Bilateral non-osteochondroma-related proximal tibiofibular synostosis.

We report the case of a 67-year-old male with bilateral proximal tibiofibular synostosis, presenting with unilateral symptoms. The patient complained of pain around the left fibular head, which was attributed to incomplete bone bridging between the proximal tibia and fibula; he underwent proximal fibular head resection, which alleviated the pain and improved knee mobility. Eleven months later, the patient continued to be pain-free and did not experience any adverse effects. An examination of this case and a review of similar cases revealed that participation in sport activities such as long-di

Magnetic resonance imaging in isolated sagittal synostosis.

Isolated fusion of the sagittal suture is the most prevalent form of craniosynostosis. Although the typical clinical appearance usually points the way to the right diagnosis, computed tomographic (CT) scans are still recommended as necessary tools for both the diagnosis of scaphocephaly and the preoperative planning. Because CT scans are accompanied by the biological effects of ionizing radiation, some authors have already postulated the use of magnetic resonance imaging (MRI) especially because MRI seems to be valuable for detecting intracranial anomalies compared with CT scans. Hence, we inv

Spring-assisted correction of sagittal suture synostosis.

PURPOSE: The purpose of this study is to describe the technique and advantages and limitations of spring-assisted cranioplasty for sagittal suture synostosis. METHODS: Preliminary data are presented of the first 41 patients treated with this technique at our institution. RESULTS: The cephalic index was 75 after surgery and dropped to 74 one year after surgery. Mean blood loss of both procedures combined was 54 ml. CONCLUSION: Spring-assisted cranioplasty requires only two small incisions and is at least as effective as other techniques with respect to the cephalic index. Blood loss, operati

Radiographic assessment of congenital C2-3 synostosis.

PURPOSE: To evaluate the morphologies of congenital C2-3 synostosis in 25 patients. METHODS: Radiographs of 11 males and 14 females aged 5 to 74 years with congenital C2-3 synostosis were reviewed. All cases were found incidentally on radiographs when presenting with neck/shoulder discomfort/pain. RESULTS: 13 of the patients had spondylosis in 21 segments: C1-2 (n=1) and C3-4 (n=1), C4-5 (n=7), C5-6 (n=9), and C6-7 (n=3). Of whom 12 had normal sagittal alignment and one had kyphotic synostosis (who developed compensatory hyperlordosis of the caudal mobile segments and subsequent spondylosis

Autosomal dominant and sporadic radio-ulnar synostosis.

We report on seven cases of congenital radio-ulnar synostosis (RUS). Five were found in the same family and two were sporadic. In six the synostosis was bilateral and consistently involved the proximal end of the radius and ulna. In the familial cases the anomaly was inherited as an autosomal dominant trait and was associated with a Dubois sign and relative shortness of metacarpals number 4 and 5 in two patients, and of number 2 in another patient, and of all phalanges of the 5th fingers. These observations suggest involvement of an ulnar developmental field. RUS does not seem to be rare in th

Long-term outcome after sagittal synostosis operations.

We evaluated 27 children who had been operated on 5-10 years previously for sagittal synostosis. The mean age at operation was 0.55 years. Twenty-two children were treated by the senior author's technique, 4 by the Pi craniectomy technique and 1 by parasagittal sagittal strip craniectomies. The mean age at follow-up was 9.6 years. The cosmetic appearance of the head was graded independently by 3 examiners. Skull radiographs were graded by 2 examiners according to the appearance and presence of beaten copper markings. The cephalic index was measured. Frequent headaches were reported in 7 of 27

[Pancraniofacial synostosis--indications for early craniofacial operation].

Premature synostosis of all major cranial sutures represents a rare craniofacial anomaly often leading to serious consequences. It is regularly associated with Kleeblattschädel-syndrome (clover-leaf skull) but can also be observed in Crouzon's craniofacial dysostosis, Apert's acrocephalosyndactyly, Pfeiffer's-syndrome and severe plagiocephaly. Since the disease not only affects the cranial vault but also the facial skeleton, we simply termed it pancraniofacial synostosis (PCS). Radical surgical therapy is frequently necessary after birth for vital indication. This study presents results and co

[Synostosis dynamics in human long tubular bones].

More than 8,000 roentgenograms and electroroentgenograms of brachial, ulnar, radiocarpal, coxofemoral, knee, talocrural joints have been studied in persons of both sex at the age from birth up to 25 years. The synostosis degree is appreciated by the six-mark system. The roentgenological data are corroborate histologically. The method of appreciation applied makes it possible to reveal cases of initially forming synostosis long before puberty. Three stages of synostosis process are defined. The first stage--slow increase of the synostosis mark. The second--quick synostosis formation. The third-

[Familial congenital radio-ulnar synostosis (author's transl)].

Radio-ulnar synostosis is a congenital bony union between radius and ulna, usually in the vicinity of the radial tuberosity. A family is described on whom this anomaly was inherited over three generations. The rare cases described in the literature were usually bilateral, in our family it was unilateral. The following aspects are discussed: the frequency of this condition, the anatomical localisation of the synostosis, their types, local soft tissue and other abnormalities, embrylogy and the hereditory pattern. Finally, methods of treatment are discussed briefly.

Metopic synostosis: Defining the temporal sequence of normal suture fusion and differentiating it from synostosis on the basis of computed tomography images.

Only the metopic suture normally fuses during early childhood; all other cranial sutures normally fuse much later in life. Despite this, metopic synostosis is one of the least common forms of craniosynostosis. The temporal sequence of normal physiologic metopic suture fusion remains undefined and controversial. Therefore, diagnosis of metopic synostosis on the basis of computed tomography images alone can prove misleading. The present study sought to determine the normal sequence of metopic suture fusion and characterize both endocranial and ectocranial suture morphology. An analysis of comput

Functional Assessment of Congenital Radioulnar Synostosis in Children.

Background  Congenital radioulnar synostosis (CRUS) is a rare condition caused by segmentation failure in embryonic life. The forearm is usually fixed in pronation, affecting the upper limb and hand functions. Treatment ranges from observation to surgical options to restore limb function and prevent disease recurrence. The study aimed to assess the functional outcome of patients with CRUS. Methods  We conducted a prospective evaluation of eight patients with CRUS between 2010 and 2020. The data involved history, physical examination, and functional assessment using the ABILHAND-Kids functional

Post-traumatic synostosis of the metatarsals - a case report.

BACKGROUND: Synostosis refers to the abnormal fusion of bones, which have varying aetiologies as well as sites which are commonly affected. Existing literature mostly describes the condition arising congenitally, and affecting the radio-ulnar joint. CASE PRESENTATION: A 57-year-old gentleman presented to our department with symptomatic malunion and synostosis of the fourth and fifth metatarsals, two years following non-operative treatment of a closed fifth metatarsal fracture. The patient subsequently underwent surgical excision of the synostosis, but had complications of recurrence at the tw

The Incidence and Morphology of Deformational Plagiocephaly in Unicoronal Synostosis.

It has been our observation that unicoronal synostosis and deformational plagiocephaly (DP) are not mutually exclusive. The incidence and phenotypical manifestations of a patient presenting with both conditions have not been studied previously. The authors performed a retrospective review of patients presenting with unicoronal synostosis, examining their 3DCT scans for the presence of DP and assessing their endocranial and orbital morphology. The authors found that 38.4% of patients with unicoronal synostosis also have DP. Ipsilateral DP is less common than contralateral DP, but these patients

Difficulties in differential diagnosis of sagittal synostosis (scaphocephaly).

UNLABELLED: Craniosynostosis is characterized by congenital absence or premature closure of skull sutures. The most common form of craniosynostosis is synostosis of sagittal suture followed by scaphocephaly. There are some head deformities similar to scaphocephaly such as positional and constitutional dolichocephaly, etc. These patients have no sagittal suture synostosis. However, there are difficulties in differential diagnosis between these deformities and scaphocephaly. OBJECTIVE: To develop differential diagnostic criteria between dolichocephalic head deformities and true scaphocephaly fo

SMAD6 is frequently mutated in nonsyndromic radioulnar synostosis.

PURPOSE: Radioulnar synostosis (RUS) can be syndromic or nonsyndromic. The genetic basis for several RUS syndromes have been reported. However, the genetic cause of nonsyndromic RUS (nsRUS) remains unknown. METHODS: We performed Giemsa (GTG) banding, Sanger sequencing, and exome sequencing on patients (n = 140) and families (n = 11) who suffered from RUS. RESULTS: GTG banding identified 10% RUS sporadic cases affected by sex chromosome aneuploidy. Sanger sequencing on candidate genes revealed noggin (NOG) rarely mutated in nsRUS. Exome sequencing identified 16 loss-of-function (LOF) and 6 mi
