Vascular function in patients with advanced heart failure and continuous-flow or pulsatile ventricular assist devices.

BACKGROUND: A significant proportion of patients with heart failure (HF) progress to an advanced stage, which is associated with a substantial increase in morbidity and mortality. These patients may be eligible for advanced treatment strategies such as mechanical circulatory support with ventricular assist devices (VAD). Vascular dysfunction is a hallmark of heart failure pathophysiology and prognosis. However, whether and to what degree the hemodynamic benefits of VADs influence vascular function remain unknown. METHODS AND RESULTS: In this study, we evaluated endothelial vascular function w

Computational modeling of blood clot lysis considering the effect of vessel wall and pulsatile blood flow.

Stroke is one of the major causes of global death, which can occur due to blockage in a blood vessel by a clot. The immediate dissolving of the clot is essential to restore the blood flow and prevent tissue necrosis. Clot dissolution can be achieved via thrombolytic therapy using plasminogen activators. In this study, a clot dissolution model is developed for a three-dimensional patient-specific carotid artery that investigates the effect of different vessel wall models on clot dissolution. The lysis pattern of the clot and hemodynamics of blood flow are evaluated using three different models

Pulsatile arterial blood pressure mimicking aortic valve opening during continuous-flow LVAD support: a case report.

BACKGROUND: Left ventricular assist devices (LVAD) have become a common treatment option in advanced heart failure. Lack of aortic valve opening during left ventricular unloading is a common complication and associated with a worse outcome. Maintaining a minimum pulse pressure is an important goal during the early postoperative period after LVAD implantation since it is commonly seen as secure sign of aortic valve opening. AIMS/OBJECTIVE: We report a case of an LVAD-supported patient with early permanent closure of the aortic valve despite a pulse pressure > 15 mmHg at all times following LVA

Distribution of Air Embolization During TEVAR Depends on Landing Zone: Insights From a Pulsatile Flow Model.

Purpose: To analyze the distribution of air bubbles in the supra-aortic vessels during thoracic stent-graft deployment in zones 2 and 3 in an aortic flow model. Materials and Methods: Ten identical, investigational, tubular, thoracic stent-grafts were deployed in a glass aortic flow model with a type I arch: 5 in zone 2 and 5 in zone 3. A pulsatile pump generated a flow of 5 L/min with systolic and diastolic pressures (±5%) of 105 and 70 mm Hg, respectively. The flow rates (±5%) were 300 mL/min in the subclavian arteries, 220 mL/min in the vertebral arteries, and 400 mL/min in the common carot

Pulsatile blood flow in total cavopulmonary connection: a comparison between Y-shaped and T-shaped geometry.

Single-ventricle anomaly is a hereditary heart disease that is characterized by anatomical malformations. The main consequence of this malformation is desaturated blood flow, which without proper treatment increases the risk of death. The classical treatment is based on a three-stage palliative procedure which should begin from the first few days of patient's life. The final stage is known as Fontan procedure, in which inferior vena cava is directly connected to pulmonary arteries without going through the ventricle. This connection is called total cavopulmonary connection (TCPC). After surger

A numerical parametric study of the mechanical action of pulsatile blood flow onto axisymmetric stenosed arteries.

In the present paper, a fluid-structure interaction model is developed, questioning how the mechanical action of the blood onto an atheromatous plaque is affected by the length and the severity of the stenosis. An axisymmetric model is considered. The fluid is assumed Newtonian. The plaque is modelled as a heterogeneous hyperelastic anisotropic solid composed of the arterial wall, the lipid core and the fibrous cap. Transient velocity and pressure conditions of actual pulsatile blood flow are prescribed. The simulation is achieved using the Arbitrary Lagrangian Eulerian scheme in the COMSOL co

A fluid--structure interaction finite element analysis of pulsatile blood flow through a compliant stenotic artery.

A new model is used to analyze the fully coupled problem of pulsatile blood flow through a compliant, axisymmetric stenotic artery using the finite element method. The model uses large displacement and large strain theory for the solid, and the full Navier-Stokes equations for the fluid. The effect of increasing area reduction on fluid dynamic and structural stresses is presented. Results show that pressure drop, peak wall shear stress, and maximum principal stress in the lesion all increase dramatically as the area reduction in the stenosis is increased from 51 to 89 percent. Further reductio

Investigation of particle penetration in a nasal maxillary ostium with optimised T-junction geometry and pulsatile flow.

Effective drug delivery to the maxillary sinus is often limited by the narrow and variable shape of the maxillary ostium. To better understand and predict how surgical changes affect drug transport, the ostium can be modelled as a simplified T-junction. The geometric configuration of these junctions plays a crucial role in managing particle flow; however, optimal design parameters remain under-explored. This paper addresses this gap, by simulating a range of radius of curvatures R c at the T-junction and oscillatory flows with pulsation frequencies of 0, 30, 45, 60 and 75 Hz to analyse their

Lower limb exercise generates pulsatile flow into the pulmonary vascular bed in the setting of the Fontan circulation.

The absence of a subpulmonary ventricle in the Fontan circulation results in non-pulsatile pulmonary blood flow. Lower limb exercise in this setting can generate pulsatile pulmonary blood flow.

Collateral flow and pulsatility during large vessel occlusions: insights from a quantitative in vitro study.

Acute ischemic stroke caused by large vessel occlusions is being increasingly treated with neurovascular interventions. The hemodynamics within the collateral system of the circle of Willis (CoW) hemodynamics play a fundamental role in therapy success. However, transient in vivo data on pathological collateral flow during large vessel occlusions are not available. Moreover, there are no flow models that accurately simulate the hemodynamic conditions in the CoW during large vessel occlusions. We used a circulatory loop to generate highly reproducible cerebrovascular-like flows and pressures and

1D versus 3D blood flow velocity and pulsatility measurements of lenticulostriate arteries at 7T MRI.

PURPOSE: 7T MRI enables measurements of blood flow velocity waveforms in small, perforating cerebral arteries. As these vessels can be tortuous, acquisition methods sensitive to flow in only one direction may not be sufficient to accurately determine the dynamic blood flow velocity. In this study, we compared 1D with 3D velocity encoding to measure the blood flow velocity and pulsatility in the lenticulostriate arteries (LSAs). METHODS: Blood flow velocity waveforms were measured in the LSAs of 18 subjects (age range: 20-74 years) using prospectively gated single-slice phase contrast (PC) MRI

The effect of angle on wall shear stresses in a LIMA to LAD anastomosis: numerical modelling of pulsatile flow.

The purpose of this study was to examine the effect of anastomotic angle on the flow patterns and wall shear distributions at the distal anastomosis of a left interior mammary artery (LIMA) graft to the left anterior descending artery (LDA). It is now well recognized that abnormal wall shear stress distributions along the anastomotic bed, around the toe, and around the heel can contribute to the focal development of intimal hyperplasia. However, the exact nature of the interaction between the dominant pulsing flow and the anastomotic angle on wall shear stresses has not been fully investigated

Osteoblasts respond to pulsatile fluid flow with short-term increases in PGE(2) but no change in mineralization.

Although there is no consensus as to the precise nature of the mechanostimulatory signals imparted to the bone cells during remodeling, it has been postulated that deformation-induced fluid flow plays a role in the mechanotransduction pathway. In vitro, osteoblasts respond to fluid shear stress with an increase in PGE(2) production; however, the long-term effects of fluid shear stress on cell proliferation and differentiation have not been examined. The goal of this study was to apply continuous pulsatile fluid shear stresses to osteoblasts and determine whether the initial production of PGE(2

A Single-Center Experience with Continuous and Pulsatile Flow Veno-Arterial Extracorporeal Membrane Oxygenation for Cardiogenic Shock.

INTRODUCTION: Pulsatile flow in veno-arterial extracorporeal membrane oxygenation (VA-ECMO) for the treatment of cardiogenic shock may yield physiological benefits over continuous flow. The primary objective of this study was to evaluate whether there are detectable differences between these modes in a small single-center cohort. METHODS: We performed a retrospective analysis of patients who underwent VA-ECMO support for cardiogenic shock at our institution between 2019 and 2023. Data collected included demographic information, disease severity scores, duration of ECMO support, and duration o

Hemodynamic assessments of unilateral pulsatile tinnitus with jugular bulb wall dehiscence using 4D flow magnetic resonance imaging.

BACKGROUND: Pulsatile tinnitus (PT) is a type of tinnitus characterized by a rhythmic sound that is synchronous with the heartbeat. One of the possible causes of PT is the jugular bulb wall dehiscence (JBWD). However, the hemodynamics of this condition are not well understood. To elucidate this issue, the present study aimed to compare the blood flow of PT patients with JBWD, PT patients with sigmoid sinus wall dehiscence (SSWD), and volunteers. METHODS: A retrospective case-control study was conducted, which enrolled patients with unilateral PT who had undergone both computed tomography angi

Chronic pulsatile tinnitus and continuous vertigo due to very delayed diagnosis of single slow-flow dural arteriovenous.

Tinnitus and vertigo are classic symptoms of inner ear disease. Dural arteriovenous fistulas (DAVF) are a rare type of acquired intracranial vascular malformation whose symptoms mimic inner ear disease, but what distinguishes it from other tinnitus is the characteristic of DAVF is pulsatile and heartbeat-synchronous. We present a 58-year-old male with chronic left-sided pulsatile tinnitus (PT) for 30 years and continuous vertigo for 3 years that took numerous consultations to establish a diagnosis after the onset of symptoms. Delay in diagnosis is caused by normal magnetic resonance imaging an
