Conduction barriers in human atrial flutter: correlation of electrophysiology and anatomy.

Animal models of atrial flutter and early mapping studies of human atrial flutter have suggested the importance of barriers in this reentrant arrhythmia. The consistency of rate and morphology of typical atrial flutter suggest a common anatomic substrate for this arrhythmia. The unique endocardial architecture of the right atrium provides anatomic barriers around which reentry occurs. In typical human atrial flutter, the crista terminalis, eustachian ridge, and tricuspid annulus have been identified as barriers to conduction. The importance of conduction barriers, methodology for defining barr

[Location of atrial and ventricular insertions of accessory atrioventricular pathways using surface ECG mapping and its importance in catheter ablation therapy].

BACKGROUND: The modern non-pharmacological therapy of the WPW syndrome by means of catheter ablation is based on the interruption of the accessory pathway(-s) by radiofrequency current energy. Destruction of the morphologic substrate of the arrhythmia alters the activation wave spread in the heart. OBJECTIVES: It was the aim of this report to demonstrate the diagnostic potential of BSM in localizing both overt and concealed accessory pathways. Presented study analyzes the alterations of the cardioelectric field by means of body surface ECG mapping in two female patients with accessory pathway

Heart ventricular activation in VAT difference maps from children with chronic kidney disease.

Children with chronic kidney disease (CKD) are affected by cardiovascular complications, including disturbances in the intraventricular conduction system. Body surface potential mapping (BSPM) is a non-invasive method of assessing the cardioelectrical field. Our aim was to investigate conduction disturbances in young CKD patients using ventricular activation time (VAT) maps. Our study comprised 22 CKD children (mean age: 13.1 ± 2.5 years) treated conservatively and 29 control patients. For each child 12-lead electrocardiogram (ECG) readings were taken, and blood pressure and serum concentratio

New endpoint for ablation of ventricular tachycardia: change in QRS morphology with pacing at protected isthmus as index of isthmus block.

INTRODUCTION: Endpoints confirming block in the critical isthmus in sinus rhythm and with pace mapping have not been established. METHODS AND RESULTS: A 44-year-old man with a history of Tetralogy of Fallot presented with recurrent ventricular tachycardia (VT). Entrainment mapping was consistent with a macroreentrant circuit rotating in a clockwise fashion under the pulmonic valve. After termination of the VT in a critical isthmus located on the conal free wall, a pace map proximal to the site of successful ablation was consistent with a change in QRS morphology. This change in QRS morphology

Role of Purkinje fibers in post-infarction ventricular tachycardia.

OBJECTIVES: The objective of this study was to assess the role of Purkinje fibers in monomorphic, post-infarction ventricular tachycardia (VT). BACKGROUND: Ventricular fibrillation and polymorphic VT in the setting of acute myocardial infarction (MI) may be triggered by ectopy arising from Purkinje fibers. METHODS: From among a group of 81 consecutive patients with post-infarction monomorphic VT referred for catheter ablation, 9 patients were identified in whom the clinical VT had a QRS duration < or =145 ms. Mapping was performed focusing on areas with Purkinje potentials. RESULTS: A total

Radiofrequency ablation of typical atrial flutter with the use of electro-anatomical mapping system CARTO.

BACKGROUND: Radiofrequency (RF) ablation of typical atrial flutter (AFL) is a well-established method of treatment. Although its efficacy is high, a search for new, more effective techniques is continuing. These new methods include modern systems for electro-anatomical mapping and new types of ablating electrodes such as cooled-tip or dual-sensor catheters. AIM: To assess the efficacy of RF ablation of typical AFL with the use of electro-anatomical mapping system CARTO and to compare the effectiveness of various ablating catheters. METHODS: The study group consisted of 60 consecutive patient

Radiofrequency ablation of idiopathic ventricular fibrillation guided by noncontact mapping.

A 32-year-old man with idiopathic ventricular fibrillation and an implantable cardioverter defibrillator presented during a ventricular fibrillation storm. Frequent monomorphic ventricular ectopics with left bundle branch block morphology were documented, some of which initiated fibrillation. He underwent noncontact mapping of the right ventricle, during which the ventricular ectopics were mapped to a site in the free wall displaying a diastolic potential 80 ms before ectopic QRS onset. Following three radiofrequency energy applications, the ectopics were abolished. After 11-month follow-up, h

Catheter ablation of frequently recurring ventricular fibrillation in a patient after aortic valve repair.

It has been demonstrated that idiopathic ventricular fibrillation (VF) can be triggered by ventricular premature beats (VPBs) arising from the Purkinje fibers. Eliminating these VPBs by radiofrequency catheter ablation prevented VF recurrences. Whether the same pathophysiology and the same treatment option exist in patients with structural heart disease is unknown. Recurrent VF was observed in a 17-year-old patient after aortic valve repair of a perforated noncoronary cusp with resulting severe aortic regurgitation. VF recurred despite therapy with various antiarrhythmic drugs. A maximum of 14

Epicardial right atrial free wall mapping in chronic atrial fibrillation. Documentation of repetitive activation with a focal spread--a hitherto unrecognised phenomenon in man.

BACKGROUND: Previous studies have shown that atrial fibrillation of recent onset in man is based on a varying number of simultaneously present activation waves reentering either themselves or each other. In the present study, right atrial activation during chronic atrial fibrillation in man was studied. METHODS AND RESULTS: In 16 patients with chronic atrial fibrillation multiple epicardial recordings of 8 s each were made at the right atrial posterior free wall and at the appendage using a 20 x 35 mm electrode array with 56 bipolar measurement points. The preferable activation pattern of eac

Characterization of radiofrequency ablation lesions by high-density unipolar, bipolar and omnipolar voltage mapping.

BACKGROUND: Radiofrequency (RF) energy is the most commonly used technique to create ablation lesions as treatment strategy for various arrhythmias. To have long-term freedom of arrhythmias, each lesion must be transmural and continuous throughout its entirety. OBJECTIVE: This study aimed to evaluate changes in unipolar, bipolar, and omnipolar potential voltages during various stages of linear RF ablation applications to define the most suitable technique to assess lesion transmurality and continuity. METHODS: Epicardial mapping of porcine hearts (N = 5) was performed during 3 stages of lesi

High-resolution structure-function mapping of intact hearts reveals altered sympathetic control of infarct border zones.

Remodeling of injured sympathetic nerves on the heart after myocardial infarction (MI) contributes to adverse outcomes such as sudden arrhythmic death, yet the underlying structural mechanisms are poorly understood. We sought to examine microstructural changes on the heart after MI and to directly link these changes with electrical dysfunction. We developed a high-resolution pipeline for anatomically precise alignment of electrical maps with structural myofiber and nerve-fiber maps created by customized computer vision algorithms. Using this integrative approach in a mouse model, we identified

Optical imaging of arrhythmias in tissue culture.

BACKGROUND: Cardiac cell cultures are becoming an important experimental system of minimal complexity that captures many of the salient features of myocardial tissue function and are simple enough that tissue parameters can be controlled systematically. Fundamental mechanisms that underlie normal and pathological electrophysiology at the tissue level can be studied. Of particular interest are spiral waves, which underlie many tachyarrhythmias and fibrillation. METHODS: Methods of patterned growth were used to control tissue structure, and contact fluorescence imaging was used to visualize the

Autocorrelation maps for optimal setting in cardiac resynchronization therapy.

BACKGROUND AND OBJECTIVE: Patients with chronic heart failure are treated with implanted devices artificially stimulating the ventricular myocardium to support the ventricular activation propagation dynamics. The criterion for stimulation/pacing timing is a shortening of the QRS duration in the ECG signal. The study suggests additional ECG parameters that could be helpful in cardiac resynchronization therapy (CRT) device pacing settings. METHODS: This issue was approached by computing and evaluating autocorrelation maps derived from body surface potential maps during the QRS complex. The auto

Ventricular tachycardia substrate mapping: What's been done and what needs to be done.

Substrate mapping is an important component of electrophysiological (EP) study for the treatment of reentrant ventricular tachycardia (VT). It is used to detect characteristics of the electrical circuit and, in particular, the location and properties of the central common pathway, aka the isthmus, where multiple circuit loops can coincide. Typically, reentrant circuits are single or double loop, but as the common pathway size increases, 4-loop patterns may emerge, consisting of 2 parallel isthmuses or a single isthmus with 4 loops. Arrhythmogenic substrate contains a mixture of scar, calcifica
