We describe a 36-year-old woman with progressive bilateral ulnar neuropathy. Sonographic and magnetic resonance imaging studies revealed extensive focal ulnar nerve enlargement at the elbow. Histological studies gave evidence of an intraneural perineurioma. Because intraneural perineurioma usually appears as a single mass lesion at sites other than typical entrapment sites, this mode of presentation is unusual. We discuss the nature of this benign tumor and the differential diagnosis of nerve enlargement. Knowledge of possible causes of nerve thickening is crucial when performing imaging in pa

Ulnar nerve entrapment neuropathy at the elbow, or the cubital tunnel syndrome, is frequently encountered in neurosurgical practice as the second most common peripheral nerve entrapment after carpal tunnel syndrome. Patients typically present with weakness or atrophy of the hand as well as paresthesias in the ulnar nerve distribution. The diagnosis can be confirmed with a careful clinical examination and electrophysiological studies. Patients who have failed conservative therapy are considered for surgery. Although a number of surgical options are available, simple decompression of the ulnar n

The incidence and causes of perioperative ulnar neuropathy in a prospective series of 203 consecutive patients were examined. Three percent of patients had ulnar neuropathy develop in the entire prospective series. The incidence was 6% in patients having total hip arthroplasty. There was a highly significant association between a tilted body position on the operating table and development of ulnar neuropathy on the contralateral side. This position rotates the arm internally and places the ulnar nerve at risk for direct compression.

Abnormal strain of the ulnar nerve over the sulcus due to an unusual sleep position is a rare cause of ulnar neuropathy at the elbow. A 57-year-old patient with Mandelung's deformity developed progressive weakness in the flexion of fingers 4 and 5 and in finger straddling on the left side. Additionally, there was slight wasting of the left hypothenar and the left interossei muscles. Motor and sensory nerve conduction studies of the left ulnar nerve showed delayed conduction velocities over the left ulnar sulcus. He preferred to sleep in a left lateral position with his head lying on a headrest

OBJECTIVES: In order to establish the focal compression in the elbow, a study of 318 patients with clinical manifestations of ulnar neuropathy was carried out. We divided the patients in three groups: I) those who presented mechanical anomalies with background of fracture, elbow valgus...; II) those who did not present mechanical anomalies, but they have been in bed long time or they have been operated...; III) those who did not have evident cause for the compression.
MATERIAL AND METHOD: Antidromic sensory conduction and segmentary motor conduction of the ulnar nerve were studied, and segmen

We operated on 16 patients for ulnar neuropathy associated with osteoarthritis of the elbow. They were all male manual workers, with an average age of 51 years at the time of surgery. The severity of the symptoms was McGowan grade 1 in five patients, grade 2 in nine and grade 3 in two. The mean follow-up was 36 months. The operation consists of resecting the osteophytes around the postcondylar groove. The shallow and narrow cubital tunnel is made deep and wide and the ulnar nerve is replaced with its surrounding soft tissues in the enlarged groove. All patients were relieved of discomfort and

INTRODUCTION: Ulnar neuropathy at the elbow (UNE) is the second most frequent entrapment syndrome. Ulnar nerve hypermobility (UNH) has been considered an anatomical variant, but recent studies suggest it may predispose to neuropathy. We investigated the role of dynamic UNH in UNE.
RESEARCH QUESTION: Does ulnar nerve hypermobility contribute to the development of ulnar neuropathy at the elbow, and can dynamic ultrasound classification of ulnar nerve instability improve diagnosis and management of UNE?
MATERIAL AND METHODS: A series of 220 adult patients with suspected UNE was retrospectively

PURPOSE: This study was designed to verify whether shear-wave elastography (SWE) can be used to differentiate ulnar neuropathy at the cubital tunnel from asymptomatic ulnar nerve or medial epicondylitis. An additional aim was to determine a cut-off value to identify patients with ulnar neuropathy.
METHODS: This study included 10 patients with ulnar neuropathy at the cubital tunnel as confirmed with electromyography (three women and seven men; mean age, 51.9 years), 10 patients with medial epicondylitis (nine women and one man; mean age, 56.1 years), and 37 patients with asymptomatic ulnar ner

OBJECTIVE: To compare pattern and parameters describing nerve thickening in ulnar neuropathy at the elbow (UNE) due to external compression in the retrocondylar groove (RTC), and entrapment under the humeroulnar aponeurosis (HUA).
METHODS: In a group of our previously reported UNE patients we ultrasonographically (US) measured ulnar nerve cross-sectional areas (CSA) on 6-8 standard locations in the elbow segment. We compared CSA patterns in both groups, and determined diagnostic utility of selected CSA based parameters.
RESULTS: We studied 79 patients (81 arms) with UNE due to external compr

Surgical treatment of ulnar nerve syndrome at the elbow gives good results in the majority of cases but there are sometimes incomplete results or recurrences that can lead to iterative procedures. This iterative surgery is responsible for painful neurological sequelae often resistant to pharmacological treatments. In these selected resistant cases, we made the choice of a surgical treatment associating a neurolysis of the ulnar nerve and wrapping with a vascularized flap modified from Lamberty and Cormack. This retrospective study focused on six cases with an average follow-up of 30 months. Th

From 1900-1995, 48 patients with ulnar nerve compression neuropathies were studied. Pinching grip, palmar adduction of the thumb, radical abduction of the index finger, and ulnar abduction of the little finger, were measured before and after operation with the new computerised intrins-o-meter. The data were processed, assessed statistically, and printed out. From the results of the four parameters mentioned above, a special Ulnar Nerve Function Profile (UNFP) was constructed. The series was subdivided in three groups A, B, C of which group A (n = 30) comprised pure ulnar nerve compression neur

INTRODUCTION: Postoperative positioning devices, such as shoulder splints, are commonly used to support and stabilize the upper limb following surgery. However, improper placement and prolonged pressure from these devices can lead to iatrogenic nerve injuries.
CASE PRESENTATION: We present a case of severe axonal injury to the median and ulnar nerves at the proximal hand in a patient following shoulder surgery. This was attributed to palm pressure from a postoperative shoulder splint with a built-in palm stress ball designed to improve circulation and healing, along with impaired pain sensati

BACKGROUND: In ulnar neuropathy at the elbow, weakness is classically a sign of severe disease. Weakness is associated with motor axonal loss as measured by decreased distal compound muscle action potential (CMAP) amplitude. Conduction block, a demyelinating phenomenon that recovers readily, can also cause weakness, creating ambiguity for the treating clinician.
METHODS: This cross-sectional study evaluated baseline blinded data collected from 2020 through 2023 from the Surgery of the Ulnar Nerve randomized controlled trial comparing in situ decompression versus transposition procedures. Adul

BACKGROUND: Many instruments have been developed to measure upper extremity disability, but few have been applied to ulnar neuropathy at the elbow (UNE).
OBJECTIVE: We measured patient outcomes following ulnar nerve decompression to (1) identify the most appropriate outcomes tools for UNE and (2) to describe outcomes following ulnar nerve decompression.
METHODS: Thirty-nine patients from 5 centers were followed prospectively after nerve decompression. Outcomes were measured preoperatively and at 6 weeks, 3 months, 6 months, and 12 months postoperatively. Each patient completed the Michigan H

BACKGROUND: Ulnar neuropathy at the elbow (UNE) is the second most prevalent neuropathy of the upper extremity after median neuropathy. Although ultrasonography (US) is increasingly used in the diagnosis of UNE, it is not clear whether it can discriminate between stages of disease severity. This study aimed to investigate the correlation between ultrasonographic findings and UNE severity.
METHODS: A systematic literature review was conducted according to PRISMA guidelines. PubMed, Embase, and Cochrane databases were searched to identify articles reporting on the relationship between US measur

BACKGROUND: This study evaluated muscle echo intensity (EI) ratio in patients with ulnar neuropathy at the elbow (UNE) and healthy controls.
METHODS: In this prospective study, 28 patients with electrodiagnostically confirmed unilateral UNE and 12 healthy controls were ultrasonographically assessed for EI ratios of the hypothenar and thenar muscles. The affected and unaffected hands between the UNE patients and controls and patient subgroups (subdivided according to electrodiagnostic severity) were compared to determine any significant differences.
RESULTS: In patients with UNE, the EI ratio

OBJECTIVES: To evaluate the sensitivity and specificity of the latency difference (DLat) between ulnar and median nerves of the arm after stimulation at the wrist; one of the easiest techniques proposed for recognizing ulnar neuropathy at the elbow (UNE). As latency difference is not a standardized technique, we set up a multicenter study to recruit large numbers of normal subjects and patients with UNE or generalized neuropathy.
METHODS: Six centers participated in the study with data obtained from three groups of participants, controls (CTRLs), patients with UNE and patients with generalize

PURPOSE: The purpose of this study is to determine whether patients diagnosed with ulnar nerve lesions (UNLs) have an increased risk of developing carpal tunnel syndrome (CTS).
METHODS: A retrospective cohort study was conducted using the TriNetX database, which contains data of >100 million patients. We identified 360,021 patients with UNLs and matched them 1:1 with controls. The incidence of CTS was assessed at 1, 2, 3, 4, and 5 years following the diagnosis of UNL. Statistical analyses were used to evaluate the development of CTS in the UNL cohort compared with controls.
RESULTS: Patients
