Reduced mobility is associated with adverse outcomes after in-hospital cardiac arrest.

OBJECTIVE: In-hospital cardiac arrest is a critical medical emergency. Knowledge of prognostic factors could assist in cardiopulmonary resuscitation decision-making. Frailty and functional status are emerging risk factors and may play a role in prognostication. The objective was to evaluate the association between reduced mobility and in-hospital cardiac arrest outcomes. METHODS: This retrospective cohort study included patients over 18 years of age with in-hospital cardiac arrest in Botucatu, Brazil, from April 2018 to December 2021. Exclusion criteria were patients with a do-not-resuscitate

Dynamic and volumetric variables of fluid responsiveness fail during immediate postresuscitation period.

OBJECTIVE: Fluid therapy after resuscitation from cardiac arrest is challenging since both hypovolemia and fluid overload may cause circulatory failure. Therefore, prediction of fluid responsiveness is a major issue in optimizing hemodynamic therapy. The aim of the present study was to evaluate the performance of stroke volume variation, pulse pressure variation, variation of Doppler-derived velocity time integral, and global end-diastolic volume index to predict fluid responsiveness in the postcardiac arrest period. DESIGN: Prospective animal study. SETTING: University-affiliated research l

Sodium nitroprusside-enhanced cardiopulmonary resuscitation improves resuscitation rates after prolonged untreated cardiac arrest in two porcine models.

OBJECTIVE: Sodium nitroprusside-enhanced cardiopulmonary resuscitation consists of active compression-decompression, an impedance threshold device, abdominal binding, and large intravenous doses of sodium nitroprusside. We hypothesize that sodium nitroprusside-enhanced cardiopulmonary resuscitation will significantly increase carotid blood flow and return of spontaneous circulation compared to standard cardiopulmonary resuscitation after prolonged ventricular fibrillation and pulseless electrical activity cardiac arrest. DESIGN: Prospective randomized animal study. SETTING: Hennepin County M

Assessment of the Effects of Sodium Nitroprusside Administered Via Intracranial Subdural Catheters on the Cerebral Blood Flow and Lactate Using Dynamic Susceptibility Contrast Magnetic Resonance Imaging and Proton Magnetic Resonance Spectroscopy in a Pig Cardiac Arrest Model.

Background Cerebral blood flow (CBF) is impaired in the early phase after return of spontaneous circulation. Sodium nitroprusside (SNP) administration via intracranial subdural catheters improves cerebral cortical microcirculation. We determined whether the SNP treatment improves CBF in the subcortical tissue and evaluated the effects of this treatment on cerebral lactate. Methods and Results Sixty minutes after return of spontaneous circulation following 14 minutes of untreated cardiac arrest, 14 minipigs randomly received 4 mg SNP or saline via intracranial subdural catheters. CBF was measur

Just the facts: Organ donation in the emergency department: When you can't save one, save eight.

A 16-year-old female presents to a community emergency room following a suicide attempt by hanging. Prehospital, on arrival of paramedics, the patient was in a pulseless electrical activity rhythm. Paramedics provided advanced cardiac life support for 20 minutes before they obtained return of spontaneous circulation. In the emergency department, she had another 25-minute cardiac arrest with ultimate return of spontaneous circulation. She is now hemodynamically stable on Levophed 0.2 µg/kg/min. Her neurological exam shows pupils to be 3 mm and fixed bilaterally, absent cough and gag, and no res

Association Between Maximum Temperature 24 Hours After Resuscitation and Survival for In-Hospital Cardiac Arrest.

BACKGROUND: Avoiding fever is a central tenet of postresuscitation care. However, few data exist describing the association between postresuscitation temperature and survival after in-hospital cardiac arrest, where response is usually prompt. METHODS: Within the GWTG-Resuscitation (Get With The Guidelines-Resuscitation) registry, we identified adults from 2015 to 2024 with an index in-hospital cardiac arrest and return of spontaneous circulation who survived at least 24 hours after resuscitation. Patients were excluded if they received therapeutic hypothermia after return of spontaneous circu

[Role of emergent coronary angiography in post-cardiac arrest care: from literature review to clinical practice].

Out-of-hospital cardiac arrest is a relatively common event. Acute coronary thrombotic events are the main trigger of sudden cardiac arrest. Mortality of patients, in whom return of spontaneous circulation is obtained, is still high, mainly due to anoxic brain injury and progressive cardiac failure. In the last years, the implementation of "post-cardiac arrest care" led to a significant improvement of hospital survival of these patients. Mild therapeutic hypothermia has become the foundation for improvement of survival with good neurological outcome after cardiac arrest. Recently, there is a g

Effects of Intrathecal Magnesium Administration on Neurologic Outcomes in a Porcine Model of Asphyxial Cardiac Arrest.

BACKGROUND: Magnesium mitigates several mechanisms involved in secondary brain injury after cardiac arrest. Because systemically administered magnesium crosses the blood-brain barrier poorly, it may be unsuitable for early post-cardiac arrest neuroprotection. We tested whether intrathecal magnesium (IT-Mg), which bypasses the blood-brain barrier, improves neurologic outcomes in a porcine model of asphyxial cardiac arrest. METHODS: Twenty-four pigs underwent asphyxial cardiac arrest and cardiopulmonary resuscitation. Thirty minutes after return of spontaneous circulation, the IT-Mg group recei

Effects of chest compressions on cardiovascular and cerebral hemodynamics in asphyxiated near-term lambs.

BACKGROUND: Chest compressions (CC) and adrenaline administration are recommended in asphyxiated newborns with persistent bradycardia despite effective ventilation. The effects of CC on cerebral blood flow in newborns at birth are unknown. Our aim was to determine the effects of CC, with or without adrenaline administration, on the return of spontaneous circulation, carotid blood flow (CBF), and carotid arterial pressure (CAP) in asphyxiated near-term lambs. METHODS: Asphyxia was induced in near-term lambs by clamping the umbilical cord and delaying ventilation onset until spontaneous circula

Low-dose tenecteplase during cardiopulmonary resuscitation due to massive pulmonary embolism: a case report and review of previously reported cases.

The case of a 29 year-old man who suffered a cardiac arrest due to a massive pulmonary embolism while he was undergoing surgical repair of a complex tibial plateau fracture is presented. After 70 min of unsuccessful cardiopulmonary resuscitation a bolus of 20 mg tenecteplase was given, with a return of spontaneous circulation 2 min after administration of the drug. Pulmonary embolism was subsequently demonstrated on a pulmonary angiogram. To our knowledge this is the first report to show that the use of a low dose of tenecteplase might be useful to achieve the return of spontaneous circulation
