Ureteral drainage with double-J catheters in obstructive uropathy during pregnancy. A report of 3 cases.

Acute dilatation of the upper urinary tract during pregnancy is a common cause of renal colics and urinary tract infections, leading, rarely, to renal insufficiency. We describe 3 cases with symptomatic hydronephrosis at 28 weeks of gestation who underwent placement of an internal urinary drainage by so-called double-J ureteral stents which rapidly reduced obstruction and symptoms and allowed pregnancies to continue to term. We conclude that double-J ureteral stenting is an effective, simple, safe, and economical method to treat acute hydronephrosis during pregnancy.

Obstructed central venous catheters. Restoring function with a 12-hour infusion of low-dose urokinase.

Thrombotic obstruction frequently prohibits infusion through or withdrawal of blood from central venous catheters and can occur in conjunction with symptomatic thrombosis of the subclavian vein. Thirty catheters were radiographically proved to be obstructed by thrombus and had not responded to at least one instillation of 5000 units of urokinase. All catheters were treated with a 12-hour infusion of urokinase at the rate of 40,000 units/hour. The obstructing thrombus was either eliminated or reduced in size in all instances and full function was restored in all but one catheter. No bleeding co

Absence of immunoreaction and cellular adhesion in a polyvinylpyrrolidone-coated ventricular catheter with choroid plexus obstruction: A case report.

BACKGROUND: While a variety of modalities are available for the treatment of hydrocephalus, ventriculoperitoneal shunting (VPS) remains the most utilized treatment. Although efficacious, VPS is susceptible to malfunction, with catheter obstruction as the primary cause of failure in pediatric patients. Prior studies have speculated that implanted catheters trigger an immune response from the central nervous system, resulting in cellular reactivity and subsequent obstruction of the device. These cells are derived from the choroid plexus (ChP), which plays an active role in immunological surveill

Computational Modeling and Simulation to Quantify the Effects of Obstructions on the Performance of Ventricular Catheters Used in Hydrocephalus Treatment.

Pediatric hydrocephalus is a debilitating condition that affects an estimated 1-2 in 1000 newborns, and there is no cure. A traditional treatment is surgical insertion of a shunt system which was designed 50 years ago, and minimal ensuing progress has been made in improving the failure rate of these devices resulting in the need for multiple brain surgeries during an affected child's lifetime for shunt replacement. A first step toward decreasing the failure rate is to optimize the ventricular catheter component of the shunt to minimize its propensity for obstruction. Given the many geometric p

[Ventricular tachycardia induced by the change of position for epidural catheter insertion in a patient with hypertrophic obstructive cardiomyopathy].

We report here a case of ventricular tachycardia (VT) induced by the change of position for insertion of an epidural catheter before the induction of general anesthesia. A 79-yr-old woman was scheduled for elective right lung surgery. Past history included hypertension and hypertrophic obstructive cardiomyopathy (HOCM), which were treated with a calcium channel blocker and an anti-arrhythmic drug. Preoperative echocardiogram (ECG) showed first degree arterio-ventricular block and left ventricular hypertrophy. Before the induction of general anesthesia, the patient was turned into the left late

Iatrogenic Ureteral Obstruction With Ureteral Orifice Polyp Secondary to Foley Catheter Misplacement: A Case Report With Cystoscopic and Imaging Findings.

INTRODUCTION: Foley catheterization is a routine urologic procedure, yet complications may arise, particularly in patients with long-term indwelling catheters. CASE PRESENTATION: We present a rare case of iatrogenic ureteral obstruction caused by inadvertent Foley catheter insertion into the right ureter in a 55-year-old male nursing home resident. The condition led to right-sided hydronephrosis and recurrent urinary tract infections. Diagnosis was established using computed tomography, which confirmed the catheter's entry into the ureter, and cystoscopy revealed polypoid lesions around the r

The shadow in the darkness: Case report on adhesive intestinal obstruction secondary to ventriculoperitoneal shunt catheter in an elderly patient.

INTRODUCTION: Shunt placement is an effective therapy for hydrocephalus. Ventriculoperitoneal shunt draining excess cerebrospinal fluid connects the cerebral ventricles to the abdominal cavity. However, intestinal obstruction may ensue as an infrequent complication of the shunt. CASE PRESENTATION: A 65 years old female patient presented with abdominal pain, abdominal bloating, and ceased passage of flatus and stool for six days. She had a history of undergoing a VP shunt procedure due to midbrain obstruction and supratentorial hydrocephalus. Conservative treatment at another local hospital co

Failed Awake Intubation for Critical Airway Obstruction Rescued With the Ventrain Device and an Arndt Exchange Catheter: A Case Report.

A 71-year-old man with advanced vocal cord carcinoma presented with severe airway obstruction. Therapeutic anticoagulation with enoxaparin complicated management. Failure of an oral awake bronchoscopic intubation was rescued by passing a guidewire through the working channel and threading an Arndt exchange catheter into the trachea under videoscopic vision. Ventilation with the Ventrain device lasting 40 minutes (15 L/min, inspiration/expiration 1:1, 15 breaths/min), during IV anesthesia with muscle paralysis, resulted in excellent blood gas values until placement of the tracheal cannula. Catheter ablation in patients with atrial fibrillation and hypertrophic obstructive cardiomyopathy: a real-world comparative outcomes study.

AIMS: Atrial fibrillation (AF) commonly coexists with hypertrophic obstructive cardiomyopathy (HOCM) and is associated with increased risks of stroke and mortality. While catheter ablation (CA) is effective in AF, outcomes in patients with HOCM remain underreported. We sought to evaluate the association between CA and long-term outcomes in patients with paroxysmal AF (PAF) and HOCM. METHODS AND RESULTS: Using the TriNetX Research Network, we identified adults (≥18 years) diagnosed with HOCM and PAF between 2015 and 2021. Patients were stratified into CA vs. non-CA groups. Chronic Obstructive Pulmonary Disease is Associated with Higher Recurrence Rates of Atrial Fibrillation Following Catheter Ablation.

BACKGROUND: Patients with atrial fibrillation (AF) and chronic obstructive pulmonary disease (COPD) are at increased risk of cardiovascular mortality compared to patients with AF alone. Consequently, employing rhythm control strategies such as AF catheter ablation could offer substantial benefits to patients with COPD. However, the impact of COPD on AF ablation outcomes is not well established. METHODS: In this single-center case control study, we retrospectively analyzed 200 patients with AF and COPD, 52 of whom underwent AF catheter ablation. Those who underwent ablation were matched with a
