Right thalamic hemorrhage resulting from high-voltage electrical injury: a case report.

A 12-year-old boy was admitted with electrical burn and loss of consciousness. On physical examination his general condition was poor. Extensive burn areas, second and third degree, were present on his face, scalp, bilateral auricles, right cervical region, shoulders, right axilla, upper region of the thorax, and proximal region of the upper extremities. The total burned surface area was about 25%. Pupils were isocoric, but response to light was bilateral poor. He was stuporous and responsive only to pain. Deep tendon reflexes were exaggerated and plantar responses were bilateral extensor. Bil

Multiple Basal Cell Carcinomas in an Adolescent with an MLPA-Detected PTCH1-Region Deletion and a Gorlin-Goltz Phenotype.

Gorlin-Goltz syndrome, also known as nevoid basal cell carcinoma syndrome or basal cell naevus syndrome, is a rare hereditary cancer-predisposition disorder most often caused by disruption of the Hedgehog signaling pathway, particularly alterations of PTCH1. Classical disease is characterized by multiple or early-onset basal cell carcinomas, odontogenic keratocysts, palmoplantar pits, skeletal anomalies and intracranial calcifications. Deletions involving chromosome region 9q22.3 may create an overlapping but broader phenotype because the deleted interval may include PTCH1 together with neighb

Neuroscience-based relational art therapy and deep brain reorienting in the treatment of dissociative identity disorder.

Art therapy (AT) has been proposed as a treatment for post-traumatic conditions, potentially by providing somatic sensory input that can (i) enhance the client's sense of self and embodiment, (ii) modulate arousal, and (iii) aid in rethinking and reframing traumatic memories. However, evidence supporting AT as a treatment for dissociative disorders remains limited. The theoretical basis for the efficacy of AT is discussed in relation to findings regarding the traumatized person's brain and mindset, as well as its altered functional network connectivity. It is crucial to consider specific alter

Speech Sound Disorders in Children: An Articulatory Phonology Perspective.

Speech Sound Disorders (SSDs) is a generic term used to describe a range of difficulties producing speech sounds in children (McLeod and Baker, 2017). The foundations of clinical assessment, classification and intervention for children with SSD have been heavily influenced by psycholinguistic theory and procedures, which largely posit a firm boundary between phonological processes and phonetics/articulation (Shriberg, 2010). Thus, in many current SSD classification systems the complex relationships between the etiology (distal), processing deficits (proximal) and the behavioral levels (speech

Speech Outcomes After Primary Palatoplasty and Presurgical Cleft Characteristics: Is There a Predictive Association?

Speech disorders related to cleft lip and palate exhibit different degrees of involvement and can occur even after primary palate repair. Hypernasality can be present as a result of velopharyngeal insufficiency, as well as nasal emission, weak pressure, articulatory errors and facial grimace, affecting speech intelligibility. Palatoplasty outcomes can be variable, and among the influencing factors are the surgical technique, the surgeon's experience, the postoperative care, and the patient/cleft characteristics. The aim of this study was to correlate speech results after primary palate repair

Ingressive speech errors: a service evaluation of speech-sound therapy in a child aged 4;6.

BACKGROUND: A pattern of ingressive substitutions for word-final sibilants can be identified in a small number of cases in child speech disorder, with growing evidence suggesting it is a phonological difficulty, despite the unusual surface form. Phonological difficulty implies a problem with the cognitive process of organizing speech into sound contrasts. AIMS: To evaluate phonological therapy approaches in the remediation of non-pulmonic speech errors. Thus, adding to evidence concerning the nature of ingressive substitutions and their remediation whilst highlighting their occurrence within

New Zealand military Veterans' perceptions on health and well-being: A qualitative study.

INTRODUCTION: The study "The psychological and physical health and well-being of New Zealand contemporary Veterans" represents one of the few large population studies conducted on contemporary Veterans in New Zealand. Included in this survey was an open-ended question allowing respondents to articulate their thoughts, opinions, or feedback on the Veteran experience. METHODS: Responses were analyzed using qualitative content analysis framework informed by Mayring's (2022) eight-step content analysis process. A hybrid deductive-inductive approach was used to generate sub-themes from participant

To Relax or Not to Relax for Anxious Youth? Findings from a Survey of Exposure Specialists.

BACKGROUND: Relaxation is a commonly used, albeit controversial, therapy technique to support anxious youth. Increasingly, clinical trial data suggest little added benefit, and potentially a contraindication, for the use of relaxation strategies for pediatric anxiety and obsessive-compulsive disorders above and beyond that of exposure therapy. However, many clinicians, even those with substantial experience delivering exposure therapy, will at times incorporate relaxation into their treatment. How and when it is best to do so remains poorly articulated in the clinical trial literature. Delinea

Central nervous system control of breathing in natural conversation turn-taking.

Conversations-hallmarks of human social interaction-rely on a deeply rooted evolutionary tool: vocalization. Vocalization productions begin with an airflow from the lungs, shaped by the vocal folds and articulators of the mouth to generate a rich variety of sounds. Speech production therefore involves tight synchronisation between respiratory and articulatory events, especially in natural social interactions. During conversations, turn-taking indeed implies respiration anticipation: lungs must be filled with enough air to produce the next speech segment. Here, we use an fMRI corpus of live, un

Examining Valued Social Participation of Disabled Older Adults Through the Frameworks of Critical Gerontology and Critical Disability Studies.

BACKGROUND AND OBJECTIVES: The importance of social participation for older adults has been well articulated. Missing from this discourse is a critical consideration of how social participation is shaped by political, economic, and social contexts that marginalize aging and disabled bodies. We bridge this gap by applying critical gerontology and critical disability frameworks to our analysis of how access to health and social services and individual and environmental factors are associated with engagement in valued social activities among disabled older adults. RESEARCH DESIGN AND METHODS: Us

Experiences with family relationships following eating disorders: a roller coaster of emotions.

BACKGROUND: Eating disorders (EDs) are life-threatening illnesses that affect both patients and their families. When a member of the family has an ED, family life is necessarily impacted, as are intrafamily relationships and communication. We aimed to shed light on patients' and their siblings' experiences with their family relationships during and after EDs. METHODS: Our study's sample included eight young-adult female patients previously diagnosed with anorexia nervosa (n = 6) or anorexia nervosa and bulimia nervosa (n = 2) along with two of their siblings. All patients received treatment f

Alexithymia in Parkinson's Disease: A Meta-analysis.

Parkinson's disease (PD) is a neurodegenerative disorder characterized by motor and non-motor manifestations, including alexithymia. This condition is defined by difficulty in recognizing, articulating, and expressing one's emotional states. In this study, we conducted a systematic review and meta-analysis to compare the prevalence of alexithymia in PD patients and a healthy population, and to identify associated demographic and clinical factors. We identified 16 observational studies through Pubmed, EMBASE, PsycINFO, and SCOPUS, selecting articles published since 2002. Data were analyzed usin

The Acoustic Voice Quality Index (AVQI) in People with Parkinson's Disease Before and After Intensive Voice and Articulation Therapies: Secondary Outcome of a Randomized Controlled Trial.

OBJECTIVES: The majority of individuals with Parkinson's disease (PD) experience voice and speech problems during the course of the disease. Despite the importance of voice quality in communication and the documented disordered voice quality in PD, few studies have explored the effects of speech treatment on this variable. STUDY DESIGN/METHODS: A parallel arm, unblinded randomized controlled trial (RCT) was conducted with two active comparators, LSVT LOUD (n = 23) and LSVT ARTIC (n = 20), and an inactive comparator group of untreated individuals with PD (n = 22). Unique shape variations of hind and midfoot bones in flatfoot subjects-A statistical shape modeling approach.

Flatfoot deformity is a prevalent hind- and midfoot disorder. Given its complexity, single-plane radiological measurements omit case-specific joint interaction and bone shape variations. Three-dimensional medical imaging assessment using statistical shape models provides a complete approach in characterizing bone shape variations unique to flatfoot condition. This study used statistical shape models to define specific bone shape variations of the subtalar, talonavicular, and calcaneocuboid joints that characterize flatfoot deformity, that differentiate them from healthy controls. Bones of the

The impact of Marfan syndrome on an Aboriginal Australian family: 'I don't like it as much as I don't like cancer'.

Marfan syndrome (MFS) is an autosomal dominantly inherited connective tissue disorder. Aortic dilatation/dissection and ectopia lentis are the most severe features, which affect physical functioning and psychological well-being. In Aboriginal Australians, there is little psychosocial research on genetic conditions. This study explored the physical, psychological, and practical impacts of MFS on Aboriginal Australians. Eighteen (8 affected and 10 unaffected) members of a large Aboriginal Australian family with MFS participated in an ethically approved study. Semi-structured qualitative intervie

Deep Brain Stimulation in KMT2B-Related Dystonia: Case Report and Review of the Literature With Special Emphasis on Dysarthria and Speech.

Objective: KMT2B-related dystonia is a progressive childhood-onset movement disorder, evolving from lower-limb focal dystonia into generalized dystonia. With increasing age, children frequently show prominent laryngeal or facial dystonia manifesting in dysarthria. Bilateral deep brain stimulation of the globus pallidus internus (GPi-DBS) is reported to be an efficient therapeutic option. Especially improvement of dystonia and regaining of independent mobility is commonly described, but detailed information about the impact of GPi-DBS on dysarthria and speech is scarce. Methods: We report the 1

What We Know About the Pathogenesis of Idiopathic Pulmonary Fibrosis.

Idiopathic pulmonary fibrosis (IPF) is a chronic progressive lung disease of unknown cause, occurring in adults, limited to the lungs and associated with the pathologic and radiologic pattern of usual interstitial pneumonia. Prognosis is poor, and most patients die of respiratory failure within 3 to 6 years from the onset of symptoms. Although our understanding of the pathogenesis of IPF has improved over the past two decades, the mechanisms responsible for this disorder have not been clearly defined. Aging is the single most important risk factor, but genetic, environmental, and diverse exoge
