Refractory thrombocytopenia in antiphospholipid syndrome.

Thrombocytopenia is the second most common manifestation of antiphospholipid syndrome (APS). It is found in approximately 22% of the patients with this disease. Often it is not severe, platelet counts usually range between 50 x 10(9)/L and 150 x 10(9)/L without bleeding problems. Yet, it does not protect patients against thrombotic events. It rarely requires treatment and, due to similarities to idiopathic thrombocytopenic purpura (ITP), similar treatment rules usually apply. In this report two patients with APS are described who presented with severe thrombocytopenia that did not respond to s

Anaphylaxis associated with antiphospholipid syndrome.

BACKGROUND: To our knowledge, no previously published reports have described food-induced anaphylaxis associated with the antiphospholipid syndrome. OBJECTIVE: We undertook a study of four patients with thrombosis associated with the antiphospholipid syndrome after each patient experienced anaphylaxis attributable to ingestion of vegetal foods. METHODS: IgE antibody levels to various foods were determined in serum specimens from the study patients, and skin prick tests with the same allergens were conducted to determine their in vivo responses. Hematologic, cardiopulmonary, vascular, and rhe

Antiphospholipid syndrome and pregnancy loss.

Antiphospholipid antibodies (aPL) are associated with recurrent miscarriage. Pregnancies that survive the first trimester risk developing pre-eclampsia, intrauterine growth retardation and fetal distress during labour. Pregnancy loss is initially caused by defective embryonic implantation and later by thrombosis of the placental vasculature. In women with aPL, thromboprophylaxis during pregnancy improves the live birth rate.

Advances in antiphospholipid (Hughes') syndrome.

Fifteen years have passed since Hughes reported the detailed clinical description of antiphospholipid syndrome (APS), and it is now recognised as one of the most common prothrombotic disorders. Its main clinical features are recurrent thrombosis (both venous and arterial), recurrent pregnancy loss and thrombocytopenia associated with the presence of antiphospholipid antibodies (aPLs). aPLs are a heterogeneous group of autoantibodies detected by either clotting or immunological assays. They include lupus anticoagulant (LA), anticardiolipin antibodies (aCL) and antibodies against other phospholi

The lupus anticoagulant/antiphospholipid syndrome.

Lupus anticoagulants and anticardiolipin antibodies have been strongly associated with the risk of thrombosis, recurrent fetal loss, thrombocytopenia, and a number of other clinical manifestations that together have been referred to as the antiphospholipid syndrome. Despite growing evidence of the significance of this relationship, the pathogenetic mechanisms involved are largely unknown. Recent data suggest strongly that lupus anticoagulants (LACs) and anticardiolipin antibodies (ACAs) are antibodies to protein-phospholipid complexes rather than to phospholipids, as had originally been though

Melanoderma revealing primary antiphospholipid syndrome.

Acute adrenal insufficiency is a rare complication of the primary antiphospholipid syndrome. We report a case of melanoderma of rapid onset revealing an addisonian crisis associated with primary antiphospholipid syndrome, which was associated with life-threatening hypercalcemia. Melanoderma does not belong to the clinical presentation of acute adrenal insufficiency. Its very rapid onset in our patient is probably due to her very high level of serum ACTH, in relation with her recent abortion.

Vasculitis associated with antiphospholipid syndrome.

Several syndromes involving antiphospholipid antibodies have been described in the literature. Although the varied clinical manifestations have been well delineated, the vascular pathophysiology in patients with these antibodies remains unclear. Vascular damage is often described as a vasculopathy; however, several case reports have described an associated vasculitis. We report two patients with manifestations of antiphospholipid antibody syndrome (APLS) and concurrent vasculitis. The first patient, a 42-year-old man, presented with abdominal pain and fevers. The second patient, a 39-year-old

[Chorea and primary antiphospholipid syndrome].

Primary antiphospholipid syndrome or PAPS is characterised by antiphospholipid antibodies and arterial and/or venous thromboses. Numerous other clinical features have been shown to be related to this syndrome. Chorea is a well known but rare phenomenon in systemic lupus erythematosus; it has been shown to be strongly related to the presence of antiphospholipid antibodies. We describe two patients with chorea that appeared to be caused by the PAPS.

S-hydroxychloroquine prevents the antiphospholipid thrombogenic complexes for antiphospholipid syndrome treatment.

Clinically used in systemic lupus erythematosus (SLE), Hydroxychloroquine (HCQ) exerts antithrombotic effects by inhibiting anti-β2-glycoprotein I (anti-β2GPI) antibody binding to phospholipid bilayers. However, HCQ is a racemic mixture, with only one enantiomer offering therapeutic benefits, while the other may contribute to toxicity. The current study evaluated the thromboprophylactic efficacy of R-enantiomer Hydroxychloroquine (R-HCQ), S-enantiomer Hydroxychloroquine (S-HCQ), and racemic HCQ (Rac-HCQ), with a focus on their impact on APS-associated markers. Both in vitro and in vivo models

Hot topics in the antiphospholipid syndrome.

Antiphospholipid syndrome (APS) presents ongoing challenges in understanding its pathogenesis, standardizing diagnostic tests, and optimizing treatment strategies. The 18th International Congress on Antiphospholipid Antibodies (ICAPA) was held in Kyoto in September 2025. This narrative meeting wrap-up summarizes selected highlights from the Congress in seven areas, including APS pathogenesis, APS diagnostics and antiphospholipid antibodies standardization, serology and biomarkers, therapeutics, obstetric APS, organ-specific manifestations, and disease activity assessment. Topics discussed incl

Controversies in the Management of Antiphospholipid Syndrome.

Strategies to prevent thrombosis in antiphospholipid antibody (aPL)-positive patients are of the utmost importance. The risk of thrombosis in patients with aPLs varies, depending on additional venous thrombosis and cardiovascular risk factors, as well as associated comorbidities. Recurrent thrombosis despite treatment with vitamin K antagonists is relatively common in daily practice. In this context, the effectiveness of the new direct oral anticoagulants in antiphospholipid syndrome is debated, as well as that of low-dose aspirin for primary thromboprophylaxis. There is an urgent unmet need t

COVID-19 and the antiphospholipid syndrome.

Coronavirus disease 2019 (COVID-19) has resulted in a global pandemic. Most COVID-19 patients are asymptomatic or have flu-like symptoms. However, around 15% of the patients may have severe disease, including unilateral or bilateral pneumonia with acute respiratory distress syndrome and progressive hypoxemia that may require mechanical ventilation assistance. A systemic inflammatory response syndrome occurs in the most severe forms of COVID-19, with multiorgan involvement which can be life threatening caused by a cytokine storm. Although what best characterizes COVID-19 are the manifestations

Complement in the Pathophysiology of the Antiphospholipid Syndrome.

The antiphospholipid syndrome (APS) is characterized by thrombosis and pregnancy morbidity in the presence of antiphospholipid antibodies (aPL). Complement is a system of enzymes and regulatory proteins of the innate immune system that plays a key role in the inflammatory response to pathogenic stimuli. The complement and coagulation pathways are closely linked, and expanding data indicate that complement may be activated in patients with aPL and function as a cofactor in the pathogenesis of aPL-associated clinical events. Complement activation by aPL generates C5a, which induces neutrophil ti

Pathophysiological insights into the antiphospholipid syndrome.

The antiphospholipid syndrome (APS) is characterized by venous and/or arterial thrombosis and severe pregnancy morbidity in presence of antiphospholipid antibodies (aPL). While there is compelling evidence that aPL cause the clinical manifestations of APS, the underlying mechanisms are still a matter of scientific debate. This is mainly related to the broad heterogeneity of aPL. There are three major types of aPL: The first one binds to (anionic) phospholipids, e.g. cardiolipin, in absence of other factors (cofactor independent aPL). The second type binds to phospholipids only in presence of p

The antiphospholipid syndrome: still an enigma.

Antiphospholipid syndrome (APS) is defined by clinical manifestations that include thrombosis and/or fetal loss or pregnancy morbidity in patients with antiphospholipid antibodies (aPL). Antiphospholipid antibodies are among the most common causes of acquired thrombophilia, but unlike most of the genetic thrombophilias are associated with both venous and arterial thrombosis. Despite an abundance of clinical and basic research on aPL, a unified mechanism that explains their prothrombotic activity has not been defined; this may reflect the heterogeneity of aPL and/or the fact that they may influ
