Annular and leaflet augmentation in Noonan's syndrome with dysplastic pulmonary valve.

Two patients with dysplastic pulmonary valves associated with Noonan's syndrome successfully underwent leaflet augmentation with pericardial membrane and annular enlargement with preservation of valve competence. Both patients are doing well at 36 and 37 months, respectively, postoperatively. Echocardiography and clinical assessment showed gradients of 12 and 16 mmHg, respectively, and negligible pulmonary valve insufficiency. The surgical technique is simple and provides an attractive alternative in patients with dysplastic pulmonary valve and small pulmonary orifice and annulus.

Recurrent neoaortic insufficiency after the switch back operation with previous repair of transposition with ventricular septal defect and aortic arch hypoplasia.

Neoaortic insufficiency is not uncommon after the arterial switch operation (ASO) for d-Transposition, yet surgery is rarely required. In a patient with worsening neoaortic regurgitation post-arterial switch and ventricular septal defect (VSD) closure, we performed a successful "switch back" operation with documented aortic valve competence on discharge echocardiography. However, recurrent severe aortic insufficiency required valve replacement, and histopathology of the excised valve indicated abnormal leaflet/vascular wall structure. We question whether the switch back operation is a viable o

Anasarca due to pulmonic valve regurgitation with low pulmonary vascular resistance.

A 57-year-old woman underwent pulmonic valvotomy for congenital pulmonic stenosis. She developed severe pulmonic insufficiency, secondary tricuspid regurgitation, and anasarca in spite of a normal pulmonary artery pressure. Insertion of a pulmonary valve prosthesis and tricuspid valve plication reversed all clinical symptoms and signs of this rare complication of pulmonary valvotomy.

Pulmonary valve preservation and restoration strategies for repair of tetralogy of Fallot.

Despite tremendous advances in surgical treatment of tetralogy of Fallot, augmenting the small right ventricular outflow tract remains a challenge. Transannular patch augmentation revolutionised surgical management, but did so at the expense of rendering patients with pulmonary insufficiency and the resulting problems associated therewith. Recent surgical efforts have focused on pulmonary valve preservation at initial correction and pulmonary valve restoration after transannular patching, with favourable results. In this manuscript, we review methods of pulmonary valve preservation and restora

The Ross procedure: outcomes at 20 years.

OBJECTIVES: Our study examines the outcomes of the Ross procedure in a cohort of 212 patients prospectively followed with clinical and echocardiographic assessments. METHODS: Patients' mean age was 34 ± 9 years; 66% were men and 82% had congenital aortic valve disease. The median follow-up was 13.8 years. Patients who had reoperations continued to be followed and entered into the survival analysis. RESULTS: There was 1 operative death as well as 9 late deaths (3 in patients who no longer had the Ross). Survival at 20 years was 93.6% and similar to the general population matched for age and s

Interpretation of cardiac pathophysiology from pressure waveform analysis: the pulmonary valve.

Although an uncommon lesion, when pulmonary stenosis is considered, pulmonary artery and right ventricular pressures should be assessed simultaneously on two-catheter pullback to appreciate the precise location of pulmonary-right ventricular pressure gradients. The case examples demonstrate that the peripheral pulmonic stenosis can mimic pulmonary valve stenosis and that pulmonary artery insufficiency may be difficult to delineate on pressure alone (as is often the case with the hemodynamics of aortic insufficiency). These hemodynamic tracings are complemented by the echocardiographic and angi

Risk factors for early pulmonary valve replacement after valve disruption in congenital pulmonary stenosis and tetralogy of Fallot.

OBJECTIVE: Congenital heart defects with a component of pulmonary stenosis are often palliated in childhood by disrupting the pulmonary valve, either by means of dilation or excision. It is unclear what factors affect a patient's ability to tolerate long-term pulmonary insufficiency before requiring pulmonary valve replacement. We analyze potential factors that are related to the interval between pulmonary valve disruption and pulmonary valve replacement. METHODS: One hundred seven patients were analyzed. They had a congenital diagnosis of pulmonary stenosis or tetralogy of Fallot, had their

Pulmonary cusp and annular extension technique for reconstruction of right ventricular outflow in tetralogy of Fallot.

An alternative pulmonary valve-sparing technique for primary repair of tetralogy of Fallot is presented. Reconstruction of the right ventricular outflow tract was accomplished by augmentation of the anterior facing pulmonary cusp, the pulmonary annulus, and the main pulmonary artery using a single autologous pericardial patch. This valve-sparing technique preserves the coaptation and suspension mechanisms of the native pulmonary cusp and therefore may minimize pulmonary insufficiency in the long term.

Distensibility of the pulmonary autograft under systemic pressure.

BACKGROUND AND AIMS OF THE STUDY: Although the Ross procedure has recently been recognized as one of the best replacements of the aortic valve, regurgitation has occasionally been observed with dilatation of the sinotubular junction (STJ) and/or basal ring (BR). This study was designed to evaluate the native distensibility of the free pulmonary root under systemic pressure. METHODS: Pulmonary arterial grafts (n = 7) were taken from pig hearts and pressurized (20-100 cmH2O) with crystal sensors attached around the STJ and BR. Dimensions of the STJ and BR were measured using a digital 3-D ultra

Tetralogy of fallot and pulmonary valve replacement: timing and techniques in the asymptomatic patient.

An increasing number of survivors of congenital heart disease will require repeat surgical intervention. In the tetralogy of Fallot population, the primary indication is for pulmonary valve replacement secondary to pulmonary regurgitation. Although pulmonary valve disruption during repair is initially well tolerated, by the second decade following repair, patients develop increased right ventricular size with potentially decreased function. It is now well accepted that patients experiencing symptoms of heart failure secondary to chronic pulmonary insufficiency undergo pulmonary valve replaceme

Absent pulmonary valve syndrome in infancy: surgery reconsidered.

Congenital absence of the pulmonary valve leaflets is generally associated with ventricular septal defect, anular pulmonary stenosis and aneurysmal dilatation of the pulmonary arteries: the absent pulmonary valve syndrome. Symptomatic infants with this syndrome suffer primarily from respiratory insufficiency caused by bronchial compression by the dilatated pulmonary arteries, and have a high mortality rate. Asymptomatic infants are thought to do well, and may have elective surgery late in childhood. Findings in four infants with absent pulmonary valve syndrome are reviewed. One symptomatic inf

Anatomical mismatch of the pulmonary autograft in the aortic root may be the cause of early aortic insufficiency after the Ross procedure.

OBJECTIVE: Early aortic insufficiency can be a problem after the Ross procedure. Anatomical mismatch and an inexact surgical technique may lead to distortion of the normal pulmonary valve geometry and subsequent incorrect leaflet coaptation and valve insufficiency. In this study, we assessed the efficacy of changing and improving the surgical technique to minimize the early pulmonary autograft valve failure. The modifications and the strategy are discussed. METHODS: From January 1995 to February 1999, a total of 77 adults underwent the Ross procedure for aortic valve replacement at Sahlgrensk

Bicuspid pulmonary valve with atrial septal defect leading to pulmonary aneurysm.

Pulmonary artery aneurysms are rare. We describe 2 adult patients with pulmonary artery aneurysm with normal pulmonary pressure associated with bicuspid pulmonary valve and atrial septal defect. One patient presented with moderate pulmonary valve stenosis and was treated with open surgery; the other patient had a small atrial septal defect and mild pulmonary valve insufficiency and is periodically still being evaluated. Hemodynamic alterations associated with a pulmonary artery aneurysm are described; the influence of additional volume overload and intrinsic wall abnormalities in pulmonary val

[Echocardiography and right heart catheterization in pulmonal hypertension].

BACKGROUND AND OBJECTIVE: Since pulmonary hypertension is defined haemodynamically with a mean pulmonary artery pressure (mPAP) of at least 25 mmHg, right heart catheterization is mandatory for diagnosis of PH. However, it remains unclear if echocardiography can always detect a PH and to what extent echocardiographic and invasive parameters correlate. We aimed to determine the frequency of right heart valve insufficiencies, the correlation of mPAP measured invasively and estimated by echocardiography and the correlation of other echocardiographic parameters with invasively measured cardiac out

4D Flow Cardiac Magnetic Resonance Uncovers the Cause of Bioprosthetic Pulmonary Valve Dysfunction.

We present a case of a 20-year-old man with tetralogy of Fallot status post-repair with a transannular patch and left pulmonary artery reconstruction at age 1 and subsequent pulmonary valve replacement with bioprosthetic valve 3 years ago. A transthoracic echocardiogram revealed severe bioprosthetic valve insufficiency. Cardiac magnetic resonance with 4-dimensional flow revealed a paravalvular leak. (Level of Difficulty: Intermediate.).

[Reconstruction of the right ventricular outflow tract in tetralogy of Fallot with a Hancock valve containing ventriculo-pulmonary conduit (author's transl)].

Between June 1975 and August 1978, 22 cases of anatomically and functionally severe tetralogy of Fallot were corrected with a valve bearing ventriculo-pulmonary Hancock conduit. The indication for use of the Hancock conduit was atresia of the pulmonary valve ring and main pulmonary artery (classified according to Sommerville and Jefferson as types I and II respectively) in 5 patients, severe hypoplasia of the pulmonary valve ring, the right ventricular outflow tract (RVOT) and the main pulmonary artery in 10, 1 patient with an abnormal right coronary artery and an acquired obstruction of the i

[Intracardiac hemodynamics after pulmonary artery valvuloplasty].

Studies of intracardiac haemodynamics were conducted in 30 patients subjected to valvuloplasty of the pulmonary artery valve. The method of continuous (for several days) right ventricle and pulmonary artery catheterization was employed. The isometric hyperfunction of the right ventricle was found to persist following the elimination of pulmonary valve stenosis. Myocardial insufficiency then develops that requires intensive cardiac therapy.

Percutaneous balloon pulmonary valvuloplasty: state of the art.

Since the first description of balloon pulmonary valvuloplasty in 1982 by Kan, the procedure has been extensively utilized by several groups of workers for relief of pulmonary valve stenosis. It is generally recommended that the procedure be performed for peak-to-peak gradients in excess of 50 mmHg. The technique involves positioning one or more balloon catheters across the stenotic valve, usually over an extra-stiff guide wire and inflating the balloons with diluted contrast material, thus producing valvotomy. The procedural details are described in this paper. The currently recommended ballo

The Damus-Stansel-Kaye procedure. Should the aortic valve or subaortic valve region be closed?

Two patients (one with transposition of the great arteries and another with Taussig-Bing anomaly) underwent the Damus-Stansel-Kaye procedure (Group I). Significant aortic valve insufficiency developed postoperatively in both patients. In contrast, seven patients with a univentricular heart and subaortic stenosis from a variety of reasons underwent creation of an aortopulmonary window (Group II), a procedure very similar to the proximal main pulmonary artery-aortic root anastomosis of the Damus-Stansel-Kaye procedure. Aortic valve insufficiency had not developed after up to 7 years of follow-up

Early echocardiographic changes after percutaneous implantation of the Edwards SAPIEN transcatheter heart valve in the pulmonary position.

OBJECTIVES: To evaluate echocardiographic changes after SAPIEN valve implantation in the pulmonary position. BACKGROUND: The feasibility of the SAPIEN transcatheter pulmonary valve (TPV) has recently been demonstrated. We evaluated changes in pulmonary valve function and the right ventricle after SAPIEN TPV placement. METHODS: We evaluated echocardiograms at baseline, discharge, 1 and 6 months after TPV placement in 33 patients from 4 centers. Pulmonary insufficiency severity was graded 0-4. TPV peak and mean gradients were measured. Right ventricular (RV) size and function were quantified u
