Prevention of visual field defect after macular hole surgery by passing air used for fluid-air exchange through water.

PURPOSE: To evaluate the effect on peripheral visual field defects occurring after macular hole surgery of passing air that is used for fluid-air exchange through water before infusion to the eye. METHODS: We used Goldmann perimetry to measure the visual fields of patients who underwent vitrectomy to manage idiopathic full-thickness macular holes using either room air or humidified air for fluid-air exchange. We retrospectively studied visual fields measured in 39 patients (group 1) who underwent vitrectomy without humidification of air and prospectively studied visual fields measured in 33 p

Silicone oil tamponade to seal macular holes without position restrictions.

OBJECTIVE: The authors performed a study to determine the effectiveness and safety of silicone oil as a substitute for gas to fill the vitreous cavity to treat macular holes. DESIGN: Multicenter, nonrandomized, interventional trial. PARTICIPANTS: Thirty-seven consecutive patients chose vitrectomy with silicone tamponade instead of gas to treat 40 eyes with stage-2 to stage-4 idiopathic age-related macular holes. Stage-2 holes constituted 40% of the holes, and stage-3 and stage-4 holes made up 60%. INTERVENTION: All eyes were treated with vitrectomy, manual detachment of the posterior vitreo

Clinical characteristics and surgical outcomes of secondary full-thickness macular holes developing after vitrectomy.

AIM: To describe the clinical features, anatomical and visual outcomes of secondary full-thickness macular holes (sFTMH) developing after pars plana vitrectomy (PPV). METHODS: The medical charts of patients from nine vitreoretinal services across the UK who, between 2009 and 2023, underwent PPV for different indications and subsequently developed sFTMH were retrospectively reviewed. Primary outcomes were clinical features, macular hole closure rate and change in best corrected visual acuity (BCVA). Patients with idiopathic FTMH (iFTMH) who underwent surgical repair in the same period acted as

A two sample Mendelian randomized study of the association of sex hormones and behavioral and clinical risk factors with macular hole.

Macular hole (MH) is a disease of the vitreoretinal interface that develops in relation to age and gender, and is 3.3 times more prevalent in females than in males. However, it remains inconclusive whether gender plays a role in the pathogenesis of MH. We adopted a two-sample Mendelian randomisation (MR) analysis to explore the relationship between free testosterone, bioavailable testosterone, oestradiol, menopause, smoking, alcohol consumption, type 2 diabetes, diastolic blood pressure, and systolic blood pressure and the risk of MH. We found that genetically predicted free testosterone level

Traumatic globe rupture after deep anterior lamellar keratoplasty-A novel management technique.

A traumatic wound dehiscence can occur many years after surgery in 2-6% of all keratoplasties. Intraocular tissue prolapse can lead to severe visual loss. Deep anterior lamellar keratoplasty (DALK) leads to higher wound stability because of the intact Descemet membrane. We report a case of a novel management of wound dehiscence following DALK. A 59-year-old patient underwent uncomplicated DALK for advanced keratoconus. Eighteen months later he experienced a traumatic wound dehiscence (globe rupture) after a fall. Visual acuity decreased to light perception on the affected eye, the iris and pro

Retinal tear and raised intraocular pressure following unintentional intraocular botulinum toxin type A injection.

CASE REPORT: A 67-year-old woman with a 30-year history of dysthyroid orbitopathy and diplopia had an inadvertent injection of botulinum toxin type A into the vitreous cavity. Full pupil mydriasis and an intraocular pressure (IOP) of 50 mm Hg in the right eye were noted after the injection. An attack of acute angle closure was initially suspected despite a patent peripheral iridotomy (PI). The episode resolved with topical and intravenous IOP-lowering agents. Postoperative retinal examination revealed a retinal tear, which was successfully treated with laser photocoagulation. Visual acuity ret
