Sickle cell disease is a prevalent autosomal recessive hemoglobinopathy with significant maternal and neonatal risks during pregnancy. Pre-pregnancy management involves counseling, baseline health assessment, genetic testing, and medication review. During pregnancy, care focuses on folic acid supplementation, serial growth scans, antenatal testing, and tailored pain management for acute crises and chronic pain. Delivery planning and peri-partum considerations involve timing, mode, and pain control. A multidisciplinary approach is critical for optimizing outcomes.

Sickle cell disease (SCD) and thalassemia (Thal) are the most common inherited, autosomal, recessive blood disorders which lead to complications such as vasoocclusion and splenomegaly. Patients who suffer from these diseases have poor quality of life and shorter life span. The most common techniques for detection of these diseases are complete blood cell count, followed by electrophoresis and high performance liquid chromatography. In this connection, the results of this paper indicate the potential of a new technique, based on spectral analysis of blood plasma and cellular components, to dete

OBJECTIVE: To characterize the craniofacial features of patients with sickle cell anemia (SCA).
MATERIALS AND METHODS: Fifty patients with SCA in the north of Minas Gerais, Brazil, were evaluated using standardized photographs (front and profile) for a subjective facial analysis and digital radiographs for a computerized cephalometric analysis. To obtain linear and angular measurements, cephalometric points were marked on the digital images by a single examiner, using the program Radiocef 6.0, and facial analyses were performed by four different orthodontists.
RESULTS: Of the population stud

BACKGROUND: The high prevalence of airway hyperresponsiveness (AHR) among children with sickle cell anemia (SCA) remains unexplained.
METHODS: To determine the relationship between AHR, features of asthma, and clinical characteristics of SCA, we conducted a multicenter, prospective cohort study of children with SCA. Dose response slope (DRS) was calculated to describe methacholine responsiveness, because 30% of participants did not achieve a 20% decrease in FEV1 after inhalation of the highest methacholine concentration, 25 mg/mL. Multiple linear regression analysis was done to identify indep

TWO MEANS OF PREVENTION: Asplenic and sickle-cell anemia patients are susceptible to infections caused by polysaccharide-encapsulated bacteria, particularly pneumococci, Haemophilus influenzae b, and meningococci. Prevention of these frequent and serious infections depends on antibiotic prophylaxis and vaccination. ANTI-POLYSACCHARIDE PNEUMOCOCCI: Vaccination Pneumo 23 is a 23-valent vaccine corresponding to more than 90% of the pneumococci strains implicated in systemic infections. The efficacy of conjugated polysaccharide vaccines for the prevention of systemic infections, particularly when

Cerebral vascular disease is a common and serious complication of sickle cell disease that mainly involves the large blood vessels of the skull base. Because recurrences are common and residual deficits severe, attention has turned to detection of preclinical cerebral involvement with the goal of preventing clinical damage. Magnetic resonance imaging (MRI), an extremely sensitive tool for detecting cerebral infarction/ischemia, has shown that 10% of asymptomatic patients exhibit white matter lesions that seem to be associated with impaired cognitive function and may be predictive of stroke; ma

An increasing number of sickle cell disease patients are deciding to bear children. The high risk of fetal and maternal complications in pregnant sickle cell disease patients mandates multidisciplinary management. Risks include spontaneous abortion, vasculorenal syndrome, fetal growth retardation, and fetal death in utero. The rates of cesarean section, maternofetal infection, and maternal death are higher than in the population at large. The diagnosis should be made prior to conception or during early pregnancy. Frequent visits with the obstetrician, hematologist, and anesthesiologist/intensi

Cerebrovascular disorders are frequently associated with sickle cell disease, mainly in homozygous children. We report the case a 25-old-patient with known sickle cell disease who presented with coma inaugurated by manifestations of intracranial hypertension. CT revealed bilateral thalamic infarcts and angiography confirmed the thrombosis of internal cerebral veins. Treatment included heparin and blood transfusion. Severe cerebral oedema resulted in the lethal outcome three days later.

BACKGROUND: The vascular wall participates in the pathogenesis of sickle cell disease. To determine whether the endothelium is activated in this disease, we studied the number, origin, and surface phenotype of circulating endothelial cells in patients with sickle cell anemia.
METHODS: We used immunohistochemical examination of buffy-coat smears to enumerate circulating endothelial cells, and we evaluated the surface phenotype by applying preparations of circulating endothelial cells. An immunofluorescence microscopy panel of antibodies was used, including a specific anti-endothelial-cell anti

We have administered Isobutyramide as a suspension over a period of 3 months, from a starting dose of 50 mg/kg/day up to 150 mg/kg/day, to four adult sickle cell (SS) anemia patients. The maximum dose was maintained for 3 weeks. The blood counts remained stable and the Hb F levels decreased slightly. The G gamma levels increased at the end of the trial, suggesting activation of the G gamma gene at the highest dose of Isobutyramide. Three patients showed a stable rate of hemolysis, while in one patient, an increase of lactate dehydrogenase occurred. None of the patients experienced pain crisis

Eighty-four hips of sickle cell anemia patients were followed up during adulthood after the occurrence during childhood of avascular necrosis of the femoral head. Roentgenograms showed residual dysplasia at completion of growth in a substantial number of cases: coxa magna was found in 11 hips, coxa plana or loss of capital sphericity in 53 hips, and true osteochondritis in 3 hips. Only 20 hips exhibited normal morphological features at completion of growth. Anomalies were found not only in the proximal femur but also in the acetabulum which was altered in 14 hips (inadequate lateral coverage i

Intravenous glucose tolerance tests (IVGTT) were performed on 9 adults with sickle cell anemia and 15 normal controls. Blood samples were collected before and 1, 3, 5, 10, 20, 30, 40, 50, and 60 min after intravenous glucose infusion and used for glucose and C-peptide assays. The mean C-peptide concentration after glucose infusion was significantly (p less than 0.05) lower for the SCA patients as compared to the controls. No significant differences were observed in the glucose concentration of IVGTT between the two groups. These results suggest an inadequate insulin secretion by the endocrine

Ten homozygous sickle cell patients, aged from 4 to 20 years, underwent a periodic red blood cells transfusion program. Indications were = one stroke or more in 4 cases, repeated severe painful crisis in 2 cases, and for others = chronic anaemia induced by hypersplenism, priapism, chronic leg ulceration, dwarfism with delayed sexual maturation. Transfusion program lasted from 6 months to 6 years. In 7 cases, a phlebotomy preceded the transfusion by the same venipuncture in order to reduce the inevitable iron overload progression. Clinical benefit was clear in the following cases: lack of painf

Cardiovascular disorders are very frequent in children with homozygous sickle cell anaemia. They are due partly to the effects of chronic anaemia on the heart and vessels and partly to other factors, such as vaso-occlusive phenomena and desaturation of arterial oxygen, which alter the cardiac function. Because of reduced oxygen tension in arterial blood, the increase in resting cardiac output is more pronounced in sickle cell anaemia than in anaemias of other origins. Beside this "anaemic heart", myocardiopathies ascribed to multiple occlusions of small coronary vessels are well documented by

Since the initial description, in 1958, of gouty arthritis occurring in association with SCA, more than 12 cases have been reported. The high proportion of women and the relatively young ages are noteworthy. Since 1968, studies of patients with SCA have shown a high prevalence of hyperuricemia, beginning during childhood. The initial event in the development of hyperuricemia presumably is increased synthesis of nucleic acids occurring as part of the erythropoietic response to hemolysis. Catabolism of the nucleic acids generates urate. Increased production of UA normally is compensated for by i

Splenic dysfunction accounts for the greatest deficit in immune function in children with homozygous SCD. This dysfunction, coupled with the natural immunologic immaturity of all young children subjects the young child with SCD to an immense risk of severe pyogenic infections. To data, experience would suggest that pneumococcal vaccines may provide only modest protection in the child with SCD less than 5 years of age. Antipneumococcal antibody responses are poor in children less than two years of age. All reported failures of pneumococcal vaccine among children with SCD have occurred in childr

A reduction of morbidity and mortality rates in homozygous sickle-cell patients was found in those with high fetal hemoglobin (HbF) levels. This factor would lead one to believe that an adequate amount of this substance would be protective to a patient with this hemoglobinopathy. This study utilizing pregnant and nongravid females, as well as males, followed for long periods of time indicates that the HbF level fluctuates with crisis. Some patients had low HbF levels with many crises and others had high amounts of HbF with no crises. However, many patients with high levels on one occasion demo
