Pupillary monitoring decreases remifentanil consumption during laparoscopic uterine surgery and improves postoperative recovery.

BACKGROUND: The aim of this paper was to explore pupillary monitoring for determining remifentanil consumption during general anesthesia and evaluating postoperative recovery quality. METHODS: Eighty patients undergoing elective laparoscopic uterine surgery were randomly divided into pupillary monitoring group (Group P) and control group (Group C). In Group P, remifentanil dosage during general anesthesia was determined according to pupil dilation reflex; in Group C, it was adjusted according to hemodynamic changes. Intraoperative remifentanil consumption and endotracheal tube extraction time

Negative pressure pulmonary oedema caused by biting and endotracheal tube occlusion--a case for oropharyngeal airways.

A patient had general anaesthesia for laparoscopic surgery. She bit on and occluded her endotracheal tube during recovery from anaesthesia. Strong inspiratory efforts during airway obstruction caused negative pressure pulmonary oedema. The pulmonary oedema resolved within 24 hours. Use of an oropharyngeal airway as a bite block could have prevented this complication.

Immersion anesthesia for extracorporeal shock wave lithotripsy. Review of two hundred twenty treatments.

Two hundred twenty extracorporeal shockwave lithotripsy (ESWL) treatments at the New York Hospital-Cornell Medical Center between September, 1984, and April, 1985, were reviewed with respect to anesthetic management. One hundred seventy-four treatments (79%) were performed under a regional anesthetic technique (RA), either with an indwelling epidural catheter (155 treatments), or with a single spinal injection (19 treatments). Forty-six treatments (21%) were performed under general anesthesia (GA). Ninety per cent of the patients were classified as ASA I or II. Hypotension during treatment, de

Delayed awakening from general anaesthesia in a patient with Hunter syndrome.

Hunter syndrome is one of a heterogeneous group of recessively inherited mucopolysaccharide storage diseases (MPS) with similar biochemical defects manifested by impairments in mucopolysaccharide catabolism with variable but progressive clinical courses. Abnormal accumulation and deposition of mucopolysaccharides in the tissue of several organs to numerous anatomical, musculoskeletal and neurological abnormalities which are known to complicate anaesthetic and airway management. Hunter syndrome has a wide variance of clinical phenotypes ranging from mild to severe. We present a patient having p

Ketone supplementation abolished isoflurane anesthesia-induced elevation in blood glucose level and increased recovery time from anesthesia in Wistar Albino Glaxo Rijswijk rats.

BACKGROUND: It has been suggested that administration of exogenous ketone supplements (EKSs) not only increases blood ketone body levels but also decreases blood glucose level and modulates isoflurane-induced anesthesia in different rodents, such as Wistar Albino Glaxo Rijswijk (WAG/Rij) rats. Thus, we investigated whether administration of EKSs can modulate the isoflurane anesthesia-generated increase in blood glucose level and the time required to recover from isoflurane-induced anesthesia. METHODS: To investigate the effect of EKSs on isoflurane anesthesia-induced changes in blood glucose

Adverse events in radiation oncology: A case series from wake up safe, the pediatric anesthesia quality improvement initiative.

BACKGROUND: Radiation therapy in pediatric patients often requires anesthesia and poses environmental challenges. Monitoring must be done remotely to limit radiation exposure to the provider. Airway access can be limited by masks or frames. Care is often delivered in relatively inaccessible locations in the hospital. While individual institutions have reported their outcomes, this case series aims to review a multicenter registry of significant adverse events and make recommendations for improved care. METHODS: Wake Up Safe: The Pediatric Quality Improvement Initiative maintains a multisite,

Emergence delirium is related to the invasiveness of strabismus surgery in preschool-age children.

OBJECTIVES: To investigate the correlation between preoperative anxiety and emergence delirium (ED), and to identify other factors that contribute to ED (such as invasiveness of surgery), in preschool-age paediatric patients undergoing strabismus surgery. METHODS: This prospective observational study enrolled Korean children (aged 2-5 years), scheduled for strabismus surgery. After completing the modified Yale Preoperative Anxiety Scale, general anaesthesia was induced. Postoperatively, Pediatric Anesthesia Emergence Delirium (PAED) scores (used as an index of the degree of ED) and a four-poi

Bispectral index-guided anesthesia in children: A systematic review and meta-analysis.

INTRODUCTION: The bispectral index (BIS) parameter is used to guide the titration of general anesthesia; however, many studies have shown conflicting results regarding its benefits in children. We aimed to perform a systematic review and meta-analysis to determine whether BIS is advantageous for pediatric patients undergoing general anesthesia. METHODS: PubMed, Embase, and Cochrane Library were systematically searched for randomized controlled trials (RCTs) comparing BIS-guided anesthesia vs. general anesthesia guided only by clinical parameters in children undergoing surgeries. We computed m

Return of spontaneous circulation after cardiopulmonary arrest in an adult horse recovering from anaesthesia.

This report describes successful cardiopulmonary resuscitation of a healthy, 525 kg, 8 years 9 months-old Quarter horse gelding. After successful pelvic limb orthopaedic surgery to excise a metatarsal bone under general anaesthesia, with the horse in left lateral recumbency, the horse was hoisted into recovery. After the horse was positioned in recovery, a routine cardiac check by thoracic auscultation was performed. Cardiopulmonary arrest (CPA) was identified by absence of cardiac sounds and pulse, as evaluated by digital palpation of the facial artery, absence of the right palpebral reflex,

Development and validation of a nomogram for predicting critical respiratory events during early anesthesia recovery in elderly patients.

BACKGROUND: Elderly patients undergoing recovery from general anesthesia face a heightened risk of critical respiratory events (CREs). Despite this, there is a notable absence of effective predictive tools tailored to this specific demographic. This study aims to develop and validate a predictive model (nomogram) to address this gap. CREs pose significant risks to elderly patients during the recovery phase from general anesthesia, making it an important issue in perioperative care. With the increasing aging population and the complexity of surgical procedures, it is crucial to develop effectiv

The Effects of Switching from Sevoflurane to Short-Term Desflurane prior to the End of General Anesthesia on Patient Emergence and Recovery: A Randomized Controlled Trial.

While sevoflurane and desflurane have been regarded as inhalation agents providing rapid induction and emergence, previous studies demonstrated the superiority of desflurane-anesthesia compared to sevoflurane-anesthesia in the postoperative recovery in obese and geriatric patients. We investigated whether a short-term switch of sevoflurane to desflurane at the end of sevoflurane-anesthesia enhances patient postoperative recovery profile in non-obese patients. We randomly divide patients undergoing elective surgery (n = 60) into two groups: sevoflurane-anesthesia group (Group-S, n = 30) and sev
