BACKGROUND: Uganda began transitioning to Dolutegravir (DTG)-based antiretroviral therapy (ART) in 2018. However, patient-level longitudinal evidence quantifying the virological impact of this transition from Ugandan facilities is limited. We evaluated whether the programmatic transition to DTG improved viral load (VL) suppression at Mildmay Hospital Uganda and assessed whether benefits were uniform across age groups and sexes.
METHODS: We conducted a retrospective cohort study using 114,406 VL measurements from 18,756 patients collected between 2016 and 2023. We defined VL suppression as ≤ 2

HIV drug resistance (HIVDR) was retrospectively characterized among 20 children and adolescents with HIV with virologic failure on non-nucleoside reverse transcriptase inhibitor (NNRTI)-based therapy, and virologic response in those switched to dolutegravir (DTG)-based therapy described. All participants had at least one NNRTI resistance mutation, most commonly K103N (N = 12) and 15 (75%) had nucleoside reverse transcriptase inhibitor (NRTI) resistance mutations, most commonly M184I/V (N = 15). Five (45%) of 11 participants who were switched to DTG-based regimens for a median of 50 months had

BACKGROUND: Weight change for women living with HIV (WLWH) who receive dolutegravir (DTG) is understudied around pregnancy. The aim of this study was to investigate the direction and magnitude of weight change among WLWH pre-, during, and post-gestation based on DTG exposure history.
METHODS: This retrospective cohort study evaluated adult pregnant WLWH receiving antenatal care between 2016 and 2022 at two clinics in Nigeria and followed them over three 9-month periods (pregestational, antenatal, and postgestational). Patients were stratified into three DTG exposure groups for each follow-up

OBJECTIVES: Cameroon adopted dolutegravir-based regimen in 2020 as the first-line treatment of HIV infection, as recommended by the World Health Organization on the basis of efficacy, high genetic barriers to drug resistance, low toxicity, and low cost. Our main objective was to evaluate the efficacy of dolutegravir-based regimen among HIV-infected children.
PATIENTS AND METHODS: We performed a retrospective hospital-based cohort study over a 5-month period at the pediatric day care unit of Bamenda Regional Hospital among HIV-infected children aged 3-15 years. Data was collected using a prede

BACKGROUND: There are limited data on DTG-associated weight gain among treatment-naïve Asian people living with HIV (PLHIV).
METHODS: Eighty-one and 100 PLHIV initiating DTG-based and efavirenz (EFV)-based antiretroviral therapy (ART), respectively, were retrospectively investigated for weight changes and lipid profiles at 6 and 12 months after ART initiation.
RESULTS: At baseline, the DTG group had a lower mean body weight (BW) (60.7 kg vs. 64.3 kg, p = 0.071) and lower mean cholesterol level (172 mg/dL vs. 185 mg/dL, p = 0.029) than the EFV group. Mean BW remained lower in the DTG group th

BACKGROUND: Treatment failure is common among children and adolescents with HIV. Antiretroviral therapy (ART) containing dolutegravir has recently been rolled out across Africa, though long-term real-world data in paediatric populations are lacking. Here, we report treatment outcomes among children and adolescents in Lesotho who transitioned from nonnucleoside reverse transcriptase inhibitor (NNRTI)-based to dolutegravir-based ART through 2 years' follow-up.
METHODS: Data were derived from two open cohort studies in Lesotho. Children and adolescents aged less than 18 years who transitioned fr

People with HIV admitted to the intensive care unit (ITU) often receive enteral nutrition via feeding tubes. Enteral feeds containing cations may interfere with dolutegravir (DTG) absorption via chelation and thus temporal spacing is recommended. This can lead to suboptimal caloric intake. We reviewed similar cases at our institution, an electronic health record and data collection, including people with HIV hospitalized in ITU with enterally administered DTG and therapeutic drug monitoring (TDM). Six cases (four males, two females, median age 34 years, median ITU admission 44.5 days, median 4

Cabotegravir is a novel human immunodeficiency virus integrase enzyme inhibitor used for prevention and treatment of HIV infection. The combinational final dosage form, as extended release injection suspension in combination with rilpivirine and as cabotegravir tablets (for lead-in therapy), was recently approved in Canada, EU and in USA and is currently seeking approval also in other countries. The subject of this investigation was to study the degradation of cabotegravir under different stress conditions as per the International Council for Harmonization (ICH) guidelines. The drug substance

OBJECTIVES: Raltegravir is used in many antiretroviral combinations, but its use in treatment-experienced patients without knowledge of baseline resistance is discussed controversially as a number of comparative studies have shown a higher rate of virological failure. However, it has been used frequently for the management of treatment failure, as it was the first integrase inhibitor to become available, and thus offered new options for patients with multiple resistance. The strategic use of raltegravir in this setting is examined in this study.
METHODS: In order to examine the efficacy of ra

Diagnosis of tuberculous meningitis (TBM) is always a challenge. We must give importance for duration of clinical manifestations. Cerebrospinal fluid (CSF) has own characteristic and it have to be control several times during the treatment. Adenosin deaminase with cut off more than 15 UI/mL and M. tuberculosis polymerase chain reaction in CSF are the most relevant diagnostic tests. Radiologic test gives diagnostic clues but do not confirm the diagnosis. In the future we can structure a score with all these elements to support the clinician in the diagnostic process. The treatment of TBM becaus

Raltegravir is a novel HIV-1 integrase inhibitor with potent in vitro activity (95% inhibitory concentration = 33 nM in 50% human serum). In vitro characterization of raltegravir inhibition potential was assessed against a panel of cytochrome P450 (CYP) enzymes. An open-label, 2-period study was conducted to assess the effect of raltegravir on the pharmacokinetics of midazolam, a sensitive CYP 3A4 probe substrate: period 1, 2.0 mg of midazolam; period 2, 400 mg of raltegravir every 12 hours for 14 days with 2.0 mg of midazolam on day 14. There was no meaningful in vitro effect of raltegravir o

BACKGROUND: Rapid Start ensures that persons with HIV initiate antiretroviral therapy in less than seven days after diagnosis. Benefits of Rapid Start include timely linkage to medical care, viral suppression in a shorter time and improved retention to medical care. Despite these benefits, there is a slow uptake of Rapid Start, in New Jersey.
OBJECTIVE: Identify barriers to Rapid Start among New Jersey providers.
METHODS: An electronic survey, consisting of 28 questions, with the following domains was administered to New Jersey providers, using Qualtrics: provider and practice characteristic

BACKGROUND: Second-line treatment of HIV-2 in resource-limited settings (RLS) is complicated by a lack of controlled trial data, limited availability of HIV-2-active antiretroviral drugs, and inadequate access to drug resistance testing. We conducted an implementation trial of a dried blood spot- (DBS) based, drug resistance genotype-informed antiretroviral therapy (ART) switching algorithm for HIV-2-infected patients in Senegal.
METHODS: HIV-2-infected adults initiating or receiving ART through the Senegalese national AIDS program were invited to participate in this single-arm trial. DBS fro

BACKGROUND: Second-generation integrase strand transfer inhibitors (InSTIs) have a high barrier to resistance and potent antiretroviral activity. They are recommended as first- or second-line (FL and SL) options in two- and three-drug regimens (2DR and 3DR) in international treatment guidelines. However, there are limited real-world data on emerging resistance at the time of virological failure (VF) with these regimens.
OBJECTIVES: The Virostar-1 study objective is to analyse the emergence of resistance-associated mutations (RAMs) over 3 years with DTG-based 2DRs and DTG- or bictegravir (BIC)

BACKGROUND: Following a successful pilot study of a three-day-per-week regimen of efavirenz, emtricitabine, and tenofovir disoproxil fumarate, we hypothesized that this strategy would sustain virological efficacy and reduce long-term toxicities.
METHODS: After a 24-week randomized phase, participants in the A-TRI-WEEK trial (ClinicalTrials.gov NCT01778413) were offered the three-day-per-week regimen. The extension was approved by the Institutional Review Board, and participants provided informed consent; follow-up continued until further participation no longer offered additional clinical or

BACKGROUND: Metabolic syndrome (MetS) is an emerging complication among people living with HIV (PLHIV) receiving long-term antiretroviral therapy (ART), particularly with protease and integrase inhibitor regimens. Early identification of high-risk individuals remains challenging, and predictive tools are limited in African settings. This study evaluated nine machine learning (ML) algorithms for predicting MetS in ART-treated adults.
METHODS: We analysed a retrospective cohort of 1,027 PLHIV without baseline MetS, followed for 144 weeks; 854 with complete data were included. Nine ML algorithms

BACKGROUND: There are limited data on how historical nucleoside reverse transcriptase inhibitor (NRTI) resistance-associated mutations (RAMs) other than M184V/I, affect the activity of B/F/TAF. We evaluated the outcomes of switching virologically suppressed (HIV-1 RNA < 50 copies/mL) individuals harbouring major RAMs from boosted protease inhibitor (bPI)-based therapy to B/F/TAF.
METHODS: Participants had various historical genotypic patterns including M184V/I, ≤2 thymidine analogue mutations (TAMs), and other NRTI RAMs (NAMs), and no integrase resistance. Baseline RAMs were explored by retro
