Anatomic and refractional correlations in earliest glaucomatous visual field defects.

PURPOSE: In glaucoma, initial visual field scotomas can be peripheral or central, whereas central scotomas are more severe and can disrupt daily activities. Individual anatomical features may influence the distribution of retinal nerve fibers and the starting site of visual field defects in glaucoma. In this study, we aimed to correlate myopia and hyperopia or anatomical variation of the disk-fovea angle with initial central or peripheral lesions in the visual field. METHODS: This cross-sectional study included patients with primary open-angle glaucoma divided into a group of isolated central

Volume perimetry: measurement in depth of visual field loss.

PURPOSE: Volume scotomas are three-dimensional regions of space that are not visible to the observer. Volume perimetry maps volume scotomas. Volume scotomas predicted from combining monocular visual fields assume known fixation locus (mainly foveal). However, fixation loci are not always known, especially with central field loss (CFL). Here we demonstrate methods for measuring and calculating volume scotomas and discuss their practical implications. METHODS: Three patients (bitemporal hemianopia, binasal scotoma, and CFL) were evaluated. Slices through the volume scotomas were measured at thr

Changes to central visual fields in cases of severe myopia in a Chinese population.

BACKGROUND: Severe myopia (SM) is commonly defined as a refractive error that exceeds -10.0 diopters (D). To date, however, studies focusing on visual field changes in eyes affected by SM remain scarce. The present study aims to characterize changes to central visual fields in the presence of SM among Chinese individuals and to provide evidence for enabling early diagnosis of primary open-angle glaucoma (POAG) in cases of SM. METHODS: The data of 26 cases (44 eyes) involving Chinese individuals with SM were collected using a Humphrey Visual Field Analyzer 750i. The following aspects were iden

Visual field defects of optic neuritis in neuromyelitis optica compared with multiple sclerosis.

BACKGROUND: Neuromyelitis optica (NMO) is an inflammatory demyelinating disease that predominantly affects the optic nerves and the spinal cord, and is possibly mediated by an immune mechanism distinct from that of multiple sclerosis (MS). Central scotoma is recognized as a characteristic visual field defect pattern of optic neuritis (ON), however, the differing pathogenic mechanisms of NMO and MS may result in different patterns of visual field defects for ON. METHODS: Medical records of 15 patients with NMO and 20 patients with MS having ON were retrospectively analyzed. A thorough systemic

Is the spectacle lens jack-in-the-box phenomenon really due to the lenses?

The calculation of the extent of the ring scotoma around positive lenses, as conventionally taught to students of clinical optics, indicates that the scotoma is of a significant size. Using knife-edge lenses this study shows that the calculations are invalid due to the finite size of the eye's entrance pupil and in many instances the scotoma is shown not to exist. The effect noticed in clinical practice is probably largely due to the frame and the edging techniques used which add to the refractive scotoma which is present with high powered lenses, large apertures and/or small pupil sizes.

Morphology of the optic nerve head in glaucomatous eyes with visual field defects in superior or inferior hemifield.

PURPOSE: To evaluate the morphology of optic nerve head (ONH) and border tissue (BT) of Elschnig in glaucomatous eyes with visual field defects in superior or inferior hemifield. METHODS: In a case-control study, we included 25 patients with superior arcuate scotoma, 25 patients with inferior arcuate scotoma, and 25 healthy controls. They received visual field testing, measurement of peripapillary retinal nerve fiber layer (RNFL) thickness, and ONH examination in a radial pattern with spectral-domain optical coherence tomography. In each ONH scan, the length of Bruch membrane opening (BMO) an

Filling in at partially deafferented visual cortex.

METHODS: Perceptual visual filling in at the corresponding area of toxoplasmic retinochoroiditis scotomas in the fellow eye was studied in 14 patients. Targets were programmed to appear inside the scotoma corresponding area in the fellow eye, and at two control points. RESULTS: The results showed faster perceptual filling-in latencies (p<0.0001) in the scotoma corresponding area than at the control points. CONCLUSIONS: Perceptual filling in is facilitated at the corresponding area of retinal scotomas. The neural mechanisms of this visual facilitation might be similar to those found following

The mode of progression of visual field defects in glaucoma.

We retrospectively studied 42 eyes of 42 patients with glaucoma to determine the pattern of progression of their visual field defects. In 33 eyes (79%) the scotomas became denser. Enlargement occurred in 22 eyes (52%) and 21 eyes (50%) developed new scotomas. Increased density of the scotomas was the only manifestation of change in ten eyes (24%), three eyes (7%) showed enlargement only, and six (14%) showed only new scotomas. Seventeen eyes (57%) with single hemifield involvement maintained a defective single hemifield throughout the follow-up period.

Multiplexing Prisms for Field Expansion.

PURPOSE: Prisms used for field expansion are limited by the optical scotoma at a prism apex (apical scotoma). For a patient with two functioning eyes, fitting prisms unilaterally allows the other eye to compensate for the apical scotoma. A monocular patient's field loss cannot be expanded with a conventional or Fresnel prism because of the apical scotoma. A newly invented optical device, the multiplexing prism (MxP), was developed to overcome the apical scotoma limitation in monocular field expansion. METHODS: A Fresnel-prism-like device with alternating prism and flat elements superimposes s

Frequency distribution of early glaucomatous visual field defects.

The optimization of visual field screening programs for glaucoma requires precise knowledge of where glaucoma defects occur and whether any relation exists between the size of defects and the associated retinal disease. The visual field results from 109 eyes of patients with early glaucomatous damage were investigated with a Friedmann Mark II Visual Field Analyser. The frequency distribution of scotomas as a function of location demonstrated that the tested field could be divided into four zones, one where scotomas occur frequently, one where scotomas occur a reasonable number of times, one wh

Comparison of visual field defects in normal-tension glaucoma and high-tension glaucoma.

In a prospective comparison of visual defects in 23 patients with normal-tension glaucoma and 23 with high-tension glaucoma, the groups were matched for equal involvement of the optic disk. F profiles on the Octopus 201 Perimeter were used to quantify thresholds at 1-degree intervals from fixation to define eccentricity, depth, and slope of the scotoma. The mean eccentricity of scotomas in the normal-tension group was 4.86 degrees from fixation; in the high-tension group it was 2.96 degrees. These differences were statistically significant (P less than .01). No statistically significant differ

Visual fields after removal of subretinal hemorrhages and neovascular membranes in age-related macular degeneration.

BACKGROUND: Surgical removal of subretinal hemorrhaging and membranes in eyes with age-related macular degeneration is feasible from a technical point of view, but no advantage over the spontaneous course of the disease has been proven. Visual acuity usually does not improve much. A realistic aim of surgical intervention may be diminution of the central scotoma size, which has not been studied. METHOD: In a prospective study visual fields were measured preoperatively and 6 weeks and 6 months postoperatively in eyes operated on for subretinal hemorrhages and membranes in exudative age-related

Visual field changes after cataract extraction: the AGIS experience.

PURPOSE: To test the hypothesis that cataract extraction in glaucomatous eyes improves overall sensitivity of visual function without affecting the size or depth of glaucomatous scotomas. DESIGN: Experimental study with no control group. METHODS: One hundred fifty-eight eyes (of 140 patients) from the Advanced Glaucoma Intervention Study with at least two reliable visual fields within a year both before and after cataract surgery were included. Average mean deviation (MD), pattern standard deviation (PSD), and corrected pattern standard deviation (CPSD) were compared before and after catarac

Visual field changes in branch retinal 'vein' occlusion.

The visual field defects in 20 consecutive patients who had branch retinal "vein" occlusions included arcuate nerve fiber bundle scotomas, central scotomas, paracentral scotomas, and segmental peripheral constriction. The role of retinal arteriolar insufficiency in the production of these field defects is discussed. The available evidence suggests that the term "branch retinal vascular occlusion" is a preferable term to describe such clinical findings.

Altered Eye Movements During Reading With Simulated Central and Peripheral Visual Field Defects.

PURPOSE: Although foveal vision provides fine spatial information, parafoveal and peripheral vision are also known to be important for efficient reading behaviors. Here we systematically investigate how different types and sizes of visual field defects affect the way visual information is acquired via eye movements during reading. METHODS: Using gaze-contingent displays, simulated scotomas were induced in 24 adults with normal or corrected-to-normal vision during a reading task. The study design included peripheral and central scotomas of varying sizes (aperture or scotoma size of 2°, 4°, 6°,
