[Effect of streptomycin placing in the fenestra of semicircular canals on the function and morphology of the normal and hydropic ears in guinea pigs].

The effect of streptomycin placed in the fenestra of lateral semicircular canal (LSC) on the function and morphology of the normal and artificial hydropic inner ears in guinea pigs was studied in order to explore the method and possibility of treating Meniere's disease by using aminoglycosides. After placing the drug in the fenestra of LSC, no change in AP threshold of ECochG in normal ears but an increase of that in hydropic ears were found caloric induced nystagmus in both groups were disappeared. Under light microscope, severe damage of vestibular epithelium of three canal cristae and utric

The Gut Microbiome May Play a Role in the Pathogenesis of Meniere's Disease.

Meniere's disease (MD) was first described 650 years ago. It is now considered to be a multifactorial disorder involving immunological mechanisms, blood-labyrinth barrier breakdown, endolymphatic hydrops, vascular compromise, and genetic susceptibility. Chronic inflammation from both innate and adaptive immunity is evident in the inner ear, with autoimmunity and allergy possibly playing a role. Despite its long history, significant knowledge gaps in its pathogenesis remain. For example, there may be root causes from elsewhere that are contributing to these pathological processes occurring in t

[Autoimmune hearing loss and a chance of its development in children: literature review and own observations].

Autoimmune sensorineural hearing loss (AiSNHL) is an uncommon auditory disorder characterized by rapidly progressive bilateral hearing loss and a positive clinical response to treatment with corticosteroids and cytostatics. The prevalence of the disease in the adult population is less than 1% among all cases of subacute and permanent sensorineural hearing loss (precise data are unknown), it is even rarer in children. AiSNHL can be primary (isolated, organ-specific) or secondary (manifestation of another systemic autoimmune disease). The pathogenesis of AiSNHL is based on the proliferation of a

Does Otosclerosis Affect Dark and Transitional Cells in the Human Vestibular Labyrinth?

HYPOTHESIS: The density of vestibular dark cells (DCs) and vestibular transitional cells (TCs) can be quantitatively decreased in human temporal bones with otosclerosis. BACKGROUND: Previous reports have shown that otosclerosis can lead to vestibular symptoms. METHODS: We examined 61 human temporal bone specimens from 52 deceased donors with otosclerosis group-with and without endosteal involvement (EI), and with and without endolymphatic hydrops (EH)-versus 25 specimens from 18 age-matched controls. Using light microscopy, we evaluated the nonsensory epithelium of the lateral semicircular c

Functional variants in NOS1 and NOS2A are not associated with progressive hearing loss in Ménière's disease in a European Caucasian population.

Hearing loss in Ménière's disease (MD) is associated with loss of spiral ganglion neurons and hair cells. In a guinea pig model of endolymphatic hydrops, nitric oxide synthases (NOS) and oxidative stress mediate loss of spiral ganglion neurons. To test the hypothesis that functional variants of NOS1 and NOS2A are associated with MD, we genotyped three functional variants of NOS1 (rs41279104, rs2682826, and a cytosine-adenosine microsatellite repeat in exon 1f) and the CCTTT repeat in the promoter of NOS2A gene (rs3833912) in two independent MD sets (273 patients in total) and 550 controls. Auditory function in women with autoimmune inner ear diseases and their offspring.

OBJECTIVE: The precise cause of congenital sensorineural hearing loss (CSNHL) is unclear in many cases. In a previous study we found that offspring from guinea pigs with autoimmune sensorineural hearing loss (ASNHL) exhibited signs of SNHL. Here we studied women with autoimmune inner ear diseases (AIED) and their offspring. Our aim was to determine if autoimmune damage may be one of the causes of CSNHL. METHODS: Thirty-eight pregnant women with AIED were recruited. Thirty-three had ASNHL; one with autoimmune delayed endolymphatic hydrops (ADEH) and four with autoimmune Meniere's disease (AIMD

[Effects of a change in environmental pressure on spontaneous nystagmus in patients with cochleovestibular diseases].

Cochlear and vestibular effects of variation of middle ear pressure have been discussed for more than 50 years. In 1975, Densert reported on temporary improvement of hearing threshold in patients suffering from Ménière's disease after reduction of air pressure in a pressure chamber. One year later Ingelstedt observed reduction of spontaneous nystagmus under this condition. The transmission of relative overpressure from the middle ear to the labyrinth with consecutive decongestion of the hydropic endolymphatic system to the saccus was thought to be the cause of this phenomenon. Meanwhile a dire

Analog model of human click-elicited SP and effects of high-pass filtering.

Recent advances in electrode design have helped to rekindle clinical interests in noninvasive electrocochleography--to enhance auditory brain stem response (ABR) recordings (namely, wave I) and to screen Meniere's disease. The salient feature of the response in suspected cases of endolymphatic hydrops is an unusually large summating potential (SP). Since the SP is a DC potential, conventional wisdom suggests the use of relatively low cutoff frequencies (i.e., long-time constants) for recording, but this may degrade the recording due to the presence of excessive low-frequency noise. However, th

Unidirectional inner ear valve implant for endolymphatic sac surgery in Meniere's disease.

In five patients with long-standing Meniere's disease, a unidirectional inner ear valve was implanted into the endolymphatic sac (ELS) to facilitate removal of excess endolymph in the hydropic state. These patients were all candidates for ablative surgery. They all had intractable vertigo and severe hearing losses. Four out of five patients had positive preoperative glycerol tests. In the patients with positive glycerol tests, the group averages before surgery were as follows: pure-tone average (PTA), 63 dB; speech reception threshold (SRT), 79 dB; and discrimination score, 27%. All of the pat

Intravenous Delayed Gadolinium-Enhanced MR Imaging of the Endolymphatic Space: A Methodological Comparative Study.

In-vivo non-invasive verification of endolymphatic hydrops (ELH) by means of intravenous delayed gadolinium (Gd) enhanced magnetic resonance imaging of the inner ear (iMRI) is rapidly developing into a standard clinical tool to investigate peripheral vestibulo-cochlear syndromes. In this context, methodological comparative studies providing standardization and comparability between labs seem even more important, but so far very few are available. One hundred eight participants [75 patients with Meniere's disease (MD; 55.2 ± 14.9 years) and 33 vestibular healthy controls (HC; 46.4 ± 15.6 years)

Noninvasive MRI assessment of cerebrospinal fluid pressure in different phases of Ménière's disease: a prospective study.

OBJECTIVES: This study aimed to determine whether there are differences of cerebrospinal fluid pressure (CSF-P) on patients with Ménière's disease (MD) during different phases. METHODS: Noninvasive CSF-P measurement was performed using MRI with a fat-suppressed fast recovery fast spin echo T2-weighted sequence, with the optic nerve subarachnoid space width (ONSASW) posterior to the globe serving as an indicator. Endolymphatic hydrops (EH) grades and hearing thresholds were analysed to investigate the potential correlations with CSF-P. RESULTS: A total of 66 participants were included. At the

Clinical doses of gadodiamide have no damaging effects on cochlear tissue in vitro and in vivo.

Gadolinium-based contrast agents (GBCAs) are widely used in systemic magnetic resonance imaging (MRI) and can be employed in otology to evaluate endolymphatic hydrops in patients with Ménière's disease. Given the heavy metal properties of gadolinium and its tendency to deposit in tissues, it is essential to assess its ototoxic risk. We evaluated the ototoxicity of gadodiamide using in vitro and in vivo models. In vitro, cochlear explants from postnatal day 3 rats were cultured for 24 h in medium containing 0, 100 (equivalent to the concentration in perilymph after intratympanic injection), 500

Assessing the efficacy of perilymphatic fistula repair surgery in alleviating vestibular symptoms and associated auditory impairments.

Perilymph Fistula (PLF), abnormal communication between the fluid-filled space of the inner ear and the air-filled space of the middle ear, is a significant cause of vestibular and auditory symptoms. This is a retrospective study of 22 cases treated with PLF repair surgery, selected based on our surgical indication. We analyzed the characteristics of these 22 cases and evaluated the efficacy of PLF repair surgery in treating vestibular and auditory symptoms. Cases with antecedent events had significantly shorter intervals before surgery. The postoperative recovery from vestibular symptoms foll

Methods for Testing the Subjective Visual Vertical during the Chronic Phase of Menière's Disease.

The subjective visual vertical (SVV) evaluates the function of the utricle, which, in patients with Menière's disease (MD), can be affected by endolymphatic hydrops. This study aimed to determine the SVV in MD patients during the chronic phase of illness compared to healthy participants. The second aim was to compare the SVV measurement tools: the analog bucket test, digital bucket test, and C-SVV© goggles. The SVV scores differed significantly between MD patients and the control group for the analog bucket test (p < 0.001) and the C-SVV® goggles (p = 0.028), but no significance was shown when
