Non-spinal-associated injuries with lumbar transverse process fractures: influence of segments, amount, and concomitant vertebral fractures.

BACKGROUND: Lumbar transverse process fractures (LTPFs) are often with concomitant non-spinal-associated injuries (NSAIs). The purpose of this study was to analyze the correlation between the NSAIs and the LTPFs of different segments, amount, and concomitant thoracolumbar/lumbar vertebral fractures. METHODS: A total of 1,181 patients with LTPFs or thoracolumbar/lumbar vertebral fractures were chosen between September 2006 and June 2010. The patients were divided into three groups: thoracolumbar/lumbar vertebral fractures without LTPFs, with associated LTPFs, and isolated LTPFs (iLTPFs). The c

Spinal cord injury after traumatic spine fracture in patients with ankylosing spinal disorders.

OBJECTIVE Spinal cord injury (SCI) is a major complication of spinal fractures in patients with ankylosing spondylitis (AS) and diffuse idiopathic skeletal hyperostosis (DISH). Due to the uncommon nature of these conditions, existing literature consists of relatively small case series without detailed neurological data. This study aims to investigate the incidence, predictors, and sequelae of SCI in patients with a traumatic fracture of the ankylosed spine. METHODS The study included all patients older than 18 years of age with AS or DISH who presented to two affiliated tertiary care centers b

Characteristics of noncontinuous multilevel spinal fractures: an age-group specific analysis.

OBJECTIVE: Noncontinuous multilevel fractures (NMF) are complex injuries characterized by spinal fractures at non-adjacent levels, with reported incidence ranging from 1.6 to 17.7% depending on imaging modalities. Typically resulting from high-energy trauma such as motor vehicle accidents, NMF can also occur due to low-energy mechanisms, especially among elderly patients. We aimed to investigate clinical characteristics and fracture morphology of NMF across age groups. METHODS: We retrospectively reviewed trauma patients treated for NMF at a tertiary center from 2014 to 2021. Demographics, di

Spinal fractures in fused spines: nonoperative treatment is a reliable alternative.

INTRODUCTION: Spinal fractures in fused spines such as in ankylosing spondylitis or DISH are typically of type B or C fractures where operative treatment is recommended. The mortality rate in non-operatively treated patients is reported to be 51%. The purpose of this study was to investigate the mortality rate, complication rate and demographics of patients following non-operatively treatment in fused spine injuries. METHODS: Between 2019 and 2021, a retrospective study was conducted including all patients who presented to our trauma center with a spinal fracture of a fused spine. Radiology a

Epidemiology of traumatic cervical spinal fractures in a general Norwegian population.

BACKGROUND: In Western countries, the typical cervical spine fracture (CS-Fx) patient has historically been a young male injured in a road traffic accident. Recent reports and daily clinical practice clearly indicate a change in the typical patient from a young male to an elderly male or female with comorbidities. This study aimed to establish contemporary population-based epidemiological data of traumatic CS-Fx for use in health-care planning and injury prevention. METHODS: This is a population-based retrospective database study (with prospectively collected data) from the Southeast Norway h

Timing of surgical fixation in traumatic spinal fractures: A systematic review.

BACKGROUND: The optimal timing of fracture fixation following spinal injury is controversial. Many spinal fractures occur as part of polytrauma requiring a complex management strategy. Whilst the decision to stabilize unstable spinal column injuries is without debate, the duration between injury and definitive fixation can impact on the incidence of post-operative complications. This study was designed to systemically summarize and compare the complication profile of early vs late stabilization of spinal injuries, in an attempt to unveil an appropriate treatment protocol for traumatic spinal f

Spinal Fractures in Patients With Ankylosing Spondylitis: Etiology, Diagnosis, and Management.

In patients with ankylosing spondylitis, the altered biomechanics of the spine increase the incidence of spinal fractures and the risk of subsequent displacement and neurologic injury. Radiographic evaluation is difficult and requires imaging of the entire spine and the use of advanced imaging. Nonsurgical treatment has a high rate of complications and is typically pursued only when surgical intervention would carry unacceptably high perioperative risks. The surgical approach depends on the fracture location and any deformity present. Anterior fixation alone has been associated with high failu

Morbidity following lower extremity fractures in men with spinal cord injury.

UNLABELLED: The Veterans Affairs Spinal Cord Dysfunction Registry from 2002 to 2007 was reviewed to determine whether men with spinal cord injury (SCI) and lower extremity fractures had an increased risk of complications compared to those without fractures. We determined that fractures are associated with significant consequences, particularly during the first month postfracture. INTRODUCTION: Despite increasing longevity, patients with SCI have a substantial number of illnesses and comorbid conditions. Lower extremity fractures are frequent events in these patients. However, whether these fr

[Trends in the treatment of spinal osteoporic fractures: vertebroplasty and kyphoplasty].

Vertebral osteoporotic fracture (VOF) is a major problem of public health. Surgical treatments such as vertebroplasty and kyphoplasty are interesting adjuvant treatments for the management of osteoporosis. A consensus proposed by the principal contributors of this management is important. Regarding the actual data, we propose a vertebroplasty or a kyphoplasty for all patients suffering of an acute VOF. If a previous kyphosis or an important local kyphosis exists, secondary to the acute VOF or others, we propose a kyphoplasty. If the VOF is older and the conservative treatment is inefficient, w

[Surgical treatment for osteoporotic spinal fractures in the thoraco-lumbar spine].

Most of osteoporotic spinal fractures can be successfully treated with conservative treatment. In some patients, however, conservative treatment is not effective and progressive vertebral collapse with neurological problems at thoraco-lumbar spine may occur. In this chapter, surgical indication and timing in patients with osteoporotic vertebral collapse are described. Surgical options including vertebroplasty, kyphoplasty, anterior reconstruction, posterior reconstruction, and combined anterior and posterior surgery are also described. The readers can understand both advantages and disadvantag

Systemic mastocytosis presenting with severe spinal osteopenia and multiple compression fractures.

Systemic mastocytosis is a rare condition in which mast cells infiltrate various organs, including the skeleton. Because the mast cell secretes various bioactive substances that may induce bone resorption, this condition may cause generalized osteoporosis. We describe a case of a 28-year-old woman who presented with a painful thoracolumbar kyphosis due to generalized osteopenia and multiple pathological compression fractures and was found to have mastocytosis. She underwent operative stabilization of her kyphotic deformity with anterior interbody fusion and posterior Cotrel-Dubousset (CD) inst

Treatment of femoral shaft fractures associated with acute spinal cord injuries.

Twenty-seven femoral shaft fractures in 23 patients with acute spinal cord injuries were reviewed for evaluation of the outcome of operative versus nonoperative treatment. Three groups were identified: 11 nonoperative, eight early operative, and eight delayed operative. Patients treated initially by nonoperative methods developed five impending nonunions (31%), which subsequently were treated by open reduction and internal fixation. One femur in each of the operative groups developed a refracture after early removal of metal fixation devices. In the delayed operative group, four patients (50%)

Surgical Treatment of Pathologic Spinal Fractures in Osteoporosis: Two Clinical Cases.

Osteoporotic spine fractures are among the most prevalent fractures in patients with osteoporosis and pose significant challenges to surgical treatment. Achieving stable fixation of osteoporotic bone is technically challenging and has become an increasing concern in spinal neurosurgery in the aging global population. Numerous studies have described the biomechanical benefits of fixation devices and techniques; however, no standard of care has been identified to provide distinct spinal stabilization in these patients. This study reviews the existing evidence and explores the complexities relate
