Miniaturization of sebaceous glands: A novel histopathological finding in pemphigus vulgaris and pemphigus foliaceus of the scalp.

BACKGROUND: Intraepidermal acantholysis is a characteristic histopathological feature of pemphigus. The histopathology of scalp biopsies in pemphigus has not been widely discussed in the literature, although the scalp is commonly involved in pemphigus. METHODS: A total of 32 consecutive patients with scalp involvement (17 with pemphigus vulgaris [PV] and 15 with pemphigus foliaceus [PF]) were enrolled into the study. The histopathological examination of scalp biopsies was performed in all patients. Tissue specimens were stained with hematoxylin and eosin. RESULTS: Acantholysis extended down

Ultrastructural alterations associated with in vivo and in vitro bound pemphigus antibodies in cultured oral epithelial cells.

Pemphigus vulgaris is characterized by bound and circulating IgG antibodies to an epithelial antigen, antibody binding resulting in acantholysis. This study examines the role of pemphigus serum in the initiation and perpetuation of acantholysis. Oral epithelial cells derived from a patient with pemphigus vulgaris were cultured in vitro in the presence of normal serum. Also, normal oral epithelial cells were cultured in pemphigus serum for between 0.5 and 72 h or exposed to pemphigus serum for 72 h and then cultured in normal serum for 2 wk. Oral epithelial cells from the pemphigus patient cont

Light and electron microscopic aspects of pemphigus herpetiformis (eosinophilic spongiosis) in comparison with other acantholytic disorders.

Histopathological and electron microscopic analyses of pemphigus herpetiformis in a 64-year old woman with a previously operated atoxic goitre and suffering from a nephrotic syndrome and a chronic non-aggressive hepatitis revealed acantholysis preceded by eosinophilic spongiosis. Light microscopically the acantholysis is characterized by the occurrence of necrobiotic epidermal cells. No dyskeratotic cells are found. The mechanism of acantholysis depends on an exuberant elongation of desmosomes, leading to their disruption, spongiotic acantholysis. A conspicuous convolution of specific tight in

A possible mechanism for phenol-induced pemphigus.

A possible mechanism for phenol-induced pemphigus lesions in genetically predisposed individuals is proposed that accounts for in vitro observations and cases of biochemical acantholysis, as well as the in vivo acantholysis in pemphigus induced by phenols. The mechanism involves the induction of interleukin-1a and tumor necrosis factor-a release by keratinocytes. These cytokines in turn have been shown to be involved in the regulation and synthesis of complement and proteases like plasminogen activator, which have been implicated in the pathogenesis of acantholysis in pemphigus vulgaris.

Localized pemphigus foliaceus exclusively involving the follicular infundibulum: a novel peau d'orange appearance.

Localized pemphigus foliaceus (PF) clinically presents as red plaques with scale and crust due to acantholysis of the epidermis. The follicular infundibulum shows acantholysis, but usually with epidermal acantholysis or at least erosion. Localized PF exclusively involving the follicular infundibulum, which shows a peau d'orange appearance, has never been reported. We describe a patient with a red plaque and peau d'orange appearance on the cheek for 20 years. Dyskeratotic acantholysis in the follicular infundibulum was overlooked when the first biopsy specimen was obtained, and a disseminated l

[A time-course observation of influence of pemphigus autoantibody upon desomosome].

OBJECTIVE: To observe the time-course changes of desmosome and its dg2/3 component in the process of acantholysis. METHOD: After setting up a pemphigus organ model and an acantholysis model by purified pemphigus autoantibody, the desmosome structures at light and electromicroscopic levels and desmoglycoprotein dg2/3 immunohistochemical stains were observed at different culture times. RESULTS: In pemphigus organ model and acantholysis model, pemphigus autoantibody only induced the increase of the intercellular space at 24 hour. Even at 48 hour, only a few desmosomes were damaged. But by 72 ho

Pemphigus vulgaris: recent advances in our understanding of its pathogenesis.

Pemphigus, a group of bullous diseases affecting the oral mucosa and the skin, is caused by antibody-mediated autoimmune reaction to desmogleins (Dsg), desmosomal transmembrane glycoproteins, leading to acantholysis. Pemphigus is classified into pemphigus vulgaris (PV), with suprabasal acantholysis, and pemphigus foliaceus (PF), with acantholysis in the more superficial epidermis. Pemphigus vulgaris is characterized by IgG autoantibodies against desmoglein 3 (Dsg 3), whereas the target of PF is Dsg1, although about 50% of PV patients also have Dsg1 autoantibodies. The clinical phenotype appear

Acantholytic Nodular Basal Cell Carcinoma.

Acantholysis is a microscopic finding describing the breakdown of desmosomes of keratinocytes and the formation of intraepithelial clefts after the loss of cohesion of keratinocytes. It can be observed in keratinocytic neoplasms, typically actinic keratoses and squamous cell carcinomas, and defines the acantholytic variants of these entities. Acantholysis has so far been reported in only 4 cases of basal cell carcinomas (BCCs), mainly of the superficial type. A case of an otherwise typical nodular BCC showing features of acantholysis is presented here. Because BCCs are keratinocytic neoplasms,

Apoptosis in pemphigus.

Pemphigus is a severe autoimmune blistering skin disease. The two main forms of pemphigus, pemphigus vulgaris and pemphigus foliaceus, are primarily caused by autoantibodies against the desmosomal cadherins desmoglein 1 and desmoglein 3. The histopathological hallmark is the loss of cell adhesion between neighbouring keratinocytes, a phenomenon called acantholysis. Various underlying mechanisms of acantholysis have been described including apoptosis. The role of apoptosis in pemphigus pathogenesis is unclear at present and the focus of this review. While characteristic signs of apoptosis can b

Dermatoses affecting desmosomes in animals: a mechanistic review of acantholytic blistering skin diseases.

Failure of desmosomal adhesion with ensuing keratinocyte separation - a phenomenon called acantholysis - can result from genetic, autoimmune or infectious proteolytic causes. Rare hereditary disorders of desmosomal formation have been identified in animals. Familial acantholysis of Angus calves and hereditary suprabasal acantholytic mechanobullous dermatosis of buffaloes appear to be similar to acantholytic epidermolysis bullosa of human beings. A genetic acantholytic dermatosis resembling human Darier disease has been rarely recognized in dogs. In autoimmune blistering dermatoses, circulating

Novel mechanisms of target cell death and survival and of therapeutic action of IVIg in Pemphigus.

Pemphigus vulgaris (PV) is a potentially lethal mucocutaneous blistering disease characterized by cell-cell detachment within the stratified epithelium (acantholysis) caused by IgG autoantibodies. Intravenous immunoglobulin (IVIg) therapy effectively treats PV, but the mechanism is not fully understood. To further understand acantholysis and the efficacy of IVIg, we measured effects of IgG fractions from PV patients on keratinocyte death processes. Using IgGs from representative PV patients who improved with IVIg, we identified apoptotic and oncotic signaling pathways in in vitro and in vivo P

Acantholytic variant of seborrheic keratosis.

From among 524 histologic specimens of seborrheic keratosis (SK), 29 showed acantholysis, not related to pemphigus, Darier's disease, actinic keratosis or acantholytic squamous cell carcinoma. Acantholysis was found in 24 (23.3%) of the irritated type of SK, almost exclusively in the squamous cell nests showing dyskeratosis and spongiosis between and around squamous eddies or horn cysts. Clinically, 65.5% of the lesions were located on the face and scalp, 20.7% on the trunk and 13.8% on the limbs. It was speculated that dyskeratotic, degenerative changes of the keratinocytes together with spon

Role of plasminogen activator in pemphigus vulgaris.

The role of plasminogen activator (PA) in the pathogenesis of acantholysis in canine pemphigus vulgaris (PV) was evaluated using differentiated cultures of canine oral keratinocytes. Both the secreted and cell-associated PA activity in cultured canine keratinocytes were completely inhibited by specific anti-urokinase antibodies. Anti-tissue type PA antibodies did not inhibit either secreted or cell-associated PA activity. Immunoblots and fibrin zymography revealed a single 57,000 molecular weight urokinase-type PA in the conditioned media of the canine oral keratinocytes. Incubation of the dif

Pemphigus.

The term pemphigus refers to a group of autoimmune intraepidermal blistering diseases of the skin and mucous membranes. Several clinical variants of pemphigus are recognized. The major histologic feature of all variants is acantholysis, the disruption of normal cell-to-cell adhesion, which leads to intraepidermal blister formation. Most patients with pemphigus demonstrate IgG autoantibodies directed against an antigen located on the surface of keratinocytes. Although the stimulus for autoantibody production is unknown, several mechanisms have been proposed to explain the pathogenesis of acanth

Immunolocalization of desmoglein I ("band 3" polypeptide) on acantholytic cells in pemphigus vulgaris, Darier's disease, and Hailey-Hailey's disease.

Acantholysis is defined as loss of coherence between epithelial cells and is histologically shown in several bullous diseases. It was postulated that desmoglein I, one of the major transmembrane glycoproteins of the desmosome, may adhere to the attachment plaque inside the cell and contribute to desmoglea outside the cell. In this study we used a well characterized antibody against desmoglein I for immunofluorescence and immunoelectron microscopic techniques on 2 cases each of pemphigus vulgaris and Darier's disease and one case of Hailey-Hailey's disease. In the normal epidermis desmosomes we

Drug-induced pemphigus.

Drug-induced pemphigus is a heterogenous group of disorders in which a drug induces acantholysis. The majority of patients have immune features of pemphigus and have a course similar to idiopathic pemphigus. Few patients do not have a detectable autoimmune process, and their eruption usually resolves with discontinuation of the associated drug. The mechanism of induction of the autoimmune process and acantholysis is not clear.

Disseminated papular acantholytic dyskeratosis.

We present two cases of elderly women with clinically atypical transient disseminated papular eruption, showing suprabasal epidermal clefts with dyskeratosis and acantholysis. The clinical picture did not correspond to either Darier's disease, Hailey's disease or Grover's acantholysis. In spite of clinical similarity the cases differed in course and outcome. Overlapping dyskeratosis and acantholysis varied in different specimens. The case with transient acantholytic lesions of short duration showed much more pronounced dyskeratosis than the other in which several relapses occurred in a 4-year

Erythromelanoderma with eosinophilic spongiosis and epidermal intercellular antibodies. Differentiation from pemphigus.

A patient with figurated, erythematous skin lesions which histologically showed eosinophilic spongiosis and an epithelial intercellular staining with IgG, but no acantholysis, developed erythromelanoderma. This case and a review of the pertinent literature indicate that the diagnosis of pemphigus does not seem to be justified even in the presence of eosinophilic spongiosis and epidermal intercellular antibodies, when the clinical signs of this disease and the typical histological feature, i.e., acantholysis, are lacking.

Signalling pathways in pemphigus vulgaris.

Acantholysis in pemphigus vulgaris is induced by binding of autoantibodies to desmoglein 3 (Dsg3). The roles of signalling pathways on development of acantholysis have recently been extensively studied. In the study by Sayar et al., recently published in Exp Dermatol, epidermal growth factor receptor (EGFR) signalling was activated in both in vivo and in vitro pemphigus vulgaris experimental models. However, while EGFR inhibitors suppressed activity of p38 mitogen-activated protein kinase (p38MAPK) linearly, they suppressed activity of c-Myc and acantholysis in a non-linear, V-shaped relations
