Effects of wildfire smoke exposure on health parameters and inflammatory responses of beef-on-dairy calves.

The increasing frequency and intensity of wildfires in the western United States have raised concerns about the health and welfare of livestock exposed to wildfire smoke. This study evaluated the physiological, clinical, and immunological responses of beef-on-dairy calves naturally exposed to wildfire smoke. A total of 18 Simmental × Jersey calves approximately 1 month of age were monitored from June to October 2022 in a commercial dairy farm in Vale, Oregon. Air quality was tracked daily, and blood samples and health scoring data were collected monthly before the wildfire (baseline; 3 samplin

Incarcerated Patients Presenting to an Academic Emergency Department: A Retrospective Review Using Payor Source for Identification.

BACKGROUND: Emergency departments (EDs) serve an important role in the care of patients who are imprisoned (incarcerated) in the federal and state prison systems, and the University of North Carolina Healthcare (UNCH) System helps provide care to these patients. Our aim was to identify and characterize this marginalized patient population by payer source, compare it with the general ED population, and, ultimately, seek ways to improve ED and follow-up care. METHODS: This was a hypothesis-generating study utilizing two separate ED sites: a tertiary referral academic ED and a smaller, academica

Baseline Hematological Parameters and Outcomes in Patients With Prostate Cancer Treated With Lu-177 PSMA Radioligand Therapy (RLT): A Multicenter Real-World Analysis.

BACKGROUND: Radioligand therapy with lutetium-177-labeled prostate-specific membrane antigen 177Lu-PSMA-617 has demonstrated survival benefits in metastatic castration-resistant prostate cancer (mCRPC), yet prognostic markers in real-world settings remain poorly defined. Baseline hematologic parameters may influence both treatment safety and outcomes, but their prognostic role is uncertain. PATIENTS AND METHODS: We conducted a multi-institutional, retrospective cohort study of 180 patients with mCRPC treated with ≥ 1 cycle of 177Lu-PSMA-617 between March 2019 and September 2024 at Stanford Un

Direct targeting for focused ultrasound thalamotomy in the treatment of movement disorders.

OBJECTIVE: Accurate targeting of the ventral intermediate nucleus (VIM) remains a critical challenge in thalamotomy for essential tremor (ET) and tremor-dominant Parkinson's disease (TDPD). Indirect atlas-based methods suffer from anatomical variability and poor visualization of thalamic substructures. Here, the authors evaluated the clinical impact of a direct targeting strategy enabled by fast gray matter acquisition T1 inversion recovery (FGATIR) imaging in MR-guided focused ultrasound (MRgFUS) thalamotomy. METHODS: The authors conducted a retrospective cohort study of adult patients who u

Relationship between reported dose and outcome in amlodipine exposures: a 25-year retrospective poison center review.

INTRODUCTION: Amlodipine, a dihydropyridine calcium channel blocker, is a growing cause of poisoning fatalities in the United States. As it takes several hours following ingestion for amlodipine peak plasma concentrations to be reached, the severity of poisoning may not be immediately apparent to clinicians. This study aimed to stratify reported amlodipine exposure doses with the severity of clinical outcomes. METHODS: We reviewed 25 years of Oregon Poison Center records to identify amlodipine exposures that included reported ingestion doses, did not involve other cardioactive co-ingestions a

Hospital birth volume and rurality: Associations with pregnancy outcomes among individuals with chronic hypertension.

INTRODUCTION: Chronic hypertension in pregnancy has doubled in prevalence over the past 15 years, but little is known about pregnancy outcomes at hospitals with different characteristics. We evaluated the association between hospital birth volume and rurality with risk of adverse pregnancy outcomes among individuals with chronic hypertension. METHODS: We conducted a population-based study using linked vital statistics and birth hospitalization discharge data from Michigan, Oregon, South Carolina (2008-2020), and Pennsylvania (2008-2018). We classified hospitals based on federal rural-urban co

Surgical management of severe refractory gastroparesis: outcomes of jejunostomy tube placement.

BACKGROUND: Surgical referrals for refractory gastroparesis (GP) are becoming more common as medical options are limited. Supplemental intestinal alimentation via feeding jejunostomy tubes (JT) is required to treat underlying malnutrition in only the most severe cases. The aim of this study was to determine predictive factors associated with successful restoration of oral nutrition after insertion of a JT for patients with severe malnutrition associated with GP. METHODS: Retrospective review of all patients who had JT inserted between November 2007 and October 2023 at The Oregon Clinic for se

Effect of Simultaneous Endothelial Keratoplasty and Cataract Surgery: A Secondary Analysis of the Descemet Endothelial Thickness Comparison Trial.

PURPOSE: Combining cataract surgery with endothelial keratoplasty (triple EK) is a common practice and may be safer because it commits the patient to only one surgery. This study aimed to determine whether outcomes of pseudophakic endothelial keratoplasty and triple EK have similar outcomes. METHODS: This was a non-prespecified secondary analysis of a multicenter, double-masked, randomized, controlled clinical trial. Enrollment centers included the Casey Eye Institute at Oregon Health and Sciences University and the Byers Eye Institute at Stanford University. Patients with damaged or diseased

Pharmacy staff-reported adaptations to naloxone provision and over-the-counter (OTC) syringe sales during the COVID-19 pandemic: Experiences across multiple states and 2 pharmacy chains.

BACKGROUND: Access to harm reduction materials was greatly disrupted during coronavirus disease 2019 (COVID-19). Community pharmacies often continued provision of harm reduction materials as part of their usual operations during the pandemic, but little is known about what, if any, adaptations were made and the perceived impact of these actions from the perspective of pharmacy staff. OBJECTIVES: We explored how pharmacy staff across 4 states in 2 major pharmacy chains adapted to the COVID-19 pandemic for ongoing naloxone and over-the-counter (OTC) syringe access and how staff perceived the pa

COMMunity of Practice And Safety Support for Navigating Pain (COMPASS-NP): study protocol for a randomized controlled trial with home care workers.

BACKGROUND: Chronic pain is a prevalent and costly problem that often has occupational origins. Home care workers (HCWs) are at high risk for work-related injuries, pain, and disability. Current treatments for chronic pain emphasize medications, which are an inadequate stand-alone treatment and can produce significant adverse effects. METHODS: In this translational study, we will adapt an established work-based injury prevention and health promotion program (COMmunity of Practice And Safety Support: COMPASS) to address the needs of HCWs experiencing chronic pain. COMPASS employs peer-led, scr

Clinical Observations in Patients With Cystic Fibrosis-Related Diabetes and Self-Reported Ototoxicity Symptoms.

PURPOSE: Persons with cystic fibrosis (PwCF) are at high risk for ototoxicity due to the routine use of intravenous aminoglycoside (IV-AG) antibiotics in respiratory infection management. Additionally, factors that contribute to ototoxicity-related symptom development and severity in PwCF are unknown. Given the increased risk of ototoxicity in people with diabetes, we explored the association between cystic fibrosis-related diabetes (CFRD) and self-reported ototoxicity symptoms (tinnitus and vestibular problems) in PwCF treated with aminoglycosides. METHOD: PwCF (N = 39; 25 females, 14 males;

A randomized controlled trial for a peer-facilitated telemedicine hepatitis c treatment intervention for people who use drugs in rural communities: study protocol for the "peer tele-HCV" study.

BACKGROUND: Hepatitis C virus (HCV) transmission is primarily driven by injection drug use, and acute HCV infection rates are increased in rural communities with substantial barriers to care. Treatment of HCV in persons who use drugs (PWUD) is cost effective, decreases high risk behaviors and HCV transmission, and achieves high rates of treatment completion and sustained viral response. Adapting HCV care delivery to utilize peer support specialists, telemedicine technology, and streamlined testing and treatment strategies can better reach rural populations living with HCV. METHODS: This is an

Bendamustine is safe and effective for lymphodepletion before tisagenlecleucel in patients with refractory or relapsed large B-cell lymphomas.

BackgroundAnti-CD19 chimeric antigen receptor T-cell immunotherapy (CAR-T) is now a standard treatment of relapsed or refractory B-cell non-Hodgkin lymphomas; however, a significant portion of patients do not respond to CAR-T and/or experience toxicities. Lymphodepleting chemotherapy is a critical component of CAR-T that enhances CAR-T-cell engraftment, expansion, cytotoxicity, and persistence. We hypothesized that the lymphodepletion regimen might affect the safety and efficacy of CAR-T.Patients and methodsWe compared the safety and efficacy of lymphodepletion using either fludarabine/cycloph

In-Home Mobility Frequency and Stability in Older Adults Living Alone With or Without MCI: Introduction of New Metrics.

Background: Older adults spend a considerable amount of time inside their residences; however, most research investigates out-of-home mobility and its health correlates. We measured indoor mobility using room-to-room transitions, tested their psychometric properties, and correlated indoor mobility with cognitive and functional status. Materials and Methods: Community-dwelling older adults living alone (n = 139; age = 78.1 ± 8.6 years) from the Oregon Center for Aging & Technology (ORCATECH) and Minority Aging Research Study (MARS) were included in the study. Two indoor mobility features were d

Enrichment of IGF-1R and PPARγ signalling pathways in orbital inflammatory diseases: steps toward understanding pathogenesis.

BACKGROUND: Orbital inflammatory disease (OID) encompasses a wide range of pathology including thyroid-associated orbitopathy (TAO), granulomatosis with polyangiitis (GPA), sarcoidosis and non-specific orbital inflammation (NSOI), accounting for up to 6% of orbital diseases. Understanding the underlying pathophysiology of OID can improve diagnosis and help target therapy. AIMS: To test the hypothesis that shared signalling pathways are activated in different forms of OID. METHODS: In this secondary analysis, pathway analysis was performed on the previously reported differentially expressed g

Evaluation of an Electronic Health Record (EHR) Tool for Integrated Behavioral Health in Primary Care.

BACKGROUND: Integrating behavioral health into primary care can improve care quality; however, most electronic health records are not designed to meet the needs of integrated teams. We worked with practices and behavioral health (BH) clinicians to design a suite of electronic health record tools to address these needs ("BH e-Suite"). The purpose of this article is to examine whether implementation of the BH e-Suite changes process of care, intermediate clinical outcomes, and patient experiences, and whether its use is acceptable to practice members and BH clinicians. METHODS: We conducted a c

Association Between Initial Opioid Prescribing Patterns and Subsequent Long-Term Use Among Opioid-Naïve Patients: A Statewide Retrospective Cohort Study.

BACKGROUND: Long-term efficacy of opioids for non-cancer pain is unproven, but risks argue for cautious prescribing. Few data suggest how long or how much opioid can be prescribed for opioid-naïve patients without inadvertently promoting long-term use. OBJECTIVE: To examine the association between initial opioid prescribing patterns and likelihood of long-term use among opioid-naïve patients. DESIGN: Retrospective cohort study; data from Oregon resident prescriptions linked to death certificates and hospital discharges. Magnetic resonance imaging and spectroscopy assessment of lower extremity skeletal muscles in boys with Duchenne muscular dystrophy: a multicenter cross sectional study.

INTRODUCTION: Duchenne muscular dystrophy (DMD) is an X-linked recessive disorder that results in functional deficits. However, these functional declines are often not able to be quantified in clinical trials for DMD until after age 7. In this study, we hypothesized that (1)H2O T2 derived using (1)H-MRS and MRI-T2 will be sensitive to muscle involvement at a young age (5-7 years) consistent with increased inflammation and muscle damage in a large cohort of DMD subjects compared to controls. METHODS: MR data were acquired from 123 boys with DMD (ages 5-14 years; mean 8.6 SD 2.2 years) and 31 h
