[New techniques for surgical treatment of postoperative male stress incontinence].

The impact of minimally invasive techniques for the treatment of postoperative male incontinence has significantly improved in recent years. These techniques are based on the continuous increase in urethral resistance. This resistance can be readjusted with balloons placed paraurethrally or with readjustable suburethral slings. The success rates depend on the preoperative degree of incontinence. With bulking agents that are transurethrally injected into the submucosa of the sphincter, continence rates between 12 and 90% can be seen. Severe complications are rare. The impact of the studies is o

Anatomic relationships of single-incision midurethral slings.

OBJECTIVE: The objective of the study was to evaluate the anatomic relationships of anchor points of single-incision midurethral slings with 2 common placement trajectories. STUDY DESIGN: In 30 female pelvic halves, a probe was introduced through a suburethral tunnel following 45° and 90° angle trajectories. The corresponding anchor points were tagged. Distances to the obturator canal, accessory obturator vessels, dorsal vein of clitoris, and external iliac vein were recorded. RESULTS: Both suburethral tunnel trajectories and their respective anchor points remained caudad to the obturator in

A microbiological and immunohistochemical analysis of periurethral and vaginal tissue in women with de novo urge symptoms after mid-urethral sling procedures--a prospective case-controlled study.

The aims were to determine whether bacteria-infection-elicited immune response after mid-urethral sling (MUS) may cause de novo urge symptoms (DNUS) and to evaluate the efficacy of suburethral sling excision for relieving the urgency. In a period of 40 months, 360 consecutive subjects with urodynamic stress incontinence had undergone one of three different MUS. Sixty-eight women with DNUS were included after exclusion. The suburethral sling of 24 of the 68 women needed to be excised because of refractory urgency. The excised vagina, or periurethral tissue, included sling of the study and contr

Correlations between Sonographic and Urodynamic Findings after Mid Urethral Sling Surgery.

PURPOSE: We studied correlations between sonographic and urodynamic findings after suburethral sling surgery. MATERIALS AND METHODS: This retrospective study was done in 141 women who underwent multichannel urodynamic testing and translabial 4-dimensional ultrasound after suburethral sling surgery between 2006 and 2016. We determined the sling-pubis gap, ie the distance between the sling and the inferior posterior margin of the pubic symphysis, during the Valsalva maneuver and the urethral motion profile. The latter was assessed by calculating the mobility vectors of 6 equidistant points alon

[Reconstructive treatment of female urethral estenosis secondary to erosion by suburethral tape].

INTRODUCTION: Female urethral stricture secondary to erosión by suburethral sling is an unfrequent problem of difficult solution. Ventral vaginal rotation flaps or buccal mucosa dorsal grafts are not useful because this type of stricture is very proximal (close to the bladder neck) and the vagina is thinned. We present our experience to manage this problem using excision of disease urethral tract, associated to bladder mucosa flap and vaginal sling using transverse vaginal flap to repair the weakened vaginal wall. MATERIAL AND METHODS: Three females with urethral stricture secondary to urethr

Development of vaginal erosion and vesicocutaneous fistula following midurethral transvaginal tape with PelviLace for treatment of stress urinary incontinence.

Stress urinary incontinence (SUI) is a common diagnosis that greatly impacts quality of life for many women. Current therapies implement the use of suburethral slings, either synthetic or xenografts, to remedy this condition. In this study, we report on a patient with SUI, treated with PelviLace Biourethral Support, who suffered multiple issues postoperatively.

The role of male slings in post prostatectomy incontinence: ICI-RS 2015.

AIMS: Post prostatectomy stress urinary incontinence can significantly impact on quality of life and is bothersome for patients. The artificial urinary sphincter is still considered the gold standard surgical treatment. Male slings are an alternative surgical option and several designs are currently available. This review will focus on efficacy, adverse events, and mechanism of action of male slings. The context of this review was a proposal at the ICI-RS meeting 2015. Following synthesis of the evidence we address the question: does it matter how male slings work? METHODS: The proposal lectu

Modified pubovaginal sling and Martius graft for repair of the recurrent vesicovaginal fistula involving the internal urinary sphincter.

A modified Martius graft interposition flap was used in conjunction with the modified pubovaginal sling bladder suspension to treat a recurrent vesicovaginal fistula involving the proximal urethra and bladder neck. The patient recovered with no incontinence, voiding dysfunction or dyspareunia. Incontinence should be anticipated with vesicovaginal fistulas involving the internal sphincter and repair should be tailored to include an organic suburethral sling.

A critical overview of the evidence base for the contemporary surgical management of stress incontinence.

PURPOSE OF REVIEW: The purpose of this article is to evaluate the evidence base for surgical management of stress urinary incontinence in women between July 2006 and December 2007. RECENT FINDINGS: The Urinary Incontinence Treatment Network and Pelvic Floor Diseases Network have contributed level 1 evidence in the management of stress urinary incontinence with multicentric randomized controlled trials. Pubovaginal sling has a higher success rate than the Burch at the expense of a higher morbidity. A prophylactic Burch procedure at the time of an abdominal sacrocolpopexy can reduce secondary s

Two-/three-dimensional transperineal sonography of complicated tape and mesh implants.

Transperineal sonography (TPS) affords dynamic evaluation of suburethral slings using tensionless vaginal tape and mesh implants used to treat complications associated with surgical procedures of the pelvic floor. This review describes and illustrates the TPS findings in postoperative complications such as recurrent prolapse, cystocele, enterocele, and rectocele. The potential use of live 3-dimensional TPS to delineate the dynamics aspects of pelvic floor disorder is also mentioned.
