Looking to the future.

Since the 7th and 13th Study Groups of the Royal College of Obstetricians and Gynaecologists met in 1977 and 1985, respectively, no meeting of this magnitude has convened to discuss the problems of spontaneous preterm labour and delivery and the associated fetomaternal mortality and morbidity. In the 17 years or so since that time, advances have been made in our understanding of the mechanisms of labour, the role of infection, the benefit of antepartum corticosteroids and the development of safer more specific tocolytics. In the future, an understanding of the genetic risk of spontaneous prete

A multicenter randomized trial of high frequency oscillatory ventilation as compared with conventional mechanical ventilation in preterm infants with respiratory failure.

A multicenter randomised trial was conducted in nine neonatal centers in Japan to re-evaluate the safety and the efficacy of high frequency oscillatory ventilation using the piston type oscillator (Hummingbird) in the treatment of respiratory failure in preterm infants weighing between 750 and 2000 g at birth. A total of 92 infants were enrolled in the study. Forty-six infants were allocated to high frequency oscillatory ventilation and 46 infants to conventional mechanical ventilation. There were no differences in sex, birth weight, gestation and Apgar score between groups. The study was begu

Mortality of very low birth weight infants by neonatal intensive care unit workload and regional group status.

BACKGROUND: Very low birth weight infants (VLBWIs) continue to face high mortality risk influenced by the care quality of neonatal intensive care units (NICUs). Under-standing the impact of workload and regional differences on these rates is crucial for improving outcomes. PURPOSE: This study aimed to assess how the structural and staffing attributes of NICUs influence the mortality rates of VLBWIs, emphasizing the significance of the availability of medical personnel and the regional distribution of care facilities. METHODS: Data from 69 Korean NICUs collected by the Korean Neonatal Network

The Term Newborn: Evaluation for Hypoxic-Ischemic Encephalopathy.

Neonatal encephalopathy due to perinatal hypoxia-ischemia (hypoxic-ischemic encephalopathy [HIE]) occurs at a rate of 1 to 3 per 1000 live births. Therapeutic hypothermia is the standard of care and the only currently available therapy to reduce the risk of death or disability in newborns with moderate to severe HIE. Hypothermia therapy needs to be initiated within 6 hours after birth in order to provide the best chance for neuroprotection. All pediatricians and delivery room attendants should be trained to recognize encephalopathy and understand the eligibility criteria for treatment. The mod

Modalities of Mechanical Ventilation: Volume-Targeted Versus Pressure-Limited.

BACKGROUND: Respiratory distress syndrome remains the most common admission diagnosis in the neonatal intensive care unit. Healthcare providers have a clear appreciation for the potential harm to pulmonary structures that have been associated with mechanical ventilation (MV) in the preterm infant. Although life sustaining, the goal is to optimally ventilate while limiting trauma to the neonatal lung in order to preserve long-term cardiopulmonary and neurodevelopmental outcomes. PURPOSE: To describe, compare, and contrast 2 primary methods of neonatal MV, pressure-limited ventilation (PLV) and

A complex urologic problem demonstrates how far pediatric urology has progressed.

OBJECTIVE: We present the experience at Connecticut Children's Medical Center (CCMC) with single-system bilateral ectopic ureters (SSBUE), a rare clinical entity which continues to challenge the pediatric urologist. Innovative diagnostic and reconstructive techniques are utilized in an effort to preserve renal function and develop urinary continence in a select group of patients. METHODS: A review of the English language literature was performed to identify all female patients with SSBUE and evaluate their treatment and outcomes. In addition, we present our experience with three cases of SSBU

Assessment of fetal growth.

Normal fetal growth is a logarithmic process, marked by rapid mitosis at its early stages and by cellular hypertrophy and the accumulation of fat, glycogen, and connective tissue later in gestation. Growth-retarding influences can alter cell number, with a symmetric pattern of IUGR resulting if they occur early. Later insults, the result of uteroplacental compromise, affect cell size and may cause an asymmetric growth retardation. Fetuses with asymmetric growth retardation are at particular risk for intrauterine fetal demise and fetal distress in labor. The assessment of fetal growth is compli

Placental insufficiency in relation to postterm pregnancy and fetal postmaturity. Evaluation of fetoplacental function; management of the postterm gravida.

As pregnancy extends post term, incidence of placental insufficiency, fetal postmaturity (dysmaturity), and fetal perinatal death increases rapidly as a consequence of reduced respiratory and nutritive placental function. Despite a compensatory fetoplacental respiratory reserve capacity, fetal distress is observed in about one third of postterm pregnancies. On a biochemical level, placental pathophysiology in postterm-postmaturity pregnancies is not well understood. Postmaturity is correlated with increased incidence of placental lesions, fetal hypoxia-asphyxia, intrauterine growth retardation

Comparison of Ultrasound and MRI for Measuring Foramen Magnum Diameter in Neonates and Infants: A Preliminary Study.

BACKGROUND: The foramen magnum is the largest opening at the skull base. Congenital skeletal disorders such as achondroplasia can cause foramen magnum stenosis, leading to neurological problems. In high-risk infants, repeated magnetic resonance imaging (MRI) is often used to screen for or monitor stenosis. Ultrasound (US), already used for neonatal spinal assessment, may offer a non-invasive method for evaluating the craniovertebral junction. However, to our knowledge, no studies have directly compared foramen magnum assessment between US and MRI. METHODS: We retrospectively reviewed neonates

Imaging and Non-Imaging Approaches for the Diagnosis and Monitoring of Necrotizing Enterocolitis-What Lies Ahead?

Necrotizing enterocolitis (NEC) remains one of the most serious gastrointestinal emergencies in preterm infants, and imaging plays a central role in diagnosis and clinical management. Historically, evaluation has relied primarily on abdominal radiography, which remains widely available and embedded in established diagnostic frameworks. However, the hallmark radiographic signs of NEC (i.e., pneumatosis intestinalis, portal venous gas, and free air) reflect relatively advanced manifestations of intestinal injury that indicate established mucosal disruption or transmural necrosis.
