COF-confined CsPbBr3 nanocomposite with CRISPR/Cas12a-driven DNA walking for ultrasensitive electrochemiluminescent detection of circulating tumor DNA.

A highly sensitive electrochemiluminescence biosensor was developed for circulating tumor DNA detection by integrating a covalent organic framework-confined CsPbBr3 nanocomposite with a CRISPR/Cas12a-driven amplification strategy. The covalent organic framework not only stabilizes CsPbBr3 nanocrystals in aqueous environments but also regulates interfacial charge transfer and enables programmable immobilization of DNA probes. Upon recognition of the target sequence, Cas12a is activated and progressively cleaves surface-tethered quencher-modified DNA strands, leading to stepwise restoration of e

Circulating Tumor DNA and Circulating Tumor Cells in Liquid Biopsy as Post-Treatment Prognostic Biomarkers in Early Breast Cancer: A Systematic Review and Meta-Analysis.

Despite advances in systemic therapy, many early breast cancer patients experience recurrence due to subclinical minimal residual disease (MRD). Circulating tumor DNA (ctDNA) and circulating tumor cells (CTCs) have emerged as promising liquid biopsy markers for post-treatment MRD detection. This pre-registered systematic review and meta-analysis (PROSPERO/PRISMA 2020) evaluated their comparative prognostic significance. Seven databases were searched from inception to March 2026. Eligible studies included early breast cancer patients undergoing post-treatment ctDNA or CTC assessment after neoad

The Effect of Unique Molecular Identifier Family Size Using Tumor-Informed Circulating Tumor DNA Analysis in Childhood Cancers.

Analysis of circulating tumor DNA (ctDNA) provides a molecular assessment that can complement routine imaging in childhood cancer management. Detailed monitoring of ctDNA levels may provide information on treatment efficacy and resistance and minimal residual disease and allows for early detection of relapse. Here, tumor-informed ctDNA analysis was applied to 90 blood plasma samples collected from eight children with malignant tumors. A total of 4 to 10 tumor-specific mutations per patient were assessed using SiMSen-Seq, a digital sequencing approach using unique molecular identifiers (UMIs).

Circulating tumor DNA monitoring and blood tumor mutational burden in patients with metastatic solid tumors treated with atezolizumab.

Immune checkpoint inhibitors are important for treatment across tumor types but are not universally effective in controlling disease. Early understanding of tumor response, or lack thereof, can inform treatment decisions. This study evaluates changes in circulating tumor DNA (ctDNA) and blood tumor mutational burden (bTMB) for associations with response to programmed cell death 1 ligand 1 (PD-L1) blockade. We sequenced cell-free DNA collected at the start of therapy, on treatment, and at the end of therapy for 153 patients treated with atezolizumab as part of the pan-tumor MyPathway study (NCT

Longitudinal Tumor-informed Circulating Tumor DNA Status Predicts Disease Upstaging and Poor Prognosis for Patients Undergoing Radical Cystectomy.

BACKGROUND AND OBJECTIVE: Decision-making on the use of neoadjuvant and adjuvant treatment for patients with bladder cancer undergoing radical cystectomy (RC) currently depends on assessment of clinical and pathological features, which lack sensitivity. Circulating tumor DNA (ctDNA) has emerged as a possible novel prognostic biomarker in the field. Our aim was to assess whether ctDNA status before RC is predictive of pathological and oncological outcomes. We also evaluated the dynamic changes in ctDNA status after RC in relation to recurrence-free survival (RFS). METHODS: We analyzed data for

Circulating Tumor DNA from Ascites as an alternative to tumor sampling for genomic profiling in ovarian cancer patients.

Genomic testing is crucial for the management of ovarian cancer. DNA from biopsies at diagnostic laparoscopies or interval debulking surgery after neoadjuvant chemotherapy, has a high failure rate. At relapse, biopsies may not be feasible. The aim of our study was to evaluate the feasibility and usefulness of measuring genomic instability score (GIS) on cell-free DNA (cfDNA) from ascites.Patients enrolled in a prospective study (NCT03010124) consented to analysis of biological samples. CfDNA was extracted from 1 to 4 ml of double-centrifuged fresh ascites. Targeted Next-generation sequencing (

Feasibility of tumor-informed circulating tumor DNA for detecting minimal residual disease in surgically resected biliary tract cancer.

BACKGROUND: We prospectively investigated the potential of tumor-informed circulating tumor DNA (ctDNA) for detecting minimal residual disease (MRD) in surgically resected biliary tract cancer (BTC). METHODS: Personalized panels were developed using individual variants identified from the whole-exome sequencing of surgical specimens from each patient. Two sequential blood samples, collected preoperatively and within 6 weeks after surgery, were analyzed. A positive ctDNA result was defined as the identification of two or more patient-specific mutations. RESULTS: A total of 18 patients were en

An alignment-free strategy for circulating tumor DNA detection and tumor fraction estimation from whole-genome sequencing data.

Circulating tumor DNA (ctDNA) is emerging as a promising biomarker for postoperative monitoring of cancer patients. Precise estimation of circulating tumor fraction is crucial for evaluating treatment effects and timely detection of disease recurrence. All current ctDNA detection methods that utilize whole-genome sequencing (WGS) data rely on the reference genome alignment of sequencing reads and often apply separate tools for detecting different variant types. However, various bioinformatic analysis confounders and the application of external variant calling tools could be avoided by analyzin

Artificial Intelligence Measured Tumor Burden and Pre-Treatment Circulating Tumor DNA in Human Papilloma Virus-Associated Oropharynx Cancer.

BACKGROUND: Artificial intelligence (AI)-based imaging analysis and circulating tumor-associated DNA (ctDNA) are both being used diagnostically in HPV-driven oropharynx squamous cell carcinoma (HPV-OPSCC). We evaluated associations between AI-measured tumor burden and ctDNA. METHODS: We analyzed 170 patients treated definitively for HPV-OPSCC. All had pre-treatment serum tumor-tissue modified viral (TTMV) ctDNA levels. An AI algorithm measured tumor and lymph nodes on CT scans. Linear regressions detected associations between ctDNA (fragments/mL) and automated volumes, clinical tumor (T) and

Current Evidence for Circulating Tumor DNA in Sarcoma: Challenges and Opportunities for Clinical Application.

Sarcomas represent a diverse group of mesenchymal tumors with high rates of recurrence after resection. While recent technical advances have enabled the detection of rare circulating tumor DNA (ctDNA) in other malignancies, the complexity and heterogeneity of sarcoma genomics have historically limited ctDNA in these cancers. This narrative review highlights the rapidly evolving evidence supporting potential clinical applications of ctDNA in common sarcoma subtypes including gastrointestinal stromal tumor, leiomyosarcoma, rhabdomyosarcoma, osteosarcoma, and Ewing sarcoma.

Circulating Tumor DNA in Upper Tract Urothelial Carcinoma: A Framework for Precision Perioperative Management.

Upper tract urothelial carcinoma (UTUC) presents distinct diagnostic and therapeutic challenges because of its rarity, anatomic constraints, frequent understaging at biopsy, and risk of systemic recurrence after radical nephroureterectomy. Current perioperative management is driven primarily by clinicopathologic risk factors, which may be insufficient to identify occult molecular residual disease (MRD) or to determine which patients are most likely to benefit from systemic therapy. This narrative review summarizes available evidence on circulating tumor DNA (ctDNA) in UTUC and related urotheli

Ultrasensitive circulating tumor DNA detection improves prognostication in triple-negative breast cancer postneoadjuvant therapy.

BACKGROUND: Current treatment guidelines for patients diagnosed with stage II to III triple-negative breast cancer (TNBC) rely on the use of neoadjuvant therapy (NAT). A critical unmet need is a sensitive test that can inform the presence of distant micrometastases to help personalize postneoadjuvant systemic therapy beyond pathological complete response (pCR). PATIENTS AND METHODS: In this study, 286 plasma samples from 86 patients from the SCANDARE study were retrospectively analyzed for circulating tumor DNA (ctDNA) at three presurgical timepoints: baseline, during and post-NAT, and during
