Permanent junctional reciprocating tachycardia in infants and children: effectiveness of medical and non-medical treatment.

BACKGROUND: The aim of this study was to identify, in children affected by permanent junctional reciprocating tachycardia (PJRT), the effective treatment. METHODS: Seventeen children (9 males, 8 females, mean age 59 +/- 62 months, median 24) affected by PJRT were referred to our Institute between the years 1987 and 2000. RESULTS: Pharmacological therapy was successfully used in 14 patients: flecainide and propranolol in 5 of them, amiodarone alone in 5 and associated with propranolol in 2, propafenone alone in 1 and in association with sotalol in 1. These drugs were given for a mean period o

Orthodromic reciprocating tachycardia and heart failure in a dog with a concealed posteroseptal accessory pathway.

A 4-month-old male Labrador Retriever was presented for recurrent bouts of pulmonary edema associated with tachycardia. Initial physical examination and echocardiography were unremarkable, and the electrocardiogram revealed only an intraventricular conduction disturbance. Subsequent recordings showed paroxysmal supraventricular tachycardia (SVT) (340 beats/min), which consistently produced pulmonary edema. The supraventricular tachycardia was unresponsive to adenosine, esmolol, and propranolol; was variable and transiently responsive to various vagal maneuvers and precordial thumps; and was al

[Junctional reciprocating tachycardia: clinical and electrophysiological characteristics, and the role of fulguration in 5 cases].

The permanent form is a variety of junctional reciprocating tachycardia that is refractory to medical treatment. The anterograde arm of the circuit is formed by the His Purkinje bundle and the retrograde conduction is through a slow conduction Accessory Atrioventricular Pathway. We report five patients with this type of arrhythmia, subjected to electrophysiological assessment. Their mean age was 37 years, all suffered from palpitations and several medical treatments had failed. During tachycardia, electrocardiogram had a negative P wave in inferior leads and RP interval was bigger than PR inte

Retrograde dual atrioventricular nodal pathway in patients with atrioventricular reciprocating tachycardia using concealed accessory pathways.

We present electrophysiological studies in two patients with atrioventricular reciprocating tachycardias. The first patient had anterograde dual atrioventricular nodal pathways with a right-sided concealed accessory pathway. The retrograde atrioventricular nodal pathway showed evidence suggestive of slow pathway properties. After block was induced with ajmaline in the accessory pathway, a typical pattern of discontinuous retrograde atrioventricular nodal conduction curves was recognized. We then observed three types of induced atrioventricular reentry. The other patient had continuous anterogr

Comparative clinical and electrophysiologic effects of adenosine triphosphate and verapamil on paroxysmal reciprocating junctional tachycardia.

The efficacy, electrophysiologic effects, and side effects of adenosine triphosphate (ATP) and verapamil in the short-term management of paroxysmal reciprocating junctional tachycardia (PRJT) were compared in 20 patients. All patients had inducible sustained PRJT during control electrophysiologic study. Fourteen patients had PRJT involving a retrograde accessory pathway, and six patients had atrioventricular (AV) nodal reentrant tachycardia ("slow-fast" type). ATP, which has a very short half-life, was first administered (10 mg iv over 1 sec) during sustained PRJT. If PRJT did not terminate wi

A modified electrocardiographic algorithm for differentiating typical atrioventricular node re-entrant tachycardia from atrioventricular reciprocating tachycardia mediated by concealed accessory pathway.

Non-invasive prediction of tachycardia mechanism is becoming clinically important in the era of catheter ablation for curing supraventricular tachycardia. Twelve-lead electrocardiograms (ECGs) during sinus rhythm and atrioventricular node re-entrant tachycardia (AVNRT) or atrioventricular reciprocating tachycardia (AVRT) with a narrow QRS complex were obtained from 154 consecutive adult patients who had received successful radiofrequency catheter ablation. The ECGs of initial 104 patients were analysed by three observers without knowledge of the electrophysiological diagnosis. The two arrhythm

A unique location of the accessory pathway in a child with permanent junctional reciprocating tachycardia.

Permanent junctional reciprocating tachycardia (PJRT) is a rare supra ventricular tachycardia (SVT) due to an accessory pathway (AP), characterized by slow and decremental retrograde conduction, which is predominantly seen in infants and children. Although the typical site of AP in PJRT is a right posteroseptal region around or just within the coronary sinus (CS), atypical sites of AP have been described. We report a rare case of PJRT in a 7-year-old girl with an AP located in the superio-paraseptal (Para-Hsian) region that was successfully ablated through a non-coronary sinus.

Influence of reciprocating tachycardia on the development of atrial fibrillation in patients with preexcitation syndrome.

BACKGROUND: We sought to evaluate the influence of atrio-ventricular reentrant tachycardia (AVRT) on atrial pressures during tachycardia and the presence of atrial fibrillation (AF) in patients with preexcitation syndrome. METHODS: The study population consisted of 88 patients (37 females, mean age 37.3 years) with left-sided accessory pathway and AVRT induced during electrophysiologic study. The AF-inducible group consisted of 32 patients with sustained episodes of AF provoked during electrophysiologic study, whereas the noninducible group comprised 56 patients without AF. RESULTS: We found

Radiofrequency catheter ablation of concealed accessory pathways in two dogs with symptomatic atrioventricular reciprocating tachycardia.

BACKGROUND: Radiofrequency catheter ablation (RFCA) is widely used as a curative therapeutic strategy in human beings with paroxysmal supraventricular tachycardia (SVT), but rarely applied in animals. This report describes successful RFCA of atrioventricular accessory pathways (AP) in two dogs with episodic weakness caused by frequent paroxysms of supraventricular tachycardia. METHODS AND RESULTS: Invasive electrophysiological studies (EPS) identified two APs in the 1st dog (right postero-septal, right posterior), and one in the 2nd dog (right posterior). Latent Mahaim fiber as a cause of antidromic reciprocating tachycardia: recognition and successful radiofrequency ablation.

The term "Mahaim fiber" usually is applied to an atriofascicular fiber that inserts distally into the right bundle branch and forms the anterograde limb of a reciprocating tachycardia. One of the features that has been used to describe the physiology of Mahaim fibers is the presence of anterograde preexcitation. We describe two patients who had a clinical tachycardia consistent with a "Mahaim tachycardia" in whom there was no evidence or minimal evidence of anterograde preexcitation during sinus rhythm or atrial pacing. In both patients, the tachycardia was rendered noninducible by radiofreque

Rapid recovery of cardiac function after catheter ablation of persistent junctional reciprocating tachycardia in children.

Four children (age range 3 weeks to 15 years) with tachycardia induced cardiomyopathy due to permanent junctional reciprocating tachycardia (PJRT) underwent successful catheter ablation of the tachyarrhythmia circuit without any complications during or after the procedure. In all four patients, the left ventricular fractional shortening improved to normal within 2 weeks of the procedure. These results argue for a more aggressive approach to the therapy of PJRT when associated with tachycardiomyopathy at a young age.

[Two to one atrioventricular block in intranodal re-entry reciprocating tachycardia. Description of 2 cases].

Atrioventricular nodal reentry tachycardia (AVNRT) is a common form of paroxysmal supraventricular tachyarrhythmia. In this tachycardia, the atrium and ventricle are not necessary links of the reentry circuit, so that the arrhythmia may persist in spite of the occurrence of 2:1 AV ratio or AV dissociation. Only a few examples of 2:1 AV block during AVNRT have been described. We report on 2 patients with a history of paroxysmal supraventricular tachycardia in whom 2:1 AV block with persistence of the arrhythmia was documented. Transesophageal electrophysiologic study was performed after pharmac

Electrophysiologic manifestations of the excitable gap of orthodromic atrioventricular reciprocating tachycardia demonstrated by single extrastimulation.

To assess the electrophysiologic characteristics of the excitable gap, 12 patients with orthodromic atrioventricular (AV) reciprocating tachycardia were studied. During tachycardia, 8 patients used a left-sided and 4 patients a right-sided anomalous bypass tract for retrograde conduction. QRS complex-synchronized single extrastimuli were delivered from high right atrium, right ventricular apex and coronary sinus, respectively, scanning the whole cycle length of tachycardia. An excitable gap was determined to be present if tachycardia resetting or tachycardia termination occurred. The duration

[Electrophysiological effects and clinical efficacy of flecainide in childhood patients with supraventricular reciprocating paroxysmal tachycardia].

The electrophysiologic effects of intravenous (i.v.) flecainide were evaluated in 13 patients (pts) with recurrent paroxysmal supraventricular tachycardia (PSVT): 6 pts had an overt accessory pathway, 2 a concealed anomalous pathway and 5 had an idionodal reentrant tachycardia (AVNRT). Another patients with overt preexcitation underwent electrophysiologic testing as part of a diagnostic investigation for syncope. After flecainide the effective refractory period of the right atrium and retrograde AV node, and anterograde and retrograde Wenckebach point significantly increased. The drug blocked

["Slowing" right bundle-branch block in reciprocating tachycardia from a latent right lateral Kent bundle].

The authors report the case of a 56 year old man with paroxysmal reciprocating tachycardia. The participation of a right lateral Kent bundle, latent in sinus rhythm and with retrograde atrioventricular conduction during tachycardia was proved by : 1) the slowing of the tachycardia rhythm and lengthening of the ventriculo-atrial conduction time by 50 ms during right bundle branch block ; 2) atrial mapping during tachycardia showing right lateral atrial pre excitation ; 3) the spontaneous termination of some attacks after a blocked Hisian depolarisation. Analysis of the mechanisms of spontaneous

A case of Wolff-Parkinson-White syndrome presenting spontaneous mutual frequent transition between atrioventricular reciprocating tachycardia and atrioventricular nodal re-entrant tachycardia.

Atrioventricular reciprocating tachycardia (AVRT) and atrioventricular nodal re-entrant tachycardia (AVNRT) can coexist and present unidirectional transition (from AVRT to AVNRT, or from AVNRT to AVRT) in a single patient. Actually, such cases have already been reported previously. However, a case with spontaneous bidirectional transition of both tachycardias during supraventricular tachycardia has never been reported. This article describes a case with spontaneous, mutual, and frequent transition between AVRT and AVNRT.

50 is the new 70: Short ventriculoatrial times are common in children with atrioventricular reciprocating tachycardia.

BACKGROUND: One of the basic electrophysiological principles of atrioventricular reciprocating tachycardia (AVRT) is that ventriculoatrial (VA) times during tachycardia are >70 ms. We hypothesized, however, that children may commonly have VA times <70 ms in AVRT. OBJECTIVE: This study sought to determine the incidence and characteristics associated with short-VA AVRT in children. METHODS: A retrospective single-center review of children with AVRT from 2000 to 2014 was performed. All patients ≤18 years of age with AVRT at electrophysiology study were included. Patients with persistent junctio

Initial depolarizing potentials in Wolff-Parkinson-White syndrome using signal-averaged electrocardiograms: correlation with reciprocating tachycardia.

Initial depolarizing potentials were examined in patients with Wolff-Parkinson-White (WPW) syndrome using signal-averaging techniques. In all, 25 WPW patients and 21 age-matched healthy children were studied. Ten of the patients were symptomatic and 15 were asymptomatic. Symptomatic patients with supraventricular tachycardias had longer durations of low-amplitude signals (LAS) < 40 mu V in the initial portion of the QRS complex (initial LAS, 49 +/- 7 ms vs. 37 +/- 9 ms, p < 0.01) and lower root-mean-square (RMS) voltage in the initial 40 ms of the QRS complex (initial RMS, 12 +/- 4 mu V vs. 23
