Histopathologic Analysis of Temporal Bones With Otosclerosis Following Cochlear Implantation.

OBJECTIVE: Analyze changes in osteoneogenesis and fibrosis following cochlear implant (CI) surgery in patients with otosclerosis and compare differences based on insertion technique. BACKGROUND: When advanced otosclerotic disease extends to the otic capsule, severe and profound sensorineural hearing loss necessitates consideration of a cochlear implant. Histopathological analysis of the human temporal bone after implantation in the patient with otosclerosis may reveal important variables that predict CI success. METHODS: Histopathological evaluation of archival human temporal bones from subj

Microanatomic analysis of the round window membrane by white light interferometry and microcomputed tomography for mechanical amplification.

OBJECTIVE: The round window membrane (RWM) is increasingly becoming a target for amplification using active middle ear implants. However, the current strategy of using available transducer tips may have negative consequences for the RWM. We investigated the microanatomy of the RWM to establish a basis for the design of the transducer tip for the RWM driver. STUDY DESIGN: Using the guinea pig as an animal model, microcomputed tomography (μCT) and white light interferometry were used to study the topography of the RWM and RW niche (RWN). The curvatures of the RWM surface were calculated using t

Labyrinthine ossificans and cochlear implants.

We report two clinical and five histopathological examples of labyrinthitis ossificans and discuss the possible cause of this condition. Diagnosis can be established by polytomography. The advent of the cochlear implant makes ossification of the cochlea (labyrinthitis ossificans) of more than academic interest, since it requires alteration of the insertion of an electrode into the scala tympani and minimizes the chances of successful stimulation. Hearing losses due to meningitis, advanced cochlear otosclerosis, and trauma are likely to be due to cochlear ossification. Therefore, it is importan

Labyrinthitis ossificans due to chronic otitis media.

Histopathology of two cases of unilateral labyrinthitis ossificans in the presence of bilateral chronic suppurative otitis media is presented. In both cases, ossification and fibrosis were limited to the scala tympani of the lower basal turn, apparently due to inflammation entering through the round window membrane. The round window was occluded by new bone and fibrous tissue presumably resulting in a conductive hearing loss in addition to the conductive loss due to middle ear lesions. Degeneration of the hair cells of the organ of Corti was limited to the lower basal turn where ossification a

Link between functional and morphological changes in the inner ear--functional changes produced by ototoxic agents and their interactions.

Common potentials used to evaluate cochlear function are the ac cochlear potential (ACCP), N1 and the positive dc endocochlear potential (EP). The ACCP is an electrical analogue of the sound stimulus; its source is the electrical activity of the cochlear hair cells. N1 is a volume conductor recorded action potential of the auditory nerve. The EP is the positive polarization of the middle compartment of the cochlea (scala media) with respect to the other compartments (the scalae tympani and vestibuli); the stria vascularis is apparently responsible for the EP. Generally, ototoxic drugs and very

Inner ear immunity.

The inner ear, like all organs, interacts with the systemic immune system via lymphatic drainage and vascular circulation to protect itself from infections and stress such as acoustic trauma. The adult mammalian inner ear including the endolymphatic sac is populated with bone-marrow derived resident macrophages. Circulating macrophages continually renew the resident macrophage population. Cells within the endolymphatic sac participate in and affect inner ear immune responses, but specific mechanisms for the interactions are unknown. Resident macrophages are present within the cochlear modiolus

Exploration of biodistribution of mRNA-delivering lipid nanoparticles in the neonatal mouse inner ear.

Hearing loss affects millions worldwide, yet hearing aids and cochlear implants have major limitations. mRNA-based gene therapy could address the underlying molecular causes, but lipid nanoparticle (LNP) delivery in the inner ear remains underexplored and its biodistribution has not been systematically characterized. Here we establish and scale up an mRNA-encapsulated LNP formulation using a design-of-experiments (DoE) approach built on predefined critical quality attributes, and map the route-dependent distribution of reporter mRNA expression in the neonatal mouse cochlea. In HEI-OC1 auditory

Responses of the endolymphatic sac to perilymphatic injections and withdrawals: evidence for the presence of a one-way valve.

Although the endolymphatic sac (ES) is thought to be a primary site for endolymph volume regulation, we have limited knowledge of how it responds to volume and pressure changes. In a prior publication, we demonstrated changes of K(+), Na(+) and endolymphatic sac potential (ESP) resulting from volume injections into, and withdrawals from, scala media of the cochlea. In the present study, we compared the influence of injections into and withdrawals from scala tympani of the cochlea on the endolymphatic sac. It is assumed that similar pressure changes are induced in endolymph and perilymph of bot

Cochlear pathology following chronic electrical stimulation of the auditory nerve: II. Deafened kittens.

The present study examines the effects of long-term electrical stimulation of the auditory nerve on cochlear histopathology and spiral ganglion cell survival in young sensorineural deafened cats. Eight kittens were deafened using kanamycin and ethacrynic acid, and implanted with bipolar or monopolar scala tympani electrodes. Following recovery from surgery the animals were unilaterally stimulated using charge balanced biphasic current pulses for 450-1730 hours over implant periods of up to four months. Charge densities varied from 0.6-0.9 microC.cm-2 geom. per phase for monopolar electrodes to

Use of multichannel cochlear implants in obstructed and obliterated cochleas.

Obstruction that occurs within the scala tympani (after meningitis or otosclerosis) has been considered a contraindication for placement of a multichannel cochlear implant electrode. Two patients who exhibited radiographic evidence of intracochlear narrowing and obliteration were implanted with multichannel electrodes. Implantation involved creation of a channel for the electrode to wrap around the modiolus. The middle ear cavity and the external auditory canal also had to be removed to gain access. The response of one of the patients was similar to that of patients with normal cochlear anatom

Effects of furosemide on intracochlear oxygen tension in the guinea pig.

The effects of intravenous furosemide on the perilymphatic oxygen tension of the scala tympani (OT) in the guinea pig and the relationship between changes in OT and changes in auditory cortical-evoked responses were studied, as well as changes in cochlear morphology. Furosemide in dosages of 25 mg/kg and 50 mg/kg caused a sharp decline in OT in addition to elevation of the threshold of auditory cortical-evoked responses. Ischemia of the cochlear stria vascularis was also found but without any damage of hair cells at the light microscopic level. Changes in OT were associated with transient thre

[Nasal infection of the domestic cat with feline herpesvirus as an animal model of experimental viral labyrinthitis].

Looking for a suitable animal model of experimental labyrinthitis we performed in vivo infections of the domestic cat with feline rhinotracheitis virus. After instillation of the virus suspension into the nose of the anaesthetized cat we could find the complete virus particles in so-called virus factories in the nasal mucosa four days later by means of electron-microscopy. The adjacent ganglia of the olfactory nerve were stained by immunohistochemical methods and showed positive viral antigen. After embedding of the cochlea for electron microscopy virus particles could be demonstrated in the i

Inner ear morphologic changes resulting from cochlear implantation.

Insertion of electrodes into the cochlea can damage the spiral ligament, organ of Corti, osseous spiral lamina, and dendrites. The amount of damage depends on the method of insertion and the amount of surgical trauma. Damage is limited to the area immediately adjacent to the electrode. Damage in the cochlea and prolonged electrical stimulation do not affect ganglion cell population. The ganglion cell population responds directly to the electrical stimulus. Examination of three auditory nerves from three cases and the cochlear nuclei from one case also reveals no effect of prolonged electrical

Adenoviral-mediated gene transfer into the scala media of the mouse cochlea in vivo.

Gene therapy may provide a way to restore inner ear function to deaf and dizzy patients. The mouse is a crucial model system for functional genomics because of the numerous genetic models for hearing loss and inner ear dysfunction. Using an advance generation, E1-/E3-/E2b- (preterminal protein-/polymerase-) Type 5 Adenovirus, we investigated several routes of virus microinjection to determine which were most reproducible in targeting the endolymphatic fluid compartment of the cochlea. We found that when adenovirus is injected via the round window, transduced cells are found only adjacent to th

Evaluation of cochlear function in an acute endolymphatic hydrops model in the guinea pig by measuring low-level DPOAEs.

During and after microinjection of artificial endolymph into scala media of the guinea pig, the 2f1- f2 -DPOAE at 4.5 kHz generated by low-level primaries was recorded. Reproducible changes were measured when 1.1 microl of artificial endolymph was injected at a rate of 1.65 nl/s (1.53-1.83). This volume corresponds with an acute endolymphatic hydrops of 23%. After the onset of injection the inner ear pressure immediately increased to a mean higher level of 22 Pa, whereas the 2f1- f2 -amplitude and -phase did not change for about 1 min. Thereafter, the amplitude decreased 2.6 dB (+/- 0.7) on av

[Aural fullness and endolymphatic hydrops].

BACKGROUND: Low-frequency modulated DPOAEs were registrated to investigate whether the subjective symptom of aural fullness can indicate an endolymphatic hydrops. METHOD: The cochlear partition is periodically moved towards scala vestibuli and scala tympani by a low-frequency suppressor tone. The level of simultaneously registrated DPOAEs is modulated depending on the phase of the suppressor. This modulation may be reduced when the displacement of the organ of Corti is inhibited by mechanical stiffening of the basilar membrane (e. g. in endolymphatic hydrops). SUBJECTS: Low-frequency modulat

Effects of intra-cochlear perfusion of salicylates on cochlear microphonic and other auditory responses in the guinea pig.

The ototoxic action of salicylate was investigated in the guinea pig by perfusion of both salicylate and bromosalicylate through scala tympani. The results qualitatively confirmed experiments using intravenous administration in cats (Stypulkowski, 1990), showing dose-dependent elevations in compound action potential (CAP) thresholds, increases in cochlear microphonics (CM) and level-dependent reductions in 2f1-f2 acoustic distortion products. The endocochlear potential was not significantly affected and iontophoretic injection of salicylate into scala media had no measurable effect on CAP thre
