint arthrodesis is commonly performed to treat hallux rigidus. Traditional stainless steel or titanium constructs (e.g., locking plates with screws) provide sufficient stiffness but are prone to permanent deformation after excessive pre-fusion loading, leading to plantar gapping and malalignment. Superelastic NiTiNOL staples capable of sustained dynamic compression (SDC) have been developed to address these shortcomings. PURPOSE: To compare the biomechanical performance of first MTP arthrodesis constructs using static fixation, SDC staples, or hyb

Segmented Forefoot Plate in Basketball Footwear: Does it Influence Performance and Foot Joint Kinematics and Kinetics?

This study examined the effects of shoes' segmented forefoot stiffness on athletic performance and ankle and metatarsophalangeal joint kinematics and kinetics in basketball movements. Seventeen university basketball players performed running vertical jumps and 5-m sprints at maximum effort with 3 basketball shoes of various forefoot plate conditions (medial plate, medial + lateral plates, and no-plate control). One-way repeated measures ANOVAs were used to examine the differences in athletic performance, joint kinematics, and joint kinetics among the 3 footwear conditions (α = .05). Results in

The medial crosssover toe: a cadaveric dissection.

UNLABELLED: The development of a medial crossover second toe (second toe crossing over the first toe) is not a rare clinical condition. It often occurs in the setting of hallux valgus, although not exclusively so. The resulting displacement of the second toe can cause pain in shoes, with surgical correction being problematic. The pathologic anatomy of this condition has not been fully described. In an effort to better understand it, dissection of a cadaveric specimen with a full crossover toe is presented. The dissection revealed findings not previously documented. They include medial displace

Properties and Function of the Flexor Hallucis Capsularis Interphalangeus Tendon.

INTRODUCTION: The fibrocartilagenous plantar plates of the forefoot are biomechanically important, forming the primary distal attachment for the plantar aponeurosis. They are integral to the function of the windlass mechanism in supporting the arches of the foot in gait. Dissection of the cadaveric hallux revealed an organised sagittal thickening of the dorsal side of the flexor hallucis longus (FHL) sheath, which attached the interphalangeal plantar plate to the metatarsophalangeal (MTP) plantar plate. A description of a similar structure was made in 1984 when it was termed the flexor halluci

Hydatid cyst of the foot: a case report.

BACKGROUND: Hydatid cyst is rarely located in soft tissues. This case of a hydatid cyst over the plantar surface of our patient's foot is one of the rarest presentations. CASE PRESENTATION: This is a case report of a 22-year-old Somali who presented with a lump over the plantar surface of his foot of 1-year duration. The diagnosis of hydatid cyst was made intraoperatively from the typical appearance of a hydatid cyst. The cyst was completely excised. No local recurrence has been detected to date. CONCLUSIONS: The rare location and uncommon incidence made the initial diagnosis of hydatid cyst

Crossed-screws provide greater tarsometatarsal fusion stability compared to compression plates.

BACKGROUND: Hallux valgus is a common deformity that is often treated with a fusion of the first tarsometatarsal (TMT) joint. Crossed-screws are currently the accepted standard but advances in plate systems present opportunities for improved clinical outcomes; however, in vitro testing should be performed prior to clinical implementation. The purpose of this study was to determine whether a locking plate with surgeon-mediated compression provides similar fusion stability compared to crossed-screws and if bone density or joint size are related to construct success. METHODS: Ten matched-pair ca

Radiographic Outcomes, Union Rates, and Complications Associated With Plantar Implant Positioning for Midfoot Arthrodesis.

BACKGROUND: Midfoot arthrodesis has long been successfully included in the treatment paradigm for a variety of pathologic foot conditions. A concern with midfoot arthrodesis is the rate of nonunion, which historically has been reported between 5% and 10%. Plantar plating has also been noted to be more biomechanically stable when compared to traditional dorsal plating in previous studies. Practical advantages of plantar plating include less dorsal skin irritation and the ability to correct flatfoot deformity from the same medial incision. The purpose of this study is to report the arthrodesis r

Grade III Turf Toe Injury in a Cowboy.

Turf toe injuries are common, particularly in athletes competing on artificial turf. This debilitating injury and its associated sequelae can affect the long-term performance of athletes and others. In this case is presented an atypical cause for development of grade III turf toe. This case presents an acute injury with significant damage to the plantar first metatarsophalangeal joint, with plantar plate rupture and tibial sesamoid retraction secondary to injury involving working calves on a ranch. The anatomy, mechanism, and associated treatments are reviewed. The anatomical and functional in

Differential contribution of lateral plantar foot ligaments to lateral column stability - A cadaver based sectioning analysis.

Lateral column (LC) instability occurs in adult acquired flatfoot deformity (AAFD). Differential ligament contribution to LC stability is unknown. The primary aim was to quantify this by using cadaver sectioning of lateral plantar ligaments. We also determined the relative contribution of each ligament to dorsal translation of the metatarsal head in the sagittal plane. 17 below-knee cadaveric specimens, preserved by vascular embalming method, were dissected to expose plantar fascia, long/short plantar ligaments (L/SPL), calcaneocuboid (CC) capsule and inferior 4th/5th tarsometatarsal (TMT) cap

Volar transfer of the lateral band with transverse retinacular ligament is effective for the correction of swan-neck deformity caused by volar plate injury of the PIP joint.

We introduced a technique with a volar transfer of the lateral band using the transverse retinacular ligament for swan-neck deformity caused by volar plate injury of the PIP joint. A 61-year-old woman injured her 5th finger and was diagnosed with a volar plate injury of the PIP joint. She presented with snapping of the finger together with the appearance of a swan-neck deformity, and surgery was performed. Dorsally located lateral bands were transferred towards the volar aspect of the finger, and their position was maintained using the transverse retinacular ligament. Improvements in the snapp

Midfoot fusion: a biomechanical comparison of plantar planting vs intramedullary screws.

BACKGROUND: Numerous reconstructive techniques for midfoot collapse secondary to Charcot neuroarthropathy have been described, but few have been studied biomechanically. The purpose of this study was to biomechanically compare 2 of the most common techniques. METHODS: Seven paired below-knee specimens were amputated through the talonavicular and calcaneocuboid joints. The nonligamentous soft tissue was stripped proximal to the metatarsal heads and disarticulated through the tarsometatarsal (TMT) joints. For each paired specimen, the TMT joints were fused by plantar plating or intramedullary s

Double plate fixation for the management of proximal interphalangeal joint instability in 30 horses (1987-2015).

REASONS FOR PERFORMING STUDY: Double plate fixation has been reported in a small number of horses with comminuted fractures of the middle phalanx. There are no published reports using this technique in the management of other injuries that destabilise the proximal interphalangeal joint (PIJ), including biaxial palmar/plantar eminence fractures and disruption of the palmar/plantar soft tissue support structures of the PIJ. Furthermore, there is a paucity of information on the long-term prognosis for horses undergoing double plate fixation to manage destabilising injuries of the PIJ. OBJECTIVES

State of the Art in Lesser Metatarsophalangeal Instability.

Advancements in lesser metatarsophalangeal (MTP) instability have involved the use of minimally invasive surgery techniques, synthetic augmentation of existing transfers, and use of arthroscopy for both diagnosing and addressing MTP disorder. Advances in imaging modalities, particularly MRI, have aided in diagnosing subtle instability. Clinical outcomes seem to be similar to traditional approaches as the indications and applicability continue to evolve.

Does Flexible Flatfoot Require Treatment?: Plantar Pressure Effects of Wearing Over-the-Counter Insoles when Walking on a Level Surface and Up and Down Stairs in Adults with Flexible Flatfoot.

BACKGROUND: Orthotic insole is a popular physiotherapy for flatfoot. However, the effects and whether flexible flatfoot needs orthotic insole treatment are not clear, and how the plantar pressure changes while walking up and down stairs has not been studied. Therefore, this study observed the plantar pressures of different walking conditions to find the answers. METHODS: Fifteen adults with flexible flatfoot and 15 adults with normal foot were examined while walking on a level surface and while walking up and down 10- and 20-cm stairs before treatment. The maximum force and the arch index wer

Comparison of simultaneous static standing balance data on a pressure mat and force plate in typical children and in children with cerebral palsy.

BACKGROUND: Balance testing is an important component in treatment planning and outcome assessment for children with Cerebral Palsy (CP). Objective measurement for static standing balance is typically conducted in motion labs utilizing force plates; however, a plantar pressure mat may prove to be a viable alternative for this type of balance assessment. METHODS: This study examined static standing balance simultaneously on a force plate and a plantar pressure mat in 30 typically developing (TD) and 30 children with CP to determine if valid measures of static standing balance could be obtained

Effects of carbon-fiber plate design on foot stress injury risk: a finite element analysis.

INTRODUCTION: Carbon-fiber plate (CFP) running shoes may alter ankle and foot biomechanical loading patterns, thereby potentially increasing the risk of foot stress injuries. However, there still a lack of systematic quantitative analysis of how multiple key design parameters (determining CFP performance) influence foot biomechanical responses. This study aimed to clarify how composite CFP influence the mechanical response of the foot and ankle, with the goal of reducing overuse injuries and providing biomechanical guidance for the structural design of CFP running shoes. METHODS: A three-dime

The distributed plantar vertical force of neutrally aligned and pes planus feet.

The distributed vertical ground reaction forces were determined for a normative population and contrasted with data from subjects with flat feet. Nineteen asymptomatic subjects, 11 with a neutrally aligned foot type (normal arch) and 8 with a pes planus foot type (low arch), were studied as they walked barefoot across a pressure plate. The pressure plate data were converted to force values at seven locations (subhallucal, five submetatarsal and subcalcaneal) on the plantar aspect of the foot. The distributed loading pattern of the plantar soft tissue throughout the stance phase of gait was det

Treatment Options for Turf Toe: A Systematic Review.

Turf toe is hyperextension injury of the plantar plate at the first metatarsophalangeal joint. Etiologies have often included sports/activities with excessive forefoot axial loading and/or violent pivotal movements. The purpose of the systematic review was to systematically review and present an overview for the current evidence-based treatment options of turf toe. Both authors systematically reviewed the PubMed and EMBASE databases from inception to April 2016 based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The level of evidence and quality of evide
