Frontal lobe seizures: electroclinical syndromes.

To define further the electroclinical manifestations of frontal lobe epilepsy (FLE), we studied 150 seizures manifested by 24 patients; 18 patients had subdural electrode arrays (SEA). The findings in these patients clearly overlapped presumably reflecting the interconnections between functionally related frontal zones; yet the manner in which the symptoms clustered and the sequence in which they occurred generally indicated the anatomic site of the epileptogenic zone. We divided the patients into three major groups: (a) those with supplementary motor seizures, (b) those with focal motor seizu

Treatment specific competence predicts outcome in cognitive therapy for social anxiety disorder.

Several studies have demonstrated a positive relationship between competence and outcome in CBT for depression but studies of CBT for anxiety disorders are lacking. The present study explores the relationship between competence and outcome in cognitive therapy (CT) for social anxiety disorder, using hierarchical linear modeling analyses (HLM). Data were drawn from a multicenter randomized controlled trial. Five trained raters evaluated videotapes of two therapy sessions per patient using the Cognitive Therapy Competence Scale for Social Phobia (CTCS-SP). Overall adherence to the treatment manu

A novel hereditary spastic paraplegia with dystonia linked to chromosome 2q24-2q31.

Spastic paraplegias (HSPs) and dystonias (DYTs) typically localize to different neuroanatomic systems. We report clinical and genetic data from large Ohio kindred with autosomal dominant (AD) HSP and DYT. Single and multipoint linkage using microsatellite and single nucleotide polymorphism array genotyping were performed on a large, multigenerational family with a novel, AD, highly penetrant neurological disease causing spasticity and DYT. Age of onset of spasticity and weakness is from the first year to the sixth decade, and age of onset of DYT from the first to third decade. There is no atax

Could you be more specific, please: self-discrepancies, affect, and variation in specificity and relevance.

Multiple studies have suggested that the use of the Selves Questionnaire to assess self-discrepancies related to specific forms of affect may be problematic. The authors tested whether greater specificity in participant characteristics, study context, and form of assessment would increase the ability to detect the hypothesized differences. The Selves Questionnaire failed to show the hypothesized relations. A modified version of the Selves Questionnaire did show some of the hypothesized relationships; however, these relationships were stronger in regard to depression than anxiety, which also co

Examining subtypes of behavior problems among 3-year-old children, Part I: investigating validity of subtypes and biological risk-factors.

This study examined 3-year-old children who were classified as hyperactive (HYP), oppositional-defiant (OD), hyperactive and oppositional defiant (HYP/OD), and non-problem based on mothers' reports of behavior. Using fathers,' teachers,' and observers' ratings of children's behavior, concurrent validity was excellent for the HYP/OD group, moderate for the HYP group, and poor for the OD group. As predicted, both the HYP/OD and HYP groups reported more prenatal/perinatal birth complications and a greater family history of hyperactivity than did non-problem children. Furthermore, the HYP/OD group

Origin and propagation of epileptic spasms delineated on electrocorticography.

PURPOSE: Ictal electrographic changes were analyzed on intracranial electrocorticography (ECoG) in children with medically refractory epileptic spasms to assess the dynamic changes of ictal discharges associated with spasms and their relation to interictal epileptiform activity and neuroimaging findings. METHODS: We studied a consecutive series of 15 children (age 0.4 to 13 years; nine girls) with clusters of epileptic spasms recorded on prolonged intracranial subdural ECoG recordings, which were being performed for subsequent cortical resection, and in total, 62 spasms were analyzed by using

Value of non-invasive testing when there are independent bitemporal seizures in the scalp EEG.

We investigated the value of non-invasive data for predicting the outcome of intracranial EEG and anterior temporal lobectomy (ATL) (follow-up>1 year) in patients who have bitemporal independent seizures in the scalp EEG. No previous report has dealt with this patient group. Independent variables were duration of epilepsy, febrile seizures, interictal and ictal scalp EEG, ictal behavior, MRI, [18F]-fluorodeoxyglucose-PET (PET) and Wada test and dependent variables were surgical outcome (seizure free or not) and localized on intracranial EEG (finding all symptomatic seizures from one temporal l

3D analysis of nystagmus during peripheral vertiginous attacks.

In order to localize the site of lesion of peripheral vertigo, 3D analysis of nystagmus during peripheral vertiginous attacks was carried out. In comparison between the three components, the horizontal component had the largest ratio in each disease. Spontaneous nystagmus was directed toward the affected side in 3 cases and to the opposite side in 15. In patients with Menière's disease (MD), all subjects had horizontal and torsional components and had almost the same slow phase velocity in these two components. In patients with vestibular neuritis (VN) and Hunt's syndrome (HS), nystagmus was d

The effect of limb-length discrepancy on gait.

We evaluated the gait of thirty-five neurologically normal children who had a limb-length discrepancy of the lower extremities that ranged from 0.8 to 15.8 per cent of the length of the long extremity (0.6 to 11.1 centimeters). The twenty-two boys and thirteen girls had an average age of thirteen years (range, eight to seventeen years). No patient had a substantial angular or rotational deformity of the lower extremities. We found no correlation between the actual discrepancy or the per cent discrepancy and any of the dependent kinematic or kinetic variables, including pelvic obliquity. Discre

Saline injection needle-knife sphincterotomy: a preliminary report.

BACKGROUND AND STUDY AIMS: Needle-knife sphincterotomy is an established technique in the management of some patients with biliary tract obstruction. However, the technique can be technically difficult in patients with very small papillae, specially when associated with distorted ampullary and duodenal anatomy. We present here a technique that may enhance successful needle-knife sphincterotomy in this situation. PATIENTS AND METHODS: Eight patients with biliary tract obstruction who had small papillae and distorted ampullary or duodenal anatomy were evaluated. All patients had undergone one o

Glaucoma Patient Preferences for Video Education on Eye Drop Technique.

SIGNIFICANCE: Glaucoma patients express a strong need for practical instruction on instilling eye drops correctly. To maximize the benefit of a video intervention to improve eye drop technique, patients recommend that video education be provided both in the clinic setting and online. PURPOSE: The purposes of this study were to (1) describe glaucoma patients' perspectives on how to improve an online eye drop technique video and how to disseminate it to other glaucoma patients and (2) examine how these perspectives differ across demographics. METHODS: Glaucoma patients (N = 43) who viewed an o

A pilot randomized clinical trial of two medication adherence and drug use interventions for HIV+ crack cocaine users.

BACKGROUND: Crack cocaine use undermines adherence to highly active antiretroviral therapy (HAART). This pilot randomized clinical trial tested the feasibility and efficacy of 2 interventions based on the Information-Motivation-Behavioral Skill model to improve HAART adherence and reduce crack cocaine problems. METHODS: Participants were 54 adults with crack cocaine use and HIV with <90% HAART adherence. Most participants were African-American (82%) heterosexual (59%), and crack cocaine dependent (92%). Average adherence was 58% in the past 2 weeks. Average viral loads (VL) were detectable (l

Factors influencing preschool-age children's acceptance of models presented as overweight.

OBJECTIVE: To investigate whether preschool-age children's ratings of an overweight model were influenced by perceptions of responsibility for weight, type of play activity (i.e., running or cards), and information (i.e., positive or neutral). METHODS: A total of 152 children (74 boys), aged 3 to 6 years, were randomly assigned to watch two videotapes; one presented information about the model (positive or neutral) and one depicted the model engaged in an activity (running or playing cards). RESULTS: Children who blamed models for being overweight provided lower acceptance ratings than those

High seizure frequency prior to antiepileptic treatment is a predictor of pharmacoresistant epilepsy in a rat model of temporal lobe epilepsy.

PURPOSE: Progress in the management of patients with medically intractable epilepsy is impeded because we do not fully understand why pharmacoresistance happens and how it can be predicted. The presence of multiple seizures prior to medical treatment has been suggested as a potential predictor of poor outcome. In the present study, we used an animal model of temporal lobe epilepsy to investigate whether pharmacoresistant rats differ in seizure frequency from pharmacoresponsive animals. METHODS: Epilepsy with spontaneous recurrent seizures (SRS) was induced by status epilepticus. Frequency of

Symptoms of epilepsy and organic brain dysfunctions in patients with acute, brief depression combined with other fluctuating psychiatric symptoms: a controlled study from an acute psychiatric department.

BACKGROUND: In psychiatric acute departments some patients present with brief depressive periods accompanied with fluctuating arrays of other psychiatric symptoms like psychosis, panic or mania. For the purpose of the present study we call this condition Acute Unstable Depressive Syndrome (AUDS). The aims of the present study were to compare clinical signs of organic brain dysfunctions and epilepsy in patients with AUDS and Major Depressive Episode (MDE). METHODS: Out of 1038 consecutive patients admitted to a psychiatric acute ward, 16 patients with AUDS and 16 age- and gender-matched MDE pa

Do controlling maternal behaviours increase state anxiety in children's responses to a social threat? A pilot study.

Past research has demonstrated a link between controlling parenting and child anxiety. However, the causal nature of this association has not yet been established since most previous studies have utilised cross-sectional designs. The aim of the current study was to implement an experimental design to examine the impact of maternal control on children's state anxiety when faced with a social threat. Mothers of 26 children aged 7-13 years were randomly allocated to conditions in which they were either required to be overly controlling or minimally controlling during preparation of a practice spe

Videotaped experiments to drop safety behaviors and self-focused attention for patients with social anxiety disorder: do they change subjective and objective evaluations of anxiety and performance?

Safety behavior (SB) and self-focused attention (SFA) have been posited as important maintenance factors in the cognitive model of social anxiety disorder (SAD). The present study reports the results of experiments to drop SB and SFA among clinically diagnosed patients with SAD employing their own idiosyncratic anxiety-provoking situations. The ratings for observable anxiety, belief in feared outcome and overall performance were better for role plays without SB and SFA than for role plays with them. The degree of drop in SFA predicted drop in observable anxiety and belief in feared outcome. Dr

Content and style of speech from mothers with schizophrenia towards their infants.

Schizophrenia can affect verbal communication and relational processes, but how it might disrupt maternal infant-directed (ID) speech is unknown. Maternal speech characteristics were coded, blind to clinical information, from brief videotaped mother-infant interactions of 14 mothers with schizophrenia and 36 mothers with similar hospitalisation but with other clinical diagnoses. Compared with the non-schizophrenia group, the speech of mothers with schizophrenia was less infant-focused. Infant-focused content was also predicted by maternal age, but not by duration of hospitalisation or infant g

'Needs expressed' and 'offers of care': an observational study of mothers with somatisation disorder and their children.

BACKGROUND: The abnormal illness behaviours characterising somatisation disorder may be learnt responses acquired through exposure to parental illness and health anxiety in childhood. In this observational STUDY WE EXPLORE THIS HYPOTHESIS BY EXAMINING PATTERNS OF INTERACTION IN MOTHERS AND THEIR SCHOOL AGE CHILDREN. METHOD: A sample of 136 mother and child pairs in 3 groups (42 mothers with somatisation disorder, 44 organically ill mothers and 50 healthy mothers) completed a battery of self-report and interview measures. Their interaction in semi-structured play tasks and a meal was videotape
