Familial Angiolipomatosis: A Report of Two Cases and Review of Clinicopathologic Features, Diagnostic Considerations, and Management Challenges in an Underserved Population.

Angiolipomas are benign mesenchymal tumors composed of mature adipose tissue with proliferative capillary vessels. Most cases are sporadic, presenting shortly after puberty as one or two small, painful "lumps" under the skin, followed by the appearance of two or three more each year, for several years. However, familial forms are rare and occur during the third through fifth decades of life on the trunk and extremities, with the forearms representing a common site of involvement. These lipomas generally affect consecutive generations, suggesting an autosomal dominant mode of inheritance, and a

Giant vascular spiradenocylindroma of the scalp clinically mimicking a vascular tumor.

We present a giant vascular spiradenocylindroma (GVSC) with a unique presentation. The diagnosis of GVSC can be clinically and radiologically elusive. The differential diagnosis includes angiolipoma, angiosarcoma, and glomus-tumor-like vascular lesions. Dermatologists should consider this variant of spiradenocylindroma in any vascular-like lesion.

Angiolipomatous Atypical Presentation of Dercum's Disease.

We describe the case of a 35-year-old man presenting with multiple painful soft tissue masses in bilateral upper extremities, the trunk, and groin. The patient had a prior history of painful lipomatosis in the left upper extremity with multiple masses removed without intraoperative or postoperative complications. Physical examination revealed multiple right upper extremity nodules that triggered reproducible pain upon palpation. Magnetic resonance imaging (MRI) confirmed the presence of multiple subcutaneous lipomatous masses with variable nonlipomatous internal signal in some of the masses. H

Compromise of the spinal canal in Proteus syndrome.

We report on 2 children with spinal stenosis and neurologic sequelae. They illustrate the 2 ways in which spinal compromise may develop in Proteus syndrome--vertebral anomalies or tumor infiltration. In one patient, spinal stenosis resulted from an angular kyphoscoliosis. In the other, cord compression resulted from infiltration of a paraspinal, intrathoracic angiolipoma.

Clinical case seminar: Fibroblast growth factor 23: a new clinical marker for oncogenic osteomalacia.

The phosphate-wasting condition, oncogenic osteomalacia, is problematic to diagnose and manage clinically due to difficulty in locating the causative tumor. Fibroblast growth factor 23 (FGF23) has recently been implicated in the pathogenesis of oncogenic osteomalacia. In this case the patient presented with clinical features typical of oncogenic osteomalacia. Removal of an angiolipoma from the thigh did not correct the clinical or biochemical abnormalities. Subsequent identification and removal of a benign giant cell tumor in the pubic ramus, however, did result in normalization of his symptom

Intramuscular hemangioma of the masseter muscle- a case report and review of literature.

INTRODUCTION: Intramuscular hemangioma, is a distinctive type of vascular tumor occurring within the skeletal muscle. Most IMH are located in the lower extremity, particularly in the muscles of the thigh and rarely in head and neck region. PRESENTATION OF CASE: 35 years old male reported with a swelling in the left cheek region since 3 years. Clinical and radiological evaluation leads to the diagnosis of Intramuscular hemangioma. Surgical excision was performed and histopathology confirmed the diagnosis. DISCUSSION: Hemangiomas of skeletal muscle represent 0.8% of all benign vascular neoplas

Dramatic effect of sirolimus on renal angiomyolipomas in a patient with tuberous sclerosis complex.

Angiomyolipomas are very common in patients with tuberous sclerosis complex (TSC) and cause substantial morbidity. Until now, arterial embolization has been the recommended treatment for symptomatic patients. We report the case of a woman with TSC and giant angiolipomas in whom sirolimus induced a dramatic reduction in bilateral renal tumors.

A case of familial angiolipomatosis with Lisch nodules.

Familial angiolipomatosis is a rare syndrome that may be confused clinically with neurofibromatosis type 1. This condition is most often inherited in an autosomal recessive manner; however, several reports have been published suggesting an autosomal dominant mode of inheritance. Angiolipomatosis, although somewhat disfiguring, is a benign condition with no known association with malignant neoplasms. This is in contradistinction to neurofibromatosis, an autosomal dominant syndrome associated with a myriad of benign and malignant neoplasms. It is, therefore, important to discriminate this entity

Infiltrating hybrid mesenchymal tumor of skeletal muscle showing lipomatous, hemangiomatous, leiomyomatous and osseous features - An unusual soft tissue tumor providing insight into the pathogenesis of lipoma variants.

Infiltrating angiolipoma and osteolipoma of the hand are rare. A 40-year-old man presented with slowly enlarging swelling of his right hand for two and half years without functional deficit but it became painful with slight limitation of movement for the past few months. Plain radiograph showed a large soft tissue swelling with specks of calcifications. Ultrasonography and magnetic resonance imaging revealed an infiltrative mass in the right palm deep to the flexor tendons. As biopsy suggested infiltrative angiolipoma of skeletal muscle, debulking of the tumor was performed. The tumor showed c

Extradural spinal lipomas. Report of two cases and review of the literature.

The authors present two cases of extradural spinal lipomas causing spinal cord compression, not associated with spinal dysraphism. Sixty-nine cases of such tumours have been reported in the literature. It is important to distinguish between two histological types of these neoplasms: angiolipoma and true lipoma. They have a different distribution in relation to age and site of occurrence. The outcome can be quite variable considering the greater surgical problems that true lipomas can cause.

Slowly progressive macrocephaly with hamartomas: a new syndrome?

We report on an 8 1/2-year-old boy with slowly progressive macrocephaly, psychomotor retardation, multiple subcutaneous angiolipomas, hypertelorism, exotropia, prolonged drooling, cutis marmorata, telangiectasia, congenital heart defect, broad thumbs and great toes, and muscle wasting. The syndrome is similar to the Bannayan-Zonana syndrome and seems to be inherited as an autosomal dominant trait. The father has partial manifestations of the syndrome.

[Gastric tumors with fatty components. CT findings and differential diagnosis].

PURPOSE: To assess the role of CT in diagnosing and characterizing gastric fatty tumors. MATERIAL AND METHODS: We reviewed the CT scans of 16 patients (8 men, 8 women, mean age 52 years) with gastric fatty tumors (11 lipomas, 3 liposarcomas, 1 angiolipoma, 1 teratoma) examined from 1990 to 1999. The differential diagnosis considered primary and secondary lipomatosis, carcinoma engulfing the perivisceral fat and thus mimicking a liposarcoma, mesenchymal gastric and primary peritoneal tumors. RESULTS: Lipomas involved the fundus (7/11), the body (3/11), the antrum (1/11). Multifocality was fou

A 20-year-old woman with chronic cough and dyspnea.

Lymphangioleiomyomatosis is a rare progressive disease characterized by abnormal smooth muscle cell proliferation leading to a diffuse cystic lung disease and extrapulmonary manifestations. Most cases are caused by mutations in the TSC1 and/or TSC2 genes, which are also associated with tuberous sclerosis complex. We describe a case of sporadic lymphangioleiomyomatosis with autosomal dominant polycystic kidney disease and renal angiolipomas in a patient who tested negative for gene mutations on the TSC1 and TSC2 gene panel.

[A case of micronodular pneumocyte hyperplasia, lymphangiomyomatosis associated with tuberous sclerosis].

We report a case of tuberous sclerosis (TS) associated with renal angiolipomas, subungual fibroma, adenoma sebaceum, pulmonary lymphangiomyomatosis (LAM), and micronodular pneumocyte hyperplasia (MNPH). A CT scan of the chest demonstrated multiple minute nodules present throughout both lung fields. Histologically, the nodules were well demarcated, and were composed of thickened, fibrotic, alveolar septa lined by pleomorphic, type II pneumocytes. This type of MNPH is extremely rare, and appears to be a hamartomatous proliferation occurring in patients with TS.

Tuberous sclerosis complex: report of two intrafamilial cases, both in mother and daughter.

Tuberous sclerosis complex (TSC) is a multisystem syndrome characterized by neurological symptoms and tumors in multiple organs including kidney, brain, skin, eyes, heart and lung. Kidney and brain are the two most frequently affected organs in TSC. TSC is an autosomal disorder with extensive clinical variability. We described TSC in a family at a mother and her daughter. We emphasized the importance of Computed Tomography in the discovery of some asymptomatic organic involvement as bilateral renal angiolipoma in the mother.

Cutaneous angiomyolipoma.

Cutaneous angiomyolipomas are rare. We report a case in a 45-year-old male with a well circumscribed lesion located on the chin. This lesion, probably hamartomatous in nature, differs from renal angiomyolipoma in terms of nonassociation with tuberous sclerosis, circumscription, and male predominance. Another characteristic feature is the absence of epithelioid cells. Differential diagnosis includes angiolipoma, angioleiomyoma, hemangioma, and myolipoma. It is distinguished from the abovementioned entities by the presence of a combination of thick-walled blood vessels, smooth muscle, and fat.

The 'Pinocchio' nasal deformity due to cavernous lymphangioma.

The 'Pinocchio' or 'Cyrano' nose is a rare condition in which deformity of the nasal tip is produced by an underlying soft tissue tumour. Previously reported cases have been due to either capillary or cavernous haemangiomas (angiolipomas). The deformity is the cause of much teasing in children. There has been debate as to whether surgical intervention is indicated, as a proportion of cases will regress spontaneously. We report a case of 'Pinocchio' nose with a lymphangioma of the nasal tip which is previously undescribed and review the options for management.
