The role of cognitive factors in differentiating individuals with somatoform disorders with and without depression.

The present study, based on a cross-sectional design, was conducted in the primary care setting with patients presenting somatic symptoms suggestive of somatoform disorders (SFD). The main aim of the study was to determine whether cognitive variables could differentiate between patients with SFD alone and those who also present depressive symptoms suggestive of depression (SFD + depression). All participants (N = 796) completed validated self-report measures to assess somatic, depressive, and/or anxiety symptoms, as well as functional impairment, quality of life (QoL), and cognitive variables

Harm avoidance and persistence are associated with somatoform disorder psychopathology: A study in Taiwan.

BACKGROUND: Whether personality features affect the development of somatoform disorders and their psychopathologies is an important issue. Aim of this study was to resolve this issue by comparing indicators of psychopathology and personality features in subjects with somatoform disorders and healthy controls. METHODS: This study recruited 148 subjects with somatoform disorders and 146 healthy controls. The severity of psychopathology was measured with the Patient Health Questionnaire-15 (PHQ-15), Health Anxiety Questionnaire (HAQ), Beck Depression Inventory-II (BDI-II), and Beck Anxiety Inven

Relationship between neural activity and immunity in patients with undifferentiated somatoform disorder.

It has been suggested that somatoform disorders are related to both the brain and the immune system, and that immune functions may be influenced by cerebral asymmetry. However, few studies have examined the relationship between brain activity and immune function in somatoform disorders. Thirty-two patients with non-medicated undifferentiated somatoform disorder were enrolled in this study. Blastogenic responses to phytohemagglutinin (PHA) were used to measure immunity. Regional cerebral perfusion was measured by 99m-Tc-ethyl cysteinate dimer single photon emission computed tomography (SPECT).

AIM: The case study of a young woman affected by moderate mental retardation and somatoform disorder not otherwise specified, with globus pharyngeus, is reported. The purpose is to show how an articulated diagnosis and treatment of individuals with mental retardation and comorbid psychopathology may produce syndromal and functional recovery. METHODS: Psychometric testing of the patient was performed, in order to monitor the prescribed, duloxetine-based, treatment. RESULTS: After six weeks of treatment the patient underwent full remission. DISCUSSION: The correct identification of psychiatri

[From "functional syndrome to "somatoform disorder". The new ICD-10 classification].

Somatization disorders are particularly common in internal and general medicine. They affect in particular the gastrointestinal tract (irritable colon, non-ulcerous dyspepsia), the cardiovascular system (cardiac phobia), the respiratory system (hyperventilation syndrome) and include chronic pain syndromes. The earlier term "functional syndrome" has been discarded in favor of the category "somatization disorders". The new ICD10 classification created the basis for this internationally accepted revision. The classification into subtypes and the diagnostic criteria for somatization disorders in a

The epidemiology and treatment of depression when it coexists with somatoform disorders, somatization, or pain.

This article reviews the relationship between depressive disorders and somatoform disorders, somatization, and pain. These disorders and symptoms are clinically interrelated, yet the nature of the interrelation is not well understood. This review of the literature from 1975 through mid-year 1990 addresses the epidemiology and treatment of these conditions and/or symptoms when they occur together. When robust criteria are used to determine which publications are included, only 14 are available that address depressive disorders, somatoform disorders, and somatization. Similarly, there are only 1

Non-pharmacological interventions for somatoform disorders and medically unexplained physical symptoms (MUPS) in adults.

BACKGROUND: Medically unexplained physical symptoms (MUPS) are physical symptoms for which no adequate medical explanation can be found after proper examination. The presence of MUPS is the key feature of conditions known as 'somatoform disorders'. Various psychological and physical therapies have been developed to treat somatoform disorders and MUPS. Although there are several reviews on non-pharmacological interventions for somatoform disorders and MUPS, a complete overview of the whole spectrum is missing. OBJECTIVES: To assess the effects of non-pharmacological interventions for somatofor

Prevalence of depression, anxiety, adjustment disorders, and somatoform disorders in patients with age-related macular degeneration in Germany.

Aims: The purpose of this study was to analyze the prevalence of depression, anxiety, adjustment disorders, and somatoform disorders in patients diagnosed with age-related macular degeneration (AMD) in Germany. Methods: This study included 7,580 patients between the ages of 40 and 90 diagnosed with AMD between January 2011 and December 2014 in 1,072 primary care practices (index date). The last follow-up was in July 2016. We also included 7,580 controls without AMD, which were matched (1:1) to the AMD cases by age, sex, type of health insurance (private or statutory), physician, and Charlson c

Opioid Therapy Discontinuation in General Practice: Associations with Adjunct Therapies and Somatoform Pain Disorders.

BACKGROUND: Chronic non-cancer pain is a common reason for consultation in primary care. Although opioid therapy may be considered a treatment option for selected patients, guideline-recommended discontinuation can be challenging for both patients and general practitioners. This study investigated predictors of opioid discontinuation in primary care. METHODS: We analyzed German claims data (2021) from patients aged ≥ 18 years with at least one oral or transdermal opioid prescription from general practitioners in the first three quarters of 2021. Exclusion criteria were malignant diseases or p

Characteristics of oligosymptomatic versus polysymptomatic presentations of somatoform disorders in patients with suspected allergies.

OBJECTIVE: Psychobehavioral characteristics of patients with somatoform disorders (SFDs), are increasingly discussed as possible positive criteria for this diagnostic group. However, little is known about psychobehavioral differences, or similarities, between the different SFD presentations, i.e., polysymptomatic [multisomatoform/somatization disorders (MSD)] versus mono- or oligosymptomatic courses [pain disorder (PD), undifferentiated somatoform disorder (USD)]. METHODS: This is a cross-sectional study including 268 consecutive allergology inpatients. After an Structured Clinical Interview

[Clinical experience of acupuncture and moxibustion in the diagnosis and treatment of persistent somatoform pain disorder].

The clinical manifestation and clinical diagnosis and treatment of acupuncture for persistent somatoform pain disorder (PSPD) were observed to explore its etiology. It is found that the causative factor of PSPD is probably relevant with pain memory induced by the negative emotions during the first attack of pain, which is in the category of "depression" in TCM. In clinical diagnosis, the medical history should be explored to discover the potential etiology and determine the diagnosis. In treatment, the root cause should be considered, focusing on mind and qi regulation and associating with ana

Somatoform respiratory disorders in children and adolescents-proposals for a practical approach to definition and classification.

Somatoform respiratory disorders represent conditions with dysfunctional breathing unexplained by structural abnormalities. This heterogeneous group includes disorders with neural dysregulation of respiration (vocal cord dysfunction) or with dysregulation of the respiratory pattern (hyperventilation, sighing dyspnea), psychogenic disorders such as unjustified anxiety of suffocation, and stereotype conditions such as throat clearing or habit cough. Many symptoms are nonspecific and largely overlap with respiratory disease symptoms of somatic etiology. Most patients will present in a nonspeciali

[Somatoform disorders with pain as the predominant symptom: results to distinguish a common group of diseases].

According to a population-based prevalence study, medically unexplained pain syndromes are highly prevalent in the German general population. With a 1-year prevalence of 8% for somatoform pain disorders and a lifetime prevalence of 12.7%, they rank among the most prevalent conditions in the community. Until now, few studies have been conducted to characterize and differentiate patients with somatoform pain disorders in more detail. The present study is the first to examine a large patient cohort from a university hospital outpatient unit with somatoform disorders presenting with pain as the pr

An investigation of temperamental traits in patients with somatoform disorder: do they belong in the affective spectrum?

BACKGROUND: About 10% of the general population report multiple and persisting physical symptoms resulting in extensive screening but with no organic explanation found. OBJECTIVE: The authors sought to determine whether these somatoform-disorder patients show characteristics of the affective disorder spectrum, with the cyclothymic temperament likely showing the highest specificity for somatoform disorder. METHOD: This study examined temperamental traits and current mood states of 44 general-hospital inpatients diagnosed with somatoform disorder. RESULTS: There was a higher prevalence of abn

Subjective symptoms and breathing pattern at rest and following hyperventilation in anxiety and somatoform disorders.

The purpose of the present study was to investigate the diagnostic specificity of bodily symptoms and respiratory behavior at rest and after a hyperventilation provocation test (HVPT) in patients that were either grouped according to the DSM classification or diagnosed as suffering from hyperventilation syndrome. Nine hundred three anxiety and somatoform patients, showing symptoms supposedly caused by psychogenic hyperventilation, and 170 healthy subjects, were studied. Breathing pattern and end-tidal CO2 concentration were recorded during breathing at rest and following a HVPT. Subjective sym

Somatoform pain disorder in a case of Klinefelter's syndrome with multiply operated lower back.

We report a case of Klinefelter's syndrome with multiply operated low back (MOB). Psychological and/or psychosocial problems related to MOB have been of recent interest in the field of orthopedic surgery. Based on psychiatric interviews, this case was diagnosed as a somatoform pain disorder of the DSM-III-R somatoform disorders. In addition to psychological problems, the pain was partly explicable by severe osteoporosis, which was prematurely caused by endocrinological disturbances associated with Klinefelter's syndrome. Patients with this syndrome are more likely to develop severe osteoporosi

Affect dysregulation and dissociation in borderline personality disorder and somatoform disorder: differentiating inhibitory and excitatory experiencing states.

Affect dysregulation and dissociation may be associated with borderline personality disorder (BPD) and somatoform disorder (SoD). In this study, both under-regulation and over-regulation of affect and positive and negative somatoform and psychoform dissociative experiences were assessed. BPD and SoD diagnoses were confirmed or ruled out in 472 psychiatric inpatients using clinical interviews and clinical multidisciplinary consensus. Affect dysregulation and dissociation were measured using self-reports. Under-regulation (but not over-regulation) of affect was moderately related to positive and
