Human atrial action potential and Ca2+ model: sinus rhythm and chronic atrial fibrillation.

RATIONALE: Understanding atrial fibrillation (AF) requires integrated understanding of ionic currents and Ca2+ transport in remodeled human atrium, but appropriate models are limited. OBJECTIVE: To study AF, we developed a new human atrial action potential (AP) model, derived from atrial experimental results and our human ventricular myocyte model. METHODS AND RESULTS: Atria versus ventricles have lower I(K1), resulting in more depolarized resting membrane potential (≈7 mV). We used higher I(to,fast) density in atrium, removed I(to,slow), and included an atrial-specific I(Kur). I(NCX) and I(

Diagnostic miscues in congenital long-QT syndrome.

BACKGROUND: Long-QT syndrome (LQTS) is a potentially lethal cardiac channelopathy that can be mistaken for palpitations, neurocardiogenic syncope, and epilepsy. Because of increased physician and public awareness of warning signs suggestive of LQTS, there is the potential for LQTS to be overdiagnosed. We sought to determine the agreement between the dismissal diagnosis from an LQTS subspecialty clinic and the original referral diagnosis. METHODS AND RESULTS: Data from the medical record were compared with data from the outside evaluation for 176 consecutive patients (121 females, median age 1

Sudden cardiac death in the young: single-center study of Bari autopsy cases.

INTRODUCTION: Sudden Cardiac Death (SCD) is one of the main causes of death in the world, with a significant impact especially on young people. Sudden Cardiac Death in the Young (SCDY) is characterized by multifactorial etiology, which includes cardiomyopathies, myocarditis, channelopathies, aortopathies and coronary artery diseases. Despite progress in prevention, a significant percentage of these deaths remain unexplained without a thorough autopsy. This study aims to SCDY cases registered between 2016 and 2024, exploring the association between type of autopsy, age, sex, causes of death and

Exome Sequencing and the Identification of New Genes and Shared Mechanisms in Polymicrogyria.

IMPORTANCE: Polymicrogyria is the most commonly diagnosed cortical malformation and is associated with neurodevelopmental sequelae including epilepsy, motor abnormalities, and cognitive deficits. Polymicrogyria frequently co-occurs with other brain malformations or as part of syndromic diseases. Past studies of polymicrogyria have defined heterogeneous genetic and nongenetic causes but have explained only a small fraction of cases. OBJECTIVE: To survey germline genetic causes of polymicrogyria in a large cohort and to consider novel polymicrogyria gene associations. DESIGN, SETTING, AND PART

An Exploration of Charge Compensating Ion Channels across the Phagocytic Vacuole of Neutrophils.

Neutrophils phagocytosing bacteria and fungi exhibit a burst of non-mitochondrial respiration that is required to kill and digest the engulfed microbes. This respiration is accomplished by the movement of electrons across the wall of the phagocytic vacuole by the neutrophil NADPH oxidase, NOX2. In this study, we have attempted to identify the non-proton ion channels or transporters involved in charge compensation by examining the effect of inhibitors on vacuolar pH and cross-sectional area, and on oxygen consumption. The chloride channel inhibitors 4-[(2-Butyl-6,7-dichloro-2-cyclopentyl-2,3-di

The non-dystrophic myotonias: molecular pathogenesis, diagnosis and treatment.

The non-dystrophic myotonias are an important group of skeletal muscle channelopathies electrophysiologically characterized by altered membrane excitability. Many distinct clinical phenotypes are now recognized and range in severity from severe neonatal myotonia with respiratory compromise through to milder late-onset myotonic muscle stiffness. Specific genetic mutations in the major skeletal muscle voltage gated chloride channel gene and in the voltage gated sodium channel gene are causative in most patients. Recent work has allowed more precise correlations between the genotype and the elect

Resilience to Endoplasmic Reticulum Stress Mitigates Calcium-Dependent Membrane Hyperexcitability Underlying Late Disease Onset in SCA6.

UNLABELLED: An enduring puzzle in many inherited neurological disorders is the late onset of symptoms despite expression of function-impairing mutant protein early in life. We examined the basis for onset of impairment in Spinocerebellar ataxia type 6 (SCA6), a canonical late-onset neurodegenerative ataxia which results from a polyglutamine expansion in the voltage gated calcium channel, Cav2.1. Cerebellar Purkinje cell spiking abnormalities are seen concurrent with motor impairment in SCA6 mice but the basis for these changes in spiking is unknown. We identify endoplasmic reticulum (ER) calci

Systemic gene expression mapping of voltage-gated calcium channel α1-subunits in cats.

BACKGROUND: In veterinary medicine, voltage-gated calcium channels (VGCCs) are key drug targets. Calcium channel blockers (CCBs) are the standard treatment for common feline diseases, such as systemic hypertension and hypertrophic cardiomyopathy. However, therapeutic strategies are limited due to the adverse effects of existing drugs like amlodipine, such as activation of the renin-angiotensin-aldosterone system. A critical lack of feline-specific data on the tissue distribution of VGCC subtypes fundamentally hinders the safe and effective application of CCBs in cats. This is because knowledge

Ion channels alterations in the forebrain of high-fat diet fed rats.

Evidence suggests that transient receptor potential (TRP) ion channels dysfunction significantly contributes to the physiopathology of metabolic and neurological disorders. Dysregulation in functions and expression in genes encoding the TRP channels cause several inherited diseases in humans (the so-called 'TRP channelopathies'), which affect the cardiovascular, renal, skeletal, and nervous systems. This study aimed to evaluate the expression of ion channels in the forebrain of rats with diet-induced obesity (DIO). DIO rats were studied after 17 weeks under a hypercaloric diet (high-fat diet,

Critical effect of Helicobacter pylori infection on the effectiveness of omeprazole for prevention of gastric or duodenal ulcers among chronic NSAID users.

Long QT syndrome (LQTS) is the prototype of the cardiac ion channelopathies which cause syncope and sudden death. LQT1, due to mutations of KCNQ1 (KVLQT1), is the most common form. This study describes the genotype-phenotype characteristics in 10 families with mutations of KCNQ1, including 5 novel mutations. One hundred and two families with a history of lethal cardiac events, 55 LQTS, 9 Brugada syndrome, 18 idiopathic ventricular fibrillation (IVF), and 20 acquired LQTS, were studied by single-strand conformational polymorphism (SSCP) and DNA sequence analyzes. Families found to have KCNQ1 mu

Left ventricular hypertrabeculation is a novel predictor of life-threatening arrhythmic events in long QT syndrome patients.

BACKGROUND: Long QT syndrome (LQTS) is a malignant cardiac channelopathy for patients with traditionally normal heart structures. Sporadic cases reported left ventricular hypertrabeculation (LVHT) was a deadly double hit in LQTS patients. Here we summarize the prevalence of LVHT within LQTS subjects and analyse the correlation of LVHT with life-threatening arrhythmic events (LAEs). METHODS: The study cohort consisted of 170 genotype-confirmed LQTS patients, which were classified to two groups, including non-left ventricular hypertrabeculation LQTS patients(LQTS-N-LVHT group) and LQTS-LVHT co-

Decreased vector magnitudes may help identify events in patients with Long QT syndrome.

INTRODUCTION: All Long QT syndrome (LQTS) patients are at elevated risk for channelopathy-induced delayed myocardial repolarization and consequently potentially life-threatening cardiac events with 90% of initial cardiac events occurring between preteen and 40 years old. Utilizing ECG and derived vectorcardiographic parameters, including T wave Vector Magnitude (TwVM) measurement data, this study attempts to determine whether TwVM from baseline ECGs is effectively predictive of future cardiac events for genotype-positive LQTS patients. METHODS: Verified carriers of established LQTS disease-ca

Ventricular Fibrillation in an Afebrile COVID-19 Patient Presenting With Transient Type-I Brugada Pattern.

COVID-19 has been associated with an increased risk of both atrial and ventricular arrhythmias. Brugada syndrome (BrS), an inherited sodium channelopathy presenting with a characteristic ECG morphology, confers a baseline risk of ventricular arrhythmias such as ventricular fibrillation (VF), especially during febrile illnesses. However, mimics of BrS, termed Brugada phenocopies (BrP), have been noted in association with fever, electrolyte abnormalities, and toxidromes outside of viral illness. Such presentations manifest the same ECG pattern, the type-I Brugada pattern (type-I BP). Thus, the a

Disruption of an EAAT-Mediated Chloride Channel in a Drosophila Model of Ataxia.

UNLABELLED: Patients with Type 6 episodic ataxia (EA6) have mutations of the excitatory amino acid transporter EAAT1 (also known as GLAST), but the underlying pathophysiological mechanism for EA6 is not known. EAAT1 is a glutamate transporter expressed by astrocytes and other glia, and it serves dual function as an anion channel. One EA6-associated mutation is a P>R substitution (EAAT1(P>R)) that in transfected cells has a reduced rate of glutamate transport and an abnormal anion conductance. We expressed this EAAT1(P>R) mutation in glial cells of Drosophila larvae and found that these larvae

Making prudent recommendations for return-to-play in adult athletes with cardiac conditions.

Clinicians who treat millions of adult athletes throughout the world may be faced with participation or return-to-play decisions in individuals with known or suspected cardiac conditions. Here we review existing published participation guidelines and analyze emerging data from ongoing registries and population-based studies pertaining to return-to-play decisions for cardiac conditions specifically affecting adult athletes. Considerations related to return-to-play decisions will vary according to age of the athlete, with inherited disorders being the main consideration in younger adult athletes
