Healing of the perineal wound after proctectomy in Crohn's disease patients: only preoperative perineal sepsis predicts poor outcome.

BACKGROUND: The aim of our study was to assess perineal wound healing in patients with Crohn's disease (CD) who undergo proctectomy or proctocolectomy with end ileostomy and to evaluate the influence of various factors including types of perineal dissection on eventual wound healing. METHODS: Data for patients with CD who underwent proctectomy or total proctocolectomy with end ileostomy from 1995 to 2012 were reviewed. The relationship between perineal wound healing and demographics, patient characteristics, and other factors was assessed using univariate and multivariate analyses. RESULTS:

Desmoid tumors do not prevent proctectomy following abdominal colectomy and ileorectal anastomosis in patients with familial adenomatous polyposis.

BACKGROUND: Elective proctocolectomy has been recommended for patients at high risk of desmoids based on the possibility that cancer in a retained rectum may be unresectable because of desmoid disease. There are no data to support the reality of this concern. OBJECTIVE: The aim of this study was to see how often proctectomy was prevented by desmoids. DESIGN: This retrospective, descriptive, database study was augmented by chart review. SETTING: This study was conducted at a hereditary colorectal cancer clinic in a tertiary referral center. PATIENTS: Those presenting for proctectomy after c

Iatrogenic colorectal perforation induced by anorectal manometry: report of two cases after restorative proctectomy for distal rectal cancer.

There are no reports regarding perforation of the colorectum induced by anorectal manometry. We report two cases of colorectal perforation that occurred during manometry in the patients undergoing restorative proctectomy for distal rectal cancer. In the first patient, computed tomography showed an extraperitoneal perforation in the pelvic cavity and a rupture of the rectal wall. A localized perforation into the retroperitoneum was managed conservatively. In the second patient, a 3 cm linear colon rupture was detected above the anastomotic site. A primary closure of the perforated colon and pro

Endorectal mucosal resection without proctectomy as an adjunct to abdominoperineal resection for nonmalignant conditions: clinical experience with five patients.

Five patients with chronic ulcerative colitis underwent endorectal excision of the rectal mucosa without removing the rectal muscle in combination with total colectomy and cutaneous ileostomy. This operative technique will cure the patient of the primary disease and obviates many of the usual unpleasant complications following total proctectomy, such as prolonged perineal drainage and sexual and bladder dysfunction. It has the further advantages of lower operative blood loss and earlier safe ambulation. On the basis of favorable clinical experience with these patients, we believe that the oper

Incidence and Risk Factors for Persistent Perineal Sinus After Proctectomy in Crohn Disease: A Single-center Retrospective Cohort Study.

OBJECTIVE: This retrospective cohort study aimed to determine the incidence of persistent perineal sinus (PPS) after proctectomy for Crohn disease (CD) and to identify associated risk factors. SUMMARY BACKGROUND DATA: Proctectomy remains necessary in a subset of patients with CD, yet impaired perineal wound healing and PPS continue to represent major sources of postoperative morbidity. METHODS: All patients undergoing proctectomy for CD between 1993 and 2023 at a tertiary referral center were included. The primary outcome was development of PPS, defined as a perineal wound remaining unhealed

Non-steroidal Anti-inflammatory Drugs and Timing-specific Anastomotic Leak Risk Following Minimally Invasive Proctectomy: A Retrospective Cohort Study.

BACKGROUND: Reports regarding postoperative administration of non-steroidal anti-inflammatory drugs (NSAIDs) and anastomotic leak (AL) after rectal surgery are conflicting. Distinctions between early and late AL have often been overlooked, leaving it uncertain whether the NSAID-related effects differ according to the timing of leak presentation. METHODS: This retrospective cohort study included 1663 patients undergoing minimally invasive rectal resection (2010-2021). NSAID exposure was defined as one or more dose within the first postoperative day. AL was classified as early (≤ 6 days) or lat

Transanal completion proctectomy after total colectomy and ileal pouch-anal anastomosis for ulcerative colitis: a modified single stapled technique.

AIM: Minimally invasive surgery has proved its efficacy for the surgical treatment of ulcerative colitis (UC). The recent evolution in single port (SP) surgery together with transanal rectal surgery could further facilitate minimally invasive surgery in UC patients. This technical note describes a technical modification for single stapled anastomoses in patients undergoing transanal completion proctectomy and ileal pouch-anal anastomosis (ta-IPAA) for UC. METHODS: A step-by-step approach of the ta-IPAA in UC is described, including pictures and a video illustration. RESULTS: We describe a ta

Experience with proctectomy to manage combat casualties sustaining catastrophic perineal blast injury complicated by invasive mucor soft-tissue infections.

Catastrophic pelviperineal injuries have become more commonplace on the modern battlefield as a result of dismounted blast mechanism and the survival benefits afforded by far-forward resuscitation and surgery. Inherent in the survival of casualties with such devastating injuries is both the risk for invasive infections and the need for complex reconstruction. We report a series of two combat casualties sustaining the constellation of catastrophic pelviperineal blast injury, anal sphincter complex destruction, complicated by invasive fungal infection managed with completion proctectomy. Single-port robotic restorative proctectomy with ileal pouch-anal anastomosis for ulcerative colitis: first clinical experience and technical insights.

BACKGROUND: Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) is the standard surgical option for patients with ulcerative colitis (UC) wishing to avoid a permanent ileostomy. Single-port (SP) robotic surgery offers enhanced dexterity and visualization, potentially overcoming the limitations of single-port laparoscopy in the confined pelvic cavity. Its role in IPAA for UC is unexplored. METHODS: Patients with ulcerative colitis undergoing SP robotic proctectomy with IPAA at a tertiary center between October 2024 and June 2025 were reviewed. Procedures followed a standardize

Adaptative changes in terminal ileum after total colectomy, mucosal proctectomy and straight ileo-anal anastomosis in chronic ulcerative colitis.

Total colectomy, mucosal proctectomy and straight ileo-anal anastomosis was performed in four adult patients with chronic ulcerative colitis (three males, one female; mean age 27.4 years). During a follow-up period of one to five years, all patients were continent, had spontaneous bowel evacuation (average 4-5 per day), and had normal bladder and sexual function. Sequential per rectal ileoscopies revealed loss of ileal folds and gradual transformation to a colonic type of mucosa. On barium studies, the anastomosed terminal ileum showed progressive ballooning and assumed a rectosigmoid appearan

Outcomes of restorative proctectomy and ileal pouch-anal anastomosis: a matched-cohort analysis of robot-assisted, laparoscopic, and open surgery.

BACKGROUND: Ileal pouch-anal anastomosis (IPAA) remains the gold standard for restorative proctocolectomy, yet its technical complexity has limited widespread minimally invasive adoption. Robotic surgery was introduced to overcome the ergonomic and visualization challenges of laparoscopy, but comparative evidence among open, laparoscopic, and robot-assisted IPAA in tertiary care remains scarce. METHODS: A matched-cohort analysis was performed on adult patients undergoing proctectomy with IPAA for ulcerative and indeterminate colitis (January 2005-September 2025) at a tertiary referral center.

Robotic Single-Port Transstomal Proctectomy and IPAA Construction With Exoscope-Assisted Transanal Transection and Single-Stapled Anastomosis for Ulcerative Colitis.

BACKGROUND: Proctectomy with IPAA is a promising indication for the single-port approach because the procedure can be performed using a single-port platform placed at the stoma site. A single-port robotic platform may address many of the challenges associated with single-incision laparoscopy. However, it lacks compatible stapling devices. Consequently, performing the distal rectal transection with linear laparoscopic staplers is suboptimal, as it often requires significant enlargement of the abdominal defect at the stoma site. The transanal transection and single-stapled anastomosis technique

Perineal wound complications after proctectomy for Crohn's disease: the impact of the omental pedicle flap and predictors of return to the operating room.

BACKGROUND: The role of omental pedicle flaps (OPFs) in reducing the risk of perineal wound complications and postoperative pelvic abscesses in patients with Crohn's disease remains unclear. This study aimed to determine whether OPFs had any effect on the incidence of perineal wound complications requiring a return to the operating room (OR) or the requirement for radiologic-guided drainage of pelvic abscesses in patients with Crohn's disease who underwent proctectomy. METHODS: This was a retrospective, single-center study. The study included patients who underwent proctectomy for Crohn's dis
