Intraoperative electrically elicited stapedius reflex threshold is related to the dosage of hypnotic drugs in general anesthesia.

Today cochlear implantation is a widely used means of treatment in deafness and severe hearing disorders in adults, children, and infants. Postoperative fitting of the externally worn speech processor is very important for successful use of the cochlear implant. However, especially in infants and young children, this fitting process can be difficult because of limited communication capabilities. The use of intraoperatively obtained stapedius reflex thresholds has been proposed for postoperative speech processor fitting, but the influence of anesthetics on threshold values needs to be taken int

Stapedius reflex evoked in free sound field in cochlear implant users compared to normal-hearing listeners.

OBJECTIVE: To determine the relation between stapedius reflex thresholds in cochlear implant users evoked once through direct electric stimulation on single channels (ESRT) and once through acoustic stimulation in free sound field. For comparison, stapedius reflex thresholds were obtained in free sound field in a normal-hearing control group. DESIGN: For each participant a new ESRT-based fitting was created. Stapedius reflex thresholds were obtained for this new fitting in free sound field for different loudness adjustments. Acoustic stimuli for eliciting the stapedius reflex were narrow band

Absent stapedius muscle and its tendon concomitant with otosclerosis: a case report on embryology, imaging, and treatment.

This study aimed to discuss the clinical implications associated with the absence of the stapedius muscle and concomitant otosclerosis. The causes of mixed hearing loss are presented based on its embryonic development, audiological tests, and high-resolution computed tomography (CT) imaging. This case describes a 35-year-old female patient diagnosed due to bilateral conductive hearing loss. The patient complained of a significant deterioration of hearing and speech understanding lasting for several months. Preoperative audiometric work-up revealed air-bone gap with the presence of Carhart's no

Electrically Evoked Stapedius Reflex Measurements in Cochlear Implantation and its Application in the Postoperative Fitting Process.

Measuring the electrically evoked stapedius reflex during the fitting of cochlear implants (CIs) provides a reliable estimation of maximum comfort levels, resulting in the programming of the CI with high hearing comfort and good speech understanding. Detection of the stapedius reflex and the required stimulation level on each implant channel is already being performed during surgery, whereby intraoperative stapedius reflexes are observed through the surgical microscope. Intraoperative stapedius reflex detection is both an indicator that the auditory nerve is responding to electrical stimulatio

Contraction of the stapedius and tensor tympani muscles explored by tympanometry and pressure measurement in the external auditory canal.

It has been suggested that tensor tympani muscle (TTM) contraction may be involved in the development of ear-related pathologies such as tinnitus, hyperacusis and otalgia, called the tonic tensor tympani syndrome (TTTS). However, as there is no precise measure of TTM function under normal and pathological states, its involvement remains speculative. When the TTM or the stapedius muscle (SM) contracts, they both generate an increase of middle ear stiffness that can be measured through middle ear admittance. However, this technique cannot differentiate the contraction between the two muscles. On

[Effect of the stapedius and tensor tympani muscles tenotomy on the quality of life of patients suffering from Ménière's disease].

Introduction: Tenotomy of the tendon of the stapedius and tensor tympani (TT) muscles is a minimal-invasive surgical therapeutic procedure in Ménière's disease (MD). It has been assumed that the TT medializes the stapes into the oval window, resulting in changes in perilymphatic pressures of the inner ear. By cutting the tendons of both middle ear muscles, they affect the pressure dynamics by not augmenting this pressure even further. Aim: The immediate and long-term investigation of the effect of middle ear muscle tenotomy on the quality of life of patients suffering from Ménière's disease, m

Effect of Probe-Tone Frequency on Ipsilateral and Contralateral Electrical Stapedius Reflex Measurement in Children With Cochlear Implants.

OBJECTIVES: The upper loudness limit of electrical stimulation in cochlear implant patients is sometimes set using electrically elicited stapedius reflex thresholds (eSRTs), especially in children for whom reporting skills may be limited. In unilateral cochlear implant patients, eSRT levels are measured typically in the contralateral unimplanted ear because the ability to measure eSRTs in the implanted ear is likely to be limited due to the cochlear implant surgery and consequential changes in middle ear dynamics. This practice is particularly limiting in the case of fitting bilaterally implan

Unique fiber phenotype composition and metabolic properties of the stapedius and tensor tympani muscles in the human middle ear.

The middle ear muscles have vital roles, yet their precise function in hearing and protection remains unclear. To better understand the function of these muscles in humans, the morphology, fiber composition, and metabolic properties of nine tensor tympani and eight stapedius muscles were analyzed with immunohistochemical, enzyme-histochemical, biochemical, and morphometric techniques. Human orofacial, jaw, extraocular, and limb muscles were used as references. The immunohistochemical analysis showed that the stapedius and tensor tympani muscles were markedly dominated by fibers expressing fast

Stapedius reflex thresholds obtained in a free sound field as an indicator for over- and understimulation in cochlear implant listeners.

OBJECTIVE: The goal of this study was to determine open field stapedius reflex thresholds (oSRTs) in CI patients with fittings based on subjective loudness ratings. A further objective was to compare these oSRTs and those of eSRT-based fittings that are similar to the oSRTs of normal hearing. DESIGN: Impedance measurements of the ear drum were taken while subjects were wearing their audio processors. The stapedius reflex was elicited by electrical stimulation transmitted through the activated CI system in response to an acoustic stimulus presented in the free sound field. STUDY SAMPLE: Subje

[The threshold levels of the stapedius reflex depending on the relationship between the gas pressure in the external auditory meatus and the middle ear].

The objective of the present work was to detect and elucidate effects of a single factor (gas pressure) responsible for the threshold levels of stapedius reflex. It was shown that the threshold reflexes were recorded at 105 dB SPL when the gas pressure in the external auditory meatus decreased by 150-200 pPa against the atmospheric pressure and when it increased by 100-150 pPa. These effects can be accounted for by the reduced compliance of the tympanic membrane and different changes in the state of the auditory ossicles undergoing compression and tension. When entering the most comfortable st

Extratympanic manometry in man. Clinical and experimental investigations of the acoustic stapedius and tensor tympani contractions in normal subjects and in patients.

New ideas and inspirations for future human investigations have emerged from the experience obtained in this study. These investigations ought to be performed for both scientific and clinical reasons. It is the author's opinion that the ETM method is extremely useful and valuable. The author intends to continue the human investigations if proper conditions are available. The ETM equipment can also be further improved for scientific and special clinical investigations. For normal clinical practice a more "simplified" ETM equipment should be developed. Clinical investigations concerning the diag

Dose-dependent suppression of the electrically elicited stapedius reflex by general anesthetics in children undergoing cochlear implant surgery.

BACKGROUND: Cochlear implants stimulate the auditory nerve to enable hearing. Determining appropriate upper and lower limits of stimulation is essential for successful cochlear implantation. The intraoperative evoked stapedius reflex threshold (ESRT) and evoked compound action potential (ECAP) are commonly used to determine the limits of implant stimulation. In this study, we evaluated the dose-related effects of sevoflurane, desflurane, isoflurane, and propofol on the intraoperative ESRT and ECAP. METHODS: Forty-four children aged 6 mo to 17 yr undergoing cochlear implantation were recruited
