Comparative analysis of the cardiotoxicity induced by doxorubicin and epirubicin and exploratory investigations of clinical parameters indicating the cardiotoxicity in healthy dogs.

Cardiotoxicity of doxorubicin (DOX) and epirubicin (EPI) has not been thoroughly compared in dogs. Fractional shortening (FS) is considered insufficient for detecting early myocardial damage induced by these drugs. This study aimed to compare the cardiotoxicity between EPI and DOX and to investigate parameters associated with histopathological findings induced by these drugs. Eight healthy beagles were randomly divided into two groups and administered either DOX or EPI until development of anthracycline-induced cardiac dysfunction (AICD), defined as consecutive FS below 30%, or until reaching

Cardiotoxicity as important differential diagnosis for reduced myocardial blood flow during Rubidium cardiac PET/CT : Cardiotoxicity in Rubidium PET/CT.

A 65-year-old woman with a history of ductal mammary carcinoma and recent autonomic dysfunction underwent a Rb-82 chloride (RbCl) cardiac PET/CT scan that showed no ischemia or scarring, but significantly reduced myocardial flow reserve (MFR) (global: 1.5) and a CAC-Score of 0. The patient's chemotherapy history (paclitaxel, carboplatin, epirubicin, pembrolizumab 2 years before) with elevated Troponin T and NT-pro-BNP levels at that time, and now reduced MFR with 0 CAC suggests cancer-therapy-related cardiotoxicity. An important differential diagnosis to the more common CAD-associated microvas

Doxorubicin-induced Cardiotoxicity in Dogs: A Narrative Review.

Doxorubicin, an anthracycline widely used in both human and veterinary oncology, is highly effective against a variety of malignancies in dogs, including lymphomas, sarcomas and carcinomas. Despite its efficacy, doxorubicin exposure can be associated with cardiotoxicity, which may present acutely or chronically, potentially leading to severe cardiac arrhythmia and congestive heart failure. Mechanisms underlying cardiotoxicity are multifactorial, including oxidative stress, mitochondrial dysfunction, DNA damage via topoisomerase IIβ, inflammation and dysregulated cell death pathways such as apo

Cell Village Captures Populational Heterogeneity of Sunitinib Cardiotoxicity.

BACKGROUND: TKIs (tyrosine kinase inhibitors) have transformed cancer therapy; however, cardiotoxicity remains a frequent adverse effect with variable susceptibility among patients. For sunitinib, a commonly prescribed TKI, the molecular programs and functional features underlying this variability are not well defined, and clinically actionable predictors of cardiotoxicity risk remain lacking. METHODS: To investigate interindividual variability in sunitinib-induced cardiotoxic responses, we implemented the Cell Village platform consisting of human induced pluripotent stem cell (iPSC)-derived

Cardiotoxicity in breast cancer patients treated with chemoradiotherapy.

With national breast cancer screening programs leading to earlier diagnosis and improved treatments, more women are achieving long-term remission. However, this has resulted in rising rates of cardiotoxicity in breast cancer survivors consequent to their cancer therapies. Anthracyclines, a cornerstone treatment in breast cancer, carry a significant risk of short- and long-term cardiotoxicity through inhibition of topoisomerase II, preventing DNA repair and increasing oxidative stress. This most commonly manifests in the form of left ventricular (LV) systolic dysfunction or heart failure, and i

Acute Hydroxychloroquine Overdose With Severe and Prolonged Cardiotoxicity.

We present a case of an overdose of hydroxychloroquine with severe toxicity and describe the hospital management of the patient. The management of large ingestions can provide significant challenges for the emergency medicine physician, as these patients can present in extremis. A 49-year-old woman presented to the emergency department (ED) two hours following a reported ingestion of 24 grams of hydroxychloroquine in a suicide attempt. The patient developed hypotension, ventricular arrhythmias including torsades de pointes, and profound hypokalemia. She was managed with intravenous fluid resus

Role of GPCR Signaling in Anthracycline-Induced Cardiotoxicity.

Anthracyclines are a class of chemotherapeutics commonly used to treat a range of cancers. Despite success in improving cancer survival rates, anthracyclines have dose-limiting cardiotoxicity that prevents more widespread clinical utility. Currently, the therapeutic options for these patients are limited to the iron-chelating agent dexrazoxane, the only FDA-approved drug for anthracycline cardiotoxicity. However, the clinical use of dexrazoxane has failed to replicate expectations from preclinical studies. A limited list of GPCRs have been identified as pathogenic in anthracycline-induced card

2-Hydroxyanthraquinone induces cardiotoxicity in zebrafish via ferroptosis.

2-Hydroxyanthraquinone (2-hATQ) is a photochemical oxidation product of anthracene (ANT), a polycyclic aromatic hydrocarbon. ANT is listed as a priority pollutant by the US Environmental Protection Agency (EPA) due to its resistance to degradation and persistence in the environment. Because of ANT's characteristics, 2-hATQ can also be ubiquitously present in the environment and is more toxic than its parent compound ANT, posing significant risks to environmental safety and human health. Nevertheless, there are limited studies addressing its toxicological effects, particularly concerning cardio

Post-transplant cyclophosphamide-induced cardiotoxicity: A comprehensive review.

Cyclophosphamide-induced cardiotoxicity, associated with its toxic metabolite acrolein, is a significant concern and unresolved issue, especially when cyclophosphamide is administrated in high doses. However, cardiotoxicity following low-dose cyclophosphamide has been also documented, especially in post-hematopoietic stem cell transplantation (post-HSCT) settings. Despite the involvement of multiple signaling pathways in cyclophosphamide-induced cardiomyopathy, the exact underlying mechanisms remain to be fully elucidated. This review outlines the current challenges of cyclophosphamide therapy

Cardiotoxicity of chloroquine and hydroxychloroquine through mitochondrial pathway.

BACKGROUND: Medical therapies can cause cardiotoxicity. Chloroquine (QC) and hydroxychloroquine (HQC) are drugs used in the treatment of malaria and skin and rheumatic disorders. These drugs were considered to help treatment of coronavirus disease (COVID-19) in 2019. Despite the low cost and availability of QC and HQC, reports indicate that this class of drugs can cause cardiotoxicity. The mechanism of this event is not well known, but evidence shows that QC and HQC can cause cardiotoxicity by affecting mitochondria and lysosomes. METHODS: Therefore, our study was designed to investigate the

Unexpected worsening of doxorubicin cardiotoxicity upon intermittent fasting.

Recently in Cell Metabolism, Ozcan et al. used preclinical and clinical data to suggest that alternate-day fasting may exacerbate the cardiotoxic effects of doxorubicin through the TFEB/GDF15 pathway, leading to myocardial atrophy and impaired cardiac function. The link between caloric intake, chemotherapy-induced cachexia, and cardiotoxicity warrants more clinical attention.

Apolipoprotein M Attenuates Anthracycline Cardiotoxicity and Lysosomal Injury.

Apolipoprotein M (ApoM) binds sphingosine-1-phosphate (S1P) and is inversely associated with mortality in human heart failure (HF). Here, we show that anthracyclines such as doxorubicin (Dox) reduce circulating ApoM in mice and humans, that ApoM is inversely associated with mortality in patients with anthracycline-induced heart failure, and ApoM heterozygosity in mice increases Dox-induced mortality. In the setting of Dox stress, our studies suggest ApoM can help sustain myocardial autophagic flux in a post-transcriptional manner, attenuate Dox cardiotoxicity, and prevent lysosomal injury.

Mitochondrial Determinants of Anti-Cancer Drug-Induced Cardiotoxicity.

Mitochondria are key organelles for the maintenance of myocardial tissue homeostasis, playing a pivotal role in adenosine triphosphate (ATP) production, calcium signaling, redox homeostasis, and thermogenesis, as well as in the regulation of crucial pathways involved in cell survival. On this basis, it is not surprising that structural and functional impairments of mitochondria can lead to contractile dysfunction, and have been widely implicated in the onset of diverse cardiovascular diseases, including ischemic cardiomyopathy, heart failure, and stroke. Several studies support mitochondrial t

Cardiotoxicity linked to anticancer agents and cardioprotective strategy.

Chemotherapy is a main treatment for cancer, and it benefits patients by controlling cancer relapse and metastasis, thereby leading to an increase in the overall survival rate. However, this treatment is associated with mild to severe side effects, one of which is cardiotoxicity. The severity of cardiotoxicity, a leading cause of cardiovascular diseases, depends on the type of cancer therapy employed and the time required for its management. A chemotherapeutic agent is used either alone or in combination with other drugs for cancer treatment. The exact mechanism of chemotherapeutic agent-induc

Interrelated In Vitro Mechanisms of Sibutramine-Induced Cardiotoxicity.

Consumption of illicit pharmaceutical products containing sibutramine has been reported to cause cardiovascular toxicity problems. This study aimed to demonstrate the toxicity profile of sibutramine, and thereby provide important implications for the development of more effective strategies in both clinical approaches and drug design studies. Action potentials (APs) were determined from freshly isolated ventricular cardiomyocytes with whole-cell configuration of current clamp as online. The maximum amplitude of APs (MAPs), the resting membrane potential (RMP), and AP duration from the repolari

Cardiotoxicity is an important complication of several cancer therapeutic agents. Several well established and newer anticancer therapies such as anthracyclines, trastuzumab and other HER2 receptor blockers, antimetabolites, alkylating agents, tyrosine kinase inhibitors (TKIs), angiogenesis inhibitors, and checkpoint inhibitors are associated with significant cardiotoxicity. Cardiovascular imaging employing radionuclide imaging, echocardiography and magnetic resonance imaging are helpful in early detection and prevention of overt heart failure secondary to cardiotoxicity of cancer therapy.
