Migration of Fish Bones to the Bile Duct Following Hepaticojejunostomy.

OBJECTIVES: Migration of fish bones into the bile duct is rare, but can cause bile duct stone formation or acute cholangitis. Herein, we examine the characteristics of this phenomenon. METHODS: This single-center, retrospective study enrolled patients with a history of fish bone extraction from the bile duct by an endoscopic procedure at our hospital between June 2016 and November 2023. Patient characteristics, treatment details, and clinical course were assessed from the electronic medical records. RESULTS: A total of 11 patients were enrolled, including 10 who underwent subtotal stomach-pr

Detection of residual stones by peroral direct cholangioscopy in patients with intrahepatic stones after hepaticojejunostomy: a prospective study (with video).

BACKGROUND AND AIMS: The difficulty in radiographic confirmation of the presence of stones remains challenging in the treatment of intrahepatic bile duct (IHBD) stones in patients after hepaticojejunostomy (HJ). Peroral direct cholangioscopy (PDCS) enables direct observation of the bile duct and is useful for detecting and removing residual stones; however, its effectiveness is not clearly established in this clinical context. METHODS: This single-center, single-arm, prospective study included 44 patients with IHBD who underwent bowel reconstruction with HJ during the study period. Stone remo

A case of gastrojejunocolic fistula with steatohepatitis.

A man in his 30s, who had undergone retrocolic Billroth II reconstruction for perforated duodenal ulcer, presented with watery diarrhea for 2 years and suspected fatty liver. He was referred to our hospital for management of chronic diarrhea, weight loss, hepatopathy and hypoalbuminemia. Initial upper and lower gastrointestinal endoscopies were negative. Since a small bowel lesion was suspected, peroral single-balloon enteroscopy was performed, which identified feces-like residue near the Billroth II anastomotic site and a connection to the colon separate from the afferent and efferent loops.

[Rapunzel Syndrome Removed with Enteroscopy in a Child].

A trichobezoar is a type of bezoar that is composed of hair. In most cases, it is confined to the stomach, but in rare cases, it may extend to the small intestine. This condition is referred to as Rapunzel syndrome. The therapeutic method for bezoar removal depends on its type, location, and size. Generally, the treatment for Rapunzel syndrome involves surgical laparotomy. Endoscopic removal has also been effective in some cases. On the other hand, complications, such as respiratory difficulty and esophageal impaction may be encountered during endoscopic removal. Until now, the successful endo

Migration of biliary stent resulting in ureteral obstruction: A case report.

This case report describes a rare complication following endoscopic retrograde cholangiopancreatography (ERCP) involving biliary stent migration, leading to ureteric obstruction. Initial management of choledocholithiasis with ERCP and stenting was followed by laparoscopic cholecystectomy. Unexpectedly, a month later, the patient presented with back pain due to the migrated stent compressing the ureter. Intervention with single balloon enteroscopy successfully resolved the issue, highlighting the importance of timely diagnosis and multidisciplinary management.

Complications in Patients with Surgically Altered Gastrointestinal Anatomy Undergoing Endoscopic Retrograde Cholangiopancreatography: 15-Year Experience at a Tertiary Care Center in Latin America.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) is a common procedure, but it poses challenges in patients with surgically altered gastrointestinal anatomy (SAGA). Alternative techniques like single-balloon enteroscopy (SBE), double-balloon enteroscopy (DBE), or push enteroscopy (PE) have been used, albeit with potential complications. Limited Latin American data exists on ERCP complications in SAGA patients. Our goal is to describe complications of ERCP in SAGA at a national referral institution. METHODS: Retrospective, single-center cohort study. All SAGA ERCP procedures p

Patient-reported experience in device-assisted enteroscopy: Conscious sedation vs. general anesthesia.

BACKGROUND AND STUDY AIMS: Device-assisted enteroscopy (DAE) plays a critical role in evaluation and treatment of small bowel disorders, with double-balloon enteroscopy (DBE) being the most widely used technique. Sedation strategies for enteroscopy remain a topic of debate, with conscious sedation (CS) and general anesthesia (GA) both being utilized. Our aim was to assess patient-reported experience measures (PREMs) for DAE, comparing CS and GA to determine their impact on patient satisfaction, procedure success, and pain perception. PATIENTS AND METHODS: PREMs were prospectively collected fr

Current insights into the diagnosis and management of small bowel bleeding.

Small bowel bleeding accounts for 5-10% of gastrointestinal (GI) hemorrhage and remains a diagnostic challenge because of its diverse etiologies and subtle clinical presentation. Advances in small bowel imaging have refined the diagnostic approach and have led to the replacement of the term obscure GI bleeding with suspected small bowel bleeding (SSBB) when findings on upper and lower endoscopy are unrevealing. The initial evaluation requires careful assessment of clinical history, comorbidities, medication use, and physical findings, which may help direct suspicion toward specific etiologies

Familial intestinal polyposis and device assisted enteroscopy: where do we stand?

INTRODUCTION: Hereditary polyposis syndromes are a group of inherited disorders associated with a high risk of developing colorectal cancer. The best known ones are familial adenomatous polyposis (FAP), Peutz-Jeghers (PJS), juvenile polyposis and Cowden syndromes, as well as conditions predisposing to cancer, such as Lynch syndrome. Some of them are characterized by an increased risk of small bowel polyps occurrence. AREAS COVERED: Literature search in PubMed was performed in November 2022 and a narrative review was carried out. Since performing small bowel polypectomy is important in such pa

Colonoscopy-assisted preplacement of guide tube for endoscopic retrograde cholangiopancreatography in patients with complex surgically altered anatomy.

BACKGROUND: Balloon-assisted enteroscopy is limited by a small working channel and few compatible accessories. Herein, we assessed a novel endoscopic retrograde cholangiopancreatography (ERCP) involving a long-type single-balloon enteroscope (200 cm length; 2.8 mm channel), requiring dual-operator manipulation. Although standard in our centre, this technique's clinical limitations compared to single-operator long-colonoscopy ERCP remain unclear. METHODS: This retrospective study enrolled patients with complex surgically altered anatomy who underwent ERCP from 2021 to 2024, divided into the si

Pancreatic Heterotopia in the Small Bowel: A Case Report and Literature Review.

Diagnosing pancreatic heterotopia (PH) is challenging, because it is a rare congenital anomaly. We report the case of a 66-year-old man who presented with ileus and jejunal PH diagnosed using single-balloon enteroscopy. Reviewing 178 cases of small intestinal PH in the literature showed that (1) PH mostly occurred at the duodenum (61.8%), followed by the jejunum (22.5%) and ileum (14.6%); (2) patients with ileal PH were younger (mean, 40.7 vs 54.6 years; P < 0.001), while those with jejunal PH were older (mean, 59.5 vs 51.7 years; P = 0.006); (3) most patients with jejunal PH were females,

[Multiple gastrointestinal stromal tumors during nilotinib treatment for chronic myelogenous leukemia in a patient with neurofibromatosis type 1].

A 60-year-old woman with neurofibromatosis type 1 presented to our hospital with melena. She reported a 1-year history of treatment with nilotinib hydrochloride hydrate for chronic myelogenous leukemia. Contrast-enhanced abdominal computed tomography revealed multiple intestinal tumors that were subsequently diagnosed as gastrointestinal stromal tumors (GIST) using single-balloon enteroscopy. Although the tumors showed no significant change over 1 year, partial jejunal resection was performed to confirm the diagnosis. Immunohistochemically, the tumors were GIST.

Embedded Ileal Fish Bone Removed via Deep Enteroscopy in a Patient with Abdominal Pain and Hematochezia: A Case Report.

Ingestion of foreign bodies is a prevalent issue in clinical practice, with fish bones being the predominant cause. While the upper gastrointestinal tract is commonly affected, small intestine impactions pose significant diagnostic challenges due to nonspecific symptoms and lack of awareness of foreign body ingestion. Herein, we describe a case presenting with recurrent, unexplained abdominal pain and hematochezia. Multiple diagnostic investigations, including esophagogastroduodenoscopy and colonoscopy, conducted over several months failed to identify the underlying cause until a retrograde si

Removal of transanastomotic pancreatic stent tubes after pancreaticoduodenectomy: a new role for double-balloon enteroscopy.

BACKGROUND: Endoscopic interventions are usually very challenging in patients with anatomic changes caused by earlier GI surgery. The recent development of the double-balloon enteroscopy (DBE) system creates an opportunity to reach the biliary tract and the pancreatic duct in postsurgical conditions. OBJECTIVE: To report a further application of DBE in surgical patients; namely, extraction of pancreatic stents placed in pancreaticojejunal anastomosis during pancreaticoduodenectomy. DESIGN: Two case reports. SETTING: A single-center experience in a tertiary-care unit. PATIENTS AND INTERVENT

Haemorrhagic ileal haemolymphangioma: a case report and review of the literature.

We describe herein a 37-year-old woman with a 2-week history of melena who was eventually diagnosed with ileal haemolymphangioma, a rare benign tumour. Local mucosal congestion and swelling were found through single-balloon enteroscopy, which showed an irregular protuberance approximately 10 cm long, located 3.2 m from the Treitz ligament. We performed a laparoscopic-assisted partial resection of the small intestine combined with intestinal adhesiolysis. According to postoperative pathology, the final diagnosis was ileal haemolymphangioma with haemorrhage.

Endoscopic ultrasound-guided internalization of a pancreaticocutaneous fistula utilizing a balloon-target technique: A case report.

RATIONALE: Endoscopic ultrasound (EUS)-guided treatment has been recently described for internalizing refractory pancreaticocutaneous fistulas (PCFs). However, the existing techniques are limited because of the difficulty in accessing nondilated pancreatic ducts or fistulas. In an attempt to overcome this limitation, we present a case where a EUS-guided intervention utilizing a balloon-target technique was employed to internalize a PCF into the stomach. PATIENT CONCERNS: A 78-year-old woman underwent percutaneous drainage and 4 percutaneous endoscopic necrosectomies for walled-off pancreatic

Biliary drainage in patients with altered anatomy: Literature review of different endoscopic approaches.

Endoscopic retrograde cholangiopancreatography (ERCP) in patients with surgically altered anatomy remains a challenging field in therapeutic endoscopy due to the complex anatomical reconstructions that limit access to the biliary tree. Over the past two decades, device-assisted enteroscopy (DAE), including single-balloon, double-balloon, and motorized spiral enteroscopy, has expanded the feasibility of ERCP in this population, with overall technical success rates generally reported between 70% and 90%. Nevertheless, these techniques are technically demanding, time-consuming, and frequently aff
