Interest of ventilation/perfusion SPECT/CT in patients with post-COVID condition: a multicenter observational study.

PURPOSE: The COVID-19 pandemic has led to a significant number of survivors experiencing persistent health issues, now recognized as long COVID. Respiratory symptoms, such as dyspnea, chronic cough, and reduced exercise capacity, are prevalent and can severely impact quality of life. Ventilation/Perfusion (V/Q) SPECT/CT is a hybrid imaging modality that offers both functional and anatomical insights and has proven valuable in evaluating pulmonary embolism (PE). However, its role in long COVID remains underexplored. METHODS: A retrospective, multicenter study was conducted using a national regi

Feasibility of lung imaging at 0.55T for assessment of interstitial lung disease in patients with pulmonary sarcoidosis.

OBJECTIVES: To assess the feasibility and diagnostic performance of radiation-free 0.55-T lung MRI for pulmonary sarcoidosis, combining morphological imaging with functional proton MRI ventilation/perfusion metrics, and to explore correlations with pulmonary function testing (PFT). MATERIALS AND METHODS: In this prospective study, 15 patients with pulmonary sarcoidosis and 30 healthy volunteers underwent 0.55-T lung MRI (bSTAR morphology; matrix-pencil based functional proton MRI). CT served as the reference standard for morphological findings (majority vote of experienced readers). Functiona

Platypnea-Orthodeoxia and Patent Foramen Ovale in a Patient in the Setting of COVID-19.

Platypnea-orthodeoxia syndrome is a rare condition characterized by positional dyspnea and hypoxemia, with symptoms presenting in the upright position and improving when recumbent. Hypoxemia in platypnea-orthodeoxia syndrome is defined as a drop in Pao2 of more than 4 mm Hg or oxygen saturation greater than 5% when moving from a supine to an upright position. Etiologic mechanisms include intracardiac or pulmonary shunts and ventilation perfusion ratio mismatch. Platypnea-orthodeoxia syndrome has been described as part of COVID-19 pneumonia sequelae, with differing physiologic mechanisms from c

Ventilatory responses to hypocapnic vertebral artery perfusion in intact and carotid body denervated dogs.

The ventilatory responses to step changes in vertebral artery PCO2 were investigated in intact and carotid body denervated dogs. The steady-state ventilatory responses of the denervated dogs were less than those of intact dogs. However, when expressed as a ratio to the control ventilation there was no difference between the two groups. While the arterial PCO2 was held at 56 mm Hg by adding CO2 to the inspired air the perfusion of the vertebral arteries was switched from the dog's own arterial supply to hypocapnic blood. The ventilation of the denervated dogs decreased at a faster rate (half ti

[Lung scintigraphy].

The S1 guideline for lung scintigraphy has been updated and extended in order to emphasize the advantages oft the method in detecting acute pulmonary embolism (PE) in the periphery oft the lung (subsegmental PE), in underlying subacute and chronic pulmonary disorders, as well as in detecting chronic LE (CTEPH). Method of choice is ventilation / perfusion (V/P) SPECT or V/P SPECT/CT with even higher specificity. Because of its high sensitivity, a threshold (V/P mismatch in at least one segment or two subsegments) is introduced to avoid overtreatment. In case of a change in the therapeutic appro

[Mechanical ventilation and weaning of patients with obstructive bronchopneumopathy].

Mechanical ventilation of chronic obstructive pulmonary disease (COPD) patients with acute respiratory failure poses certain problems specific to the pathophysiology of this condition. These difficulties arise during the mechanical ventilation phase as well as during weaning. A thorough knowledge of the mechanisms involved and of the means of minimizing complications allows the vast majority of these patients to be weaned successfully. This article reviews some of these problems and possible solutions.

Chronic obstructive pulmonary disease and sleep.

The control of breathing in patients with chronic obstructive pulmonary disease (COPD) follows the same basic principles as in normal subjects, both awake and asleep, with an expected lower feedback response during sleep. This impacts nocturnal gas exchange and sleep quality most profoundly in patients with more severe COPD, as multiple factors come into play. Hypoventilation causes the most important gas-exchange alteration in COPD patients, leading to hypercapnia and hypoxemia, especially during rapid-eye-movement sleep, when marked respiratory muscle atonia occurs. The hypoxia leads to incr

[Relationship between hypercapnia and hypoxemia in chronic obstructive respiratory insufficiency].

Hypercapnia is common in chronic respiratory failure (IRCO), and may be further increased in a significant way by oxygen therapy, used for severe hypoxaemia in acute exacerbations. The determinants of PaCO2 are metabolic (hence importance of alkalosis) and ventilatory. In chronic airflow obstruction, CO2 production and ventilation are normal; thus the factor responsible for hypercapnia is essentially the fraction of total ventilation lost in the anatomical and alveolar (VD/VT ratio) dead space, whose effect on PaCO2 is all the more marked on account of the high starting point. From the time of

Chest compressions for bradycardia or asystole in neonates.

When effective ventilation fails to establish a heart rate of greater than 60 bpm, cardiac compressions should be initiated to improve perfusion. The 2-thumb method is the most effective and least fatiguing technique. A ratio of 3 compressions to 1 breath is recommended to provide adequate ventilation, the most common cause of newborn cardiovascular collapse. Interruptions in compressions should be limited to not diminishing the perfusion generated. Oxygen (100%) is recommended during compressions and can be reduced once adequate heart rate and oxygen saturation are achieved. Limited clinical

Pulse oximetry in pediatric practice.

The introduction of pulse oximetry in clinical practice has allowed for simple, noninvasive, and reasonably accurate estimation of arterial oxygen saturation. Pulse oximetry is routinely used in the emergency department, the pediatric ward, and in pediatric intensive and perioperative care. However, clinically relevant principles and inherent limitations of the method are not always well understood by health care professionals caring for children. The calculation of the percentage of arterial oxyhemoglobin is based on the distinct characteristics of light absorption in the red and infrared spe

Imaging obstructed ventilation with NMR using inert fluorinated gases.

We partially obstructed the left bronchi of rats and imaged an inert insoluble gas, SF(6), in the lungs with NMR using a technique that clearly differentiates obstructed and normal ventilation. When the inhaled fraction of O(2) is high, SF(6) concentrates dramatically in regions of the lung with low ventilation-to-perfusion ratios (VA/Q); therefore, these regions are brighter in an image than where VA/Q values are normal or high. A second image, made when the inhaled fraction of O(2) is low, serves as a reference because the SF(6) fraction is nearly uniform, regardless of VA/Q. The quotient of

[Combination of scimitar syndrome and horseshoe lung. A rare but typical finding--case report and review of the literature].

The scimitar syndrome is a rare congenital cardiopulmonary malformation. In association with a horseshoe lung it is extremely uncommon. We describe a child with typical scimitar syndrome (anomalous right pulmonary venous return, hypoplasia of the right lung with dextro-position of the heart and aberrant systemic arterial supply of the right lung), associated with a horseshoe lung. No clinical symptoms are present, surgical management is not necessary. This is the 20th case in addition to the 19 reported cases with scimitar syndrome associated with horseshoe lung. Eight other additional cases o

Ventilation-perfusion relationships in alligators.

We measured V/Q distributions with the multiple inert gas elimination technique in five anesthetized artificially ventilated American alligators at 24 degrees C. The overall V/Q was relatively high (2.0 to 7.2) because of continuous artificial ventilation. The V/Q distributions were usually unimodal (log standard deviation of Q distribution averaged 0.47 +/- 0.09 [SD] and there was considerable shunt (28.5 +/- 10.3% of cardiac output). The data are consistent with a pulmonary, rather than central cardiovascular shunt. We also detected a molecular weight dependent limitation to inert gas elimin

[Isotope studies in the detection of pulmonary embolism].

The regional absence of perfusion in the territory situated downstream of a lung embolus can be visualized using a gamma camera and 99m Technetium labelled macroaggregated albumin of albumin microspheres. This is a simple and atraumatic procedure. Following intravenous injection these particles are blocked in the pulmonary capillaries, their distribution in the lungs being proportional to the regional perfusion. A normal perfusion scan virtually excludes pulmonary embolism. Pulmonary embolism typically causes a wedge shaped perfusion defect at one or more sites. However, a regional perfusion d

[Intraoperative monitoring in artificial respiration of premature and newborn infants. II. Monitoring of arterial oxygenation].

Monitoring of adequate arterial oxygenation serves to avoid periods of hypoxaemia and hyperoxaemia with potentially life threatening or organ-damaging sequelae. Basic clinical monitoring, i.e., inspection and auscultation, is mandatory. In all infants, paO2 may be continuously and indirectly monitored by measurement of transcutaneous pO2 (tcpO2). The use of pulse oximetry for non-invasive measurement of arterial oxygen saturation (SaO2) is still undergoing clinical testing. Invasive monitoring of gas exchange is essential in prolonged or intrathoracic interventions as well as in neonates with

Analyses of maximum cardiopulmonary performance during exposure to acute hypoxia at simulated altitude--sea level to 5000 meters (760-404 mm Hg).

The maximum cardiopulmonary performance of seven healthy male subjects was studied repeatedly in graded hypoxia at ambient pressures ranging from 760 to 404 mm Hg (sea level to 5000 m of simulated altitude). Using this approach it has been possible to not only establish a reproducible value for VO2max, but to determine an equation which may be used to predict the VO2 at altitude for healthy, unacclimatized males exercising to exhaustion. Moreover, we have attempted to explain the limits to pulmonary ventilation at decreasing levels of PO2 by comparing a given VO2max (STPD) to the corresponding

[Regional pulmonary function in unilateral sero-fibrinous pleural effusions and changes under the influence of treatment (author's transl)].

The regional distribution of ventilation (Vr), perfusion (Qr) (expressed in absolute values) and the regional distribution of ventilated lung volume, of ventilation and of perfusion (expressed as %) were studied using Xenon 133 in 13 subjects with a unilateral pleural effusion of proven or presumed tuberculous origin. The examinations were performed in a sitting position. The initial examination was carried out after as complete an evacuation as possible of the fluid effusion. Eight subjects underwent a follow-up examination after two months treatment with tuberculostatic agents combined with
