Clinical Features and Outcomes of Infections Caused by Metallo-β-Lactamase-Producing Enterobacterales: A 3-Year Prospective Study From an Endemic Area.

BACKGROUND: Metallo-β-lactamase (MBL)-producing Enterobacterales are increasing worldwide. Our aim was to describe clinical features, treatments, and outcomes of infections by MBL-Enterobacterales. METHODS: A prospective observational study conducted in the Pisa University Hospital (January 2019 to October 2022) included patients with MBL-producing Enterobacterales infections. The primary outcome measure was the 30-day mortality rate. Multivariable Cox regression analysis was performed to identify factors associated with that mortality rate, and adjusted hazard ratios (aHRs) and 95% confidenc

Determinants of antimicrobial resistance in biosolids: A systematic review, database, and meta-analysis.

Biosolids can provide a nutrient rich soil amendment, particularly for poor soils and semi-arid or drought-prone areas. However, there are concerns that sludge and biosolids could be a source of propagation and exposure to AMR determinants such as antibiotic resistant bacteria (ARB), and antibiotic resistance genes (ARGs). To inform risk assessment efforts, a systematic literature review was performed to build a comprehensive spreadsheet database of ARB and ARG concentrations in biosolids (and some sludges specified as intended for land application), along with 69 other quantitative and qualit

Whole genome sequence analysis of Aeromonas spp. isolated from ready-to-eat seafood: antimicrobial resistance and virulence factors.

Aeromonas are widespread in aquatic environments and are considered emerging pathogens in humans and animals. Multidrug resistant (MDR) Aeromonas circulating in the aquatic environment and food production chain can potentially disseminate antimicrobial resistance (AMR) to humans via the foodborne route. In this study, we aimed to investigate AMR and virulence factors of 22 Aeromonas strains isolated from ready-to-eat (RTE) seafood. A multilocus phylogenetic analysis (MLPA) using the concatenated sequences of six housekeeping genes (gyrB, rpoD, gyrA, recA, dnaJ, and dnaX) in the 22 Aeromonas ge

Coexistence of tmexCD-toprJ, blaNDM-1, and blaIMP-4 in One Plasmid Carried by Clinical Klebsiella spp.

In clinical practice, carbapenems and tigecycline are considered significant options for treating infections caused by multidrug-resistant Klebsiella spp. The continual evolution of resistance mechanisms to carbapenems and tigecycline is shattering the present condition. Meanwhile, convergence of the two resistance mechanisms in a single strain has been reported repeatedly, posing a significant threat to public health and safety. In this study, two carbapenem- and tigecycline-resistant Klebsiella species were obtained from patients and investigated using antimicrobial susceptibility testing, c

Genomic comparisons of Escherichia coli ST131 from Australia.

Escherichia coli ST131 is a globally dispersed extraintestinal pathogenic E. coli lineage contributing significantly to hospital and community acquired urinary tract and bloodstream infections. Here we describe a detailed phylogenetic analysis of the whole genome sequences of 284 Australian ST131 E. coli isolates from diverse sources, including clinical, food and companion animals, wildlife and the environment. Our phylogeny and the results of single nucleotide polymorphism (SNP) analysis show the typical ST131 clade distribution with clades A, B and C clearly displayed, but no niche associati

Emerging reservoirs of metallo-β-lactamase genes among non-aeruginosa Pseudomonas and Stutzerimonas species in a Spanish tertiary hospital.

INTRODUCTION: Pseudomonas is one of the largest genera among Gram-negative bacteria, currently comprising nearly 400 valid species. Among them, non-aeruginosa Pseudomonas species, together with Stutzerimonas-a genus recently separated from Pseudomonas-are becoming important reservoirs of antimicrobial resistance genes. However, accurate species-level identification of these organisms remains challenging when relying on the methods routinely implemented in most clinical microbiology laboratories, such as MALDI-TOF mass spectrometry. Therefore, this study aimed to identify and characterize metal

Fluoroquinolone resistant Escherichia coli in community: molecular analyses of resistance genes, virulence factors, and antimicrobial susceptibility test.

INTRODUCTION: Most urinary tract infections in all populations are caused by gram -negative, facultative anaerobic, uropathogenic E. coli (UPEC). Multidrug resistance (MDR) to first line antibiotics recommended for treatment of UTI is a global health problem. The misuse of antimicrobial agents for UTI treatment can lead to selection of bacterial MDR, able to transfer mechanism of resistance through mobile genetic elements (MGEs), such as transposons, integrons and conjugative plasmids. This study aims to characterize MLST, virulence and resistance genes to quinolones and β-lactams of UPEC samp

Novel bacterial hosts and mobile genetic structure of tet(X) variants in tetracycline-contaminated aquatic environment uncovered by culture and long-read metagenomics.

Clinically important tigecycline (3rd-generation tetracycline) resistance tet(X) variants were inferred to have evolutionarily originated from environmental bacteria, and have been recognized among environment, human and animals. However, genetic basis for environmental proliferation and dissemination of tet(X) variants remains ambiguous. This study profiled tet(X) variants at gene, contig, isolate, and community levels in environmental community subjected to long-term stepwise increasing oxytetracycline (1st-generation tetracycline) or tigecycline pressure using long-term microcosm experiment

Comprehensive genomic analysis of multidrug-resistant Pseudomonas aeruginosa isolates from companion animals in Germany.

Pseudomonas (P.) aeruginosa is a highly adaptable, opportunistic nosocomial pathogen that poses significant risks to public health and veterinary medicine. This bacterium carries a wide range of antimicrobial resistance (AMR) determinants and produces various virulence factors that enable it to invade hosts and increase disease severity. Recognised as a One Health pathogen, P. aeruginosa can be isolated from multiple sources, including humans, animals, food, and the environment. Despite its importance in clinical settings, there are still limited genomic and epidemiological data on P. aerugino
