A randomized double-blind study of caspofungin versus fluconazole for the treatment of esophageal candidiasis.

BACKGROUND: Candida esophagitis remains an important cause of morbidity in patients with advanced human immunodeficiency virus (HIV) infection. Fluconazole is widely regarded as the treatment of choice for this condition. METHODS: The efficacy and safety of caspofungin were compared with fluconazole in adult patients with Candida esophagitis in a double-blind randomized trial. Eligible patients had symptoms compatible with esophagitis, endoscopic demonstration of mucosal plaques, and microscopic demonstration of Candida from the esophageal lesions. Patients were randomly assigned to receive c

Diverse genetic pathways drive convergent fluconazole and caspofungin resistance phenotypes in Candida albicans.

The emergence of antifungal resistance in Candida albicans is a major worldwide public health threat, particularly for immunocompromised patients. Antifungal resistance may arise through uniform resistance mechanisms or through variable evolutionary routes. The aim of this study was to investigate whether antifungal resistance arises in C. albicans through shared or isolate-specific mechanisms, in other words, following convergent or divergent adaptive pathways and genetic mechanisms. Six laboratory-induced fluconazole-resistant and six caspofungin-resistant isolates were analyzed using whole-

Suspected Caspofungin-Induced Visual Toxicity Manifesting as Temporary Decreased Vision: A Case Report.

INTRODUCTION: Caspofungin is an echinocandin antifungal agent widely used for the treatment of invasive candidiasis because of its broad antifungal activity. Although ocular adverse events are listed among uncommon reactions in the Summary of Product Characteristics, detailed clinical descriptions of acute visual impairment temporally associated with caspofungin therapy are scarce. CASE PRESENTATION: We report the case of a 21-year-old man who developed candidemia due to Candida guilliermondii following complex spinal surgery. Antifungal therapy with caspofungin was initiated in accordance wi

Caspofungin pharmacokinetics in a child undergoing continuous veno-venous haemodiafiltration: a case report.

We report the case of a 13-year-old with systemic lupus erythematosus who received caspofungin for disseminated invasive aspergillosis and underwent continuous renal replacement therapy (CRRT) for acute kidney failure and fluid overload. The RRT membrane was made with polyacrylonitrile. During two days, blood samples were collected before and after the filter and from the effluent line to measure caspofungin concentrations and estimate total and CRRT (CLRRT) clearances. We found a total clearance of 13 mL/min and a mean (range) CLRRT of 10.1 (5.9-14.9) mL/min. Compared to previous studies, the

Caspofungin at sub-inhibitory concentration promotes the formation of Candida albicans persister cells.

AIMS: Low caspofungin exposure is frequently encountered in patients with invasive candidiasis caused by Candida albicans. This study aimed to investigate the effects of caspofungin on C. albicans at sub-inhibitory concentrations. METHODS AND RESULTS: First, a comparative transcriptomics analysis was performed on C. albicans receiving caspofungin at sub-minimum inhibitory concentrations (sub-MICs). The results showed that caspofungin significantly changed the mRNA expression profile in DAY185, with DE-mRNAs enriched in the functions of cell wall biosynthesis, metabolism, etc. Subsequently, ce

Transcriptional regulation of the caspofungin-induced cell wall damage response in Candida albicans.

The human fungal pathogen Candida albicans maintains pathogenic and commensal states primarily through cell wall functions. The echinocandin antifungal drug caspofungin inhibits cell wall synthesis and is widely used in treating disseminated candidiasis. Signaling pathways are critical in coordinating the adaptive response to cell wall damage (CWD). C. albicans executes a robust transcriptional program following caspofungin-induced CWD. A comprehensive analysis of signaling pathways at the transcriptional level facilitates the identification of prospective genes for functional characterization

Low penetration of caspofungin into cerebrospinal fluid following intravenous administration of standard doses.

The kinetics of caspofungin (CAS) in cerebrospinal fluid (CSF) following intravenous (i.v.) administration has been studied exclusively in animal models. Human data are missing so far. In this study, 13 CSF samples were obtained at different time points following i.v. infusion of CAS in ten paediatric haemato-/oncological patients (age range 1.0-14.2 years, median 8.6 years) without signs of central nervous system (CNS) infection (n = 10 samples) or with infectious meningitis (n = 3 samples). Serum samples were obtained concurrently. Liquid chromatography-tandem mass spectrometry was used for

Effect of fluid loading during hypovolaemic shock on caspofungin pharmacokinetic parameters in pig.

INTRODUCTION: Caspofungin treatment is frequently initiated in shock patients. In the present study, we investigated the influence of hypovolaemic shock requiring fluid loading on the plasma and pulmonary pharmacokinetic parameters of caspofungin in the pig. METHODS: After being anaesthetised and mechanically ventilated, 12 pigs were bled to induce a two-hour deep shock and resuscitated using normal saline based on haemodynamic goals. A one-hour infusion of 70 mg of caspofungin was started at the beginning of the resuscitation period. The lungs were removed four hours after caspofungin admini

Caspofungin prolongs survival of transiently neutropenic rats with advanced-stage invasive pulmonary aspergillosis.

A high-dose-step-down strategy for caspofungin treatment was evaluated in an experimental model of advanced-stage invasive pulmonary aspergillosis. The therapeutic efficacy of caspofungin in relation to the severity of invasive pulmonary infection caused by Aspergillus fumigatus in transiently neutropenic rats was investigated by using rat survival and the decrease in the fungal burden as the parameters of efficacy. When treatment was started at either 16 h or 24 h after fungal inoculation, caspofungin administered intraperitoneally at 4 mg/kg of body weight/day for 10 days was highly effectiv

Analysis of the Localization of Schizosaccharomyces pombe Glucan Synthases in the Presence of the Antifungal Agent Caspofungin.

In recent years, invasive fungal infections have emerged as a common source of infections in immunosuppressed patients. All fungal cells are surrounded by a cell wall that is essential for cell integrity and survival. It prevents cell death and lysis resulting from high internal turgor pressure. Since the cell wall is not present in animal cells, it is an ideal target for selective invasive fungal infection treatments. The antifungal family known as echinocandins, which specifically inhibit the synthesis of the cell wall β(13)glucan, has been established as an alternative treatment for mycoses

Heterogeneity in the transcriptional response of the human pathogen Aspergillus fumigatus to the antifungal agent caspofungin.

Aspergillus fumigatus is the main causative agent of invasive pulmonary aspergillosis (IPA), a severe disease that affects immunosuppressed patients worldwide. The fungistatic drug caspofungin (CSP) is the second line of therapy against IPA but has increasingly been used against clinical strains that are resistant to azoles, the first line antifungal therapy. In high concentrations, CSP induces a tolerance phenotype with partial reestablishment of fungal growth called CSP paradoxical effect (CPE), resulting from a change in the composition of the cell wall. An increasing number of studies has

Effects of caspofungin, tolcapone and other FDA-approved medications on MRSA susceptibility to vancomycin.

OBJECTIVES: Vancomycin is a first-line antibiotic for the treatment of invasive infections in humans caused by methicillin-resistant Staphylococcus aureus (MRSA). Based on the premise that antibiotic combinations can exhibit synergistic and antagonistic interactions, medications used for the treatment of infection and other medical conditions were evaluated for their ability to alter MRSA susceptibility to vancomycin. METHODS: A chemical library comprised of 1237 pharmacological agents was evaluated in a 96-well plate format for its ability to inhibit MRSA growth in combination with half the

Utilization and comparative effectiveness of caspofungin and voriconazole early after market approval in the U.S.

OBJECTIVES: Both caspofungin and voriconazole were initially approved by the FDA with very narrow indications. Our aim was to evaluate the utilization patterns and comparative effectiveness of these agents early after marketing before any labeling change occurred. METHODS: This was a retrospective cohort study utilizing a large healthcare database in the United States. Patients who received at least one dose of systemic antifungal agent between the years 2001 and 2003 were included. Information was available for each hospital-day including underlying conditions, medications, procedures and di

Complete heart block in a neutropenic patient with aspergillosis: An unusual adverse effect of caspofungins.

We present a case of complete heart block (CHB) in a 58-year-old female patient with acute myeloid leukemia (AML) with no past history of cardiac disease, who received caspofungin in the treatment of disseminated fungal infection. To our knowledge, this is the first case of CHB associated with caspofungins. Subsequent to induction chemotherapy the patient developed invasive pulmonary aspergillosis with sudden tachypnea, dyspnoea, fever, bilateral pulmonary infiltrates and acute respiratory insufficiency consequent to neutropenia with ANC<500. Disseminated candidemia refractory to caspofungin therapy in an infant with extremely low birth weight.

Systemic fungal infections have high morbidity and mortality rates in neonates, especially neonates with an extremely low birth weight (ELBW). Here, we describe a 21-day-old ELBW female infant with an amphotericin B-unresponsive congenital Candida albicans infection that was treated with caspofungin. Blood sterilization was performed during the first episode, but a second episode of candidemia occurred after the discontinuation of caspofungin. Blood sterilization was again performed during the second round of caspofungin treatment, but fungal endocarditis and renal fungal balls still developed
